BACKGROUND: Premature birth poses significant health challenges, including respiratory distress syndrome (RDS). Corticosteroids reduce the incidence of RDS, but higher dexamethasone doses may lead to adverse neonatal outcomes, such as growth restriction and neurodevelopmental issues. Determining the appropriate dose is crucial to balance efficacy and safety. Dexamethasone is inexpensive and widely available in most low- and middle-income countries. This study aims to compare the efficacy and safety of 4-mg, 5-mg and 6-mg dexamethasone in preventing RDS among preterm infants. This trial aims to

Introduction Neonatal seizures are a significant neurological manifestation in the early days of life. They can cause long-term developmental disabilities and increase mortality rates if not properly managed. They can also be a sign of underlying neurological disorders, metabolic disturbances, or hypoxic-ischemic injury. Among the numerous etiologies, birth asphyxia, neonatal meningitis, and biochemical abnormalities are widespread. Biochemical abnormalities occur either as an underlying cause or as an associated abnormality. So biochemical anomalies should be excluded in every case of neonata

BACKGROUND: In addition to hyperglycemia, women with hypoglycemia identified during pregnancy have a higher risk of adverse pregnancy outcomes. However, there is limited evidence of the association between prepregnant hypoglycemia and adverse pregnancy outcomes in women without pre-existing diabetes. This study aims to explore the association between maternal preconception hypoglycemia and adverse pregnancy outcomes among childbearing-aged women in China.
METHODS AND FINDINGS: This was a retrospective cohort study of the National Free Preconception Checkup Project (NFPCP), including women who

BACKGROUND AND OBJECTIVES: Labor is a physically demanding and painful process that may lead to fat breakdown, ketone accumulation, and ketosis, potentially resulting in metabolic acidosis. Proper management of oral intake during labor helps mitigate this risk. We reviewed the published impact of oral intake management during labor on maternal and neonatal outcomes.
METHODS AND STUDY DESIGN: The scoping review used Arksey and O'Malley's methodological framework. The systematic search was conducted using PubMed, Web of Science, Embase, Cochrane Library, Scopus, CNKI, and CINAHL Complete databa

BACKGROUND: Most patients with pre-labor rupture of membranes (PROM) enter labor spontaneously at term. However, maternal and neonatal complications increase with prolonged PROM. Induction of labor (IOL) is therefore often recommended, but the optimal agent is still debated. Low-dose oral misoprostol (OM) is safe, effective, cheap, and easy to administer. However, there is little data regarding its role in PROM at term.
OBJECTIVE: To determine the safety and efficacy of OM compared to Intravenous Oxytocin for IOL for PROM at term.
STUDY DESIGN: A cohort study compared the perinatal outcomes

Maturity-onset diabetes of the young (MODY)-a monogenic form of diabetes-accounts for approximately 1-2% of all diabetes cases, with GCK-MODY being the second most commonly diagnosed type. Although the inherited nature of the disease implies that the interplay between maternal glycemia and fetal genotype directly influences neonatal outcomes, clinical guidelines for MODY-complicated pregnancies remain underdeveloped. A systematic literature search in the PubMed, Scopus, Web of Science, and Cochrane databases was conducted following the PRISMA guidelines. The study protocol has been logged in t

Sotos syndrome is an autosomal dominant disorder resulting from pathogenic variants of the NSD1 gene. In this study, we present five Chinese paediatric cases, including two previously unreported NSD1 variants: a nonsense mutation (c.1486A > T p. Lys496*) and a missense mutation (c.6086C > T) (p. Thr2029Ile) respectively. Additionally, we analyzed the genotypic and phenotypic spectrum of 23 Chinese children with molecularly confirmed Sotos syndrome. Patients exhibited characteristic craniofacial features and significant overgrowth. All patients showed DD/ID and five patients (21.7%) showed symp

CONTEXT: The postnatal management of preterm infants at birth may influence their clinical course in the short, medium, and long term. The concept of the "Golden Hour" (GH) has emerged in neonatology, aiming to standardize this management.
OBJECTIVE: We conducted a meta-analysis to assess GH's impact on early clinical outcomes and on the comorbidities of prematurity.
DATA SOURCES: Pubmed, Embase, Scopus, and Cochrane Library were searched without any restriction.

OBJECTIVES: Hypoglycemia is a widespread pediatric emergency that can manifest in various ways. One of the most critical symptoms is the occurrence of seizures. Recognizing these episodes promptly is essential in managing the condition effectively. Physicians' apprehension, specifically pediatricians', can lead to early diagnosis and improve the prognosis by decreasing the neurologic aftermath. By presenting data and analysis on the prevalence of hypoglycemic seizures, triggers, and neurologic side effects, we plan to raise awareness of the issue. This study intends to demonstrate the leading

Maternal health has a profound impact on fetal development, influencing the risk of pediatric endocrine disorders both directly and indirectly through various biological and environmental mechanisms. Throughout pregnancy, several endocrine disorders can arise or be exacerbated due to the physiological changes that occur. An in-depth review of articles with evidence-based research discussing the significant effects of maternal endocrinopathies and endocrine disruptors on fetal development and infant health was conducted in this review paper. The most common endocrine disorder during pregnancy i

INTRODUCTION: Mauritius is among the countries with the highest disease burden globally in diabetes mellitus (DM), ranking twelfth in terms of disease impact. Though data is available for DM in Mauritius, there is a lack of data and research on gestational DM (GDM) in Mauritius. This study aimed to bridge this gap to determine the sociodemographic details of GDM patients in the Mauritian context, the prevalence of GDM in Mauritius, identify the risk factors predisposing Mauritian women to GDM, and assess maternal and fetal/neonatal complications of GDM in Mauritius.
METHODS: A cross-sectional

UNLABELLED: SLC44A1 gene variants (MIM # 618868) are associated with a choline transporter deficiency with a rare autosomal recessive genetic disorder characterized by neurodegeneration, childhood-onset with ataxia, tremor, optic atrophy, and cognitive decline. Variants in the SLC44A1 gene are considered to be responsible for the syndrome. We reported a four-month-old baby with neonatal cholestasis and liver failure, but neurological development and examination were normal. During the patient's initial physical examination, height, weight, and head circumference were < -2 SDS. He was alert, wi

Fatty acid oxidation disorders (FAODs) are rare inherited metabolic diseases that can present with various features including cardiomyopathy, arrhythmias, hypoketotic hypoglycemia, and liver dysfunction. Their clinical and radiological manifestations may at times overlap with acquired conditions such as viral myocarditis and multisystem inflammatory syndrome in children (MIS-C), making timely diagnosis challenging. We describe a 10-month-old infant boy with undiagnosed long-chain FAOD (LC-FAOD) who presented with sudden cardiac arrest following a viral illness. The patient's family history was

Carnitine transport and cycle disorders (CCD) are a group of metabolic disorders characterized by either carnitine depletion or dysfunction in the carnitine cycle, a critical process for the transport of fatty acids into the mitochondria and their subsequent β-oxidation. Clinically, CCD can manifest with a wide range of symptoms, including hypoketotic hypoglycemia, which may be accompanied by signs of liver dysfunction, hepatic steatosis, myopathy and cardiomyopathy. Biochemical diagnosis typically involves measuring carnitine and acylcarnitine levels in blood, alongside organic acid profiling

AIM: Different hypoglycaemia screening guidelines for infants born small for gestational age (SGA) are applied throughout the world. This narrative review analysed the published guidelines and evaluated the characteristic features of hypoglycaemia in SGA infants to assess the relevance of the issue.
METHODS: PubMed and the Cochrane Database of Systematic Reviews were reviewed for papers regarding hypoglycaemia and hypoglycaemia screening guidelines in term and near-term SGA infants. We also searched government-related websites and national healthcare institution websites for guidelines.

Neonatal cholestasis can be caused by several conditions, with biliary atresia being the major cause. Genetic and endocrinological etiologies represent other possibilities, with most of them requiring a rapid diagnosis and a specific treatment. We describe a neonatal case of severe cholestasis with low gamma glutamyl transferase in a child presenting with multiple abnormalities, including pituitary stalk interruption syndrome and consequent hypopituitarism. The cholestasis was rapidly resolved with hormone therapy. Genetic analysis showed a de novo 17q chromosome deletion, including the HNF1β

Genome-wide paternal uniparental isodisomy mosaicism (GWpUPIDM) is an extremely rare condition characterized by varying proportions of an androgenetic cell line across different tissues. It is primarily associated with severe congenital hyperinsulinism (CHI), Beckwith-Wiedemann syndrome (BWS) stigmata, a high risk (69-79%) of developing neoplasia and, in some cases, additional manifestations of multilocus paternal imprinting disorders (MPIDs). We herein report the first Mexican/Latin American female patient GWpUPIDM presenting with non-syndromic CHI requiring subtotal pancreatectomy and persis
