Airborne Mucorales in the Netherlands largely mirror Dutch clinical isolates causing mucormycosis.

We determined the optimal method to culture Mucorales species from air samples. Subsequently, we investigated the diversity of Mucorales species in Dutch air and compared these species with those causing mucormycosis in patients. We optimized the Mucorales culturing protocol by testing different growth conditions with samples from a newly developed air sampling approach. We used 120 air samples taken throughout the Netherlands in the project called Schimmelradar (September-October 2023). These samples were supplemented with additional air samples taken in the Netherlands (February 2024). The M

Repurposing antiviral agents against mucormycosis.

Mucormycosis is a life-threatening fungal infection with limited treatment options and high mortality rates among immunocompromised individuals. To identify new therapeutic strategies, we screened a library of 618 antiviral compounds against Rhizopus delemar both alone and in combination with amphotericin B (AmB) to search for agents with intrinsic antifungal activity or the ability to enhance AmB's efficacy. Four candidates, IMB-301, U18666A, BLT-1, and obefazimod, showed potent in vitro effects, with three sustaining growth suppression comparable to AmB for up to 48 h in time-kill assays. Th

Real-World Performance of Aspergillus and Mucorales Polymerase Chain Reaction Assays on Bronchoalveolar Lavage Fluid in Immunocompromised Children and Young Adults.

We describe use of Aspergillus and Mucorales polymerase chain reaction (PCRs) in 54 immunocompromised children and young adults undergoing 66 bronchoscopies. Test positivity was 2/54 (3.2%) and 1/59 (1.7%) for Aspergillus and Mucorales. For Aspergillus PCRs, 4/54 (7.4%) positively impacted management, compared to 4/59 (6.8%) for Mucorales. Fungal PCRs may influence care of immunocompromised patients, though overall impact was limited.

Survival and renal outcomes of mucormycosis infection and its treatment regimens in resource-limited settings: a retrospective case series.

Mucormycosis is a rapidly progressive opportunistic fungal infection associated with high morbidity and mortality, especially in immunocompromised patients. In resource-limited settings, management is further hindered by delayed diagnosis and restricted access to safer antifungal formulations. To evaluate survival and renal outcomes among patients with mucormycosis treated with different antifungal regimens. A retrospective review was conducted on 19 patients diagnosed with mucormycosis. Clinical features, comorbidities, antifungal therapy, complications, and outcomes were analyzed. Patients w

Health-related quality of life after orbital exenteration for COVID-19-associated rhino-orbital-cerebral mucormycosis in South India: a cross-sectional SF-36 study and systematic review.

PURPOSE: Rhino-orbital-cerebral mucormycosis (ROCM) surged during the COVID-19 pandemic, often necessitating orbital exenteration for disease control. While survival outcomes are increasingly reported, long-term health-related quality of life (HRQoL) among survivors remains poorly characterized, particularly in low- and middle-income settings. This study evaluated HRQoL using the SF-36 among survivors of COVID-19-associated ROCM who underwent orbital exenteration. METHODS: In this cross-sectional study, adult survivors of orbital exenteration for COVID-19-associated ROCM treated at a tertiary

Rapid and reliable diagnosis of mucormycosis using colorimetric loop-mediated isothermal amplification.

Current diagnostic approaches for mucormycosis are often limited by low sensitivity and prolonged turnaround times, which result in delayed treatment and poor clinical outcomes. We developed a novel diagnostic method utilizing a colorimetric loop-mediated isothermal amplification (LAMP) assay for the rapid and sensitive detection of mucormycosis. The assay incorporates specifically designed primers capable of detecting as low as 0.001 picograms (pg) of spiked genomic DNA from Mucorales fungi. This LAMP assay demonstrated a high sensitivity of 98% and a 100% specificity of detecting fungal ribo

Protoellisomyces batrachophilus, gen. et sp. nov., a novel member of the Mucor circinelloides complex causing mucormycosis in a toad.

Mucormycosis is an opportunistic infection that affects humans and animals, being caused by moulds of the order Mucorales. In amphibians, mucormycosis has been well documented in association with Mucor amphibiorum. In this study, histopathological examination of cutaneous nodules in a Japanese common toad (Bufo japonicus) revealed fungal granulomas and sporangia. Phylogenetic analyses of five partial gene fragments, namely the internal transcribed spacer, mini chromosome maintenance complex component 7 (mcm7), largest subunit of RNA polymerase II (rpb1), 20S ribosomal RNA accumulation protein

ICU-specific determinants of mortality in critically ill patients with infective endocarditis : Mortality predictors in ICU patients with infective endocarditis.

OBJECTIVE: Infective endocarditis (IE) remains a life-threatening infectious disease characterized by high morbidity and mortality despite substantial advances in diagnostic and therapeutic strategies (Eur Heart J 44:3948-4042, 2023, Eur J Clin Microbiol Infect Dis 44:1325-1333, 2025). Patients requiring intensive care unit (ICU) admission represent the most severe clinical spectrum of IE and continue to exhibit markedly elevated mortality rates (Klimik Derg 32:2-116, Klimik Derg 36:216-225, 2023). Early identification of mortality predictors in critically ill patients is essential for optimiz

Identification and Validation of Rhizopus-Specific Glucoamylase as a Novel Biomarker for Diagnosing Mucormycosis.

BACKGROUND: Mucormycosis is a life-threatening invasive fungal infection. Current diagnosis relies on insensitive techniques with prolonged turnaround times, underscoring the need for rapid noninvasive diagnostic tools. The current study aimed to identify the antigens of Rhizopus arrhizus that can facilitate serodiagnosis of mucormycosis and assess their diagnostic potential in patients. METHODS: This was a 2-stage systematic study combining antigen discovery and clinical validation. Using immunoproteomics, we identified immunoreactive protein spots in R. arrhizus secretory extract, using poo

Rhino-orbito-cerebral mucormycosis in new-onset type 1 diabetes mellitus: a case-based short review.

Mucormycosis is an aggressive angioinvasive fungal infection that commonly involves the paranasal sinuses, orbit, lungs, and central nervous system (CNS). Patients with diabetic ketoacidosis (DKA) are uniquely susceptible because hyperglycemia and acidosis enhance fungal invasion and blunt innate immune defenses. We present a brief vignette of rhino-orbito-cerebral mucormycosis in a patient with recent diagnosis of type 1 diabetes mellitus (DM), and use it to frame a short review of epidemiology, pathogenesis, diagnosis, and management. Rapid recognition, urgent surgical debridement, prompt in

A case report of rhino-orbital-cerebral invasive infection caused by co-infection with Pleurostoma ootheca and Mucorales.

Pleurostoma species are rare opportunistic fungal pathogens that primarily infects immunocompromised hosts, causing phaeohyphomycosis, manifesting as localised superficial or deep infections, pulmonary infections, central nervous system infections, or disseminated diseases. Fungi of the order Mucorales are known to cause rapidly progressive infections in the lungs, rhino-orbital-cerebral regions, and skin, with high mortality rates. We report the first case of rhino-orbital co-infection by these pathogens in a diabetic patient, resulting in irreversible vision loss. The patient presented with

Primary gastric mucormycosis presenting as upper gastrointestinal bleeding.

Mucormycosis is a rare, aggressive fungal infection associated with high mortality, particularly in immuno-compromised individuals. Gastric involvement, though uncommon, poses significant diagnostic and therapeutic challenges that frequently result in poor clinical outcomes. Our case emphasizes the importance of including uncommon infectious aetiology in the evaluation of upper GI bleeding, particularly in immuno-compromised patients, to facilitate timely intervention and potentially improve outcomes.

Mid-term Clinical and Radiological Outcome of the BioBall Adapter System in Revision Total Hip Arthroplasty: A Retrospective Study.

AIM AND BACKGROUND: Revision total hip arthroplasty (rTHA) is a technically demanding procedure, particularly when the femoral stem is well-fixed. Removal of a stable stem increases operative time, morbidity, and risk of complications. The BioBall modular head-neck adapter system allows intraoperative restoration of leg length, offset, and version without stem removal. This study evaluates the mid-term clinical and radiological outcomes of the BioBall system in revision THA. METHODS: A retrospective, single-center observational case series was conducted including all rTHAs performed at Wright

Clinical and Radiological Predictors of Invasive Fungal Sinusitis.

BACKGROUND: Invasive fungal sinusitis (IFS) is associated with high mortality rates, chiefly because it is often diagnosed late in the course of disease when infection has spread to the orbit, skull base, or intracranially. We aimed to define early clinical and radiologic indicators of IFS, which can be integrated into a prediction model and clinical management algorithms. METHODS: Patients with proven or probable IFS were identified in 2 tertiary-level hospitals as part of prospective monitoring of invasive mycoses. Patients with similar underlying medical conditions who underwent sinus comp

Advances in the Diagnosis of Invasive Pulmonary Mold Infections: Focus on Diagnostic Performance and Cost-Effectiveness of Diagnostic Tests.

Invasive pulmonary mold infections (IPMIs) are critical complications in immunocompromised patients, contributing significantly to morbidity and mortality. Diagnosing pathogens like Aspergillus species (spp.) and the Mucorales remains challenging due to non-specific clinical presentations and the limitations of traditional culture methods. This review provides an up-to-date synopsis of IPMI diagnostic tools, focusing on their diagnostic performance, turnaround time (TAT), and cost-effectiveness. We conducted a narrative review of the current literature regarding clinical evaluation, radiograph

Primary mediastinal mucormycosis presenting with hoarseness: a case report.

BACKGROUND: Mucormycosis is a rare, rapidly progressive fungal infection caused by molds of the order Mucorales, most commonly Rhizopus species. While it predominantly affects immunocompromised patients, mediastinal mucormycosis is exceptionally rare, with fewer than 50 cases reported, and has a high mortality rate. Diagnosis is challenging owing to nonspecific clinical and radiologic findings, often mimicking malignancies such as lymphoma or thymoma. This study aims to report a rare case of primary mediastinal mucormycosis presenting with hoarseness and to highlight the diagnostic challenges
