Immune Phenotypes in Patients With Invasive Mould Infection Support the Use of PD-1 Inhibition as Potential Treatment Option.

BACKGROUND: Invasive mould infections (IMI) cause substantial morbidity and mortality in populations at risk. Novel treatment approaches are urgently needed. Targeting immune checkpoints may reverse hyporesponsiveness of the innate and adaptive immune systems. METHODS: In this prospective, observational study, we investigated immune checkpoint expression levels on immune cells in patients with invasive aspergillosis (IA; n = 25) and mucormycosis (MU; n = 7). Healthy controls (HC; n = 5) and patients with matched haematological diseases but without IMI served as control populations (CP; n = 10

Mucormycosis coinfection in patients with proven aspergillosis.

Data on the coinfection of molds are limited. Therefore, we systematically investigated mucormycosis coinfection in patients with morphologically proven aspergillosis. The medical records of adult patients with proven aspergillosis and available formalin-fixed paraffin-embedded (FFPE) tissue sections were retrospectively reviewed at a tertiary hospital between January 2019 and July 2024. The fungal culture results were reviewed and polymerase chain reaction (PCR) was performed to detect Aspergillus- and Mucorales-specific DNA using FFPE tissues. A positive Mucorales PCR test was confirmed when

The first report of pseudoaneurysm secondary to Cunninghamella bertholletiae infection in a Haematopoietic stem cell transplantation recipient: a case report and literature review.

BACKGROUND: Mucormycosis is a life-threatening complication that occurs in haematopoietic stem cell transplantation (HSCT) recipients. Among mucormycosis patients, those infected with Cunninghamella bertholletiae have the poorest prognosis because of the high virulence and angioinvasive nature of this organism. Despite its high vascular invasiveness, pseudoaneurysm caused by C. bertholletiae in HSCT recipients has rarely been reported. CASE PRESENTATION: An 8-year-old HSCT recipient experienced recurrent fever, cough and pain after HSCT. Teicoplanin, acyclovir, posaconazole and caspofungin we

Molecular characterization and virulence of fungal pathogens associated with mass mortalities in hilsa Shad (Tenualosa ilisha).

The rising incidence of invasive microbial infections and the expanding spectrum of pathogens make early and accurate identification of the causative pathogen a daunting task. Accurate diagnosis enables the identification and characterization of microbes and offers new possibilities for the control and prevention of infectious diseases. For the first time, we isolated and characterized three fungal species, viz., Mucor circinelloides, Fusarium equiseti, and F. incarnatum from hilsa (Tenualosa ilisha). The characterization was based on fungus morphology (in vitro culture and microscopy), molecu

Development and evaluation of a Pan-Mucorales Real-time PCR and a multiplex Real-time PCR for detection and identification of Rhizopus arrhizus, Rhizopus microsporus, and Mucor spp. in clinical specimens.

UNLABELLED: Mucormycosis is a life-threatening infection associated with high morbidity and mortality. Rapid and accurate diagnosis is essential for improving patient outcomes. Conventional diagnostic methods, such as histopathology and culture, are limited by low sensitivity and prolonged turnaround times, while commercial polymerase chain reaction (PCR) assays are costly and may lack specific genus or species targets. Here, we present a novel molecular diagnostic workflow to facilitate the rapid detection of Mucorales directly from clinical specimens. This workflow integrates two in-house in

Disguised Threat: Rhino-Orbital Mucormycosis Masquerading as Acute Dacryocystitis.

Mucormycosis is an aggressive, life-threatening fungal infection predominantly affecting immunocompromised individuals. It typically manifests as rhino-orbital or rhino-cerebral disease. However, rarely, its initial presentation may closely resemble an innocuous condition like acute dacryocystitis, which is prone to result in higher morbidity rates. Herein, we report a case of a 52-year-old diabetic patient who presented with symptoms suggestive of acute dacryocystitis. Broad-spectrum antibiotics were started, but the patient did not improve with therapy. Further investigations, including imag

Sensitivity and specificity of immunohistochemistry for the diagnosis of filamentous fungal infections.

OBJECTIVES: Many fungal species share overlapping morphologic features in tissue sections, preventing reliable identification and optimal treatment. We sought to determine whether immunohistochemistry (IHC) using a panel of commercially available antibodies could effectively distinguish between fungi commonly encountered in anatomic pathology specimens. METHODS: Anti-Aspergillus, anti-Rhizopus, and anti-Candida IHC was performed on formalin-fixed, paraffin-embedded tissue sections from 24 cases with fungal infections identified by culture or sequencing (including 4 polyfungal infections). Leading developments in basic research on aspergillosis and mucormycosis.

This editorial presents an overview of notable contributions in basic research on aspergillosis and mucormycosis published between 2022 and 2024. Basic research in aspergillosis saw major advances in the field of genomics brought about by inexpensive whole-genome sequencing of hundreds of isolates. This has deepened our understanding of Aspergillus fumigatus population structure, gene diversity, and the evolution of azole antifungal resistance. Basic research in mucormycosis saw interesting developments in our understanding of the interactions between Rhizopus microsporus and endosymbiotic bac

Detection of Mucorales antigen in bronchoalveolar lavage samples using a newly developed lateral-flow device.

A murine IgG2b monoclonal antibody, named TG11, binding to an extracellular polysaccharide antigen secreted by all Mucorales fungi has been recently developed and integrated into a lateral-flow device (TG11-LFD). The aim of this study was to establish the clinical performance of TG11-LFD on bronchoalveolar lavage (BAL) fluids for the diagnosis of mucormycosis. Thirteen BAL samples from 13 patients with mucormycosis, all of which tested positive for Mucorales qPCR (Mucor/Rhizopus [n = 5], Lichtheimia [n = 2], Rhizomucor [n = 5], and Cunninghamella [n = 1]), were used to assess the TG11-LFD.
