Fetal Hepatosplenomegaly: Stepwise Diagnostic Framework, Diagnostic Approach to Fetal Hepatosplenomegaly.

Background/Objectives: Fetal hepatosplenomegaly is an uncommon but clinically significant prenatal finding associated with a wide range of heterogeneous conditions, including congenital infections, fetal anemia, genetic syndromes, metabolic disorders, and other, less common abnormalities. Because of its nonspecific presentation and diverse etiologies, it represents a diagnostic challenge that requires a structured, multidisciplinary approach. This review aims to present a practical, clinically oriented, stepwise approach to the prenatal evaluation of fetal hepatosplenomegaly, integrating curre

Congenital Skin Sloughing in the Setting of Hydrops Fetalis Secondary to Congenital Diffuse Lymphangiectasia Mimicking Epidermolysis Bullosa.

We present a case where the combination of severe hydrops fetalis, lymphangiectasia, and skin desquamation created a clinical picture that masqueraded as epidermolysis bullosa (EB). A neonate presented at birth with severe hydrops fetalis and extensive skin sloughing. Skin biopsy findings were consistent with EB; however, the clinical course and genetic testing were inconsistent with this diagnosis, and the postmortem examination revealed the underlying cause to be severe congenital diffuse lymphangiectasia. It is important for clinicians to consider edema bulla and tissue hypoxia as a cause o

Premature Ductus Arteriosus Constriction and Pulmonary Trunk Dilation in a Case of Non-Immune Hydrops Fetalis With Trisomy 21.

Premature constriction of the ductus arteriosus accompanied by the dilation of the pulmonary trunk constitutes a rare congenital heart anomaly that may be underestimated and can result in fetal hydrops and ultimately death if not treated; therefore, it necessitates fetal post-mortem evaluation in all cases of legal abortion or fetal death.

Intrauterine interventions for severe fetal anemia due to parvovirus B19 national outbreak.

OBJECTIVE: Parvovirus B19 infection during pregnancy can cause severe fetal anemia, hydrops fetalis, and fetal death. This study describes our experience managing ten cases of severe fetal anemia due to parvovirus B19 infection during the 2023-2024 national outbreak in Israel. METHODS: This single-center descriptive cohort study included ten singleton pregnancies with fetal anemia (MCA-PSV >1.5 MoM) requiring intrauterine transfusion. Parvovirus B19 infection was confirmed by maternal IgM serology and/or amniotic fluid PCR. For pregnancies <20 weeks of gestation, we implemented a modified int

Diagnosis and management of primary fetal pleural effusion: A narrative review.

Primary fetal pleural effusion involves the accumulation of fluid in the fetal pleural space, posing considerable challenges for perinatal diagnosis and management. Primary effusions, emerging from the maldevelopment of lymphatic vessels, differ significantly from secondary effusions, which are associated with a spectrum of anomalies and conditions, affecting clinical approach and prognosis. The diagnosis of primary fetal pleural effusion is one of exclusion. The natural history of the condition ranges from spontaneous resolution in mild cases to progression to life-threatening complications.

Prenatal radiofrequency ablation for bronchopulmonary sequestration: a case report of successful fetal intervention and longitudinal outcomes.

INTRODUCTION: Bronchopulmonary sequestration (BPS), a rare congenital pulmonary malformation, is characterized by nonfunctional pulmonary parenchyma with aberrant systemic arterial supply (typically from the aorta or its major branches) and absence of communication with the tracheobronchial tree. Large BPS lesions pose significant fetal risks, including hydrops fetalis, mediastinal shift, and polyhydramnios, necessitating timely prenatal intervention to prevent fetal demise. CASE PRESENTATION: We present a case of successful ultrasound-guided radiofrequency ablation (RFA) of the feeding arter

Third trimester hydrops fetalis as the presentation leads to prenatal diagnosis of familial hemophagocytic lymphohistiocytosis.

OBJECTIVE: We first report a case of familial hemophagocytic lymphohistiocytosis (FHL) diagnosed in utero due to third trimester hydrops fetalis. CASE REPORT: A 28-year-old G1P0 woman had a normal nuchal translucency scan and 2 second trimester ultrasounds at 18 and 24 weeks. The third trimester ultrasound at 30 weeks showed mild polyhydramnios. The follow-up ultrasound at 32 weeks showed features of hydrops fetalis with pleural effusions, ascites, skin edema and polyhydramnios. Fetal blood sampling via cordocentesis showed a mild anemia (hemoglobin-100 g/L), thrombocytopenia (platelets-12×10

Invasive in-utero cardiac intervention in the hydropic fetus may improve survival: results from the International Fetal Cardiac Intervention Registry (IFCIR).

OBJECTIVE: To describe the pregnancy and postnatal outcomes of a large, retrospectively analyzed case-cohort series of pregnancies complicated by congenital heart disease (CHD) and fetal hydrops that were assessed for potential for fetal cardiac intervention (FCI). METHODS: The International Fetal Cardiac Intervention Registry (IFCIR) collects diagnostic and procedure-related data on maternal-fetal dyads with CHD that are considered by fetal specialists to be potentially capable of benefiting from FCI, with data submitted voluntarily by local institutions. For this retrospective analysis, the

Incremental yield of genome sequencing after exome sequencing for nonimmune hydrops fetalis spectrum.

OBJECTIVE: Exome sequencing (ES) detects a monogenic disorder in approximately 30% of pregnancies with unexplained nonimmune hydrops fetalis spectrum (NIHFS), defined as the presence of one or more pathologic fetal fluid collection. Since genome sequencing (GS) can identify additional genetic variants not detectable by ES, we sought to determine the proportion of cases with new reportable findings on GS following nondiagnostic standard of care testing, including ES, for NIHFS. STUDY DESIGN: Cohort study of pregnancies with NIHFS that had nondiagnostic results of karyotype and/or chromosomal m

[Ultrasound diagnosis of fetal infections: current possibilities and limitations].

INTRODUCTION: Fetal infections, particularly those caused by TORCH agents (toxoplasmosis, other [syphilis, varicella-zoster, parvovirus B19], rubella, cytomegalovirus, herpes), may significantly affect fetal growth and development, resulting in a substantial risk of morbidity and mortality. Since infections are mostly preventable causes of congenital anomalies, early diagnosis relies on serological and molecular testing and fetal ultrasound. OBJECTIVE: The aim of this study was to assess the prevalence of fetal infections and their detectable ultrasound features. METHOD: This retrospective s

Impaired lymphatic drainage in a child with hydrops fetalis and a novel variant in the PROX1 gene: case report and review of literature.

PROX1 is a gene that encodes a protein that may play a key role in the development of the lymphatic system. This report describes impaired lymphatic drainage, with non-immune hydrops fetalis, congenital bilateral chylothorax, chylous ascites, and the need for peritoneal dialysis in a preterm neonate born at 32 weeks of gestation. The child died at about seven months of life. Clinical Exome Sequencing revealed the novel heterozygous variant c.502C > T in the PROX1 gene, which determines the change p.Arg168Trp at the protein level. This de novo missense variant was not detected in the analyzed p

Society for Maternal-Fetal Medicine Consult Series #75: Evaluation and management of non-immune hydrops fetalis.

Non-immune hydrops fetalis (NIHF) can result from a multitude of underlying causes, such as fetal genetic diseases, congenital anomalies, infections, fetal arrhythmias, placental tumors, monochorionic twin complications, and other disorders. Management is complex and rooted in the preferences of the pregnant individual and the known or suspected etiology of NIHF. In this Consult, we review general considerations and a contemporary approach to the diagnosis, evaluation, and management of NIHF, providing recommendations based on the available evidence. The following are the Society for Maternal-

Burden of Rhesus isoimmunization and pregnancy outcomes: a cross-sectional study conducted at Kenyatta National Hospital, Kenya.

INTRODUCTION: the ABO and Rhesus systems remain the most clinically significant blood group antigens on red blood cell membranes. Rhesus isoimmunization occurs when maternal Rh antibodies in a RhD-negative woman react with red blood cells of an RhD-positive fetus, leading to adverse fetal and neonatal outcomes. METHODS: a cross-sectional review of 194 medical records of RhD-negative pregnant women managed at Kenyatta National Hospital (KNH) between 2013 and 2019 was conducted. Data on sociodemographic, obstetric, and clinical characteristics were extracted and analyzed using SPSS version 23.

Maternal Isoimmunization-Associated Fetal Anemia After First-Trimester Abortion: A Case Report and Literature Review.

Isoimmunization and fetal anemia pose considerable challenges in obstetrics, especially when there is a documented history of RhD incompatibility or other antibodies that may threaten fetal well-being. These conditions frequently result in serious complications, including hydrops fetalis, fetal anemia, and, in certain situations, fetal loss. Effective management of such pregnancies needs careful monitoring and prompt interventions, such as intrauterine transfusion (IUT), along with thorough post-delivery follow-up to safeguard both maternal and neonatal health. Due to ongoing uncertainties con

The Importance of a Multidisciplinary Team Approach in a Rare Case of Antenatally Diagnosed Tonne-Kalscheuer Syndrome.

Tonne-Kalscheuer syndrome (TOKAS) is a rare X-linked neurodevelopmental disorder with very few reported cases. We describe the case of a male neonate born at 27 weeks' gestation to a 41-year-old mother, with a diagnosis of TOKAS established antenatally through whole-exome sequencing after ultrasound revealed nuchal translucency, hydrops fetalis, structural cardiac abnormalities, and severe fetal growth restriction. The infant survived for nearly two hours after birth and was cared for and held by his parents and extended family. This case highlights the essential role of multidisciplinary coll
