We report a 21-year-old farmer with a 4-year history of a nodular plaque with fistulas and induration of adjacent skin. The lesion had been treated surgically at another hospital, but recurred 2 years later. Black, charcoal-like grains were observed draining through the fistulas. A biopsy specimen showed brown grains with filaments in an abscess surrounded by macrophages, giant cells, and lymphocytes. Culture demonstrated small white colonies of Madurella mycetomatis. The patient was treated with itraconazole for 6 months, followed by surgery. TNP was initiated in the immediate post-operative

Eumycetoma is a chronic granulomatous infection of the skin, subcutaneous tissue, fascia, and bone caused by true fungi. Most commonly, it affects the foot or hand. Fungi commonly reported to cause eumycetoma are Madurella mycetomatis, Madurella grisea, Phialophora jeanselmei, Cephalosporium recifei, etc. There have been several previous reports of human invasive infections by Paecilomyces lilacinus causing endophthalmitis, keratitis, chronic sinusitis, skin and soft tissue infections, and catheter-related infections. We report a case of eumycetoma caused by P. lilacinus. To our knowledge, thi

The aim of this prospective study was to evaluate the use of the cell block technique as a safe, accurate and quick tool for the cytodiagnosis of mycetoma. The study included 240 patients with suspected mycetoma. The suspected lesion was aspirated and the aspirated material was processed to form cell blocks. The cell blocks were processed as described for routine tissue histopathological examination. Haematoxylin/eosin-stained sections were reviewed to identify the morphological features of the mycetoma grains and the different inflammatory tissue reactions. The findings were compared with tho

We report on 722 children with confirmed mycetoma seen at the Mycetoma Research Centre, Khartoum, Sudan during May 1991 to March 2009. There were 531 males (73.5%) and 191 females (26.5%) with an age range of 4-17 years (mean 13.7+/-2.9 years); most were students. The majority of patients were from Central and Western Sudan. The disease duration ranged between 6 months and 14 years (mean 2.27+/-2.12 years). Most of the patients had eumycetoma (79.1%). The clinical course was typical in the majority of the patients. Family history of mycetoma was reported in 15% of patients. The foot was affect

PURPOSE OF REVIEW: Eumycetoma, phaeohyphomycosis and chromoblastomycosis are subcutaneous mycoses having in common the fact that they are acquired as a result of penetrating trauma to the skin and may be caused by a large variety of fungi. This article will review recent data regarding the epidemiology and treatment of these infections.
RECENT FINDINGS: Recent epidemiologic observations in these mycoses include an increased incidence of phaeohyphomycosis in immunosuppressed patients, the association of polymorphisms in genes involved in innate immunity, the occurrence of eumycetoma caused by

UNLABELLED: The literature states that proper management of Madura foot strongly depends on accurate identification of the causative organism. We describe the case of a patient with Madura foot who was diagnosed with eumycetoma after the dot-in-circle sign, which has been previously reported in regard to magnetic resonance images of this lesion, was observed on both ultrasound and magnetic resonance images.
LEVEL OF CLINICAL EVIDENCE: 4.

BACKGROUND: Mycetoma is a late-stage clinical manifestation of a subcutaneous infection produced by bacteria (actinomycetoma) or fungi (eumycetoma). Only a few articles have described the morphologic appearance of this uncommon pathology on cytology. The distinction between eumycetoma and actinomycetoma in fine needle aspiration cytology (FNAC) is as accurate as in histopathology, as demonstrated in the present case.
CASE: A 30-year-old man presented with a large swelling on his left foot with a discharging sinus. FNAC of the swelling yielded pus-like material. Initial Papanicolaou and Giemsa

Invasive fungal infection negatively influences the morbidity and mortality in heavily immuno-incompetent patients. Diagnosis of non-Aspergillus mould infections remains challenging despite application of a wide spectrum of non-culture-based microbiological techniques. Invasive diagnostic procedures are often essential. In this article, we present the case of a 15-month-old boy diagnosed with Rhizopus microsporus var. rhizopodiformis liver mycetoma during induction chemotherapy for acute promyelocytic leukaemia. Following surgery, he was effectively treated with a combination of ABLC and posac

A 57-year-old male presented with dermatosis of the dorsum of the foot consisting of tumefaction, deformity and sinus tract formation. The direct examination of exudates as well as the biopsy tissue, demonstrated the presence of black granules. A dematiaceous fungus was isolated from the lesions and was identified by ribosomal DNA sequencing as Cladophialophora bantiana. This is the second report of this fungus as an etiologic agent of eumycetoma in humans. Clinical and mycologic cure was achieved after 20 months of treatment with itraconazole at a starting dose of 300 mg/day that was tapered

Nocardia spp are gram-positive, aerobic, acid-fast bacteria which exist as saprophytes in nature. Invasive disseminated infections are particularly common in immunocompromised or debilitated hosts. Superficial infections with Nocardia spp occur as a result of local trauma and contamination of the wound. Clinically, it presents as acute infection (abscesses or cellulitis), mycetoma, or sporotrichoid infection. Differential diagnosis includes eumycetoma, chromomycosis, blastomycosis, coccidioidomycosis, sporotrichosis, tuberculosis, botryomycosis, syphilis, yaws, and neoplasia. Its diagnosis is

OBJECTIVES: Eumycetoma caused by Madurella mycetomatis is treated with surgery and high doses of itraconazole and ketoconazole. These agents are toxic, and new therapies are required.
METHODS: MICs were determined for artemisinin and tea tree oil, two natural herbal compounds.
RESULTS: Artemisinin was not active against M. mycetomatis, but tea tree oil did inhibit its growth. Since tea tree oil's prime component easily penetrates the skin, tea tree oil could be a useful agent in the treatment of eumycetoma.
CONCLUSIONS: Tea tree oil is active in vitro against M. mycetomatis.

Eumycetoma in the cerebellopontine angle region is extremely uncommon with no case being reported as per an extensive review of the literature by the authors. The authors report a case of cerebellopontine angle eumycetoma in a young female managed by subtotal decompression and antifungal treatment. The pre-operative diagnosis of eumycetoma in this location is extremely difficult and the role of histopathology is very important to characterize this uncommon lesion. The prognosis of this bizarre pathology is dismal despite all treatment modalities as compared to the usual tumors of the cerebello

Mycetoma a worldwide disease frequently occurs in the tropics with the highest prevalence being in Africa. Madurella mycetomatis is the main causative organism of human eumycetoma in Sudan. The legs and feet were commonly the sites of the infection. A 22-year-old lady was presented with painful abdominal swelling around a previous caesarian section scar. A provisional diagnosis of obstructed incisional hernia was put. Histopathological examination revealed macroscopically four masses of soft tissue. Microscopic sections showed grains of Madurella mycetomatis.

PURPOSE OF REVIEW: The present review highlights an orphan infectious disease in alarming need of international recognition. While money is being invested to develop new broad-spectrum antimicrobial drugs to treat infection in general, improvement in the management of complicated infections such as mycetoma receives little support.
RECENT FINDINGS: Many case presentations describe single-center experience in the management of mycetoma. Unfortunately, randomized and blinded clinical studies into the efficacy of antimicrobial treatment are desperately lacking. Response to medical treatment is u

Mycetoma is a chronic, local, granulomatous, subcutaneous, slowly progressive inflammatory disease caused by true fungi (eumycetoma) or filamentous bacteria (actinomycetoma), with the classic presentation involving tumefaction, multiple draining sinuses, and grain-filled pus. Both forms of mycetoma present as a progressive, subcutaneous swelling, although actinomycetoma has a more rapid course. Although not unheard of, mycetoma is an unusual cause of pedal osteomyelitis. We present a case report, complete with magnetic resonance images, of subcutaneous nodular calcaneal mycetoma osteomyelitis,

One of the hallmarks of eumycetoma is the formation of fungal grains, which are secreted by multiple sinuses in infected tissues. Madurella mycetomatis grains are black. This black colour was shown to be due to the presence of melanin. Melanin can be produced through various biochemical pathways. It appeared that M. mycetomatis melanisation could be blocked by inhibitors of the pyo- and dihydroxynaphthalene (DHN)-melanin pathways but not by inhibitors of the dihydroxyphenylalanine (L-DOPA)-melanin pathway. Melanin isolated from M. mycetomatis cells provides in vitro protection against the kill

Madura foot or mycetoma is endemic in many developing countries. It is occasionally seen within the United States due to increasing international travel but it may sometimes be acquired within US soil. Herein, we present a case of a patient with a diagnosis of mycetoma acquired through trauma to the foot. In addition, we discuss the epidemiology, etiological agents, clinical presentation, diagnosis, and treatment of mycetomas. Clinicians need to recognize mycetoma early and institute treatment promptly to reduce the substantial morbidity associated with this devastating infection.
