Protocol: A multi-factorial, multi-centre study, for biomarker identification in healthy controls for comparison to babies with moderate-severe NESHIE.

Neonatal encephalopathy suspected to be hypoxic-ischaemic encephalopathy (NESHIE) remains a leading cause of neonatal mortality and long-term neurodevelopmental impairment, particularly in low- and middle-income countries. While therapeutic hypothermia reduces mortality in moderate to severe cases, a significant proportion of affected infants continue to experience adverse neurological outcomes. This multi-centre observational study aims to elucidate the clinical and biological mechanisms underlying NESHIE by conducting a comprehensive comparative analysis of neonates with moderate to severe N

Pharmacokinetics-based management of newborns affected by hypoxic-ischemic encephalopathy undergoing therapeutic hypothermia.

Perinatal asphyxia (PA) is caused by reduced blood flow or oxygen levels around the time of birth. Neonatal hypoxic-ischemic encephalopathy (HIE) specifically refers to the neurological damage resulting from PA and ischemia and is the most frequent cause of mortality and morbility in newborns. Therapeutic hypothermia (TH) is nowadays the standard of care to treat newborns with hypoxic-ischemic encephalopathy (HIE). However, pharmacokinetic (PK) changes during TH can impact the efficacy and safety of administered drugs. In this narrative review, we critically examined the main PK changes that o

Mitochondrial dysfunction in neonatal brain injury: from molecular mechanisms to therapeutic interventions.

BACKGROUND: Neonatal brain injury, including hypoxic-ischemic encephalopathy, preterm brain injury, and neonatal infectious brain injury, remains a major cause of death and long-term neurodevelopmental disability worldwide. The immature brain is highly dependent on oxidative metabolism yet particularly vulnerable to energy failure and oxidative stress, placing mitochondria at the core of injury cascades. By integrating disturbances in energy production, redox balance, calcium homeostasis, and cell death signaling, mitochondrial dysfunction is increasingly recognized as a unifying driver of div

Temporal and cell-specific changes to cellular iron sequestration and lipid peroxidation in a murine model of neonatal hypoxic-ischemic brain injury.

BACKGROUND: Iron accumulation and lipid peroxidation are pathophysiologic mechanisms that contribute to neonatal hypoxic-ischemic (HI) brain injury. Characterization of spatiotemporal changes in these processes will help elucidate their role in ischemic neuronal injury as an initial step towards developing targeted interventions. METHODS: HI was induced in post-natal day 9 mice using the modified-Vannucci model. Hippocampal tissue from ipsilateral HI exposed, contralateral hypoxia exposed and sham animals was collected at 6 h, 24 h, 72 h, 7d and 90d post-HI. Tissue was evaluated for cell deat

Neonatal hypoxic-ischemic encephalopathy online registry in Shenzhen: protocol for a multicentre, prospective, open, observational cohort study.

INTRODUCTION: Hypoxic-ischaemic encephalopathy (HIE), a leading cause of neurological disability in neonates, is managed with therapeutic hypothermia, yet systematic data on its aetiology and prognosis are lacking, particularly in China. Herein, our study addresses this gap by establishing a prospective registry in Shenzhen and aims to clinically characterise HIE, identify its risk factors and document its long-term outcomes. METHODS AND ANALYSIS: We will recruit 200 neonates with HIE from 10 hospitals in Shenzhen, China, between January 2025 and December 2028. In parallel, clinical data will

Intravenous administration of ex vivo expanded human umbilical cord blood-derived CD34⁺ cells in a preterm hypoxic-ischemic encephalopathy mouse model.

Neonatal hypoxic-ischemic encephalopathy (HIE) in preterm infants lacks effective treatment and remains a leading cause of long-term neurological impairment. Autologous umbilical cord blood (UCB)-derived CD34⁺ cell therapy shows therapeutic potential; however, limited UCB volume in preterm infants often yields insufficient cells for clinical use. Recent advances in ex vivo expansion using chemically defined, albumin- and cytokine-free systems enable efficient proliferation of CD34⁺ hematopoietic stem cells. We evaluated the safety and efficacy of intravenously administered, ex vivo expanded CD

Enteral nutrition in neonates: current evidence from clinical trials and evolving strategies.

Despite advances in neonatal intensive care and increased availability of randomized clinical trial data, optimal enteral nutrition strategies remain debated, with significant gaps between evidence and practice. We reviewed current evidence for enteral feeding in neonates. Recent clinical trials support the early progression of enteral feeding within 96 h of birth and faster advancement rates of 30-40 mL/kg/day in preterm infants. Evidence demonstrates that these strategies reduce the time to full feeds and decrease the risk of sepsis without increasing the risk of necrotizing enterocolitis. A

Patient 1 was a male neonate who, at 3 hours of life, presented with respiratory distress, hypotonia, and ventilator dependence. Genetic testing revealed a DMPK gene CTG repeat expansion (13/>83). Patient 2 was a male neonate who presented at 2 days of life after resuscitation for perinatal asphyxia, with hypotonia and ventilator dependence, complicated by hypoxic-ischemic encephalopathy and diaphragmatic eventration, which appears to represent the first such combination reported in China. Genetic testing showed a DMPK gene CTG repeat expansion (12/>83). Both cases were diagnosed with congenit

Application of the African Philosophy Ubuntu to the Care of Families With Infants Undergoing Therapeutic Hypothermia for the Treatment of Hypoxic-Ischemic Encephalopathy.

Neonatal intensive care units throughout the world strive to provide family-integrated care guided by the family-centred care philosophy. Under the family-integrated model of care, families are considered a member of the healthcare team and the primary caregivers to their newborn. Families experiencing intensive care related to their newborn acquiring hypoxic-ischemic encephalopathy shortly before, during or after birth, are commonly prevented from holding or participating in the care of their newborn in the first few days of life. These restrictive practices are the very antithesis of family-

Therapeutic hypothermia in newborns: evidence-based guidelines from a systematic review.

Hypoxic-ischemic encephalopathy represents a major cause of neonatal mortality and long-term neurodevelopmental impairment. Therapeutic hypothermia has become the standard of care, significantly improving survival and outcomes. Nevertheless, its clinical implementation often lacks a standardized operational framework to ensure consistent efficacy and safety. The present work aimed to develop GRADE-based operational guidelines for therapeutic hypothermia, offering neonatologists a structured approach for identifying, treating, and monitoring newborns throughout the procedure. A panel of experts

Maternal Inflammation Alters Nuclear and Mitochondrial DNA Methylation Patterns in Neonatal Brain Monocytes.

Neonatal hypoxic ischemic encephalopathy (HIE) is a common birth complication that can cause death or lifelong disabling conditions like cerebral palsy, epilepsy, and autism. It is well established that maternal infection and inflammation are significant risk factors for HIE but reasons for this increase in neurological risk to the offspring remain unknown. Inflammation or infection are associated with epigenetic changes and may contribute to the increased risk of neurodevelopmental disability in exposed offspring. Here, we analyzed and compared DNA methylation patterns in brain monocytes isol

Low-level laser therapy enhances neuroprotection and functional recovery after neonatal hypoxic-ischemic injury via potential activation of Nrf2/HO-1 and Akt pathways.

BACKGROUND: Neonatal hypoxic–ischemic encephalopathy (HIE) is a leading cause of neurodevelopmental disability. Low-level laser therapy (LLLT), or photobiomodulation, has shown neuroprotective potential, but its long-term effects and region-specific responses remain poorly defined. METHODS: Neonatal rats subjected to hypoxic–ischemic brain damage (HIBD) received transcranial 810-nm LLLT (24 mW/cm², 10 min/session, twice daily for three days). Histological, immunofluorescence, and Western blot analyses assessed neuronal integrity, glial activation, and signaling pathway changes. Sensorimotor fu

Neuroplacental Interactions in early human development: Insights from selective acute neuronal necrosis.

INTRODUCTION: The correct development of the central nervous system depends largely on the last two trimesters of gestation and the early neonatal period. Neurological lesions are common in autopsies, being one of the most relevant, selective acute neuronal necrosis, a manifestation of hypoxic-ischemic encephalopathy. This study aimed to analyse the distribution of selective acute neuronal necrosis (SANN) and its association with placental lesions, clinical factors and other central nervous system (CNS) abnormalities. METHODS: From 1090 autopsies performed, 205 cases (18.8 %) with SANN were i
