BACKGROUND: Neonatal hypoxic-ischemic encephalopathy (HIE) is a major cause of neonatal death and long-term neurodevelopmental impairment. Secondary neuroinflammation and inflammasome-mediated pyroptosis substantially contribute to progressive brain injury. Procyanidin B2 (PCB2), a plant-derived proanthocyanidin abundant in several traditional botanicals, exhibits anti-inflammatory activity, but its role in neonatal hypoxic-ischemic brain injury remains unclear.
OBJECTIVE: To determine whether PCB2 protects against neonatal hypoxic-ischemic brain damage (HIBD) in rat and to clarify whether th

BACKGROUND: Neonatal hypoxic-ischemic encephalopathy (HIE) is a major perinatal neurological disorder with limited objective early diagnostic markers, while conventional point resolved selective spectroscopy (PRESS)-based 1H-magnetic resonance spectroscopy (1H-MRS), though widely used, is suboptimal for detecting short-T2 metabolites critical to early brain injury assessment. This study aimed to investigate the feasibility of spin echo full intensity acquired localization (SPECIAL) spectra on newborn rat brain with HIE model, and to explore the reliable metabolic markers in rat brain and compa

UNLABELLED: Hypoxic-ischemic encephalopathy (HIE) often results in multi-organ damage. Liver injury following HIE is typically characterized by time-dependent altered patterns of alanine aminotransferase (ALT), aspartate aminotransferase (AST) or total bilirubin (TB) concentrations. This study aims to describe age-dependent patterns of liver injury biomarkers during and after therapeutic hypothermia (TH). Liver injury biomarkers (ALT, AST, TB) over the first 10 days of postnatal age (PNA) from six cohorts, including 428 neonates with moderate-to-severe HIE treated with TH were pooled. Statisti

Early prediction of neurodevelopmental (ND) impairment in neonates with hypoxic-ischemic encephalopathy (HIE) is essential for timely intervention, particularly in infants treated with therapeutic hypothermia (TH). We developed an Extreme Gradient Boosting-based prediction model using neonatal brain magnetic resonance imaging volumetrics and clinical features in 89 full-term HIE infants treated with TH. ND impairment was defined as a Bayley-III composite score <85 at 18 to 24 months. Model performance was evaluated using stratified 5-fold cross-validation, and interpretability was assessed usi

BACKGROUND: Neonatal hypoxic-ischemic encephalopathy (HIE), caused by perinatal asphyxia, is a severe neurological disorder in which neuroinflammation contributes substantially to secondary brain injury and functional impairment. Although apelin-13 has demonstrated anti-inflammatory and neuroprotective effects in central nervous system injury models, its role in modulating neuroinflammation following HIE remains poorly understood. This study aimed to investigate the anti-inflammatory effects of apelin-13 in HIE and to elucidate the underlying mechanisms.
METHODS: Neonatal rats were subjected

Hyperpolarization chemistry based on reversible exchange of parahydrogen, also known as Signal Amplification By Reversible Exchange (SABRE), is a particularly simple approach to attain high levels of nuclear spin hyperpolarization, which can enhance NMR and MRI signals by many orders of magnitude. SABRE has received significant attention in the scientific community since its inception because of its relative experimental simplicity and its broad applicability to a wide range of molecules, however in vivo detection of molecular probes hyperpolarized by SABRE has remained elusive. Here we descri

Hypoxic-ischemic encephalopathy (HIE) is caused by impaired oxygen delivery or inadequate blood flow to the brain that is sustained for a critical duration. Neuroimaging plays a major role in the evaluation of suspected HIE or acute encephalopathy of unclear etiology. MR imaging is the imaging modality of choice, helping estimate the overall acuity, extent, and severity of the injury. This article outlines the basic mechanisms that lead to selective vulnerability in HIE and describes how MRI findings evolve over time.

Spinal muscular atrophy with congenital bone fractures is a rare, severe neuromuscular disorder with autosomal recessive inheritance, characterised by hypotonia, congenital contractures, and respiratory distress. We present the case of a newborn girl with a homozygous mutation in the ASCC1 gene, who was diagnosed with hypoxic-ischaemic encephalopathy after birth and underwent therapeutic hypothermia (TH). Although TH did not cause any side effects, it also did not improve the prognosis or quality of life of the patient. The decision whether to perform TH in neonates with congenital or genetic

Neonatal hypoxic-ischemic encephalopathy (HIE) may induce substantial neuronal damage. In particular, an overactivation of microglia following HIE represents a pathogenically important process. Previous studies have shown that microglial inhibitors can exert neuroprotective effects in HIE; however, the specific mechanisms underlying these effects have not yet been elucidated. Ligation of the left common carotid artery and exposure to 5% O2 were utilized to produce an HIE model in rats. A number of experimental approaches were then used to determine the effect of a microglial inhibition, achiev

Neonatal encephalopathy refers to disturbed neurological function in the neonatal period and has multiple potential aetiologies. A systematic review showed that neonatal encephalopathy, hypoxic-ischaemic encephalopathy, and perinatal asphyxia are often used interchangeably in clinical trials and communications. We aimed to establish an international, consensus-based definition as a preliminary step towards standardising terminology. Findings from a systematic review of definitions guided the creation of a real-time Delphi survey. Three consensus meetings were held to finalise the definition, w

Cerebral medullary veins are involved in a wide range of pathologies affecting the central nervous system during childhood. The aim of this article is to provide an anatomical overview of the cerebral venous system and to review the main conditions affecting the medullary veins in this age group, outlining their pathophysiology and the characteristic imaging findings across different imaging modalities. Vascular causes are the most common, but medullary vein involvement can also be seen in hypoxic-ischaemic encephalopathy, malformative syndromes, and inflammatory or infectious diseases.

BACKGROUND: Integrating stem cell therapies into clinical settings faces several challenges, particularly in achieving the high cell yields necessary for attaining therapeutic doses. Preconditioning with hypoxic conditions has shown promise in enhancing the mesenchymal stem cells (MSCs)’ reparative capabilities of the central nervous system. Recent evidence suggests that oxygen concentration and exposure duration can shape MSCs’ phenotypes, supporting the need for further optimization of this strategy in a way to achieve maximal repair. METHODS: Wistar Han rat pups from both sexes underwent hy

Hypoxic-ischemic encephalopathy (HIE) is a major cause of neonatal death and long-term neurodevelopmental impairment. Therapeutic hypothermia is an effective intervention to improve the prognosis of neonates with moderate to severe HIE, and its standardized management is crucial for optimizing outcomes. Based on domestic and international evidence and expert opinion, the Evidence-Based Medicine Group, Neonatologist Society, Chinese Medical Doctor Association developed the "Expert consensus on the clinical management of therapeutic hypothermia for neonatal hypoxic-ischemic encephalopathy (2026)

UNLABELLED: Analgesia, sedation and developmental care are key components of supportive management for newborns with hypoxic-ischemic encephalopathy (HIE) undergoing therapeutic hypothermia (TH). However, evidence-based recommendations remain limited, and practices may vary substantially between centers. We aimed to describe current practices in analgesia, sedation, and developmental care during TH across French neonatal intensive care units (NICUs). We conducted a nationwide cross-sectional survey using a standardized electronic questionnaire distributed to all NICUs performing TH in France.
