Mild Neonatal Hypoxia Targets Synaptic Maturation, Disrupts Adult Hippocampal Learning and Memory, and Is Associated with CK2-Mediated Loss of Synaptic Calcium-Activated Potassium Channel KCNN2 Activity.

Preterm infants frequently sustain brief hypoxic insults of unclear clinical significance. Since preterm survivors commonly sustain lifelong memory impairment without apparent gray matter injury, we tested whether mild hypoxia alone without ischemia could persistently disrupt adult hippocampal learning and memory mechanisms without causing brain injury. We developed a neonatal mouse model of mild hypoxia that generated clinically relevant oxygen desaturation, but without responses typically associated with hypoxia-ischemia including bradycardia, seizures, neuroinflammation, and neuronal or gli

Neonatal Oculomotor, Optomotor, Vestibulo-ocular, Protective Blink, and Pupillary Light Reflexes Following Perinatal Brain Trauma: Implications for Early Visual Rehabilitation in Mild Traumatic Brain Injury and Concussion.

Perinatal brain trauma, including hypoxic-ischemic encephalopathy and mechanical injury during pregnancy, labor, or delivery, represents an early-acquired brain injury that disrupts the identical subcortical, brainstem, cerebellar, and emerging cortical visual, oculomotor, and vestibulo-ocular pathways commonly impaired in mild traumatic brain injury (mTBI) or concussion. This topical review examines five key neonatal reflexes: optomotor (optokinetic nystagmus [OKN], head-turning, fix-and-follow), oculomotor (reflexive saccades, doll's eye), vestibulo-ocular (VOR), pupillary light (PLR includi

Management of Hypoxemic Respiratory Failure in the Newborn During Therapeutic Hypothermia.

Therapeutic hypothermia (TH) is the gold standard for the management of hypoxic ischemic encephalopathy (HIE) in newborns. Both hypoxia and TH may contribute to an increase in pulmonary vascular resistance (PVR), resulting in persistent pulmonary hypertension of the newborn. Infants with worsening hypoxemic respiratory failure in the setting of HIE and TH may require optimization of respiratory support to achieve adequate lung recruitment, surfactant, and inhaled nitric oxide to improve oxygenation and ventilation strategies to avoid acidosis and hypocapnia. Furthermore, management of systemic

Maternal and neonatal outcomes following emergency cesarean section or operative vaginal delivery for fetal distress in the second stage of labor.

BACKGROUND: Operative vaginal delivery and emergency cesarean section are critical interventions used in the second stage of labor when fetal distress threatens maternal or neonatal well-being. Prompt recognition of fetal compromise and timely decision-making are essential to ensure safe delivery outcomes while minimizing complications for both mother and baby. The aim of this study is to compare maternal and neonatal outcomes between operative vaginal delivery and emergency cesarean section in cases of fetal distress occurring during the second stage of labor. METHODS: This is a retrospectiv

Prophylactic antiseizure medication administration practices in neonates with hypoxic-ischaemic encephalopathy: a scoping review protocol.

INTRODUCTION: Hypoxic-ischaemic encephalopathy (HIE) is a leading cause of neonatal mortality and long-term neurodevelopmental impairment worldwide. Neonatal seizures are common in HIE and are associated with adverse outcomes. While antiseizure medications are routinely used for treatment, their prophylactic use in the absence of clinically evident seizures remains controversial, with limited consensus and variable clinical practices. Therefore, this review will map the existing evidence concerning the utilisation of antiseizure medication for prophylaxis among neonates with HIE. METHODS AND

The role of persistent inflammation in failed recovery after perinatal brain injury: is resolution the cure?

Perinatal brain injury (PBI) is a major predictor of neurological disability. Commonly associated with prematurity, infection, stroke, hypoxia-ischemia, hemorrhage, and/or toxin exposure, PBI triggers acute and persistent systemic inflammation. There are many stages of vulnerability to PBI during development including pregnancy, birth - term and preterm, and neonatal age. The vulnerable stages can compound inflammation through injury to the placental-fetal-brain axis, adaptive and innate immune system development, neural-immune communication, and central nervous system maturation. Neonates exh

Early arterial blood gas parameters, clinical features, and neuroimaging findings in neonates undergoing therapeutic hypothermia for suspected hypoxic-ischemic encephalopathy: a retrospective single-center study.

BACKGROUND: Hypoxic-ischemic encephalopathy (HIE) remains an important cause of neonatal mortality and long-term neurodevelopmental impairment. Although therapeutic hypothermia is the standard treatment for term and near-term infants with suspected moderate-to-severe HIE, the relationships among early blood gas parameters, clinical findings, and radiologic abnormalities in cooled neonates are not fully defined. METHODS: This retrospective observational study included neonates aged 0-28 days who underwent therapeutic hypothermia for suspected HIE at any neonatal intensive care unit and were su

Human umbilical cord mesenchymal stem cells alleviate hypoxic-ischemia-induced white-matter injury in neonatal rats by regulating polarization of microglia.

The protective impact and unique mechanisms of human umbilical cord mesenchymal stem cells (HUC-MSCs) transplantation following hypoxic-ischemic (HI)-induced brain white-matter injury (WMI) were explored. To establish a WMI model, Sprague-Dawley rats with three days after birth underwent unilateral carotid artery ligation, followed by hypoxic exposure (8% oxygen and 92% nitrogen). Subsequently, HUC-MSC transplantation was performed into the lateral ventricle. Molecular and behavioral experiments were conducted to assess how it would influence NLRP3 inflammasome activation, M1/M2 microglial pol

Hypothermia and Stem Cells Confer Limited Protection to Mammillary Bodies in a Neonatal Mouse Model of Hypoxia-Ischemia.

BACKGROUND: Hypoxic-ischemic (HI) brain injury due to perinatal asphyxia causes long-term neurological morbidity in newborns. HI affects brain areas including the cortex, hippocampus, and basal ganglia. More recently the mammillary bodies (MBs), critical for memory, were also shown to be affected in patients, despite treatment with therapeutic hypothermia (TH). Here we characterized the kinetics and extent of MB damage in an established neonatal HI mouse model and evaluated the therapeutic potential of TH or a novel regenerative intervention: intranasal mesenchymal stem cell (MSC) therapy. β-hydroxybutyrate alleviates motor impairment and neurological damage in hypoxic-ischemic encephalopathy mice.

Hypoxic-ischemic encephalopathy (HIE) is a major reason for neonatal mortality and long-term neurological deficits, with limited efficacy from current standard treatments like therapeutic hypothermia. β-hydroxybutyrate (BHB) is an abundant ketone body synthesized in the liver, possesses energy supply, anti-inflammatory, antioxidant, and anti-apoptotic activities. This study aims to investigate the effect of exogenous BHB on motor impairments and brain damage in HIE mice. Ten-day-old mouse pups underwent left common carotid artery ligation followed by hypoxia to induce HIE. Control underwent Sh

Blood sLOX-1 Is Beneficial for Determining Severity of Neonatal Hypoxic-Ischemic Encephalopathy: A Nationwide Prospective Cohort Study.

Background/Objectives: Neonatal hypoxic-ischemic encephalopathy (HIE) is a major cause of neurodevelopmental diseases. In a previous retrospective study, we revealed that the levels of the soluble form of lectin-like oxidized low-density lipoprotein receptor-1 (sLOX-1) are linked to HIE severity. We conducted a prospective investigation to evaluate the association between sLOX-1 levels and HIE severity using the Sarnat staging system, as well as a multicenter study to assess neurodevelopmental sequelae in the short term. Methods: A total of 193 infants were enrolled in the study between April

Neonatal encephalopathy mortality rate in Aotearoa, New Zealand: review of national data 2010-2020.

BACKGROUND: Neonatal encephalopathy (NE) is a condition that affects newborn and can lead to long-term neurological impairments and death. Hypoxic ischaemic encephalopathy is the subset of NE with evidence of a hypoxic ischaemic insult. In Aotearoa New Zealand, national data have been collected for infants ≥37 weeks since 2010 and ≥35 weeks from 2016. Although the mortality rate associated with NE has not been a focus of annual reports, it is an important outcome for counselling whānau (families) and for international comparison. METHODS: The Perinatal and Maternal Mortality Review Committee

Inhaled nitric oxide and hypoxic brain injury in newborn piglets.

BACKGROUND: Inhaled nitric oxide (iNO) is widely used as a pulmonary vasodilator to treat infants with pulmonary hypertension (PHN). Hypoxia (Hx) and acidosis increase pulmonary vascular resistance and contribute to PHN. Perinatal asphyxia and resultant hypoxic-ischemic encephalopathy (HIE) remain major causes of neonatal death and neurological impairment, and the impact of iNO on neurodevelopmental outcomes after HIE is unclear. Although hypothermia is the standard neuroprotective therapy for HIE, many infants still do not survive or continue to experience neurological impairments despite tre

Comparing severe maternal morbidity and neonatal outcomes between Somali and non-Somali black birthing cohorts.

BACKGROUND: Black birthing individuals experience worse perinatal outcomes compared to White counterparts, yet within-group differences among Black populations remain understudied. The U.S. Black population is heterogeneous, encompassing long-established U.S.-born communities and immigrants from across the African diaspora, whose health profiles may differ. Somali immigrants represent the largest African-born population in many U.S. communities and face documented challenges including cultural adaptation and language barriers. However, severe maternal morbidity (SMM) has not been specifically

Drugs and Cell-Based Therapies for the Prevention and Treatment of Neonatal Brain Injury.

The international burden of neonatal brain injury (NBI) remains significant with enormous potential for therapeutic intervention. Recent large trials have demonstrated no neuroprotection with either erythropoietin or darbepoetin for term or pre-term brain injury. Neuroprotective strategies with strong evidence are currently limited to antenatal steroids, antenatal magnesium, and supportive care for pre-term NBI and therapeutic hypothermia for term hypoxic-ischemic encephalopathy. Of the pharmacologic strategies currently being investigated clinically, the agents with the highest likelihood of

From maternal nutrition to neonatal neuroprotection: Passion fruit stilbenes in hypoxia-ischemia.

BACKGROUND: Neonatal hypoxia-ischemia (HI) remains a major cause of perinatal morbidity and mortality, leading to lifelong neurological sequelae. Therapeutic hypothermia is the current standard of care but provides only partial protection and must be applied within a narrow therapeutic window. Complementary or preventive neuroprotective strategies are therefore needed. Maternal supplementation with plant-derived polyphenols has shown promising effects in reducing oxidative stress, neuroinflammation, and neuronal loss in experimental models of neonatal HI. Passion fruit extract, rich in stilben
