Clinical and molecular spectrum of congenital disorders of glycosylation in 80 Egyptian patients.

Congenital disorders of glycosylation (CDG) represent a diverse group of inborn errors of metabolism caused by defective glycosylation processes, usually characterized by multiorgan involvement and overlapping clinical symptoms. This study aimed to decipher the clinical and genetic landscape of CDG in Egypt to enumerate the most common CDG types and unveil potentially new candidate genes. The study included 80 patients from 57 families with molecular findings consistent with CDG. Consanguinity was noted in 90% of the families. The most common clinical features were global developmental delay (

Clinical characteristics and genetic analysis of children with corpus callosum abnormalities.

BACKGROUND: Abnormalities of the corpus callosum (ACC) are common in pediatric populations. Classification by its morphology did not correlate well with the clinical characteristics of the patients. We aim to analyze the clinical and genetic characteristics of ACC, with categorizing them based on whether they are congenital lesions or secondary changes to other brain lesions. METHODS: This retrospective study analyzed the clinical, demographic, and genetic characteristics of 35 children and adolescents whose brain magnetic resonance imaging revealed ACC at our institution between August 2016

Aesthetic judgement and appreciation in agenesis of the corpus callosum.

The corpus callosum has been associated with some of the most complex processes of cognition, including creativity and imagination. Agenesis of the corpus callosum (ACC) is a congenital disorder, affecting approximately 1:4000 persons, in which the corpus callosum fails to develop. Even when FSIQ is in the average range, ACC results in a characteristic neuropsychological syndrome of reduced interhemispheric transfer of sensory-motor information, reduced cognitive processing speed, difficulties in complex reasoning and novel problem-solving, and reduced elaborative imagination and creative capa

First Detection of 1p36 Deletion by Whole-Exome Sequencing in a Tunisian Patient.

STUDY OBJECTIVE: We reported a rare case of 1p36 deletion syndrome diagnosed using whole-exome sequencing (WES) in a Tunisian neonate, and to highlight the utility of WES in detecting structural variants, particularly in resource-limited settings. METHODS: Clinical and genetic investigations were conducted on a female neonate presenting with a severe polymalformative syndrome. WES was performed to detect potential genetic abnormalities, followed by validation through fluorescence in situ hybridization (FISH). Variant annotation and classification were done in accordance with ACMG guidelines.

Progressive neuroinflammation and deficits in motor function in a mouse model with an Epg5 pathogenic variant of Vici syndrome.

Vici syndrome (VS) is a rare pediatric genetic disorder characterized by profound developmental delay, seizures, immune deficits, cardiomyopathy and progressive motor dysfunction. This devastating condition is caused by pathogenic variants in the EPG5 gene, which encodes a regulator of autophagy, leading to the accumulation of toxic intracellular material and widespread cellular dysfunction. Less-severe EPG5 pathogenic variants have recently been linked to rare familial forms of Parkinson's disease, suggesting deficits in EPG5 function drive a range of neurodegenerative disorders. Currently, t

A Patient With Intellectual Disability, Agenesis of Corpus Callosum, and Congenital Heart Disease Associated With Chromosome 10p11.2 Microdeletion.

DeSanto-Shinawi syndrome is a rare genetic disorder caused by pathogenic variants or deletions involving the WAC gene, located on chromosome 10p12.1, and is characterized by developmental delay, intellectual disability, and distinctive dysmorphic features. In addition to deletions encompassing WAC, several proximal deletions on chromosome 10 that exclude WAC have also been reported. Here, we describe a patient with a microdeletion of chromosome 10p11.23-p11.21 spanning approximately 4.2 Mb. The patient exhibited intellectual disability, agenesis of the corpus callosum, and congenital heart dis

Loss of Zmiz1 in Mice Leads to Impaired Cortical Development and Autistic-Like Behaviors.

BACKGROUND: De novo mutations in transcriptional regulators are emerging as key risk factors contributing to the etiology of neurodevelopmental disorders. Human genetic studies have recently identified ZMIZ1 and its de novo mutations as a cause of a neurodevelopmental syndrome strongly associated with intellectual disability, autism, attention-deficit/hyperactivity disorder, microcephaly, and other developmental anomalies. However, the role of ZMIZ1 in brain development or how ZMIZ1 mutations cause neurological phenotypes is unknown. METHODS: We generated forebrain-specific Zmiz1 mutant mice

Expanding the clinical and immunological phenotypes of COPB1 deficiency.

PURPOSE: COPB1 encodes the coatomer subunit beta protein, which is essential for brain development and intracellular protein trafficking. Homozygous mutations cause Baralle-Macken syndrome that characterized by global developmental delay, severe intellectual disability, and early-onset cataracts. Although immunodeficiency has been observed in patients with COPB1 deficiency, the immunological phenotype remains incompletely characterized. Here, we comprehensively describe the clinical features and delineate the immunological phenotype associated with COPB1 mutations. METHODS: We performed detai

Clinical Heterogeneity in a Scandinavian FMR1 Premutation Carrier Cohort and Basal Ganglia Atrophy in FXTAS.

Pathological CGG expansions in the FMR1 gene, encompassing premutations (55–200 repeats) and full range mutations (> 200 repeats), cause a spectrum of complex and incurable disorders. Premutation carriers (PMC), particularly men, face the risk of developing fragile X-associated tremor/ataxia syndrome (FXTAS). Other conditions associated with premutations in FMR1 include both Fragile X-associated primary ovarian insufficiency (FXPOI), and Fragile X-associated neuropsychiatric disorders (FXAND). Hyperintensities in the middle cerebellar peduncle (MCP), known as the MCP sign, have been considered

Fetal Neuroimaging in Aicardi Syndrome: A Case Report and Literature Review.

Aicardi syndrome (AIC) is a congenital condition involving characteristic neurological and ocular abnormalities. We describe a 30-year-old pregnant woman referred at 32 weeks of gestation after an ultrasound at 29 weeks showed corpus callosum agenesis, an interhemispheric arachnoid cyst, and colpocephaly. These findings raised prenatal suspicion of AIC. Postnatal evaluation confirmed the diagnosis with the identification of chorioretinal lacunae on fundoscopic examination. In this case, the prenatal imaging findings were key to recognizing a pattern suggestive of AIC, while postnatal assessmen

The secret connections of the brain: a connectomic reserve for neuroplasticity?

Here, we propose a new concept linked to neuroplasticity and cognitive reserve: 'Connectomic reserve'. It is suggested and discussed as a hypothesis, aiming to stimulate experimental testing, taking as an example the exuberant circuits formed during development by commissural neurons and their projections. Upon reviewing the developmental strategies of the formation of callosal circuits, from axonal emission by cortical cell bodies to the choice of trajectories within the ipsi and contralateral hemispheres, we identified the formation of many ectopic and heterotopic projections. Ectopic projec

Diffusion tensor imaging of cerebellar peduncles and association tracts in a case of primary complete callosal agenesis.

The corpus callosum is the largest white matter tract in the brain, connecting the left and right cerebral hemispheres. Callosal agenesis is a condition where this tract fails to form during neurodevelopment. Despite this, individuals with callosal agenesis can present as cognitively and behaviorally asymptomatic, retaining some alternate form of interhemispheric communication. Afferent and efferent pathways linking the cortex to the contralateral cerebellum, such as the corticopontocerebellar tract (CPCT) and dentatorubrothalamic tract (DRTT), could serve as compensatory routes in individuals

Prevalence of epilepsy and structural brain anomalies in spina bifida aperta.

INTRODUCTION: Spina bifida aperta (SBA) is frequently associated with a wide range of structural anomalies in the brain, some of these thought to be related to epilepsy. However, the relationship between these is poorly studied. This study aims to assess the prevalence of brain anomalies and epilepsy in SBA patients and explore the nature and magnitude of their association. METHODS: We conducted a retrospective cross-sectional cohort study including all consecutive postnatally treated SBA patients, managed at Erasmus MC Sophia Children's Hospital, Rotterdam from January 1st 2000 up to June 1s

Multiple midline brain anomalies in a CMV-exposed fetus with successful surgical management: a case report and literature review.

INTRODUCTION: Congenital arachnoid cysts (ACs) are cerebrospinal fluid collections resulting from abnormal splitting of the arachnoid membrane during embryogenesis. They may occur as isolated lesions or in association with other central nervous system (CNS) malformations. Congenital cytomegalovirus (cCMV) infection is a leading infectious cause of neurodevelopmental impairment and may contribute to complex CNS anomalies. PRESENTATION OF CASE: A term male infant was diagnosed prenatally at 30 weeks with midline cystic lesions, ventriculomegaly, and absent corpus callosum. Postnatal magnetic re
