Fibrodysplasia ossificans progressiva in children: diagnostic pitfalls and ACVR1 genotype-phenotype spectrum.

UNLABELLED: Fibrodysplasia ossificans progressiva (FOP) is a rare multisystem disorder with the triad of congenital hallux valgus, flare-ups and progressive heterotopic ossification. The flare-ups (episodes of painful soft-tissue swelling) start in early childhood, and progress to ossification with cumulative functional disability. We describe the cohort of FOP from a single tertiary center in Türkiye and highlight the phenotypic variabilities and diagnostic pitfalls. The subjects included 10 affected individuals (7 females and 3 males, aged 10 days to 20 years), whose diagnosis was confirmed

[Effects of Thap6 gene knockout on emotional and social behaviors in mice].

OBJECTIVES: THAP domain-containing protein 6 (THAP6) is a member of the THAP family and contains a conserved THAP zinc-finger domain, allowing it to regulate gene expression as a transcription factor. Previous studies have shown that THAP6 may be involved in macrophage activation and transcriptional regulation related to social stimuli; however, whether THAP6 affects emotional behavior and brain tissue structure has not yet been reported. This study aims to clarify the effects of Thap6 gene deletion on emotional behaviors in mice, preliminarily explore the effects of Thap6 gene knockout on mou

Chronic pain and stress: Transdiagnostic meta-analytic evidence of convergent network signature with PTSD.

Chronic pain is increasingly conceptualized within a stress-related framework. However, it remains unclear whether chronic pain and prototypical stress-related conditions-such as post-traumatic stress disorder (PTSD)-show convergence in their morphometric alterations and underlying normative functional connectivity profiles. To this end, we conducted a pre-registered transdiagnostic meta-analytic study of gray matter volume alterations in chronic pain (60 studies) and PTSD (20 studies), testing convergence at two complementary levels: direct anatomical overlap and network-level convergence wit

Neonatal developmental and epileptic encephalopathy with movement disorder and arthrogryposis: A shared phenotype across brain-expressed sodium channelopathies.

OBJECTIVE: Neonatal developmental and epileptic encephalopathy with movement disorder and arthrogryposis (NDEEMA) represents the most severe end of the gain-of-function (GOF) SCN1A disorder spectrum. Sporadic cases of congenital arthrogryposis have also been reported in individuals with SCN2A-, SCN3A-, and SCN8A-related developmental and epileptic encephalopathy. Here, we investigated whether NDEEMA occurs in other brain-expressed sodium channelopathies and characterized its features. METHODS: Individuals with the clinical phenotype of NDEEMA were identified through internal databases, an int

Beta-blockers improve neurological outcomes of cerebral cavernous malformations: a systematic review.

Cerebral cavernous malformations (CCMs) are vascular lesions in the brain, presenting risks of intracerebral hemorrhage (ICH) and non-hemorrhagic focal neurological deficit (NH-FND). Currently, no effective medical therapy exists for CCMs, and treatment options limited to surgical resection and stereotactic radiosurgery. Beta-blockers, particularly propranolol, have shown promise in treating similar vascular conditions and may have therapeutic potential for CCMs. We conducted a systematic search of PubMed, Web of Science, EMBASE, Cochrane Central, and ClinicalTrials.gov (March 2024) for random

A single-cell multiomic analysis identifies molecular and gene-regulatory mechanisms dysregulated in developing Down syndrome neocortex.

Down syndrome (DS) is the most common genetic cause of intellectual disability, yet the cellular and molecular mechanisms driving this developmental disorder remain unclear. In this study, we profiled human mid-gestation neocortex with snMultiomics across 26 donors. We observed a reduction in neural progenitors and corticothalamic neurons and an increase of intratelencephalic neurons, accompanied by accelerated neuronal specification. We uncovered widespread changes in gene expression, chromatin accessibility, and cell interaction networks affecting neurogenesis, specification, and maturation;

Impaired Glur2 palmitoylation in cerebellar Purkinje cells of a Friedreich ataxia mouse model.

Friedreich's ataxia (FRDA) is a neurodegenerative disorder caused by frataxin deficiency, characterized by progressive cerebellar dysfunction and neuronal loss. Although synaptic abnormalities are an early feature of FRDA, the molecular mechanisms linking frataxin deficiency to synaptic dysfunction remain incompletely defined. In this study, we investigated post-translational regulation of the AMPA receptor subunit GluR2 in the cerebellum of the frataxin knockdown (FRDAkd) mouse model. GluR2 protein levels are reduced early following frataxin knockdown, despite unchanged mRNA expression and pr

Association between upper airway obstruction and pulmonary hypertension in children with Down syndrome.

PURPOSE OF REVIEW: Down syndrome (DS) is the most common nonlethal chromosomal aneuploidy, affecting 1 in 700 live births. Pulmonary hypertension (PH) occurs in approximately 25% of children with DS and contributes to a 10% mortality rate within 3  years of diagnosis. Despite obstructive sleep apnea (OSA) affecting up to 80% of children with DS, the specific contribution of upper airway obstruction to PH in this population remains poorly characterized. This review synthesizes current evidence to address that gap. RECENT FINDINGS: Children with DS develop multilevel upper airway obstruction du

Involvement of Tumor Suppressor Candidate 3 in Mouse Survival, Craniofacial, and Cortical Development.

Tumor Suppressor Candidate 3 (TUSC3) is an integral component of the oligosaccharyltransferase complex and is required for the N-glycosylation of proteins. While TUSC3 has been implicated in cancer and autosomal recessive non-syndromic intellectual disability, some patients also exhibit distinct facial features. However, the role of Tusc3 in craniofacial development is unknown. Here, we report a patient presenting with cleft lip and palate who was found to carry a homozygous variant in the TUSC3 promoter region in addition to a maternally inherited chromosomal duplication. In order to evaluate

Refining Electrophysiological Criteria in CIDP and MMN: Toward a Unified Definition of Motor Conduction Block and Proximal Temporal Dispersion.

BACKGROUND: The definition of motor conduction block (CB) and proximal temporal dispersion (TD) for the diagnosis of CIDP and MMN remains debated, particularly regarding whether CB should be based on amplitude or area reduction, and whether proximal TD is best assessed using negative peak or total CMAP duration. METHODS: Nerve conduction waveforms from 106 CIDP, 87 MMN, and 234 controls were analyzed. Motor CB was defined by ≥ 30%, ≥ 40%, or ≥ 50% amplitude or area reduction. Proximal TD was defined as a ≥ 30% increase in negative peak duration, total CMAP duration, or either criterion. Sensi

Active subperiosteal versus passive subdural 24-hour drainage following single burr hole evacuation of chronic subdural haematoma (the SUPERDURA trial): protocol for a multicentre, randomised non-inferiority trial.

INTRODUCTION: The main treatment of chronic subdural haematoma (CSDH) is neurosurgical evacuation with subsequent drainage. However, consensus on optimal drain modality and placement is lacking. AIM: To examine whether 24-hour active subperiosteal drainage is non-inferior to 24-hour passive subdural drainage after a single burr hole evacuation of a symptomatic CSDH. METHODS AND ANALYSIS: SUPERDURA is a multicentre randomised non-inferiority trial encompassing all neurosurgical units in Denmark. Adult patients with symptomatic CSDH admitted to a Danish neurosurgical unit for single burr hole

Nervonic acid supplementation mitigates disease severity biomarkers in adrenoleukodystrophy.

X-linked adrenoleukodystrophy (ALD) is a severe neurometabolic disorder caused by mutations in the ABCD1 gene, leading to impaired peroxisomal β-oxidation of very long-chain fatty acids (VLCFAs). The accumulation of saturated VLCFAs, predominantly C26:0, in plasma and across all tissues, contributes to adrenal dysfunction and progressive neurodegeneration. No approved therapy addresses the diverse spectrum of ALD manifestations, underscoring the urgent need for safe, accessible, and preventive treatments. Nervonic acid (NA), a monounsaturated fatty acid, is potentially beneficial for ALD throu

Induction of ferroptotic and amyloidogenic signatures linked to Alzheimer's disease by chemically distinct air pollutants.

Air pollution (AirP) exposure is associated with increased risk of Alzheimer's disease (AD), yet AirP is chemically heterogeneous, complicating identification of shared pathogenic drivers. We compared acute cortical responses to two chemically distinct but metal-rich AirP sources, diesel exhaust particles (DEP) and World Trade Center (WTC) dust, and contrasted them with woodsmoke (WS), a particulate exposure containing substantially lower metal content. Despite major differences in particle composition and size, DEP and WTC elicited highly convergent transcriptomic responses, sharing more than

Cenobamate and Trazodone Combination Suppresses Seizures, EEG Alterations, Preserves Neuronal Architecture and Normalizes the BDNF/TrkB Expression in PTZ‑Kindled Mice.

Polytherapy is increasingly needed to overcome the limitations of monotherapy in epilepsy and its comorbidities. Therefore, we investigated the effect of cenobamate (CNB, 30 mg/kg) and trazodone (TZ, 10 mg/kg), alone and in combination, in PTZ-kindling model, where mice received 11 doses of pentylenetetrazole (PTZ, 40 mg/kg) every other day for three weeks. Behavioral seizure scoring, electroencephalographic (EEG) changes at the 1st, 5th, and 11th injections, molecular markers, and hippocampal histopathology were assessed. TZ + CNB suppressed seizure progression with no tonic-clonic seizures o

Characterizing brain structural and functional abnormalities in borderline personality disorder by seed-based d mapping approach.

Borderline personality disorder (BPD) is a mental disorder characterized by extreme instability in mood, interpersonal relationships, self-image, and behavioral patterns. Its core symptoms include intense mood swings, strained interpersonal relationships, disturbed self-identity, impulsive behaviors, and cognitive and perceptual abnormalities. The etiology of BPD is complex, involving genetic, neurobiological, and psychosocial factors. Among neurobiological factors, neuroimaging studies have consistently reported inconsistent findings regarding amygdala volume in BPD patients-some MRI studies

Hemizygous loss-of-function variants of EIF1AX are associated with a syndromic neurodevelopmental disorder.

Pathogenic variants of genes encoding initiation factors can cause neurological diseases, including neurodevelopmental disorders and brain abnormalities. The eukaryotic translation initiation factor 1 A, X-linked (EIF1AX) is a gene located at Xp22.12 that plays an important role in the regulation of translation initiation. Here, we identified de novo hemizygous EIF1AX variants in male individuals with neurodevelopmental disorders and explored their possible involvement in these neurological disorders. We performed trio-based exome or whole genome sequencing in four families. The pathogenicity

Resting-state brain age is associated with cognitive and sensorimotor abnormalities in schizophrenia spectrum disorders: a validation & longitudinal study.

Brain-age models use neuroimaging features to predict chronological age. The resulting brain-age gap (BAG), defined as predicted brain age minus chronological age, quantifies deviations from age-expected brain characteristics. Structural brain-age acceleration is well established in schizophrenia spectrum disorders (SSD), but the utility of resting-state functional connectivity (rs-FC)-based brain age remains unclear. Here, we trained rs-FC brain-age models on aggregated lifespan data from healthy controls (n ≈ 2200) and evaluated them in four independent SSD case-control cohorts. Across indep
