UNLABELLED: Chromosome 1q21.1 copy number variants (CNVs) are associated with a broad and variable clinical spectrum encompassing neurodevelopmental, anthropometric, and systemic anomalies. The incomplete penetrance and variable expressivity of these rearrangements pose significant challenges for clinical interpretation and genotype-phenotype correlation. We retrospectively analyzed 42 pediatric patients with 1q21.1 CNVs, identified by chromosomal microarray analysis (CMA), evaluated at a single tertiary center between 2019 and 2025. Patients were classified according to CNV type and localizat

BACKGROUND: Benign enlargement of the subarachnoid space (BESS) is a condition characterized by widened subarachnoid spaces, typically associated with macrocephaly or increasing head circumference during infancy or early childhood. Often described as idiopathic and self-limiting, BESS has also been associated with developmental delays and spontaneous subdural collections. Yet, complete interrogation of these associations is limited by lack of normative subarachnoid space characterization and standardized diagnostic criteria.
OBJECTIVE: This project aimed to review and synthesize literature on

BACKGROUND: Trio Rho guanine nucleotide exchange factor (TRIO) encodes the guanine nucleotide exchange factor (GEF) for RHOA and RAC1 GTPases, which plays a critical role in neurodevelopment. Pathogenic variants in the TRIO gene are predominantly associated with two autosomal dominant neurodevelopmental disorders: intellectual developmental disorder 44 with microcephaly (MRD44) and intellectual developmental disorder 63 with macrocephaly (MRD63). However, the genotype-phenotype correlation of TRIO-related disorders and the impacts of TRIO variants on early-life disease progression remain uncle

PTEN hamartoma tumour syndrome (PHTS), a rare disease caused by germline heterozygous PTEN variants, is associated with multi-organ/tissue overgrowth, autism spectrum disorder and increased cancer risk. Phenotypic variability in PHTS is partly due to diverse PTEN variants and the protein's multifaceted functions. PTEN is primarily a phosphatidylinositol(3,4,5)trisphosphate (PIP3) phosphatase regulating PI3K/AKT signalling but also maintains chromosomal stability through nuclear functions such as double-stranded (ds)DNA damage repair. Here, we show that PTEN-R173C, a pathogenic variant frequent

PTEN hamartoma tumor syndrome (PHTS) is a rare genetic condition associated with neurodevelopmental disorders, macrocephaly, and increased cancer risk. We report the case of a four-year-old girl with congenital hypothyroidism, progressive macrocephaly, and global developmental delay, later diagnosed with autism spectrum disorder (ASD). Brain MRI revealed megalencephaly with prominent extra-axial spaces and a diffusely thickened corpus callosum. Genetic testing identified a novel frameshift variant in the PTEN gene. This case highlights clinical findings that should raise suspicion for PHTS and

Rare brain disorders often present with changes in brain volume, and variation in brain volume is known to be highly heritable. Recent work studying brain volume variation has largely focused on common variants and structural variants. Rare variants often have large effect sizes and clearer connections to biological mechanisms, but the role of rare variants has not been extensively studied. We performed rare-variant gene aggregation analysis for total brain volume and 43 regional brain volume phenotypes (n = 50,061) to identify genes associated with brain volume variation through loss-of-funct

OBJECTIVE: Pathogenic variants in Kaptin (KPTN) cause KPTN-related disorder (KRD). KPTN modulates mTOR signaling activation within the KICSTOR complex in response to cellular amino acid levels. We define the clinical spectrum and investigate the developmental pathogenesis of KRD.
METHODS: We report the genotype and clinical phenotype of 71 KRD individuals (28 female subjects, ages 1 to 55 years) including 48 newly identified KRD individuals. The effects of Kptn knockout on brain development were assayed in vitro and in vivo.
RESULTS: We defined 15 novel KPTN variants. Intellectual disability

Fragile X syndrome (FXS) is the leading cause of inherited intellectual disability and autism. One common physical feature of FXS is macrocephaly, a condition typically associated with brain overgrowth and dysfunction; however, the mechanisms underlying its occurrence are unclear. Here, we uncover a role for the fragile X messenger ribonucleoprotein (FMRP) in regulating tissue growth through the Minibrain (Mnb) kinase, also known as DYRK1A, a gene up-regulated in Down syndrome and mutated in a specific form of autism. Using fly models of FXS, we find that Drosophila FMRP (dFmrp) suppresses the

The nonsense-mediated mRNA decay (NMD) pathway is a translation-dependent mechanism that maintains cellular RNA homeostasis by eliminating mRNAs containing a premature termination codon (PTC). NMD also targets ~ 15% of all physiological coding and non-coding spliced transcripts, thereby playing a significant role in shaping the cellular transcriptome. SMG5 is one of several NMD effector proteins required for the cleavage of mRNAs. In a female patient with craniofacial dysmorphism, developmental delay, severe growth retardation, and relative macrocephaly, we identified the homozygous synonymous

Megalencephaly (ME) is a malformation of cortical development defined by an enlarged brain. Individuals with ME often suffer from drug resistant epilepsy, intellectual disability, and autism spectrum disorder. Several clinical ME subtypes result from pathogenic variants in mTOR pathway genes (MPG) which cause diffuse brain overgrowth likely as a consequence of hyperactive mechanistic target of rapamycin (mTOR) signaling during brain development. Unfortunately, resected surgical or post-mortem ME brain tissue specimens are not widely available, and thus, there is only limited understanding of t

Biallelic pathogenic variants in STRADA (STE20-related adaptor alpha), an upstream regulator of the mechanistic target of rapamycin (mTOR) pathway, result in megalencephaly, drug-resistant epilepsy, and severe intellectual disability. This study explores how mTOR pathway hyperactivity alters cell fate specification in dorsal and ventral forebrain development using STRADA knockout human stem cell-derived brain organoids. In both dorsal and ventral forebrain STRADA knockout organoids, neurogenesis is delayed, with a predilection for progenitor renewal, increased proliferation and an expanded out

For pediatricians to recognize when there is medical concern, it is very important to have a basic understanding of what to expect for typical development and evidence-based definitions and criteria for flagging growth concerns. In this article, we review basic terminology such as growth faltering, malnutrition, short stature, macrocephaly, and microcephaly to provide a quick reference guide.

Macrocephaly, defined as a large head size, has a very broad differential and sometimes can be challenging to differentiate as benign versus indicative of pathology. In this review, we outline a stepwise approach to improving diagnosis of neurogenetic disorders versus other causes using the example of PTEN (phosphatase and TENsin homolog) Hamartoma Tumor Syndrome (PHTS). PHTS is a multiple hamartoma syndrome with medical management implications including the need for tumor surveillance, but often cases are not diagnosed until later in adulthood. Our review emphasizes the utility of using head

Malformations of cortical development are complex and diverse. They are an etiology for drug-resistant epilepsy, and their imaging presentation may be striking or quite subtle. There are multiple schemes to classify this pathology group. One of the most well known is to group disorders based on the type of embryologic error that leads to each malformation. They can also be grouped based on imaging appearance, with the morphology of the involved cerebral cortex acting as the first major distinction.

An uncommon inherited autosomal recessive disorder, Van der Knaap disease is also referred to as megalencephalic leukoencephalopathy with subcortical cysts (MLC). Megalencephaly, which either develops at birth or during infancy, along with seizures and a mild motor development delay, is its defining characteristic. Leukodystrophy and subcortical cystic degeneration are the two MRI abnormalities that are indicative of the illness and typically provide the key to diagnosis. The disease is more common in certain ethnicities, like the Aggarwal community in India, where marriages within the communi

Background/Objectives: Autism spectrum disorder (ASD) is a neurodevelopmental disease with both clinical and genetic heterogeneity. Several loss-of-function variants in the chromodomain helicase DNA-binding protein 8 (CHD8) gene have been identified in individuals with ASD and/or developmental delay/intellectual disability. These are associated with specific clinical manifestations, including overgrowth, macrocephaly, sleep disturbance, and gastrointestinal problems. Methods: We performed clinical exome sequencing in a female patient with ASD and macrocephaly. RNA analysis from peripheral bloo

We report a prenatal case of rare dysplastic megalencephaly detected via ultrasound and magnetic resonance imaging (MRI), showing progressive macrocephaly and abnormal cortical development. Whole-exome sequencing (WES) identified a somatic AKT1 variant (c.49G>A), confirming the diagnosis. This case highlights the integration of imaging and genetic analysis in diagnosing fetal megalencephaly, underscoring the role of mTOR pathway disorders. Pregnancy was terminated after counseling.

Nuclear factor I (NFI) transcription factors regulate neural stem and progenitor differentiation during brain development. While NFIA, NFIB, and NFIX are linked to neurodevelopmental disorders, the role of NFIC (MIM: 600729) in human disease remains unclear. This study aimed to determine whether NFIC contributes to a neurodevelopmental syndrome, define its phenotype, and assess dosage-dependent effects. We established the first cohort of 11 individuals, including NFIC deletions and single nucleotide variants. Genotype-phenotype correlations, including critical region mapping, were performed. M

Syndromic megalencephaly comprises a heterogeneous group of genetic disorders characterized by abnormal brain overgrowth and variable structural brain abnormalities. Prenatal recognition remains challenging because imaging findings overlap with those of isolated macrocephaly, while genotype-phenotype correlations are incompletely understood. This review summarizes the current literature on fetal magnetic resonance imaging (MRI) findings in molecularly confirmed syndromic megalencephaly and discusses their relationship with the underlying genetic pathways. The available evidence indicates that
