Coarctation-like features from skeletal anomalies: recognising extrinsic compression in fetal echocardiography.

Prenatal diagnosis of coarctation of the aorta (CoA) remains a challenging diagnosis in fetal cardiology. In rare situations, extrinsic compression of the aorta can produce a pseudocoarctation-like appearance, characterised by elongation and kinking of the aorta. This appearance is rarely recognised prenatally and is not well documented. We report a fetus at 24+4 weeks with ventricular disproportion and arch kinking, initially raising suspicion for CoA. Echocardiography demonstrated preserved haemodynamics and no discrete narrowing, but findings were consistent with extrinsic compression of th

A Global Update on the Status of Prevention of Folic Acid-Preventable Spina Bifida and Anencephaly in Year 2024.

BACKGROUND: Spina bifida and anencephaly are major birth defects, and the majority of them can be prevented through maternal intake of folic acid (vitamin B9) during early pregnancy. We estimated the proportion of folic acid-preventable spina bifida and anencephaly (FAP SBA) prevented globally through mandatory folic acid fortification policy in the year 2024 for cereal grains (i.e., wheat flour, maize flour, and rice). METHODS: We used country-specific fortification data from the Food Fortification Initiative for year 2024. Seventy countries had mandatory cereal grain fortification adding fo

Effectiveness of closed incisional negative pressure wound therapy in pediatric spinal dysraphism: a single-center retrospective case series.

INTRODUCTION: Postoperative wound complications - including surgical site infection (SSI), cerebrospinal fluid (CSF) leakage, and wound dehiscence - remain a significant challenge following surgery for spinal dysraphism, particularly in neonates and infants. Closed incisional negative pressure wound therapy (ciNPWT) has demonstrated benefit in several surgical subspecialties; however, evidence regarding its routine use in pediatric spinal dysraphism surgery is limited. METHODS: A retrospective review was conducted of all pediatric patients (age < 18 years) who underwent surgery for spinal dys

Unmasking pathogen traits for chronic colonization in neurogenic bladder.

Individuals with neurogenic bladder are susceptible to chronic bacterial colonization and urinary tract infections. Neurogenic bladder arises from conditions including spinal cord injuries and spina bifida. We established a longitudinal cohort of 77 children and young adults with spina bifida. Using enhanced urine culture, 16S rRNA sequencing, and whole-genome sequencing of bacterial isolates, we characterized the urine microbiota. We also retrieved prior bacterial isolates from Vanderbilt's MicroVU biobank, enabling 5-year evolutionary comparisons within subjects. Urine samples showed high ab

Sleep-related breathing disorders beyond infancy following open spina bifida repair.

In patients with repaired open spina bifida, there are limited data on respiratory outcomes beyond the first year of life. We hypothesize that after 1 year of age there is 1) resolution of sleep-related breathing disorders (SRBD), 2) improvement of oxygenation and 3) no difference in SRBD between prenatally vs. postnatally repaired groups. We reviewed 67 patients with repaired open spina bifida and polysomnography (PSG) after age 1 year seen at Children's Hospital Los Angeles between 2015-2025. Demographics, neural tube defect location, surgery type, PSG results, and respiratory support data w

A prospective crossover study comparing ICCS-recommended and Palmer-adjusted filling rates in children with spina bifida.

OBJECTIVE: This study aimed to investigate whether the maximal cystometric capacity (MCC) in children with spina bifida (SB) is indeed lower, as predicted by the Palmer formula, and to evaluate the impact of different bladder filling rates on urodynamic parameters. MATERIALS AND METHODS: This prospective, randomized, two-sequence crossover-controlled study included 70 children aged 3-18 years with spina bifida under regular follow-up. In Group 1, the first two bladder fillings were performed at the ICCS-recommended rate, and the third at 75% of that rate (Palmer formula). In Group 2, the sequ

Urologic outcomes in children with closed spinal dysraphism with and without untethering of the spinal cord.

PURPOSE: Closed spinal dysraphism (CSD) comprises a heterogeneous group of congenital anomalies that may remain asymptomatic or present with neurological and urological dysfunction. This study aimed to evaluate long-term urological outcomes in children with isolated CSD and to compare outcomes between patients managed conservatively and those who underwent spinal cord untethering (SCU). METHODS: We conducted a retrospective cohort study of children with CSD treated at Helsinki University Hospital between 1990 and 2015. RESULTS: Forty-three patients were followed for a median of 11.0 years. U

Prenatal Diagnosis of Craniorachischisis Totalis Following an Initially Normal First-Trimester Ultrasound in a Woman With Two Previous Caesarean Births: A Case Report.

Craniorachischisis totalis is the most severe form of open neural tube defect and combines acrania/anencephaly with extensive open spinal dysraphism. It is rare, lethal, and usually identifiable prenatally, but diagnostic labeling may evolve across pregnancy as imaging findings become clearer. We report the case of a 33-year-old multiparous woman, gravida 4 para 2+1, with two previous caesarean births, whose booking ultrasound at 8 weeks and 4 days of amenorrhea showed a single live intrauterine embryo with no documented structural abnormality. She later underwent targeted fetal anomaly scanni

Spina Bifida Occulta, Syringomyelia, and Diastematomyelia in a Toddler: A Two-Year Case Report.

Spinal dysraphisms are a heterogeneous group of congenital spinal cord anomalies resulting from defective neurulation. They include both open and closed forms, with spina bifida (SB) being the most clinically recognized. These conditions frequently coexist with other anomalies, such as syringomyelia and diastematomyelia, significantly complicating clinical presentation and management. We report the case of a male neonate born at term with a lumbar spinal defect consistent with SB occulta. Initial evaluation revealed a closed, skin-colored lesion with preserved neurological function. Early spin

Surgical management of anterior sacral meningoceles: a case series and systematic review.

PURPOSE: Anterior sacral meningoceles (ASMs) are rare forms of spinal dysraphism caused by anterior herniation of meninges through a sacral defect. Surgical management is the treatment of choice, yet the optimal approach remains debated. This study compares outcomes of three main surgical approaches-anterior transabdominal, posterior transsacral, posterior sagittal (PSARP-type)-focusing on the efficacy and safety of the PSARP-type. METHODS: A retrospective single-center analysis was conducted on eight patients treated for ASM between 2008 and 2025. Demographic, clinical, radiological, functio

Prevalence of spina bifida across the lifespan in the USA.

AIM: To estimate the prevalence of spina bifida across the lifespan, the number of people with spina bifida by state, and variations in spina bifida prevalence by insurance type, state, and selected demographics. METHOD: This was an observational study that used administrative insurance data. We used three public-payer data sets (Medicaid Fee for Service, Medicare, and Medicare Advantage) and one large employer-sponsored insurance database (Health Care Cost Institute [HCCI]) to estimate all-age prevalence of spina bifida in the USA. Claims data from 2017 to 2019 were used to identify spina bi

Congenital Cervical Vertebral and Spinal Cord Anomalies.

Congenital cervical vertebral anomalies in horses are structural changes in the neck vertebrae that develop during the fetal stage. These anomalies can vary in their severity and effects. Some horses may not show any clinical signs and can live normally, while others may develop serious problems such as spinal cord compression. This compression can lead to issues like ataxia or even paralysis, affecting the horse's mobility and quality of life. Recognizing and diagnosing these anomalies early are very important, as it helps guide decisions about the best treatment options and overall managemen

Pseudo-osteoporosis on bone densitometry due to congenital spinal dysraphism.

A patient in his late adolescence was referred for evaluation of low and apparently declining spinal bone density on dual-energy X-ray absorptiometry (DXA). Review of the DXA images revealed normal bone mineral density at other body sites but exceedingly low lumbar spine Z-scores which appeared to get progressively lower over time. The DXA result was explained by congenital spinal dysraphism with absent posterior spinous elements, rather than true low bone mineral content. Recognition of this structural artefact prevented unnecessary interventions. The case underscores the importance of correl

Hemimyelomeningocele: a systematic review and meta-analysis of the literature.

Hemimyelomeningocele (HMM) is a rare split cord malformation where only one hemicord forms a myelomeningocele-like sac, and the opposite hemicord undergoes normal neurulation. We aimed to compile all published HMM cases to concisely summarize embryology, presentation, imaging, associated anomalies, management, and outcomes of the disorder. Following PRISMA 2020, we searched PubMed/MEDLINE, Scopus, Web of Science, and Google Scholar (1968-Feb 2025) for studies with confirmed human HMM, extracting clinical, radiological, surgical, and follow-up data. Of 688 records screened, 25 articles met incl

Prenatal Closure of Myeloschisis vs Myelomeningocele: Insights From the fMMC Consortium Registry.

BACKGROUND AND OBJECTIVES: Myelomeningocele is defined by the presence of a fluid-filled sac that contains the neural placode and cerebrospinal fluid (CSF) at the level of the spinal defect. Alternatively, the term myeloschisis is used when the neural placode is not contained within a CSF-filled sac. While both are eligible for prenatal closure, few studies have characterized the impact of the type of lesion on postnatal outcomes. This study compares the outcomes of these 2 types of open spinal dysraphism using data from the Fetal Myelomeningocele Consortium registry sponsored by the North Ame
