Surgical treatment and outcomes of neural tube defects at the university of Gondar Comprehensive Specialized Hospital, Ethiopia: a two-year retrospective review.

INTRODUCTION: Spina bifida, particularly myelomeningocele (MMC), is one of the most common congenital central nervous system abnormalities compatible with life. Despite global efforts to reduce its incidence through folic acid fortification, the burden remains high in low-resource settings like Ethiopia. This study aimed to analyze the pattern of presentation, clinical characteristics, management, and outcomes of spina bifida spectrum disorders (SBSDs) at the University of Gondar Comprehensive Specialized Hospital. METHODS: A retrospective cross-sectional study was conducted on 240 neonates a

Regional anesthesia combined with esketamine analgesia and remimazolam sedation as a salvage strategy for intertrochanteric femur fracture surgery in a high-risk patient: A case report.

RATIONALE: The anesthetic management of intertrochanteric fractures in patients with dual contraindications to neuraxial and general anesthesia is highly challenging. We illustrate a unique salvage multimodal strategy combining a comprehensive ultrasound-guided regional blockade with esketamine-based sedation to manage such complex cases. PATIENT CONCERNS: A 55-year-old female (147 cm, 45 kg) was admitted following a slip-and-fall accident resulting in severe right hip pain and limited mobility. Due to her complex medical history, the patient and her family expressed significant anxiety regar

Oxidative stress and antioxidative defense in fetuses and newborn children with spina bifida in cerebrospinal fluid.

BACKGROUND: Spina bifida remains the most prevalent defect of the central nervous system and second most common congenital birth anomaly. Oxidative stress is one of the key factors affecting the extent of tissue damage during fetal life. Harmful effects of Reactive Oxygen Species (ROS) are exacerbated by rapid growth and development of tissues, coupled with weak antioxidant defenses, and lead to protein, lipid and DNA damage, ultimately resulting in cell death through apoptosis or necrosis. METHODS: A total of 62 samples of the cerebrospinal fluid from children with spina bifida were collecte

Familial caudal regression syndrome with complex spinal dysraphism presenting as recurrent cutaneous infection.

BACKGROUND: Caudal regression syndrome (CRS) is a rare congenital disorder characterized by varying degrees of sacral agenesis and spinal dysraphism. Although most cases are sporadic and associated with maternal diabetes, familial occurrence with variable phenotypic expressivity across generations has rarely been described. CASE DESCRIPTION: We report a 12-year-old boy with CRS presenting with recurrent midline pustular lesions since birth, intermittent urinary incontinence, and lower-limb pain. Magnetic resonance imaging revealed sacral agenesis with preservation of S1, a low-lying conus med

Microsurgical Untethering of Pediatric Lipomyelomeningocele: A Stepwise, Photo-Illustrated Technical Note.

Lipomyelomeningocele (LMMC) is one of the most common forms of occult spinal dysraphism, with an estimated incidence of 3-6 per 100,000 live births, and microsurgical untethering remains the cornerstone of management for symptomatic and selected at-risk children. The operation is technically demanding: reported rates of long-term symptomatic re-tethering after partial resection still reach 15-25%, and the surgeon must balance adequate untethering against preservation of the placode and lumbosacral nerve roots. In this technical note, we present a stepwise, illustrated description of our instit

Advanced neurosurgical intervention of recurrent tethered cord syndrome with split cord malformation type 1: an illustrative case.

PURPOSE: Recurrent tethered cord syndrome (TCS) with split cord malformation (SCM) is a rare presentation with distinct surgical challenges to successful patient outcomes. SCMs are rare and can progress to cause significant neurological deficits including pain, weakness, sensory disturbance, bowel and/or bladder dysfunction. In these circumstances, surgical management of TSC with SCM is indicated. Reoperation for recurrent TCS, particularly in the context of retained or regrown bony septa and prior spinal instrumentation, is technically demanding and carries elevated risk. ILLUSTRATIVE CASE:

Rare Case of Anterior Sacral Meningocele in a 30-Year-Old Male: Surgical Excision Using the Posterior Sagittal Approach.

Anterior sacral meningocele (ASM) is an uncommon congenital spinal abnormality in which the meningeal sac herniates through an abnormality in the anterior sacrum into the presacral space. It is more observed in females and usually asymptomatic; large ASMs may cause pressure effects on nearby pelvic structures. A 30-year-old male presented with prolonged constipation and recurrent episodes of urinary retention, which developed to complete urinary obstruction. Ultrasound demonstrated a large presacral cystic mass, and MRI confirmed the diagnosis of an ASM. Pelvic x-ray revealed the classic Scimi

Bowel continence in adult individuals with spina bifida: Who takes ownership of care?

INTRODUCTION: Neurogenic bowel dysfunction is a significant health and quality-of-life concern for individuals with spina bifida. To date, little is known about how these individuals manage continence issues once they reach independence in adulthood. The purpose of this study is to better understand how adults with spina bifida manage their bowel continence, surgical complications, and coordination of care. STUDY DESIGN: An anonymous web-based survey was distributed through the Spina Bifida Association (SBA) e-mail listserv and various social media groups. Data regarding demographics, bowel m

Spinal intradural epidermoid cysts in pediatric patients: illustrative cases and systematic review.

OBJECTIVE: Spinal intradural epidermoid cysts (sIECs) are rare, benign neoplasms comprising < 3% of intraspinal tumors in children. Their nonspecific clinical presentation and rarity often lead to diagnostic delays, particularly in the pediatric population. Authors of this study present 2 pediatric sIEC cases and conduct a systematic review of the literature to characterize the clinical features, management strategies, and outcomes of pediatric sIEC. METHODS: A systematic review was conducted according to PRISMA guidelines, searching the PubMed, Embase, Web of Science, and Cochrane databases

Microsurgical Resection of an Inclusion Cyst in a Fetal Myeloschisis Patient: A Surgical Anatomy and Technical Note.

BACKGROUND AND OBJECTIVES: Inclusion cysts (ICs) are a recognized sequela of myeloschisis closure, particularly in patients who undergo in utero repair. These lesions and their management present a unique challenge in the context of tethered cord surgery. Early detection and appropriate surgical management are essential for reversing or preventing further neurological decline. METHODS: We present the case of a 22-month-old girl who underwent fetal myeloschisis repair at 23 weeks' gestation. During routine screening, she was found to have progressive neurogenic bowel and bladder dysfunction, a

[Transition of Patients with Neurogenic Bladder Dysfunction: Challenges and Unmet Needs in Adult Care].

Transition describes the structured process by which adolescents are transferred from pediatric and family-centered, and thus predominantly externally determined, care to patient-centered, self-determined adult care. In patients with spina bifida (SB), transition is particularly relevant from a urological perspective, as neurogenic bladder and bowel dysfunction frequently require chronic pharmacological treatment, permanent bladder management (including intermittent self-catheterization), and lifelong monitoring of kidney and bladder function. At the same time, physical limitations and frequen

Habitat-shifting predatory and herbivorous reef fishes exhibit spatial partitioning despite shared biophysical drivers of seascape use.

CONTEXT: Climate change and anthropogenic stressors are degrading coastal biogenic habitats, disrupting the vital pathways fish use to shift from nearshore nurseries to offshore reef environments. Although fishes are critical for ecosystem health, spatial management rarely accounts for the interconnectedness of fish habitat needs across their life cycle. OBJECTIVES: To examine and compare drivers of habitat use for subadult size classes of five habitat-shifting fishes from two feeding guilds: mesopredators and herbivores. To assess the multi-species suitability of existing management zones.
