From Gastrulation to Malformation: Understanding the Embryology of Split Cord Malformations for Obstetric Practice.

BACKGROUND: Split cord malformation (SCM) is an uncommon congenital anomaly in which the spinal cord is longitudinally divided into two hemicords separated by a fibrous, cartilaginous, or osseous septum. Historically described as diastematomyelia or diplomyelia, the unified classification proposed by Pang et al. [Neurosurgery. 1992;31(3):451-80] distinguishes two main subtypes: type I, characterized by two hemicords within separate dural sacs divided by a rigid spur, and type II, in which hemicords share a single dural sac separated by a fibrous band. Composite SCM, involving multiple nonconti

Intradural posterior bony spur causing tethered cord syndrome without split cord malformation: A rare caudal developmental variant.

BACKGROUND: Tethered cord syndrome (TCS) is most commonly associated with congenital spinal dysraphism, thickened filum terminale, lipomas, or split cord malformations (SCM ). Bony causes of tethering are typically described in association with diastematomyelia. Isolated intradural bony spurs without cord splitting or extradural communication are exceptionally rare. CASE DESCRIPTION: We report a young adult female presenting with long-standing progressive lower limb sensory disturbances, gait tightness, and urinary dysfunction. A delayed neurological diagnosis resulted in multiple orthopedic

Rare Case of Multifocal Spinal Hemangioblastomas With Holocord Syringo-Hydromyelia in an Adolescent Male.

Hemangioblastomas are benign, highly vascular tumors of the central nervous system. Although they most commonly arise in the cerebellum, they may also involve the brainstem and spinal cord. The presence of multiple spinal hemangioblastomas is frequently associated with von Hippel-Lindau (VHL) syndrome; however, sporadic cases without an underlying genetic syndrome can occur. Characteristic imaging features include avid post-contrast enhancement, associated syrinx formation, and intratumoral or peritumoral flow voids. Spinal hemangioblastomas account for a small proportion of intramedullary spi

Coarctation-like features from skeletal anomalies: recognising extrinsic compression in fetal echocardiography.

Prenatal diagnosis of coarctation of the aorta (CoA) remains a challenging diagnosis in fetal cardiology. In rare situations, extrinsic compression of the aorta can produce a pseudocoarctation-like appearance, characterised by elongation and kinking of the aorta. This appearance is rarely recognised prenatally and is not well documented. We report a fetus at 24+4 weeks with ventricular disproportion and arch kinking, initially raising suspicion for CoA. Echocardiography demonstrated preserved haemodynamics and no discrete narrowing, but findings were consistent with extrinsic compression of th

Foramen Magnum Decompression for Retro-Odontoid Pseudotumor in an Elderly Patient With Chiari Malformation Type I: A Case Report.

Chiari malformation type I (CM-I) is uncommon in elderly patients, and surgical experience in this population remains limited. Long-term outcomes after surgery, particularly following prolonged conservative observation, have rarely been reported. We present an 82-year-old woman with CM-I who developed neurological deterioration due to a retro-odontoid pseudotumor (ROP) after two decades of clinical stability. Foramen magnum decompression (FMD) was performed without postoperative complications, resulting in marked neurological improvement. She was discharged, and long-term follow-up demonstrate

Spina Bifida Occulta, Syringomyelia, and Diastematomyelia in a Toddler: A Two-Year Case Report.

Spinal dysraphisms are a heterogeneous group of congenital spinal cord anomalies resulting from defective neurulation. They include both open and closed forms, with spina bifida (SB) being the most clinically recognized. These conditions frequently coexist with other anomalies, such as syringomyelia and diastematomyelia, significantly complicating clinical presentation and management. We report the case of a male neonate born at term with a lumbar spinal defect consistent with SB occulta. Initial evaluation revealed a closed, skin-colored lesion with preserved neurological function. Early spin

18F-FDG PET/CT Findings in a Case of Primary Leptomeningeal Melanomatosis With Diastematomyelia.

Primary leptomeningeal melanomatosis is a rare and aggressive variant of primary central nervous system melanoma. Diagnosis is often challenging due to nonspecific radiologic features. We hereby report the 18F-FDG PET/CT and MRI findings in a case of primary leptomeningeal melanomatosis in a 47-year-old woman with a previously unidentified diastematomyelia. Spinal MRI showed diffuse abnormal signal. 18F-FDG PET/CT showed hypermetabolism most notably in the lumbar and caudal portion of the spinal cord, which was helpful to identify an appropriate site for biopsy.

Hemimyelomeningocele: a systematic review and meta-analysis of the literature.

Hemimyelomeningocele (HMM) is a rare split cord malformation where only one hemicord forms a myelomeningocele-like sac, and the opposite hemicord undergoes normal neurulation. We aimed to compile all published HMM cases to concisely summarize embryology, presentation, imaging, associated anomalies, management, and outcomes of the disorder. Following PRISMA 2020, we searched PubMed/MEDLINE, Scopus, Web of Science, and Google Scholar (1968-Feb 2025) for studies with confirmed human HMM, extracting clinical, radiological, surgical, and follow-up data. Of 688 records screened, 25 articles met incl

Surgical fixation of a unilateral bar and unsegmented butterfly vertebra in a patient with diastematomyelia and a tethered cord: illustrative case.

BACKGROUND: There is a paucity of literature to guide the surgical management of the simultaneous presentation of a diastematomyelia, two hemicords, and a unilateral bar unsegmented vertebra, a form of formation failure. OBSERVATIONS: An 11-year-old female presented with acute-onset back pain, urinary incontinence, and lower extremity numbness. Evaluation revealed T7 diastematomyelia with a tethered cord and a right-sided T6-11 nonsegmented unilateral bar, resulting in thoracolumbar levoscoliosis (maximal Cobb angle of 49°). Orthopedic manifestations as presenting symptoms in children with neuroblastoma: a retrospective case series and clinical review.

INTRODUCTION: Neuroblastoma is the second most common childhood malignancy. Only a minority of children with metastatic disease present initially to orthopedic surgeons, despite musculoskeletal complaints such as back pain, limb pain, limp, extremity swelling, or findings mimicking osteomyelitis. These vague and nonspecific presentations increase the risk of delayed diagnosis. AIMS: To study orthopedic manifestations that present as the initial presenting symptoms of neuroblastoma in children and to characterize their clinical, radiological, and laboratory profiles. Congenital myelomeningocele containing two hemicords with placodes in a patient with complex split spinal cord malformation: illustrative case.

BACKGROUND: The unified theory of split spinal cord malformation (SCM) comprises type I, with each one of the hemicords in one own dural tube, separated by a bone spur, and type II, with two hemicords in one dural tube, separated by a nonrigid septum. This classification is still evolving, with denominations SCM type 0.5 proposed for incomplete forms, SCM type 1.5 for lesions combining features of SCM types I and II, and SCM type III for the composite lesions occurring on different spinal levels. OBSERVATIONS: A newborn male patient presented with a complex SCM with two placodes, one in each

Research Progress on the Pathogenesis and Diagnostic and Therapeutic Potential of Ciliopathies Regulated by IFT172.

IFT172 is a core component of intraflagellar transport complex B (IFT-B), and pathogenic IFT172 variants disrupt ciliary transport, receptor localization, and tissue-specific signaling. This review summarizes evidence linking IFT172 dysfunction to neurological, retinal, skeletal, renal, and syndromic ciliopathy phenotypes, with emphasis on Bardet-Biedl syndrome, Joubert-spectrum disease, orofaciodigital syndrome, Mainzer-Saldino syndrome, and retinal degeneration. Current data support a primary role for IFT172 in IFT-train assembly, tip remodeling, anterograde-to-retrograde transition, and cil

Adult-Onset Craniotubular Dysplasia, Ikegawa Type: A Report of a Case From Bahrain With a Novel Compound Heterozygous TMEM53 Mutation and Literature Review.

Craniotubular dysplasia, Ikegawa type (CTDI), is an ultra-rare sclerosing bone disorder associated with pathogenic mutations in the TMEM53 gene. The condition disrupts the negative regulation of ossification, leading to progressive skeletal abnormalities, short stature, and early-onset visual impairment secondary to optic nerve compression. Since its initial description in 2021, only a limited number of cases have been reported, nearly all presenting in childhood. Adult-onset disease remains exceptionally rare and poorly characterized. In this report, we describe a 38-year-old Bahraini gentlem

X-linked ARSL chondrodysplasia punctata: expanding the spectrum with a severe multisystem phenotype.

OBJECTIVES: X-linked chondrodysplasia punctata type 1 (CDPX1) is a rare skeletal dysplasia caused by pathogenic variants in ARSL (Arylsulfatase L), formerly known as ARSE (Arylsulfatase E), resulting in defective arylsulfatase E activity and abnormal cartilage matrix formation. It is typically defined by stippled epiphyses, nasomaxillary hypoplasia, and brachytelephalangy, but the condition has phenotypic variability. We report an infant with a pathogenic ARSL variant (NM_000047.3:c.1442C>T; p.Thr481Met) to highlight an exceptionally severe and complex phenotype. CASE PRESENTATION: A male inf

Advanced neurosurgical intervention of recurrent tethered cord syndrome with split cord malformation type 1: an illustrative case.

PURPOSE: Recurrent tethered cord syndrome (TCS) with split cord malformation (SCM) is a rare presentation with distinct surgical challenges to successful patient outcomes. SCMs are rare and can progress to cause significant neurological deficits including pain, weakness, sensory disturbance, bowel and/or bladder dysfunction. In these circumstances, surgical management of TSC with SCM is indicated. Reoperation for recurrent TCS, particularly in the context of retained or regrown bony septa and prior spinal instrumentation, is technically demanding and carries elevated risk. ILLUSTRATIVE CASE:

Contemporary Outcomes of Thoracic Endovascular Aortic Repair in Patients with Syndromic Genetic Aortopathy: A Multi-Centre National Study.

BACKGROUND: Thoracic aortic aneurysms and dissection in individuals with syndromic genetic aortopathy (SGA) due to Marfan syndrome, Loeys-Dietz syndrome, and vascular Ehlers-Danlos syndrome are especially challenging. This is due to the impaired aortic architecture secondary to the genetic variants. Previous studies have analyzed the outcomes after open surgery in this population. However, there is little information on their outcomes after thoracic endovascular aortic repair (TEVAR). Recent studies have demonstrated high rates of subsequent secondary interventions in these patients treated wi

Elucidating the genetic architecture of yield components in pyramidale wheat (Triticum turgidum L.): a comparative approach using QTL analysis and genomic selection.

BACKGROUND: Diabetic retinopathy (DR) is a major microvascular complication of diabetes mellitus and a leading cause of visual impairment. While dyslipidemia has been implicated in DR pathogenesis, the relationship between high-density lipoprotein cholesterol (HDL-C) and DR remains controversial. This study aimed to investigate the association between HDL-C levels and DR prevalence in diabetic patients type 2 diabetes mellitus (T2DM). METHODS: This study is the second analysis based on a cross-sectional studv. A total of 2001 (858 men and 1143 women) diabetic patients who visited the diabetic

Branchial cleft cyst of the cervicothoracic spine.

A branchial cleft cyst (BCC) is a neck mass, present from birth, caused by incomplete development of the branchial arches during fetal development. It commonly presents as a solitary, painless mass in the neck of a child or young adult. We present a case of a large intramedullary BCC in a patient with Klippel-Feil syndrome (KFS). A male in his 60s presented with long-lasting neck pain and fast-progressing paraparesis. MRI showed a large intramedullary cystic tumour in the cervicothoracic spine with accompanying cervical diastematomyelia and extensive cervical vertebrae fusion. The patient was

Hydranencephaly and porencephaly predominated among neuropathologic findings in stillborn and neonatal piglets during the first major outbreak of Japanese encephalitis in Australia.

In 2022, piggeries in southeastern Australia experienced an increase in reproductive losses and occasional neurologic disease in neonates with up to 100% mortality in some litters. Molecular testing identified a genotype IV Japanese encephalitis virus (JEV) as the etiological agent, detected for the first time in Australia. Necropsy of 235 JEV-positive domestic piglets, mostly stillbirths with 30% mummified, revealed hydranencephaly-porencephaly impacting the dorsal cerebrum as the most common lesion. Microscopically, this was characterized by varying degrees of parenchymal collapse as a resul
