Research Progress on the Pathogenesis and Diagnostic and Therapeutic Potential of Ciliopathies Regulated by IFT172.

IFT172 is a core component of intraflagellar transport complex B (IFT-B), and pathogenic IFT172 variants disrupt ciliary transport, receptor localization, and tissue-specific signaling. This review summarizes evidence linking IFT172 dysfunction to neurological, retinal, skeletal, renal, and syndromic ciliopathy phenotypes, with emphasis on Bardet-Biedl syndrome, Joubert-spectrum disease, orofaciodigital syndrome, Mainzer-Saldino syndrome, and retinal degeneration. Current data support a primary role for IFT172 in IFT-train assembly, tip remodeling, anterograde-to-retrograde transition, and cil

Adult-Onset Craniotubular Dysplasia, Ikegawa Type: A Report of a Case From Bahrain With a Novel Compound Heterozygous TMEM53 Mutation and Literature Review.

Craniotubular dysplasia, Ikegawa type (CTDI), is an ultra-rare sclerosing bone disorder associated with pathogenic mutations in the TMEM53 gene. The condition disrupts the negative regulation of ossification, leading to progressive skeletal abnormalities, short stature, and early-onset visual impairment secondary to optic nerve compression. Since its initial description in 2021, only a limited number of cases have been reported, nearly all presenting in childhood. Adult-onset disease remains exceptionally rare and poorly characterized. In this report, we describe a 38-year-old Bahraini gentlem

X-linked ARSL chondrodysplasia punctata: expanding the spectrum with a severe multisystem phenotype.

OBJECTIVES: X-linked chondrodysplasia punctata type 1 (CDPX1) is a rare skeletal dysplasia caused by pathogenic variants in ARSL (Arylsulfatase L), formerly known as ARSE (Arylsulfatase E), resulting in defective arylsulfatase E activity and abnormal cartilage matrix formation. It is typically defined by stippled epiphyses, nasomaxillary hypoplasia, and brachytelephalangy, but the condition has phenotypic variability. We report an infant with a pathogenic ARSL variant (NM_000047.3:c.1442C>T; p.Thr481Met) to highlight an exceptionally severe and complex phenotype. CASE PRESENTATION: A male inf

Elucidating the genetic architecture of yield components in pyramidale wheat (Triticum turgidum L.): a comparative approach using QTL analysis and genomic selection.

BACKGROUND: Diabetic retinopathy (DR) is a major microvascular complication of diabetes mellitus and a leading cause of visual impairment. While dyslipidemia has been implicated in DR pathogenesis, the relationship between high-density lipoprotein cholesterol (HDL-C) and DR remains controversial. This study aimed to investigate the association between HDL-C levels and DR prevalence in diabetic patients type 2 diabetes mellitus (T2DM). METHODS: This study is the second analysis based on a cross-sectional studv. A total of 2001 (858 men and 1143 women) diabetic patients who visited the diabetic

Hydranencephaly and porencephaly predominated among neuropathologic findings in stillborn and neonatal piglets during the first major outbreak of Japanese encephalitis in Australia.

In 2022, piggeries in southeastern Australia experienced an increase in reproductive losses and occasional neurologic disease in neonates with up to 100% mortality in some litters. Molecular testing identified a genotype IV Japanese encephalitis virus (JEV) as the etiological agent, detected for the first time in Australia. Necropsy of 235 JEV-positive domestic piglets, mostly stillbirths with 30% mummified, revealed hydranencephaly-porencephaly impacting the dorsal cerebrum as the most common lesion. Microscopically, this was characterized by varying degrees of parenchymal collapse as a resul

A Novel Subtype of Spondylocostal Dysplasia Associated With a Heterozygous Missense FLNA Variant.

BACKGROUND: Spondylocostal dysplasia (SCD) is characterized by vertebral defects and rib abnormalities. Following radiological diagnosis, further genetic testing is conducted to confirm the mutant loci and identify the subtype of SCD. While seven loci potentially associated with SCD have been identified, rare cases remain unexplained. CASE PRESENTATIONS: A 37-year-old female diagnosed with SCD at birth was reported in this study. She exhibited scoliosis and thoracic asymmetry, along with a left-sided bilateral breast deformity. Imaging analysis revealed congenital scoliosis with a lack of seg

Dorsal 3D-printed plate stabilisation of atlantoaxial instability associated with atlanto-occipital overlap in three dogs.

Atlantoaxial instability is a common condition in toy breed dogs, sometimes associated with occipital dysplasia. Surgical stabilisation is challenging due to the small bone size and proximity to the spinal cord. This case report describes three dogs treated with a custom 3D-printed dorsal plate for atlantoaxial instability, considering the presence of atlanto-occipital overlap. Each patient underwent pre- and postoperative computed tomography, and follow-up was obtained via telephone for the surviving dogs. The use of a dorsal 3D-printed plate appears to offer a feasible and clinically accepta

Imaging of Craniovertebral Junction Instability, Fixation, and Stenosis in Children.

Craniovertebral junction (CVJ) instability, fixation, and stenosis in children are closely related conditions that are often challenging to diagnose and are associated with significant morbidity. Groups at higher risk for CVJ abnormalities include children with trisomy 21, juvenile idiopathic arthritis, upper respiratory infection or other inflammatory conditions of the head and neck, and certain skeletal dysplasias. Radiography, CT, and MRI play complementary roles in the evaluation of pathologic conditions of the CVJ. CVJ morphometry is helpful in characterizing osseous relationships and sug

Myelomeningocele in a newborn with VACTERL association.

VACTERL association refers to the concomitant occurrence of congenital malformations such as vertebral defects, anorectal malformations, cardiac anomalies, tracheoesophageal fistula, renal anomalies, and limb defects. This case describes a newborn girl who presented with anomalies compatible with this association, such as scoliosis, sacral dysgenesis, right lower phocomelia, and multicystic renal dysplasia, also associated with myelomeningocele, which was surprising as it occurred concomitantly with the other congenital malformations. Although neural tube defects are not part of the classic cr

Pathologies of the cervical spine in skeletal syndromes and dysplasias.

Skeletal syndromes and dysplasias include more than 150 entities, most often of genetic origin. Some of them cause abnormalities in the cervical spine, with or without instability, distortion or compression of the spinal cord. These abnormalities must be detected and treated if necessary because they can have serious consequences such as quadriplegia. Up to 30% of patients with Down syndrome are affected by occipitocervical or atlantoaxial instability. Dynamic cervical spine radiographs are the most common screening tool. Mucopolysaccharidoses (MPS) are a group of inherited lysosomal storage d

Cervicothoracic Kyphosis and Spinal Cord Compression in Hurler Syndrome.

INTRODUCTION: Hurler syndrome is characterized by significant spinal abnormalities, including cervical instability, odontoid dysplasia, and spinal cord compression. Spinal compression and kyphosis at the cervicothoracic (C-T) junction are also observed but less commonly reported. The present study was conducted to determine the prevalence of C-T kyphosis and spinal cord compression at the C-T junction in patients with Hurler syndrome. In addition, we aimed to evaluate the significance of C-T kyphosis and spinal cord compression in these patients in terms of clinical symptoms or neuromonitoring

Brain tissue microstructure in a prospective, longitudinal, population-based cohort of preterm and term-born young adults.

BACKGROUND: Fifteen million infants annually are born prematurely, placing them at high risk for life-long adverse neurodevelopmental outcomes. Whether brain tissue abnormalities that accompany preterm birth persist into young adulthood and are associated with long-term cognitive or psychiatric outcomes is not known. METHODS: From infancy into young adulthood, we followed a population-based sample of consecutively identified preterm infants and their matched term controls. The preterm group was born at an average gestational age of 31.5 ± 2.6 weeks. We obtained Diffusion Tensor Imaging scans

Foetal achondroplasia: Prenatal diagnosis, outcome and perspectives.

BACKGROUND: Achondroplasia, due to a specific pathogenic variant in FGFR3, is the most common viable skeletal dysplasia and the diagnosis is mostly done in the prenatal period. Since 2021, the use of Vosoritide, a specific treatment for achondroplasia, validated in phase 3 placebo-controlled trials, has been recommended to significantly increase the height of children and infants. In the light of these new therapeutic prospects, a complete understanding of the pathophysiology of skeletal damages occurring from foetal life is required. OBJECTIVES: To describe foetal imaging and the antenatal a

Cervical spinal decompression and fusion in the setting of Wolcott-Rallison Syndrome: a rare pediatric indication and its surgical considerations.

Wolcott-Rallison Syndrome is an extremely rare syndrome characterized by infantile non-autoimmune diabetes, extensive skeletal dysplasia, and multi-organ failure requiring transplant. Prognosis is very poor, and as such, surgical intervention for symptomatic cervical spine compromise in pediatric patients has not been widely reported in part due to their high fragility. We report a complex case of Wolcott-Rallison Syndrome that presented with cervical myelopathy due to cervicomedullary compression and the exceptional surgical considerations required for successful intervention.

Vertebral and spinal malformations in small brachycephalic dog breeds: Current knowledge and remaining questions.

Small brachycephalic dog breeds, such as the French bulldog, English bulldog and pug have become increasingly popular. These breeds are predisposed to a variety of vertebral and spinal malformations, including hemivertebra, caudal articular process dysplasia, transitional vertebra, cranial thoracic vertebral canal stenosis, spinal arachnoid diverticulum and meningeal fibrosis. Recent studies have provided new insights into the prevalence, anatomical characteristics, pathophysiology and treatment of these conditions. Thoracic hemivertebra, caudal articular process dysplasia, transitional verteb

Pseudoachondroplasia associated with os odontoideum and retro-odontoid mass: case-based update.

PURPOSE: Pseudoachondroplasia is a rare skeletal dysplasia caused by a mutation in the COMP gene. Infants with pseudoachondroplasia present with rhizomelic dwarfism. Pseudoachondroplasia can resemble achondroplasia, which also presents with a phenotype of rhizomelic dwarfism. The differentiation between these two conditions is important because while both can present with cervicomedullary compromise, the underlying pathophysiology and its management is entirely different. RESULTS: We present a case of a 13-year-old female patient with pseudoachondroplasia who presented with cervical myelopath

A pictorial review of the radiographic skeletal findings in Morquio syndrome (mucopolysaccharidosis type IV).

Morquio syndrome, also known as Morquio-Brailsford syndrome or mucopolysaccharidosis type IV (MPS IV), is a subgroup of mucopolysaccharidosis. It is an autosomal recessive lysosomal storage disorder. Two subtypes of Morquio syndrome have been identified. In MPS IVA, a deficiency in N-acetylgalactosamine-6-sulfate sulfatase interrupts the normal metabolic pathway of degrading glycosaminoglycans. Accumulated undigested glycosaminoglycans in the tissue and bones result in complications leading to severe skeletal deformity. In MPS IVB, a deficiency in beta-galactosidase results in a milder phenoty
