Clinical and MRI morphometric characterization of symptomatic cerebellar tonsillar ectopia.

BACKGROUND: Cerebellar tonsils normally extend up to 5 mm below the foramen magnum (FM). Some individuals with tonsillar ectopia (TE) measuring under 5 mm may present with symptoms of Chiari type 1 malformation (CM1). We analyzed MRI data of the posterior cranial fossa (PCF) in affected patients to assess TE's impact on clinical manifestations. METHODS: The retrospective study examined 83 adults with 2-4 mm symptomatic cerebellar TE without syringomyelia (SM), 16 with syndromic CM1 without SM, and 19 controls. Participants with TE were divided into three groups: syndromic (TEs, n = 15), incom

Mosaicism for Cohen-Gibson Syndrome in a Female Individual With Overgrowth and Neurodevelopmental Delay.

BACKGROUND: The Embryonic Ectoderm Development (EED) gene encodes a core component of the Polycomb Repressive Complex 2 (PRC2), which regulates chromatin structure and plays a key role in embryonic development and cell differentiation. Pathogenic variants in EED are associated with overgrowth-intellectual disability syndrome (Cohen-Gibson syndrome, OMIM #617561), though reported cases are rare and typically involve heterozygous germline variants. This study presents an individual with confirmed postzygotic mosaicism in EED, contributing to diagnostic complexity. METHODS: A 12-year-old girl wi

Variants in CEP135 Cause Congenital Microcephaly and Primary Ciliary Dyskinesia.

INTRODUCTION: Primary ciliary dyskinesia (PCD), a disorder of motile ciliary dysfunction causing chronic respiratory infections, may also rarely present with aspects of non-motile ciliary dysfunction, including retinitis, central nervous system malformations, skeletal dysplasia, and sensorineural hearing loss. However, congenital microcephaly has not been previously associated with PCD. METHODS: We identified three children with congenital microcephaly and chronic respiratory disease consistent with PCD. Comprehensive PCD diagnostic testing and whole exome sequencing were performed. Character

Advances in PMP22 sequence variant-associated hereditary neuropathy with liability to pressure palsies: from gene dosage effects to mechanism-informed variant interpretation.

Hereditary neuropathy with liability to pressure palsies (HNPP) is an autosomal dominant neuropathy classically caused by a heterozygous deletion of the 17p11.2-p12 region encompassing PMP22. However, PMP22 sequence variants can produce HNPP-like or overlapping phenotypes, complicating variant interpretation. This narrative review synthesizes clinical, genetic, functional, and experimental evidence relevant to PMP22 sequence variant-associated HNPP. PMP22 sequence variants may cause quantitative loss of functional protein or qualitative impairment through defective folding, trafficking, membra

Dose-dependent neurotoxic effects of monosodium glutamate on neural tube development in chicken embryos.

BACKGROUND: Considering the excitatory neurotransmitter role of glutamate in the central nervous system and the widespread use of monosodium glutamate (MSG) as a food additive, this study aimed to evaluate the effects of MSG on neural tube defect formation in chicken embryos. MATERIALS AND METHODS: In this study, fertile, specific pathogen free eggs were used. Dosage calculation was based on the provisional tolerable intake of MSG of 30 mg/kg body weight per day reported in the literature. Doses were calculated as Group A: saline, Group B: 30 mg/kg, Group C: 60 mg/kg. Monosodium glutamate was

Targeting KIT prevents brain arteriovenous malformations driven by ALK1-deficient angiogenic endothelial cells.

Hereditary hemorrhagic telangiectasia type 2 (HHT2), caused by mutations in ACVRL1 (also known as ALK1), is characterized by brain arteriovenous malformations (bAVMs), abnormal artery-vein connections for which treatment options remain limited. Despite evidence of endothelial cell (EC) heterogeneity, its role in bAVM pathogenesis remains poorly defined. Using EC-specific inducible Alk1-knockout mice (Alk1iECKO) and regionally resolved single-cell RNA sequencing, we showed that loss of ALK1 signaling induces bAVMs predominantly in the perineural vascular plexus. This process is driven by the em

Hereditary Spastic Paraplegia Type 11: A Comprehensive Case Report.

Hereditary spastic paraplegia (HSP) is a heterogeneous group of genetically determined diseases, characterized by progressive spastic paraparesis of the lower limbs, associated with degeneration of the corticospinal tract and the posterior column of the spinal cord. Currently known to be about 70 genes are involved in HSP, though new potentially pathogenic variants are being reported regularly. Almost all patterns of inheritance like autosomal dominant, autosomal recessive, X-linked, and mitochondrial have been found in families of HSP patients. However, autosomal recessive form, hereditary sp

Burden and social context of caregivers of children and adolescents with Charcot-Marie-Tooth disease.

BACKGROUND: In pediatric Charcot-Marie-Tooth (CMT) disease, caregivers play an important role in coordinating care and promoting child well-being. Therefore, understanding caregiver burden and social context is key for treatment planning. OBJECTIVE: To investigate caregiver burden, family income, and level of schooling among caregivers of children and adolescents with CMT disease, and to examine correlations between caregiver burden, patient age, clinical impairment, and caregiver age. METHODS: A cross-sectional study was conducted with 58 patients (aged 3-18 years) and their primary caregiv

Hemorrhagic sellar lesion mimicking pituitary adenoma apoplexy: MRI follow-up suggesting Rathke cleft cyst.

Rathke cleft cysts (RCCs) are benign epithelial sellar and suprasellar lesions arising from remnants of Rathke pouch. Although typically incidental and asymptomatic, intracystic hemorrhage is rare and may mimic pituitary adenoma apoplexy. We report a 19-year-old woman presenting with one week of progressive severe headache. CT demonstrated sellar enlargement with suprasellar extension. MRI showed a hemorrhagic sellar lesion centered in the pars intermedia/posterior pituitary with fluid-fluid levels, dependent T2 hypointense components, susceptibility-related signal loss, and suprasellar extens

Genome-wide CRISPRi screen in human iNeurons identifies novel negative mTOR regulator genes associated with focal cortical dysplasia.

Focal cortical dysplasia (FCD) is a common cause of focal epilepsy that typically results from brain mosaic mutations in the mTOR cell signaling pathway. To identify new potential FCD genes, we developed an in vitro CRISPRi screen in human neurons and used FACS enrichment based on the FCD biomarker, phosphorylated S6 ribosomal protein (pS6). Using whole-genome (110,000 gRNAs) and candidate (129 gRNAs) libraries, we discovered 6 new genes in which loss of function significantly increases pS6 levels: LRRC4, EIF3A, TSN, HIP1, PIK3R3, and URI1. Further analysis of the mTOR pathway showed that only

The role of nimodipine in preventing cerebral ischaemia and neurological deterioration in cerebral arteriovenous malformations: a systematic review.

INTRODUCTION: Cerebral arteriovenous malformations (cAVMs) are congenital cerebrovascular anomalies that are associated with a high risk of intracerebral bleeding, which may be exacerbated by cerebral vasospasm (CVS). Despite advances in treatment modalities such as microsurgical resection, stereotactic radiosurgery, and endovascular embolization, CVS remains a significant concern, often precipitating significant neurological morbidity and mortality. Nimodipine, a calcium channel blocker, is widely used in aneurysmal subarachnoid haemorrhage (aSAH) to reduce neurological deficits. However, its

Fetal Structural Anomalies and Vasa Previa Newly Identified After an Unremarkable Mid-Trimester Systematic Ultrasound Screening: Clinical Significance and Follow-Up Findings in 47 Cases.

BACKGROUND: Some fetal structural anomalies and cases of vasa previa may not be detectable until after mid-trimester ultrasound screening. This study evaluated the spectrum, sonographic features, and perinatal outcomes of these newly detected abnormalities. METHODS: We retrospectively reviewed 14 579 singleton pregnancies that underwent standardized mid-trimester ultrasound screening with no structural abnormalities detected at that time. Among these, 47 fetuses had anomalies newly identified before delivery. One additional pregnancy, in which multiple malformations were already detected at m

Oropouche virus causes severe congenital disease in embryonic mice.

The recent outbreak of Oropouche virus (OROV) in Latin-America with unprecedented reports of vertical transmission resulting in congenital malformations and fetal death, redefines the threat posed by this arbovirus. We discover that injection of a low inoculum of the epidemic OROVIRCCS-SCDC_1/2024 strain into the placenta of Swiss mice results in viral replication across embryonic organs, in particular the brain. Infected embryos present with multi-organ pathology, pneumonitis, myocarditis, steatohepatitis and severe neuropathology, including microcephaly, ventriculomegaly and neural loss. Neu
