Cell division cycle 42 binding protein beta as a plasma-based biomarker for cerebral cavernous malformations.

OBJECTIVE: Cerebral cavernous malformations (CCMs) are groups of blood vessels that develop abnormally in both the brain and/or spinal cord. Currently, MRI and/or CT are the primary methods for assessing CCMs. Plasma-based biomarkers could serve as a complement to standard imaging techniques by providing a quantitative and molecular-based technique to detect disease at lower cost. Therefore, the authors evaluated cell division cycle 42 binding protein beta (CDC42BPB) as a potential novel plasma biomarker for CCMs. METHODS: Plasma samples were obtained from patients with pathological analysis-

Radiation-induced intracranial neoplasms after stereotactic radiosurgery for brain arteriovenous malformations: a retrospective multicenter cohort study.

OBJECTIVE: Stereotactic radiosurgery (SRS) is a well-established treatment for brain arteriovenous malformations (bAVMs). However, data on radiation-induced neoplasms (RINs) following SRS in patients with bAVMs remain limited. In this study, authors aimed to evaluate the incidence of newly developed intracranial RINs following SRS for bAVMs. METHODS: The authors retrospectively analyzed anonymized data for patients who underwent SRS for bAVMs between 1990 and 2019 at 18 Gamma Knife centers in Japan. Patients with fewer than 3 years of radiological follow-up, prior radiotherapy, or genetic tum

Spectrum of dominant Charcot-Marie-Tooth disease due to SLC12A6 variants.

BACKGROUND: Heterozygous variants in SLC12A6 have recently been shown to cause dominant Charcot-Marie-Tooth disease (CMT). We aim to characterise the phenotype of patients with previously reported and novel heterozygous variants in the gene and understand any genotype-phenotype correlation. METHODS: Patients were clinically and genetically assessed in sites from Europe, Australia, Brazil and the USA. All patients underwent whole exome or whole genome sequencing. Variants were classified using American College of Medical Genetics and Genomics criteria. RESULTS: Twenty-three individuals from 1

A qualitative study of enablers and barriers to healthy eating in adults with Charcot Marie Tooth disease using the Theoretical Domains Framework and COM-B model.

Charcot-Marie-Tooth disease (CMT) is a progressive neurological disorder affecting the peripheral nervous system, with symptoms such as fatigue, pain and muscle weakness that can hinder engagement in health-promoting behaviours, including healthy eating. This study explored enablers and barriers to healthy eating among adults with CMT, using the Theoretical Domains Framework (TDF) and the COM-B model. Twenty-two semi-structured interviews were conducted with 17 women and 5 men (aged 25-73). Data were analysed using framework analysis. Identified barriers included symptom-related fatigue, parti

Genetic commonalities between rare subtypes of ALS and CMT: insights into molecular mechanisms of neurodegeneration.

Amyotrophic lateral sclerosis (ALS) and Charcot-Marie-Tooth disease (CMT) are two distinct neurodegenerative disorders. While ALS is characterised by rapidly progressive motor neuron degeneration, leading to severe complications and death, CMT as a peripheral neuropathy is less severe, and patients have a longer life span, although with a compromised quality of life. Despite their clinical differences, current knowledge suggests that familial ALS (fALS) and CMT may share common genetic and molecular mechanisms. We aimed to identify shared genes mutations and molecular pathways between fALS and

Seizure control after radiosurgical treatment in patients with cerebral arteriovenous malformations: An observational study.

OBJECTIVE: Seizures are among the most common symptoms of cerebral arteriovenous malformations (AVMs). Although the main goal of AVM treatment remains complete obliteration, seizure control has become an important treatment aspect in recent years. Thus, we analyzed seizure control following Gamma Knife Radiosurgery (GKRS) treatment at our department in patients with AVMs. METHODS: We analyzed 130 patients with AVMs who had at least one seizure before treatment with GKRS at our tertiary referral center between 1992 and 2022. We analyzed predictors for treatment success, including AVM size, Vir

Deep characterization of refractory epilepsy due to mild malformation of cortical development with oligodendroglial hyperplasia (MOGHE) and insights into the role of invasive monitoring.

OBJECTIVE: Epilepsy surgery is an effective treatment option for patients with medically refractory epilepsy due to mild malformation of cortical development with oligodendroglial hyperplasia (MOGHE). The success of surgery depends on the accurate localization of the epileptogenic zone, which can be challenging due to the subtle imaging features. The aim of this project was to provide an in-depth electro-clinical characterization of MOGHE in patients with medically intractable epilepsy, and to assess the role of stereo-electroencephalography (SEEG) in tailoring the resection and optimizing sur

Patient-derived neural organoids reveal developmental impairments associated with a novel GJB1 mutation in X-linked Charcot-Marie-Tooth disease.

Charcot-Marie-Tooth disease (CMT) is one of the most prevalent inherited peripheral neuropathies. CMT type X1 (CMTX1), caused by mutations in the GJB1 gene, represents the most common X-linked subtype with central nervous system (CNS) involvement. Here, we report the identification and functional characterization of a novel GJB1 variant (c.554C > T, p.Thr185Ile) in a CMTX1-affected family and its pathogenic impact using patient-derived induced pluripotent stem cells (iPSCs) and three-dimensional (3D) neural organoid models. The GJB1 gene encodes connexin 32 (Cx32), a gap junction protein. Immu

Characteristics, pathophysiology, and management of Galassi III arachnoid cysts: a scoping review protocol.

OBJECTIVE: This scoping review aims to map the existing evidence on Galassi III arachnoid cysts and identify knowledge gaps in the current literature. INTRODUCTION: Galassi III arachnoid cysts are rare intracranial lesions characterized by significant midline displacement and associated with space-occupying effects. Despite their rarity, they have higher intervention rates than other types, yet optimal management remains unclear. Understanding their prevalence, pathophysiology, clinical characteristics, natural history, management strategies, and outcomes is critical for enhancing research on

Flow-sensitive black blood sequence in evaluating impairment of cerebral venous drainage in patients with intracranial dural arteriovenous fistulas.

BACKGROUND AND PURPOSE: Intracranial dural arteriovenous fistulas (DAVFs) sometimes cause impaired cerebral venous drainage (ICVD), leading to severe neurological outcomes. While digital subtraction angiography (DSA) is the diagnostic gold standard, the assessment of ICVD through the “pseudophlebitic pattern (PPP)” can be subjective and difficult. This study aimed to evaluate the utility of the flow-sensitive black-blood (FSBB) MRI sequence for detecting ICVD in DAVF patients by comparing it with findings from cerebral angiography. MATERIALS AND METHODS: We retrospectively analyzed 25 patients

Transfistulous coil occlusion of high-grade tentorial dural arteriovenous fistulas: An effective alternative to liquid embolic agents.

Endovascular therapy (EVT) of dural arteriovenous fistulas (DAVF) includes transarterial and transvenous techniques, or EVT combined with surgery. The transfistulous approach (TFA) allows occlusion of the proximal venous outflow via transarterial catheterization through the AV shunt. We report five cases of tentorial DAVF treated using this technique, with complete anatomical occlusion in 4 patients, and near-complete occlusion in one. Clinical cure was achieved in 4 patients; one patient with advanced dementia showed no change in symptoms. No complications were observed. The TFA is safe and h

Patient journey with Charcot-Marie-Tooth Disease - A German patient survey study.

BACKGROUND: Charcot-Marie-Tooth (CMT) and related disorders represent one of the largest groups of inherited neurological disorders. Long considered incurable, the first disease-modifying treatments are currently being evaluated in clinical trials. However, frequent misdiagnosis or delayed recognition are common, hindering timely treatment necessary to prevent permanent disability. In this study, we conducted a survey among patients from our CMT clinic and the German patient advocate group to capture patients’ perspective on the diagnostic journey and to explore solutions for earlier diagnosis

EXOSC10 haploinsufficiency causes primary microcephaly by derepression of Sonic hedgehog signalling.

The evolutionarily conserved RNA exosome complex modulates gene expression during development. Mutations in RNA exosome complex subunits have been implicated in various human brain disorders, suggesting that defects in RNA decay are linked to impaired neural development. In our study, we identified de novo variants of EXOSC10 in microcephalic individuals. The patient's phenotype can be replicated by heterozygous conditional knockout of Exosc10 in the developing mouse forebrain. The heterozygous loss of Exosc10 in the developing mouse cortex leads to increased cell cycle exit, with a premature

Microsurgical Treatment of Ethmoidal Dural Arteriovenous Fistula.

A 78-year-old male patient presented with an episode of confusion. Computed tomography angiography (CTA) revealed an abnormal collection of vessels in the basal frontal region, and diagnostic cerebral angiogram revealed an ethmoidal dural arteriovenous fistula (dAVF). dAVFs are rare intracranial vascular lesions, comprising approximately 2%-3% of all dAVFs.1-3 These fistulas are primarily supplied by the ethmoidal branches of the ophthalmic artery.1-3 A high-grade ethmoidal dural arteriovenous fistula characterized by cortical venous drainage or associated symptoms requires treatment.4 Interve
