Lash Ptosis and Entropion after Congenital Ptosis Surgery: Mechanisms and Treatment.

PURPOSE: This perspective proposes a clinically useful framework for understanding upper eyelid lash ptosis and entropion after congenital ptosis surgery, linking potential mechanisms to prevention and management. METHODS: A focused literature review was performed. RESULTS: Postoperative lash ptosis and entropion likely reflect the differential interplay of multiple biomechanical factors: lamellar imbalance, suboptimal fixation geometry, tarsal buckling, excessive pretarsal dissection, and patient-specific predisposition, rather than a single technical error. Understanding these mechanisms m

Early Expanded Polytetrafluoroethylene Sheet Removal Due to Postoperative Infection in Frontalis Suspension Surgery Preserves Eyelid Elevation and Curvature.

In frontalis muscle suspension surgery for treating congenital ptosis, the use of expanded polytetrafluoroethylene (ePTFE) sheets often yields stable postoperative outcomes; however, it also carries the risks of foreign body reactions and infection, with few reports describing the detailed course of treatment in such cases. A 15-year-old girl was referred for management of right congenital ptosis. Preoperative margin reflex distance-1 (MRD-1) measurements were 1.5 mm on the right and 4.5 mm on the left, with corresponding levator function of 5 mm and 14 mm on the right and left, respectively.

Correction of Congenital Ptosis With Preservation of Single-Eyelid Morphology: A Modified Palpebral Margin Incision Technique.

BACKGROUND: Conventional anterior ptosis correction typically creates an iatrogenic double eyelid, which may be undesirable for patients who wish to preserve single-eyelid morphology. It may also leave a conspicuous scar. This study evaluates a modified eyelid-margin incision technique to correct ptosis while concealing the scar and maintaining a single-eyelid appearance. METHODS: The authors retrospectively reviewed 21 patients (33 eyes) with congenital ptosis who underwent ptosis correction by a palpebral margin incision. Preoperative and postoperative margin reflex distance 1 (MRD1) and pa

From blink to care: smartphone video-based functional analysis and personalized management in pediatric blepharoptosis.

Early detection of congenital ptosis is critical to prevent visual and psychosocial impairment in children, yet clinical assessment is challenged by limited patient cooperation and specialist availability. In this prospective, multicenter study, we developed and validated a smartphone-based system comprising three modules: morphological assessment, functional analysis, and a domain-adapted dialogue model, using 3164 blink clips and 1,229 facial images. The morphological module showed high measurement accuracy with intraclass correlation coefficients over 0.90 versus manual assessments. The fun

Histopathological Characteristics of the Levator Muscle in Congenital and Acquired Ptosis With Varying Levator Function.

PURPOSE: Levator function (LF) is a key parameter in the evaluation and surgical planning of blepharoptosis; however, it does not always correlate with clinical severity or outcomes. This study aimed to compare the histopathological characteristics of the levator palpebrae superioris (LPS) muscle in congenital and acquired (involutional) ptosis with different LF levels. METHODS: This retrospective study included 38 eyelids from patients with congenital or acquired ptosis who underwent anterior levator surgery. Patients were categorized into 3 groups based on LF: congenital ptosis with LF >5 m

Modified Müller's muscle conjunctival resection combined with levator plication in moderate to severe congenital ptosis with poor levator function.

To present a modified surgical technique for the correction of moderate to severe congenital ptosis with poor levator function. This prospective case series included 34 eyes from 34 patients with unilateral moderate to severe congenital ptosis, defined as marginal reflex distance-1 (MRD-1, the distance from the corneal light reflex to the upper eyelid margin in primary gaze) ≤ 2 mm and levator function (LF) ≤ 5 mm. The surgical approach combined Müller's muscle conjunctival resection with an extended range (12-17 mm) and levator plication. An "ideal" response was defined as a postoperative int
