Endovascular stenting of bilateral hypoplastic jugular bulb in a toddler with elevated intracranial pressure: a case report.

We report a 3-year-old boy with macrocephaly, papilledema, and headaches associated with bilateral hypoplastic jugular bulbs leading to intracranial hypertension. Initial imaging was misinterpreted as idiopathic intracranial hypertension, and medical therapy with acetazolamide failed. Repeat MRI/MRV demonstrated thrombosis of the right transverse sinus with orbital collateral drainage. Angiography confirmed occlusion of the right jugular bulb and severe stenosis of the left. Due to progressive symptoms and developmental risk, rescue endovascular stenting of the right internal jugular vein was

Congenital cystic eye and coloboma with a cyst hybrid phenotype: a developmental continuum.

A 2-day-old boy presented with a large orbital mass, which showed radiological and pathological overlap between congenital cystic eye (CCE) and coloboma with cyst (CwC). Initially identified on prenatal imaging at 21 weeks, the patient underwent postnatal clinical and radiologic evaluation followed by histopathologic analysis of the enucleated eye. The coexistence of both anomalies within a single globe challenges the traditional view of CCE and CwC as distinct entities and supports a unifying theory of a developmental spectrum of early ocular malformations.

Cavernous venous malformation complicated by a secondary spontaneous arteriovenous fistula of the eyelid.

Periorbital and orbital cavernous venous malformations (CVMs) are low-flow vascular malformations, whereas arteriovenous fistulas (AVFs) represent high-flow vascular malformations. The authors report a case of a 55-year-old female with no history of prior trauma, presenting with a chronic, progressive, painless, pulsatile left upper eyelid mass. Ophthalmic examination revealed a palpable thrill, and orbital imaging suggested a vascular lesion associated with an arteriovenous fistula (AVF) located in the left upper eyelid. Diagnostic cerebral angiography confirmed a composite lesion comprising

Carotid cavernous fistula in a young adult with Bannayan-Riley-Ruvalcaba syndrome.

Direct carotid-cavernous fistulas (CCFs) are an uncommon but recognized vascular complication of inherited connective tissue disorders. We report a direct CCF in a 22-year-old patient with Bannayan-Riley-Ruvalcaba syndrome (BRRS), a rare disorder within the phosphatase and tensin homolog (PTEN) hamartoma tumor syndrome (PHTS) spectrum. The patient, diagnosed with BRRS at age two based on the classic triad of macrocephaly, abdominal lipomas, and genital lentiginous macules, presented with acute unilateral eyelid swelling and chemosis in the absence of trauma, visual deficits, or afferent pupill

First Successful Le Fort III Distraction in a Patient With Beare-Stevenson Cutis Gyrata Syndrome.

OBJECTIVE: Beare-Stevenson cutis gyrata syndrome (BSS) is a genetic disorder caused by mutations in the FGFR2 gene. It is characterized by craniosynostosis, midface hypoplasia, Chiari malformations, and other extensive congenital abnormalities. We performed a Le Fort III distraction procedure on a girl with this condition and achieved favorable outcomes. To our knowledge, this report describes the world's first successful Le Fort III distraction osteogenesis occurring in a patient with BSS. METHODS: The patient was a 5-year-old girl who was diagnosed with BSS shortly after birth and underwent

A Rare Case Report on Holoprosencephaly With Cyclopia: Jimma University Medical Center, Ethiopia.

BACKGROUND: Holoprosencephaly is a congenital brain malformation resulting from incomplete division of the forebrain (prosencephalon) into two hemispheres during early embryonic development, typically during the 4th-6th week of gestation. Cyclopia is a rare and fatal form of holoprosencephaly characterized by severe craniofacial abnormalities resulting from incomplete cleavage of the embryonic prosencephalon, leading to the failure of separation of the orbital cavities. The prevalence is 1.31 per 10,000 for live births and 1:250 for spontaneous abortions. CASE PRESENTATION: This is a case rep

Pediatric growing orbital roof fracture with ptosis: a case report.

The majority of pediatric skull fractures are minimally displaced and can usually be managed conservatively. A rare but serious complication is the development of a growing skull fracture (GSF), that may lead to significant sequelae including orbital meningoencephalocele or neurological impairments. We report a 6-year-old boy who initially presented with right-sided traumatic ptosis and a minimally displaced right orbital roof "blow-up" fracture following minor head trauma. This injury subsequently progressed into a growing orbital roof fracture, complicated by the development of an orbital me

En-bloc posterior vault distraction including the foramen magnum for Chiari I malformation in severe syndromic craniosynostosis: First cases and literature review.

This case series describes two patients with severe Crouzon syndrome and persistent Chiari I malformation. Initial treatment with standard posterior vault distraction osteogenesis and frontoorbital advancement failed to resolve the patient's symptoms. One patient developed severe syringomyelia and myelopathy refractory even to standard suboccipital decompression. Given the persistent posterior fossa constriction, an en-bloc posterior vault distraction incorporating the foramen magnum was performed over 21 days, achieving an expansion of 20 mm. This approach resulted in complete symptom resolut

Isolated frontosphenoidal suture synostoses: report of two cases and literature review.

INTRODUCTION: Anterior plagiocephaly is most commonly attributed to unilateral coronal synostosis. However, isolated premature fusion of the frontosphenoidal suture (FSC) represents a rare and often misdiagnosed cause due to its overlapping clinical presentation. Accurate diagnosis is essential, as treatment strategies and prognoses differ significantly. OBJECTIVES: To present a comprehensive literature review of FSC and to report two novel clinical cases-one managed with open fronto-orbital advancement and the other with endoscopic suturectomy. METHODS: A systematic review of reported FSC c

Engineering of genetically encoded programmable calcium channel inhibitory binders.

Store-operated Ca2+ release-activated Ca2+ (CRAC) channels, composed of STIM and ORAI, are essential for immune and developmental processes, and their dysregulation underlies channelopathies such as Stormorken syndrome. Here, we report the engineering of genetically encoded CRAC channel inhibitory binders (CRABs) derived from the ORAI C-terminal tail. Guided by deep mutational scanning, we optimize a membrane-anchored CRAB variant that potently inhibits Ca2+ influx and NFAT signaling, and rescues thrombocytopenia-like phenotypes in a zebrafish model of Stormorken syndrome. To enable tunable in

Public Insurance Is Associated With Increased Emergency Visits After Craniosynostosis Surgery.

INTRODUCTION: Postoperative emergency department (ED) visits are common across various procedures, often due to surgical complications, pain, and patient uncertainty about normal surgical recovery. While issues such as infections or dehiscence require acute medical management in the ED setting, many visit reasons are driven by manageable postoperative symptoms like fever or swelling. In this study, we aim to identify the most common reasons for acute care visits within 30 d following craniosynostosis surgery, as to inform postoperative care and address health-care utilization. METHODS: A retr

Comparative Outcomes of Fronto-Orbital Advancement and Strip Craniectomy in the Management of Metopic Craniosynostosis.

OBJECTIVE: Metopic craniosynostosis results in characteristic trigonocephaly and may impair cranial growth. Surgical correction is traditionally performed using fronto-orbital advancement (FOAR), while strip craniectomy with postoperative helmet therapy (SC) has emerged as a minimally invasive alternative. This study compares perioperative outcomes and postoperative cranial shape between FOAR and SC. METHODS: A retrospective cohort study was conducted at a tertiary pediatric craniofacial unit. Patients with isolated metopic craniosynostosis treated with FOAR or SC between 2015 and 2024 were i

Severe Plagiocephaly Due to Isolated Left Frontosphenoidal Craniosynostosis: An Exceptional Presentation.

Craniosynostosis is a condition characterized by premature fusion of cranial sutures, leading to abnormal head shapes and potential developmental and neurological complications. Although craniosynostosis typically involves major cranial sutures, isolated frontosphenoidal suture fusion represents an exceptionally rare entity. In this case report, we present a 2-year-old girl with a rare occurrence of early closure of the left frontosphenoidal suture, resulting in significant left-sided plagiocephaly. After frontal-orbital advancement surgery with barrel-stave osteotomies, the patient experience

Unilateral Versus Bilateral Fronto-Orbital Advancement in Non-Syndromic Unicoronal Craniosynostosis: A Systematic Review and Pooled Data Analysis.

BACKGROUND: Nonsyndromic unicoronal craniosynostosis (NS-UCS) is characterized by asymmetric craniofacial deformity and remains one of the most challenging entities in craniofacial surgery. Fronto-orbital advancement (FOAR) may be performed either unilaterally or bilaterally, yet the optimal approach remains controversial. The present study aims to compare surgical outcomes, complication profiles, and long-term stability between unilateral and bilateral FOAR to clarify selection criteria for surgical technique. METHODS: A systematic review was conducted using the PubMed, Web of Science, and C

Advantages of the double-incision approach in endoscopic management of unicoronal synostosis.

Endoscopic strip craniectomy followed by postoperative helmet therapy has become a well-established treatment for single-suture craniosynostosis. Several incision patterns have been described in the medical literature. In this study, we describe a double-incision endoscopic strip craniectomy technique for unicoronal synostosis and discuss its technical pearls and potential advantages. Our endoscopic strip craniectomy technique for unicoronal synostosis uses two incisions. Patients typically present ipsilateral forehead flattening, contralateral frontal bossing, and harlequin eye deformity, wit

Pre- and postoperative cranial shape analysis in Craniosynostosis: The role of mechanical loading during CT imaging.

BACKGROUND: Craniosynostosis is a congenital disorder characterized by premature fusion of one or more cranial sutures, resulting in abnormal skull growth and, in some cases, elevated intracranial pressure. Accurate assessment of cranial morphology is essential for diagnosis, surgical planning, and evaluation of postoperative outcomes. Three-dimensional computed tomography allows detailed visualization of cranial shape; however, measurements may be influenced by external forces during image acquisition, such as contact between the occipital region and the supporting surface. It remains unclear
