Horizontal Palpebral Fissure Best Predicts Subjective Facial Asymmetry in Unilateral Anophthalmia/Microphthalmia.

PURPOSE: Congenital anophthalmia/microphthalmia are developmental eye disorders with variable severity. The absence of a normal-sized eye can cause facial asymmetry. Outcome is often reported as relative horizontal palpebral fissure (rHPF) and/or orbital dimensions. Yet, the predictive value of these measurements is unknown. This study aims to test the relation between facial and ultrasound measurements and subjective outcomes. METHODS: In this retrospective study, 3D facial scans of 31 patients with unilateral an/microphthalmia were analyzed. The scans were obtained using the Vectra scanner,

Compound genetic burden in oculo-facio-cardio-dental (OFCD) syndrome: surgical risk stratification with co-occurring BCOR and MYLK mutations.

INTRODUCTION: Oculo-facio-cardio-dental (OFCD) syndrome is a rare X-linked dominant disorder caused by pathogenic BCOR variants and characterized by congenital cataracts, microcornea, and secondary glaucoma. Multilocus pathogenic variation (MPV), in which independent variants contribute to disease burden, can further complicate syndromic presentations and alter surgical planning. This case highlights the importance of longitudinal genetic evaluation in complex ophthalmic disease. METHODS: Comprehensive ophthalmic evaluation, multimodal imaging, and systemic assessment were performed. Trio exo

OTX2 Syndromes.

Patients with OTX2 mutations have highly variable phenotypes, as the transcription factor is key in the development of retinal, pituitary gland, inner ear, and craniofacial structures. There are two recognized syndromes caused by OTX2 mutations: syndromic microphthalmia type 5 (MCOPS5), and combined pituitary hormone deficiency type 6 (CPHD6).

XEN 63 in a case of refractory glaucoma secondary to microphthalmia and aphakia.

Microphthalmia (MO) predisposes to glaucoma and congenital cataracts (CC). Cataract removal in MO is characterized by several complications such as aphakic glaucoma (AG). We report the case of a 33-year-old male with MO, aphakia, and refractory glaucoma who underwent inferonasal ab-interno XEN 63 implantation, after posterior vitrectomy. Whole Exome Sequencing (WES) analysis was performed identifying the likely pathogenic variant c.197G>A (p.Cys66Tyr) in heterozygosity in the PAX6 gene related to Peters anomaly (PA) and the variant of unclear significance c.353T>C (p.Leu118Pro) in heterozygosi

Congenital microphthalmos with orbital dermoid cyst extending to frontal sinus.

A child presented with a chronic mucus discharging fistula from a mass over the right eyelid. There was a diagnostic dilemma about the mucus discharge from the mass which was probably thought to be secreted from the frontal sinus. The lesion caused a noticeable swelling in the eyelid with potential implications for surrounding structures. During surgical exploration, a large dermoid cyst was found occupying the entire orbit and extending into the frontal sinus, which was then thoroughly curetted and packed with fat to prevent recurrence and maintain structural integrity. On further exploration

[Pediatric cataract-part 2 : Surgical treatment].

Pediatric cataract surgery is very different from adult cataract surgery. A profound understanding of surgical technique, age at time of surgery, combined ocular and systemic diseases to be associated with long-term functional or morphological outcomes is important for planning and performing treatment of cataracts in children. Increased intra- and postoperative risk and individual amblyogenic risk mainly affect surgical versus nonsurgical treatment decisions. The most common long-term complication in children following congenital cataract surgery is aphakic glaucoma. It is well established th

A biometric survey of known and prospective murine models of posterior microphthalmia-nanophthalmia.

Posterior microphthalmia and nanophthalmia are related genetic conditions that disrupt ocular growth. Here, we conducted a biometric analysis of mouse models to assess shared features of these diseases. Three known microphthalmia alleles (Mfrprd6, Prss56glcr4, and Adipor1tm1Dgen) and two prospective alleles (C1qtnf5tm1.1(KOMP)Vlcg and Prss56em2(IMPC)J) were introgressed onto the C57BL/6J (B6) genetic background and compared to B6 mice at 1 through 12 months of age. Biometric parameters obtained using optical coherence tomography were analyzed statistically to identify strain differences. Fundu

Oculo-facio-cardio-dental (OFCD) syndrome: a case report.

BACKGROUND: Oculo-facio-cardio-dental (OFCD) syndrome is a rare condition that affects the eyes, face, heart, and teeth of patients. One notable dental characteristic of OFCD is radiculomegaly, or root gigantism, which highlights the role of dentists in detecting this syndrome. OFCD is an X-linked dominant syndrome that results from a variant in the BCOR gene. Our study presents the first documented case of OFCD in Vietnam and reports a novel BCOR gene variant observed in this case. CASE PRESENTATION: A 19-year-old Vietnamese female patient with an extremely long root with an abscess was clin

Microphthalmia and congenital cataract in two patients with Stickler syndrome type II: a case report.

BACKGROUND: Stickler syndrome (STL) is a collagenopathy caused by pathogenic variants in collagen-coding genes, mainly COL2A1 or COL11A1 associated with Stickler syndrome type 1 (STL1) or type 2 (STL2), respectively. Affected individuals manifest ocular, auditory, articular, and craniofacial findings in varying degrees. Previous literature and case reports describe high variability in clinical findings for patients with STL. With this case report, we broaden the clinical spectrum of the phenotype. MATERIALS AND METHODS: Case report on two members of a family (mother and son) including clinica

Insight into small eyes: a practical description from phenotypes presentations to the management.

This narrative review aimed to have an algorithmic approach to microphthalmos by a systematic search. The definition can be related to a number of special phenotypes. In the more challenging cases of complex microphthalmos, relative anterior microphthalmos, and nanophthalmos, the surgeon can approach these cases more safely if they have a deep understanding of the anatomical variations and ideal formulae for intraocular lens computation and knows how to avoid intra- and post-operative complications. In this article, we review the criteria by which we recognize and describe pre-, intra-, and po

A founder variant expands the phenotype of WNT7B-related PDAC syndrome.

Pulmonary hypoplasia, Diaphragmatic anomalies, Anophthalmia/microphthalmia, and Cardiac defects (PDAC) syndrome is a genetically heterogeneous multiple congenital malformation syndrome. Although pathogenic variants in RARB and STRA6 are established causes of PDAC, many PDAC cases remain unsolved at the molecular level. Recently, we proposed biallelic WNT7B variants as a novel etiology based on several families with typical features of PDAC syndrome albeit with variable expressivity. Here, we report three patients from two families that share a novel founder variant in WNT7B (c.739C > T; Arg247

Structural Variant Disrupting the Expression of the Remote FOXC1 Gene in a Patient with Syndromic Complex Microphthalmia.

Ocular malformations (OMs) arise from early defects during embryonic eye development. Despite the identification of over 100 genes linked to this heterogeneous group of disorders, the genetic cause remains unknown for half of the individuals following Whole-Exome Sequencing. Diagnosis procedures are further hampered by the difficulty of studying samples from clinically relevant tissue, which is one of the main obstacles in OMs. Whole-Genome Sequencing (WGS) to screen for non-coding regions and structural variants may unveil new diagnoses for OM individuals. In this study, we report a patient e

High Clinical Exome Sequencing Diagnostic Rates and Novel Phenotypic Expansions for Nonisolated Microphthalmia, Anophthalmia, and Coloboma.

PURPOSE: A molecular diagnosis is only made in a subset of individuals with nonisolated microphthalmia, anophthalmia, and coloboma (MAC). This may be due to underutilization of clinical (whole) exome sequencing (cES) and an incomplete understanding of the genes that cause MAC. The purpose of this study is to determine the efficacy of cES in cases of nonisolated MAC and to identify new MAC phenotypic expansions. METHODS: We determined the efficacy of cES in 189 individuals with nonisolated MAC. We then used cES data, a validated machine learning algorithm, and previously published expression d
