Research Progress on the Pathogenesis and Diagnostic and Therapeutic Potential of Ciliopathies Regulated by IFT172.

IFT172 is a core component of intraflagellar transport complex B (IFT-B), and pathogenic IFT172 variants disrupt ciliary transport, receptor localization, and tissue-specific signaling. This review summarizes evidence linking IFT172 dysfunction to neurological, retinal, skeletal, renal, and syndromic ciliopathy phenotypes, with emphasis on Bardet-Biedl syndrome, Joubert-spectrum disease, orofaciodigital syndrome, Mainzer-Saldino syndrome, and retinal degeneration. Current data support a primary role for IFT172 in IFT-train assembly, tip remodeling, anterograde-to-retrograde transition, and cil

Sleep Disordered Breathing in Joubert Syndrome: Disease Spectrum in a Pediatric Cohort.

BACKGROUND: Joubert syndrome (JS) is a rare, autosomal recessive condition with multisystem manifestations. Sleep disordered breathing (SDB) is a prominent feature, with patients characteristically presenting with a mixture of hyperpnea and apnea. Reports describing whether the respiratory abnormalities change as patients grow older are lacking. METHODS: We describe the clinical course of 21 children with JS followed over 16 years in a tertiary pediatric hospital. RESULTS: Fourteen children underwent polysomnography, while 11 had multiple repeat studies. Patients exhibited a range of SDB, in

Characterization of Usher Syndrome Type 2-Associated Proteins in the Retina via Affinity Purification-Mass Spectrometry.

Usher syndrome is the leading cause of inherited deaf-blindness, with type 2 (Usher syndrome type 2, USH2) being the most common form. USH2A, ADGRV1, and WHRN are the three known USH2 causative genes, which are also linked to isolated retinal degeneration and hearing loss. These genes encode usherin, ADGRV1, and whirlin, respectively, collectively called USH2 proteins. These proteins form a multiprotein complex (USH2 complex) at the periciliary membrane in retinal photoreceptors and at the stereociliary ankle link in inner ear hair cells. The molecular function of the USH2 complex and its dise

Incontinentia pigmenti: exploring the oral manifestations.

Incontinentia pigmenti (IP) is a rare genetic condition which is X-linked and predominantly affects the structures arising from neuroectodermal origin such as the skin, teeth, eyes, hair, nails and central nervous system. The dermatological manifestations are seen as the first developing signs. The dental manifestations are also commonly seen. Here we discuss a case of a girl in her early adolescence who was diagnosed with a case of IP. The dental features present in this case were typical, including missing permanent teeth, retained deciduous teeth, microdontia and delayed eruption.

Silent Chiari Type I Malformation Presenting With Markedly Asymmetrical Papilledema Detected on Routine Ophthalmic Examination: A Pediatric Case Report.

Chiari type I malformation (CMI) is a congenital hindbrain anomaly that may disrupt cerebrospinal fluid (CSF) dynamics and lead to intracranial hypertension. Although papilledema is a well-recognized manifestation of elevated intracranial pressure, it is typically bilateral and symmetric. Asymmetrical papilledema is uncommon, particularly in pediatric patients, and may pose a diagnostic challenge. We report a 10-year-old boy with no neurological or visual complaints who was referred after asymmetrical optic disc edema was detected during a routine ophthalmic examination. Visual acuity was pres

A large intraorbital arachnoid cyst with preserved visual acuity in a 3-year-old female: a case report and review of the literature.

BACKGROUND: Arachnoid cysts (ACs) are the most common brain cysts, comprising 1% of intracranial lesions, with a higher prevalence in children (2.6%) than adults (0.3%-1.7%). Intraorbital ACs are extremely rare, with only a few cases reported to date, especially in children. While most ACs do not exhibit any symptoms, some cases of symptomatic ACs require surgical intervention to prevent brain damage. This article presents a unique case of a large intraorbital AC with preserved visual acuity in a 3-year-old female with a concise literature review of all intraorbital AC cases reported to date.

Nonsense Mutation in USH2A Exon-13 Activates the Innate Immune Response in Müller Glial Cells.

Pathological USH2A mutations cause Usher syndrome type II, characterized by progressive retinitis pigmentosa and hearing and balance impairment. This study aims to investigate the cellular mechanisms underlying USH2A-related retinal degeneration using human induced pluripotent stem cell (hiPSC)-derived retinal organoids. The introduction of a homozygous nonsense mutation in the USH2A hotspot exon-13 resulted in normal photoreceptor development but loss of ciliary localization of usherin long form B and its interacting proteins, ADGRV1 and whirlin. Anatomical and clinical factors associated with multiple-stage embolization of brain arteriovenous malformation as a stand alone treatment.

INTRODUCTION: Embolization is one of the modalities used to treat brain arteriovenous malformations (bAVMs), which often require a multiple-stage approach. Factors influencing the need for a multiple-stage embolization are poorly defined and not very well understood. The aim of this study was to establish those factors. PATIENTS AND METHODS: We analysed 117 consecutive patients undergoing embolization due to bAVM with intention to treat. Patients undergoing embolization prior to surgery and with history of previous interventions or stereotactic radiosurgery were excluded. We used univariate a

A novel case of Heimler syndrome in a young child with compound heterozygous PEX26 mutations: clinical and genetic insights with literature review.

Heimler syndrome is an ultra-rare autosomal recessive disorder belonging to the spectrum of peroxisomal disorders. Heimler syndrome is characterized by sensorineural hearing loss, retinal dystrophy and dental anomalies. It is caused by hypomorphic mutations in peroxisomal biogenesis genes, namely PEX1, PEX6, PEX26, and PEX10. Fewer than 30 cases have been reported to date.We report the case of a male infant born at 36 weeks of gestation with congenital bilateral sensorineural hearing loss, confirmed by abnormal auditory brainstem responses and managed with early bilateral cochlear implantation

Mesenchymal drift in ciliopathy iPSC-derived RPE reveals a convergent pathogenic cell state.

Ciliopathies comprise a spectrum of disorders involving mutations in over 150 genes affecting the primary cilium, with retinal degeneration as a prominent feature driven by concomitant developmental and maturation defects in photoreceptors and the retinal pigment epithelium (RPE). Current single-gene-targeted therapeutic approaches are expensive with limited scalability. We hypothesize that downstream of primary cilium dysfunction, mutation-agnostic shared pathways initiate tissue defects. To test this hypothesis, we here develop induced pluripotent stem cell-derived RPE models (iRPE) from nin
