Vitreoretinal dysplasia masquerading as Peters' anomaly.

PURPOSE: Vitreoretinal dysplasia is characterized by a congenital bilateral nonattachment of the retina. In some cases, anteroposition of the iris and lens can lead to corneolenticular contact and corneal opacity, masquerading as Peters' anomaly. We report 3 cases of vitreoretinal dysplasia initially diagnosed as Peters' anomaly. METHODS: Case report. RESULTS: Case 1: In a 3-year-old boy with bilateral corneal opacities, high frequency ultrasound showed iridolenticular adherences as in Peters' anomaly, but color Doppler imaging of the posterior pole revealed a bilateral total retinal detachm

Early vitrectomy effective for bilateral combined anterior and posterior persistent fetal vasculature syndrome.

PURPOSE: The purpose of this study was to review our surgical experience with patients with bilateral combined anterior and posterior persistent fetal vasculature syndrome (PFVS). METHODS: We retrospectively reviewed the charts of all patients seen in our tertiary care pediatric retinal practice from 1988 to 2008 with a potential diagnosis of bilateral PFVS with posterior involvement. Clinical diagnosis required the presence of either bilateral persistent hyaloidal stalk tissue with retinal involvement or bilateral dense retrolental fibrovascular plaques (usually with no posterior view preope

Choroidal neovascular membrane in persistent fetal vasculature syndrome managed with intravitreal pegaptanib sodium in an infant.

PURPOSE: The purpose of this study was to report the successful management of a case of choroidal neovascular membrane secondary to persistent fetal vasculature syndrome in an infant using intravitreal pegaptanib sodium (Macugen). METHODS: A 14-month-old male infant developed a peripapillary choroidal neovascular membrane 10 months after lens-sparing vitrectomy that he underwent for persistent fetal vasculature syndrome and confirmed on angiography and optical coherence tomography. A single dose of intravitreal pegaptanib sodium (0.3 mg) was administered after informed consent was obtained. A

Stickler syndrome and the vitreous phenotype: mutations in COL2A1 and COL11A1.

Stickler syndrome is a dominantly inherited disorder affecting the fibrillar type II/XI collagen molecules expressed in vitreous and cartilage. Mutations have been found in COL2A1, COL11A1 and COL11A2. It has a highly variable phenotype that can include midline clefting, hearing loss, premature osteoarthritis, congenital high myopia and blindness through retinal detachment. Although the systemic phenotype is highly variable, the vitreous phenotype has been used successfully to differentiate between patients with mutations in these different genes. Mutations in COL2A1 usually result in a congen

[Stickler syndrome with rhegmatogenous retinal detachment].

BACKGROUND: Stickler syndrome is an autosomal dominant disease characterized by various disorders of the eyes and the connective tissues throughout the body. It can arise from a mutation in the collagen associated gene. We present a case of Stickler syndrome with rhegmatogenous retinal detachment. CASE: A 10-years-old boy was referred to us with rhegmatogenous retinal detachment of the right eye. His family history included eye disease and a cleft palate. He had high myopia, vitreous liquefaction and lattice degeneration in the both eye. He also had a cleft palate and a broad nasal bridge. Hi

Bilateral persistent fetal vasculature in an adult: Clinical manifestations and surgical outcomes.

UNLABELLED: A 23-year-old man presented with bilateral persistent fetal vasculature (PFV). Examination revealed bilateral cataracts, lens subluxation, elongated ciliary processes, and persistent hyaloid vessel remnants with mild posterior segment involvement. Extracapsular cataract extraction combined with posterior capsulectomy and anterior vitrectomy was performed in the right eye; lensectomy, posterior capsulectomy, anterior vitrectomy, superior peripheral iridectomy, and anterior chamber intraocular lens implantation were performed in the left eye. Postoperatively, the corrected distance v

Antivascular endothelial growth factor treatment in pseudoxanthoma elasticum patients.

The eye in patients with pseudoxanthoma elasticum, an autosomal recessive disease, shows several lesions, like peau d'orange, angioid streaks, comet lesions, and paired hyperpigmented smudges. The most devastating ocular complication is the development of choroidal neovascularizations (CNV). This exudative disease of the central retina leads to loss of visual acuity, and results of treatments in the past have been disappointing. From the present evidence it may be concluded that intravitreal antivascular endothelial growth factor (anti-VEGF) therapy with ranibizumab or bevacizumaab is benefici

Antivascular endothelial growth factor in hereditary dystrophies.

Choroidal neovascularization can rarely complicate the course of a number of chorioretinal hereditary dystrophies leading to an even further impaired vision function. In recent years, several case reports and case series have shown that intravitreal injections of antivascular endothelial growth factor drugs can be effective in treating subfoveal choroidal neovascularization secondary to chorioretinal dystrophies either improving vision, or at least halting its progressive loss. Additional studies are warranted to confirm the initial positive response and to assess the best therapeutic regimen.

Proliferative retinopathy in a child with congenital cyanotic heart disease.

In congenital cyanotic heart disease, oxygenated and deoxygenated blood mixes, and oxygen saturation of the arterial blood is not maintained. As a result, an ischemic environment prevails in the entire body. While various ocular findings have been described in patients with cyanotic heart disease, proliferative retinopathy has not been previously noted. We report a child with congenital cyanotic heart disease and multiple fibrovascular fronds over the fundus consistent with proliferative retinopathy.

Bilateral persistent hyperplastic primary vitreous.

A case of bilateral persistent hyperplastic primary vitreous (PHPV) in a 3-month-old male infant, who had bilateral leukokoria, is presented. The child was referred for imaging with a clinical suspicion of retinoblastoma. Gray-scale ultrasound evaluation revealed an echogenic band in the posterior segment of both globes, extending from the posterior surface of the lens capsule to the optic disc. Doppler examination revealed the presence of arterial flow in the band in both globes. Associated echogenic hemorrhage was also seen, which was confirmed by computed tomography. Most cases of PHPV are

Prospects for treatment of pediatric vitreoretinal diseases with vascular endothelial growth factor inhibition.

While angiogenesis inhibitors are already widely used to treat retinal disease in adults, only limited reports are currently available for the use of anti-VEGF in pediatric vitreoretinal diseases such as retinopathy of prematurity, Coats' disease, familial exudative vitreoretinopathy and retinopathy of incontinentia pigmenti. The limited trials of anti-VEGF therapy for pediatric vitreoretinal diseases are promising, although more extensive controlled trials will be needed to confirm their safety and efficacy. This paper will examine the current evidence for use of anti-VEGF therapy in a number
