Long-term surgical outcomes of pediatric retinal detachment associated with primary congenital glaucoma.

PURPOSE: Primary congenital glaucoma (PCG) is an extremely rare disease with an incidence of one in 10,000 live births. Increasing axial length due to PCG predisposes to retinal thinning and retinal detachment (RD). The present study aims to determine the long-term surgical outcomes of pediatric RD associated with PCG. METHODS: This was a retrospective study including pediatric patients who underwent vitrectomy with silicone oil injection for RD associated with PCG. Ocular examination reports and surgical findings of patients were analyzed to assess the anatomical and functional outcomes of t

A Landmark Case of Childhood Glaucoma Care in Bangladesh: Gonioscopy-Assisted Transluminal Trabeculotomy in Primary Congenital Glaucoma.

Primary congenital glaucoma (PCG) is a challenging condition requiring early surgical intervention to prevent visual impairment. Gonioscopy-assisted transluminal trabeculotomy (GATT) is a novel minimally invasive procedure that presents a promising alternative for the management of childhood glaucoma, utilizing either a microcatheter or suture (e.g., 5-0 prolene). We present a case of a three-year-old male from consanguineous parents diagnosed with bilateral PCG, who underwent suture-GATT with 5-0 prolene in his left eye. A six-month postoperative follow-up showed no complications and good con

A 15-year experience with keratoplasty in the management of paediatric corneal diseases: indications and clinical outcomes in Malaysia.

INTRODUCTION: Corneal blindness is a leading yet preventable cause of childhood blindness worldwide. Despite the need for corneal transplantation in paediatric cases, comprehensive data on its aetiology, clinical outcomes, and graft survival in Malaysia remain scarce. This study reviews the indications and outcomes of paediatric keratoplasty at Hospital Kuala Lumpur over the past 15 years. MATERIALS AND METHODS: We conducted a retrospective evaluation of paediatric patients (less than 12-years-old) who underwent keratoplasty in Hospital Kuala Lumpur, from January 2008 to December 2022. We ana

Glaucoma in retinopathy of prematurity: A review.

Glaucoma in retinopathy of prematurity (ROP) is a challenge. It is typically secondary, though it may be associated with primary congenital glaucoma. Secondary causes are multifactorial, with mechanisms and severity potentially linked to the stage of ROP, time of presentation, or interventions for its treatment. Anecdotal reports of glaucoma following anti-vascular endothelial growth factor injection exist, but conclusive evidence is lacking. The most important cause of glaucoma in ROP is secondary angle closure in patients with stage 5 ROP. Clinical features such as corneal clouding and incre

Pathogenic variants identification in primary congenital glaucoma patients using whole exome sequencing.

Primary Congenital Glaucoma (PCG) is a severe form of glaucoma that affects infants and young children that damage and causes vision impairment. Despite being a well-known condition, the genetic basis of PCG, particularly in highly consanguineous populations like the Pashtun community, still needs to be explored. Six consanguineous Pashtun families (PCG-01, PCG-02, PCG-03, PCG-04, PCG-05, & PCG-07) suffering from PCG were recruited for whole exome sequencing. A prioritization strategy was employed to identify variants in known PCG-related genes, primarily focusing on CYP1B1. Sanger sequencing

The Role of Axial Length and Intraocular Pressure in the Follow-Up of Growing Children with Primary Congenital Glaucoma.

Purpose: The aim of this study was to investigate the factors that affect ocular growth in children with primary congenital glaucoma (PCG) who have undergone surgical treatment. We designed an analysis like that proposed by Al-Obaida et al. to assess whether similar results could be obtained in the Caucasian population. Methods: Data on axial length (AxL), intraocular pressure (IOP), and corneal diameter of PCG patients were recorded and analyzed. Results: Patients' age was significantly influencing the increase in AxL. Also, levels of IOP, number of glaucoma medications, and number of surgeri

Semi-automated evaluation of trabecular meshwork and schlemm canal imaged with OCT in children with normal eyes vs. primary congenital glaucoma.

Maldevelopment of Schlemm canal (SC) and trabecular meshwork (TM) is proposed in the pathogenesis of primary congenital glaucoma (PCG). This study aims to evaluate a semi-automated segmentation algorithm of SC and TM in pediatric patients scanned using overhead-mounted optical coherence tomography (OCT). Twenty-four children (33 eyes: 17 PCG, 16 normal), with mean age 1.3 ± 1.08 years, had anterior segment OCT imaging. Manual segmentation of SC and TM on individual B-scans represented the gold standard and was compared to semi-automated segmentation of the same structures. Intraclass correlati

Enhanced Aqueous Humor Outflow Following Trabeculotomy in Primary Congenital Glaucoma Eyes.

In this study, we compared aqueous humor outflow (AHO) pathway patterns using aqueous angiography before circumferential trabeculotomy and again at subsequent follow-up, 1–2 months later, in eyes with primary congenital glaucoma (PCG; n=6). Pre-surgical and follow-up images were qualitatively and quantitatively compared. Trabeculotomy successfully lowered intraocular pressure (IOP) (28±1.3 to 11.8±2.0 mmHg; p <0.001). Qualitatively, few AHO pathways were seen at baseline that improved at follow-up. After trabeculotomy, there was an increased number of AHO branches (p=0.016) and branch junction

Management practices and surgical techniques for ab externo less invasive glaucoma surgery: a literature review and expert recommendations.

PURPOSE: Subconjunctival bleb-forming devices (SBD) can provide greater intraocular pressure reductions than minimally invasive glaucoma surgery (MIGS) whilst remaining less invasive than traditional surgical techniques. However, variations in management practices and surgical techniques indicate the need for concise and clear guidance on these procedures in diverse patient populations. Here we describe current recommendations on the use of PRESERFLO MicroShunt, an ab externo SBD, according to a review of current literature and the opinions of 20 international glaucoma experts. METHODS: A lit

Preliminary results of lateral interaction and radial deformity hyperacuity in children with glaucoma.

This study aimed to assess lateral spatial interaction (LSI) and radial deformity hyperacuity (RDH) in children with glaucoma and compare them with a normal control group. A cross-sectional study was conducted on both eyes of 16 participants (8 children with glaucoma and 10 age-matched control). LSI and RDH were collected in the same day. Children with glaucoma exhibited a lower interaction effect (F = 4.48; p = 0.035) compared to age-matched control children. Differences were observed in measurement conditions with and without flanks, except for frequency 0.8 cpd. Additionally, radial deforma
