A Meier-Gorlin syndrome mutation impairs the loading of the MCM2-7 complex during DNA replication initiation.

Chromatin loading of the hexameric replicative helicase MCM2-7 complex requires coordinated interactions with the origin recognition complex (ORC), CDC6, and CDT1. MCM2-7 not bound to DNA forms a single hexamer (SH) with an open DNA entry gate between MCM2 and MCM5. Two MCM2-7 SHs can be loaded sequentially to form the double hexamer (DH) that encircles the DNA duplex. Activated MCM2-7 then unwinds DNA and initiates DNA replication. Our cryoelectron microscopy analyses show that a fraction of human MCM2-7 without DNA exists as DH. Unexpectedly, we find that the MCM3 winged helix domain (WHD) d

The Influence Pathway of the Burden on Caregivers of Children With Congenital Ear Malformations: An Analysis of the Mediating Effects of Social Support and Coping Mechanisms.

BACKGROUND: Congenital auricular malformations, including microtia, result in a significant caregiving burden for parents or guardians. This study examines the caregiving burden, social support, and coping strategies among primary caregivers of children with congenital ear malformations and explores the factors influencing this burden. OBJECTIVE: This study aimed to assess the level of caregiving burden, social support, and coping strategies among primary caregivers of children with congenital ear malformations, and to examine the mediating roles of social support and coping strategies in the

Surgical and Postoperative Radiotherapy for Keloids on Congenital Microtia Reconstructed Auricles: Site-specific Incision Design.

BACKGROUND: Patients who undergo auricular reconstruction have a high risk of developing keloids. The traditional incision designs for keloid excision may appear inadequate to accommodate the biomechanical characteristics of different subunits of reconstructed ears. METHODS: A retrospective cohort of 86 patients (90 ears) with auricular keloids post-reconstruction from January 2020 to January 2024 underwent the keloid core removal surgery. We designed different incisions based on specific sites: earlobe, helix, crus of helix, and others. 24-48 h after surgery, all participants underwent super

The application of a simple and efficient negative suction system in two-staged microtia reconstruction.

Achieving a well-defined auricular contour is a core goal of microtia reconstruction, which highly relies on effective postoperative exudate drainage and stable adhesion between the cartilage framework and overlying skin. This study introduced a simplified, high-efficiency negative pressure suction system for exudate drainage in autogenous rib cartilage microtia reconstruction. The system enables controlled negative pressure suction, facilitating tight adhesion between the cartilage framework and skin. A retrospective analysis was conducted on clinical and postoperative data of patients who un

The application of the wound protector device "lap-protector" in costal cartilage harvest for auricular reconstruction to improve outcomes.

The study presents experience in using a wound protector device named lap-protector during costal cartilage harvest in auricular reconstruction in order to improve outcomes, particularly final scar quality and length. The present study retrospectively comprised fifty-five patients who underwent costal cartilage harvesting for auricular reconstruction were admitted between June to August 2022. The author divided the patients into 2 groups according to whether the lap-protector was used or not: group 1 underwent costal cartilage with the minimal invasive conventional technique by using the lap-p

Modern methods of cartilaginous microtia reconstruction.

PURPOSE OF REVIEW: The purpose of this manuscript is to review current literature of modern cartilaginous microtia reconstruction techniques and describe evolving biotechnological advancements within this field. We additionally offer opinions on technical refinements from our institutional experience with autologous rib cartilage reconstruction performed for nearly 30 years. RECENT FINDINGS: Autologous rib cartilage reconstruction is the gold standard for microtia treatment. There is active research in developing alternative framework materials, including allogenic and tissue-engineered carti

Current Evidence of the Effect of Breastfeeding on Ear Molding Outcomes: A Scoping Review.

OBJECTIVE: To provide an overview on auricular molding and to investigate whether breastfeeding is associated with greater cartilage malleability or improved perinatal auricular molding outcomes. DATA SOURCES: Embase, MEDLINE, and CENTRAL databases. METHODS: Three databases were comprehensively searched for treatment of congenital auricular malformations or deformations with auricular molding. Extracted data included study design, patient demographics and auricular anomalies, interventions and outcomes measures, and references to the effect of estrogen and breastfeeding on cartilage malleabi

Accuracy of fiducial-based augmented reality in auricular reconstruction.

OBJECTIVES: Reconstruction with autologous costal cartilage remains the gold standard surgical treatment for total or partial ear reconstruction. The different auricular subunits are sculpted from costal cartilage and assembled. The assembly requires a particular skillset and training, including a thorough three-dimensional (3D) understanding of auricular anatomy. The correct positioning of the ear framework can be particularly difficult in patients with hemifacial microsomia because of facial asymmetry. We developed a digital workflow that can help the surgeon in the correct positioning of th

Hair Removal with 755 nm Alexandrite Laser via Subcutaneous Route in Stage-III Ear Reconstruction by Expansion.

BACKGROUND: Congenital microtia reconstruction often introduces unwanted hair onto the new auricle. Laser hair removal can address this issue, but conventional external treatments require multiple sessions, increasing the overall treatment time, discomfort, and cost. Preclinical studies suggest that delivering laser energy from the flap's underside may improve efficiency by directly targeting hair follicles. We evaluated an intraoperative subcutaneous laser depilation approach applied during stage-III surgery to improve hair removal in this setting. METHODS: A prospective cohort study include

Evolution of Craft Skills in Ear Reconstruction: A Narrative Review From the Perspectives of an Anaplastologist and a Plastic Surgeon.

Auricular reconstruction encompasses both prosthetic and surgical methodologies, each requiring a distinct combination of scientific knowledge, manual skill, and aesthetic judgment. This narrative review traces the evolution of craft skills across both disciplines, from their earliest recorded applications to contemporary digital and tissue-engineering approaches. A narrative review was conducted using PubMed/MEDLINE, Scopus, Embase, and Google Scholar (August 2025-February 2026). Searches combined MeSH and free-text terms across auricular anatomy, autologous and alloplastic reconstruction, pr

Auricular cartilage tissue engineering: from making cartilage to regenerating a shape-stable elastic organ.

Auricular cartilage tissue engineering has long been grouped under the broader category of craniofacial cartilage reconstruction, but this classification is becoming insufficient. Unlike articular cartilage repair, which mainly aims to restore load-bearing function and relieve pain, auricular reconstruction must reproduce a complex three-dimensional structure, preserve elastic recoil, resist long-term contraction, tolerate soft-tissue coverage, and remain surgically and aesthetically acceptable over time. These demands make auricular cartilage engineering fundamentally distinct from generic ca

OBJECTIVE: To retrospectively evaluate the effects of targeted nursing centered on a game-based model on self-concept, anxiety, and quality of life in children undergoing auricular reconstruction for congenital microtia. METHODS: This retrospective observational study included medical records from children with congenital microtia who underwent auricular reconstruction between January 2022 and December 2025. Medical records were categorized into a control group and an intervention group according to the perioperative nursing model documented during hospitalization. The control group received
