Our Evolved Technique for Microtia Reconstruction: An Advanced Form of the Yotsuyanagi Method.

BACKGROUND: Reconstruction of microtia remains a complex challenge in plastic surgery, particularly in achieving both aesthetic and functional outcomes. Since the introduction of costal cartilage grafting, various techniques have evolved, including the Nagata method. However, limitations such as rigidity of the framework, high cartilage volume requirement, and complications from temporoparietal fascia (TPF) flaps persist. METHODS: The authors present our refined technique for lobule-type microtia reconstruction, now known in Japan as the Yotsuyanagi Method. This approach emphasizes a hybrid c

Endoscopic-Assisted Harvest of Temporoparietal Fascia Flap Through a Single Incision for Single-Stage Auricular Reconstruction With Porous Polyethylene Framework: How I Do It.

Single-stage auricular reconstruction using a porous polyethylene (PPE) framework with temporoparietal fascia flap (TPFF) coverage has become increasingly popular for microtia correction. This technique reduces the number of operative stages and avoids donor-site morbidity from rib cartilage harvest. We describe our refined approach combining PPE framework implantation with endoscopic-assisted TPFF harvest via a single small incision. Preoperative planning includes 256-slice contrast-enhanced MSCT angiography and Doppler mapping to delineate the branching pattern of the superficial temporal ar

Influence of vascular anatomy and perfusion on complications in the first stage of total auricular reconstruction.

BACKGROUND: The aesthetic quality of total auricular reconstruction for microtia has improved significantly in recent years, resulting in favourable outcomes for many patients. However, complications such as skin necrosis and infection remain. Adequate skin perfusion is essential to prevent these complications, but the vascular anatomy of patients with microtia is poorly understood. We evaluated the relationship between vascular anatomy assessed by indocyanine green fluorescence angiography performed during the first stage of autologous total ear reconstruction for microtia and postoperative c

Advanced Care for Management of Microtia Using a Two-Stage Autologous Costal Cartilage Technique for Auricular Reconstruction: Surgical Method, Outcomes, and Future Directions.

Auricular reconstruction for microtia remains one of the most technically demanding procedures in craniofacial surgery. The authors present a refined, two-stage approach using autologous costal cartilage. This method, building on the work of Tanzer, Brent, Nagata, and Firmin, demonstrates high aesthetic fidelity and improved patient quality of life. The authors' technique incorporates innovations in framework symmetry, postoperative care, a standardised splinting protocol following the second stage of ear reconstruction, and outcome evaluation using validated health-related quality of life (HR

A Modified Two-Flap Method for First-Stage Ear Reconstruction in Lobule-Type Microtia.

BACKGROUND: Autologous ear reconstruction typically involves a lengthy treatment period, often spanning 2 to 4 stages or utilizing skin expansion, to achieve satisfactory results. Consequently, an alternative procedure capable of shortening this treatment period would be advantageous for patients. We have made significant enhancements to the safety and outcomes of single-stage autologous ear reconstruction through a multitude of technical improvements. Herein, we propose a novel procedure capable of reconstructing a complete ear contour with postauricular sulcus in the first stage, with the op

A Predictive Model for Size-Specific Temporoparietal Fascia Flap Planning in Porous Polyethylene-Based Auricular Reconstruction.

Temporoparietal fascia (TPF) flaps are routinely harvested in a standardized size for auricular reconstruction using porous polyethylene implants, regardless of implant dimensions. This approach may lead to unnecessary over-dissection and increased morbidity, particularly in patients for whom a smaller implant is appropriate. The authors aimed to develop a predictive model for size-specific TPF flap planning based on implant dimensions. Commercially available "one-piece" porous polyethylene auricular implants, from 3 size categories ("small," "medium," and "large"), were analyzed. Using a plia

Genetic characteristics associated with isolated Microtia revealed through whole exome sequencing of 201 pedigrees.

Microtia is one of the most common congenital craniofacial malformations, characterized by the maldevelopment of the external and middle ear. While numerous genes have been implicated in syndromic forms of microtia, the genetic underpinnings of isolated microtia remain poorly understood. In this study, we conducted whole exome sequencing (WES) on 201 pedigrees with isolated microtia to investigate its genetic basis. Bioinformatics analysis identified 1362 deleterious variants corresponding to 332 candidate genes, including 40 previously associated with microtia-related phenotypes. Among these,

Single-Stage Ear Reconstruction Simultaneous With Piezoelectric Osia® Bone Conduction Implantation.

BACKGROUND: Single-stage, alloplastic ear reconstruction is often met with the challenge of coordinating bone-anchored hearing aid implantation. Modern digital piezoelectric stimulation, such as the Cochlear™ Osia® System (Sydney, Australia), contains integrated circuitry preventing the use of monopolar cautery after implantation and is larger than other devices, thus presenting several surgical challenges during simultaneous implantation. This study presents the first series of patients outlining a stepwise technique for safe and effective single-stage ear reconstruction and Osia implantation

A Practical Guide to Microtia Scoring: Step-by-Step.

Microtia/aural atresia reconstruction surgery focuses on improving hearing and cosmesis in patients with microtia, aural atresia, and aural stenosis. Preoperative evaluation of microtia with high-resolution CT plays an important role in assessing the potential for the successful surgical restoration of hearing. Given the challenges of the procedure, surgical candidates are carefully selected, taking into account the CT imaging findings. The Jahrsdoerfer scoring system is a semiquantitative tool used in the selection of suitable surgical candidates to predict the postoperative hearing outcome i
