AIMS: Status epilepticus is common in children with severe falciparum malaria and is associated with poor outcome. Phenytoin is often used to control status epilepticus, but its water-soluble prodrug, fosphenytoin, may be more useful as it is easier to administer. We studied the pharmacokinetics and clinical effects of phenytoin and fosphenytoin sodium in children with severe falciparum malaria and status epilepticus.
METHODS: Children received intravenous (i.v.) phenytoin as a 18 mg kg-1 loading dose infused over 20 min followed by a 2.5 mg x kg(-1) 12 hourly maintenance dose infused over 5

BACKGROUND: In Australia, healthcare providers face significant challenges in meeting the niche health needs of refugees. Social determinants of health, including limited health literacy and finances, complicate refugees' access to treatment for conditions, such as chronic hepatitis B (CHB). Understanding the strategies utilised by a specialised general practice for refugees can offer guidance for treating this population. The aim of this study was to explore how primary care is currently managing CHB care tailored to refugees and to identify areas for potential improvements.

Purpura fulminans and peripheral gangrene are rare and severe complications of malaria, resulting from microvascular thrombosis driven by endothelial injury, cytokine-mediated coagulation and depletion of anticoagulants. Because there are no effective strategies to prevent progression to gangrene, the most important part of the management is initiating effective antimalarial therapy as soon as possible. In this report, the authors describe the case of a 66-year-old woman who returned from Angola without antimalarial chemoprophylaxis and presented with cerebral malaria due to Plasmodium falcipa

Cerebral malaria is a major complication of Plasmodium falciparum infection that occurs upon the sequestration of infected red blood cells (iRBCs) in brain capillaries, resulting in the loss of endothelial barrier integrity, brain swelling, and frequently long-term sequelae or death. P. falciparum-iRBCs cause the disruption of human brain microvascular endothelial cell barrier integrity in vitro, mimicking the microenvironment of cerebral malaria, yet the specific mechanisms mediating this process remain unknown. Upon infection of the host RBCs, P. falciparum produces hemozoin, a crystal form

Cerebral malaria (CM) is a severe neurological complication of Plasmodium falciparum infection associated with endothelial activation, neuroinflammation, and disruption of the blood-brain barrier (BBB). While curcumin (Cur) possesses antiplasmodial and anti-inflammatory properties, its therapeutic potential is limited by poor aqueous solubility, rapid metabolism, and insufficient brain bioavailability. In this study, we hypothesized that polysaccharide-driven surface engineering could regulate nanoparticles (NPs) interfacial properties to improve BBB compatibility and antiplasmodial efficacy.

INTRODUCTION: Severe Plasmodium falciparum malaria is a potentially fatal illness associated with diverse systemic and neurological manifestations. While cerebral malaria is a wellrecognized complication, hemorrhagic stroke remains an exceptionally uncommon presentation. We report a rare case of severe malaria complicated by an acute left intracerebral hematoma, resulting in right-sided hemiparesis with facial nerve palsy. This report adds to the limited existing literature on malaria-associated hemorrhagic stroke and underscores its neurovascular implications.

Cerebral malaria (CM) is the most lethal complication of malaria, causing severe impairment of brain function. Although the precise mechanisms leading to CM remain unknown, the adhesion of infected erythrocytes to the cerebral endothelium, mediated by PfEMP1 proteins, is considered a key factor. These proteins, encoded by var genes, are divided into several groups and contain specific domain cassettes (DC) already known to be associated with CM. In this study, we compared the human and parasite proteomes of Plasmodium falciparum infected erythrocytes from nineteen samples from the NeuroCM coho

BACKGROUND: Malaria continues to be a significant global health challenge, caused predominantly by Plasmodium falciparum and Plasmodium vivax. While its classical manifestations are well described, rare and atypical organ-specific complications are increasingly recognized and may mimic other tropical diseases, delaying diagnosis and management.
OBJECTIVE: This study aimed to identify the spectrum of rare clinical and hematological manifestations of malaria and describe patient outcomes following appropriate therapy.
METHODS: This retrospective case series analyzed twelve adult patients with

BACKGROUND: Cerebral malaria (CM) is a life-threatening neurological complication of Plasmodium falciparum infection characterized by excessive inflammation, blood-brain barrier (BBB) disruption, and immune dysregulation. Macrophage-mediated inflammatory responses play a central role in CM pathogenesis, where imbalanced activation contributes to disease progression and tissue damage. However, integrated analyses combining macrophage surface phenotyping with transcriptional profiling remain limited, restricting comprehensive understanding of immune modulation during CM.

BACKGROUND: In non-endemic settings, imported malaria poses diagnostic challenges due to non-specific presentations and lack of immunity, especially in visiting friends and relatives (VFRs). Focusing on the Western Friuli (Italy) observatory, this work aims to optimize triage by identifying early severity predictors. Through the analysis of clinical parameters, paediatric cerebral cases, and host genetics - including an illustrative case of HbS/β-thalassaemia - we evaluate the drivers of Plasmodium falciparum progression to improve patient management.

BACKGROUND: Malaria-associated acute kidney injury (MAKI) represents a severe complication of malaria, increasingly documented in both Plasmodium falciparum and Plasmodium vivax infections across endemic regions.
METHODS: A systematic review and metaanalysis using PubMed, EMBASE, and Cochrane CENTRAL was conducted to identify studies published between 2000 and 2024. This review evaluates the existing literature on MAKI in adult population, aiming to synthesize data on demographics, Plasmodium species, kidney involvement, dialysis need, mortality, kidney histopathology, and long-term kidney ou

RATIONALE: Plasmodium falciparum infection can lead to acute thrombocytopenia, severe hemolytic anemia, and acute liver and kidney failure, among which the fatality rate of cerebral malaria is as high as 20% to 30%. Continuous bedside blood purification technology is an important intervention measure for severe malaria. The artificial liver blood purification technology, with its multimode clearance advantage, is widely used in the treatment of critical conditions such as liver failure, sepsis, and novel coronavirus infection. We described a case of severe cerebral malaria complicated with sev

Cerebral malaria (CM) remains one of the most severe complications of Plasmodium falciparum infection, often leading to death within days and leaving survivors with lasting cognitive damage. Since current treatments target the parasite but offer little to no neuroprotection, we explored whether existing neuroprotective drugs could help fill this gap. Six clinically-approved drugs, viz. topiramate, rasagiline, vinpocetine, cilostazol, piracetam, and rivastigmine were tested for both antimalarial and anti-cerebral malaria activity. Screening against P. falciparum in vitro showed that vinpocetine

BACKGROUND: Cerebral malaria (CM) is a serious complication of Plasmodium falciparum malaria that causes coma and, frequently, death. In malaria-endemic settings, a large percentage of the population presents with incidental P falciparum malaria parasitemia. In the absence of specific biomarkers for CM, when these individuals suffer from bacterial or viral infections causing coma, they are frequently misdiagnosed with CM.
METHODS: We have tested the specificity for CM of 2 candidate biomarkers for severe malaria, angiopoietin-like 4 (ANGPTL4) and inhibin-βE (INHBE), which are secreted by endo

OBJECTIVE: Jacobabad, located in the northern region of Sindh, is characterized by an extremely hot and humid climate and is classified as a malaria-endemic area, with an annual parasite incidence (API) of 31 per 1000 population in 2024. The present study investigates the incidence of malaria among patients admitted to our hospital along with comparison of clinical and haematological profile of severe and uncomplicated malaria in the region and to determine any association with age. To the best of our knowledge, no prior study has systematically documented the malaria burden in this region.

The malaria parasite mitochondrial proteins are critical targets for antimalarial drugs, however, the emergence of drug resistance against the existing protein targets necessitates novel therapeutic approaches. In this study, we profiled mitochondrial sense and natural antisense transcripts (NATs) in 22 Plasmodium falciparum clinical isolates, classified into three disease groups - uncomplicated malaria (UNC, n = 6), cerebral malaria (CM, n = 4), and hepatic dysfunction (HD, n = 12), using a custom, strand-aware 60 K microarray, and validated the transcriptome using pooled, strand-specific RNA

BACKGROUND: Malaria, caused by parasites of the Plasmodium genus and transmitted by Anopheles mosquitoes, remains a global public health issue, with a high incidence in tropical and subtropical regions. Although Plasmodium falciparum is the main cause of severe cases, Plasmodium vivax can also lead to serious complications, including cerebral malaria.
CASE PRESENTATION: This case report presents the case of a 43-year-old woman from the southwestern Pacific coast, Colombia, who developed severe neurological symptoms with an initially unclear diagnosis. A thick blood smear confirmed P. vivax in

BACKGROUND: Plasmodium falciparum erythrocyte membrane protein 1 (PfEMP1) antigens play an important role in host immune system evasion and parasite sequestration. The PfEMP1 antigenic domains that drive immune-mediated protection from severe malaria have not been identified. We hypothesized that Malian children with severe malaria lack serorecognition to distinct subsets of PfEMP1s compared with matched uncomplicated malaria controls.
METHODS: We developed a custom protein microarray including 79 fragments from PfEMP1s associated with severe malaria, including domain cassettes (DCs) 1, 4, 5,
