Patient satisfaction after surgery to correct the prominent ear relates to the degree of retroposition of the ear. Angular breaks in the cartilage seen with other cartilage-transecting procedures (and even after conchal resections), which are noticed by patients, led to the development of this technique. It is an easy operative technique that may be modified to suit all prominent ears, including the constricted cup ear, and it does not leave any sharp, visible ridges in the region of the anti-helix or concha. The procedure combines and modifies a number of techniques that have been described p

The authors report their experience in the surgical treatment of 96 patients with prominent ear. Most of the patients are less than 30 years old--75.3%. Various operation techniques are used. We noticed that the best cosmetic result was obtained by combination of two or more basic otoplasty techniques and that approach was applied in 60.5%. There were no major complications. Three patients have undergone second operation.

Saethre-Chotzen syndrome (acrocephalo-syndactyly type III, ACS III) is an autosomal dominant craniosynostosis with brachydactyly, soft tissue syndactyly and facial dysmorphism including ptosis, facial asymmetry and prominent ear crura. ACS III has been mapped to chromosome 7p21-22. Of interest, TWIST, the human counterpart of the murine Twist gene, has been localized on chromosome 7p21 as well. The Twist gene product is a transcription factor containing a basic helix-loop-helix (b-HLH) domain, required in head mesenchyme for cranial neural tube morphogenesis in mice. The co-localisation of ACS

Postoperative nausea and vomiting (PONV) frequently follows prominent ear correction under general anaesthesia in children. In a prospective, single-blind study, we compared the incidence of PONV after propofol infusion anaesthesia with that following thiopentone induction and isoflurane maintenance in 30 children aged from four to 14 years randomly allocated to one of two groups. All the children were mechanically ventilated. Anaesthesia was supplemented in both groups with nitrous oxide and infiltration of the ears using a mixture of bupivacaine, adrenaline, and hyaluronidase. One child rece

Saethre-Chotzen syndrome, a common autosomal dominant craniosynostosis in humans, is characterized by brachydactyly, soft tissue syndactyly and facial dysmorphism including ptosis, facial asymmetry, and prominent ear crura. Previously, we identified a yeast artificial chromosome that encompassed the breakpoint of an apparently balanced t(6;7) (q16.2;p15.3) translocation associated with a mild form of Saethre-Chotzen syndrome. We now describe, at the DNA sequence level, the region on chromosome 7 affected by this translocation event. The rearrangement occurred approximately 5 kb 3' of the human

Based upon a study of the embryologic anatomy of the external ear, the authors have suggested that the antitragus muscle be called the auricularis inferioris muscle which has a double nerve supply and a double function. Abnormal traction of this muscle can vary the final shape of the ear, dragging the border of the antitragus downward and everting it. This scaphoid and prominent ear deformity can be corrected by precise surgery described here by the authors.

PURPOSE: The literature illustrates a large number of different techniques for the correction of the prominent ear, and the great variety available is indicative of how difficult it is to achieve satisfactory results in all cases. This report describes a procedure for successfully treating such patients.
PATIENTS AND METHODS: Forty patients were treated with a surgical procedure creating a new anthelical plica based on the Stenström and Mustardé method and producing an anatomic cavity in the mastoid region where the concha can be repositioned according to the Furnas method. The tension of the

We report two children and a young adult who developed unilateral facial palsy shortly after injury to the external ear. In two instances the paralysis followed a prominent ear correction and in the other a laceration to the concha. The trauma-triggered facial palsy was most likely idiopathic although the anatomy of the facial nerve near the ear leads one to speculate on a possible pathway of a virally induced palsy (Bell's palsy). Each patient recovered over a period of 6 months.

It is a common fallacy that the prominent ear correction is easy; on the contrary it is difficult to obtain a perfect result, as indicated by the continuous growth of new methods. For a long time we have used Mustardè's mattress suture technique (Francesconi) but quite often the result was incomplete and not infrequently there was recurrence. In recent years, in order to obtain a more complete and permanent correction, we have added to the mattress sutures the superficial scratching of the anterior aspect of the cartilage as suggested by Stenstrom. A well formed contour of the antihelix and of

Contrary to a prominent ear, the deformity of the over-convex antihelix shows a very acute antihelix angle and helix depression and retreat. The patients usually have a desire to have such deformity corrected though it is not severe. In recent years we have corrected the over-conves antihelix using postauricular flap and cartilage with satisfactory results.

A prospective, randomised, double blind trial was carried out to test whether or not the application of topical anaesthetic cream (EMLA cream) before infiltration of local anaesthetic would decrease the discomfort of correction of prominent ears under local anaesthetic. 23 patients were entered into the trial. They acted as their own controls, one ear having EMLA cream applied approximately 2 h prior to surgery, and the other Aqueous cream. Immediately after surgery, the patients were asked to complete two scales describing the amount of pain they had felt in each ear, both at the time of inje

A simplified and knifeless technique for otoplasty is presented. Correction of the prominent ear is easily achieved by subcutaneous and transcartilaginous nonabsorbable permanent sutures after cartilage resilience is weakened by needle scoring. This technique was very successful in 11 consecutive cases with up to 30 months of followup.

The oral and maxillofacial surgeon is aware of the need to evaluate and treat the entire facial complex. To gain maximum esthetic results, excessive ear prominence occasionally needs to be addressed. Excessive ear prominence can be the result of failure of scapha folding, conchal hypertrophy, conchal malposition, or a combination of these deformities. Management of this problem is based on the accurate diagnosis of the deformity and a thorough understanding of the basic techniques that address them. This article presents an overview of otoplasty for the treatment of prominent ears.

In completing a satisfactory repair of the prominent ear, the shape of the antihelical fold is often the key to the aesthetic outcome. Correction involves not just creating a fold in the antihelix but also establishing a smooth sweep or antihelical curve anterior to posterior as well. The essential components described include accurate scoring of the antihelical fold followed by placement of mattress fixation sutures, with emphasis on obliquely placed sutures to control the final curve and shape of the antihelix. Sutures placed obliquely can solve the problem of persistent upper-pole prominenc

Non-hypoplastic congenital ear deformities are amenable to non-surgical treatment with splinting in the neonatal period. It is therefore important to determine when the deformity occurs. 100 postal questionnaires were sent to parents to study, retrospectively, the evolution of the prominent ear deformity in their children. 79 questionnaires were returned. As remembered by parents, the deformity was first noted at birth in 48 (61%) children, in 68 (86%) by 6 months and in all cases by 5 years. Therefore a large number of patients with prominent ears could be treated effectively, non surgically,

The effectiveness of surgery to correct prominent ears in relieving the psychosocial distress of children has been analysed in this prospective study. 30 children and their families were assessed preoperatively and again 12 months postoperatively. It was found that whilst prominent ear correction improved the well-being of 90% of the children, there was a small group of acutely distressed children who remained dissatisfied with outcome. These were children who were socially isolated prior to surgery. Careful screening and referral back to the family doctor of the more distressed children is re

Thirty patients were assigned to two equal groups to compare the effects of ear infiltration with bupivacaine versus lignocaine prior to correction of prominent ears. There was a statistically significant decrease in narcotic requirement in the first 4 h postoperatively in the bupivacaine group, associated with a decrease in the antiemetic requirement. The choice of local anaesthetic did not significantly affect the incidence of vomiting.
