BACKGROUND: In Australia, healthcare providers face significant challenges in meeting the niche health needs of refugees. Social determinants of health, including limited health literacy and finances, complicate refugees' access to treatment for conditions, such as chronic hepatitis B (CHB). Understanding the strategies utilised by a specialised general practice for refugees can offer guidance for treating this population. The aim of this study was to explore how primary care is currently managing CHB care tailored to refugees and to identify areas for potential improvements.
METHODS: Between

Purpura fulminans and peripheral gangrene are rare and severe complications of malaria, resulting from microvascular thrombosis driven by endothelial injury, cytokine-mediated coagulation and depletion of anticoagulants. Because there are no effective strategies to prevent progression to gangrene, the most important part of the management is initiating effective antimalarial therapy as soon as possible. In this report, the authors describe the case of a 66-year-old woman who returned from Angola without antimalarial chemoprophylaxis and presented with cerebral malaria due to Plasmodium falcipa

Severe Plasmodium falciparum malaria remains a major cause of morbidity in children, particularly among non-immune travelers. Intravenous artesunate is the treatment of choice for severe malaria due to its rapid parasite clearance and improved safety profile. However, post-artemisinin delayed hemolysis (PADH) is increasingly recognized as a delayed adverse effect, typically occurring seven to 30 days after treatment. Recognition of PADH is crucial to avoid misdiagnosis, guide monitoring, and ensure timely intervention. A previously healthy nine-year-old boy presented with severe P. falciparum

Severe Plasmodium falciparum malaria may be complicated by disseminated intravascular coagulation (DIC) with heterogeneous clinical manifestations. We report the case of a 61-year-old man with recent travel to Angola who presented with severe Plasmodium falciparum malaria complicated by DIC with a predominant thrombotic phenotype. On admission, he exhibited profound thrombocytopenia, markedly elevated D-dimer levels, metabolic acidosis, shock, and evidence of multiorgan dysfunction. Peripheral blood smear demonstrated parasitemia exceeding 25%. Despite prompt initiation of intravenous artesuna

BACKGROUND: Malaria continues to be a significant contributor to morbidity among children in tropical countries. Increasing evidence suggests that elevated uric acid levels may reflect inflammatory activity and disease severity in malaria. The aim of this study was to measure serum uric acid (SUA) levels in children with complicated malaria and to examine their association with clinical outcomes and biochemical markers of hepatic dysfunction.
METHODS: A hospital-based cross-sectional investigation was carried out in the Pediatrics Department of a tertiary care teaching hospital in eastern Ind

Malaria, primarily caused by Plasmodium falciparum, remains a critical global health challenge, particularly in sub-Saharan Africa. Genetic factors such as angiotensin-converting enzyme (ACE) I/D polymorphism have been implicated in influencing malaria severity and outcomes. We report the case of a 40-year-old woman from Seychelles, residing in Ivory Coast, who developed severe P. falciparum malaria complicated by thrombosis. Laboratory findings revealed hyperparasitemia, acute kidney injury, and metabolic acidosis. Genetic testing through real time PCR identified the ACE I/D polymorphism, alo

Co-infections of dengue and malaria are uncommon but clinically significant due to overlapping presentations and divergent management strategies. This article presents a case report of a 25-year-old adult female patient admitted to the critical care unit (CCU) of a tertiary care center in Jharkhand, India, with co-infection of Plasmodium falciparum malaria and dengue virus. The patient exhibited varying degrees of multiorgan failure, including severe respiratory failure necessitating mechanical ventilation, hepatic dysfunction, and hematological abnormalities. The diagnosis of dengue was estab

BACKGROUND: Malaria continues to be a significant global health challenge, caused predominantly by Plasmodium falciparum and Plasmodium vivax. While its classical manifestations are well described, rare and atypical organ-specific complications are increasingly recognized and may mimic other tropical diseases, delaying diagnosis and management.
OBJECTIVE: This study aimed to identify the spectrum of rare clinical and hematological manifestations of malaria and describe patient outcomes following appropriate therapy.
METHODS: This retrospective case series analyzed twelve adult patients with

Malaria remains a major global public health challenge, particularly in endemic regions and among travelers. Mixed-species malarial co-infections in these areas are frequently underdiagnosed or underestimated due to their lower incidence, especially in vulnerable pediatric populations. Such co-infections often result in severe complications and delayed targeted therapy. Here, we report the case of an 18-month-old male infant who presented with a low-grade fever, abdominal pain, severe anemia, and hepatosplenomegaly. Given the regional endemicity and season, Plasmodium vivax infection was initi

Background Acute kidney injury (AKI) is a frequent and serious complication of severe malaria (SM), contributing significantly to morbidity and mortality. In endemic regions like Sudan, AKI complicates the clinical course of SM, yet detailed demographic, clinical, and outcome data remain limited. Aim This study aimed to characterize the demographic profile, clinical features, management strategies, and outcomes of AKI in patients with SM in Sudan. Methods A cross-sectional analysis of 100 SM patients with AKI was conducted in Kosti Teaching Hospital from February to May 2025. Data on demograph

BACKGROUND: Cerebral malaria (CM) is a life-threatening neurological complication of Plasmodium falciparum infection characterized by excessive inflammation, blood-brain barrier (BBB) disruption, and immune dysregulation. Macrophage-mediated inflammatory responses play a central role in CM pathogenesis, where imbalanced activation contributes to disease progression and tissue damage. However, integrated analyses combining macrophage surface phenotyping with transcriptional profiling remain limited, restricting comprehensive understanding of immune modulation during CM.
OBJECTIVE: To investiga

Two licensed vaccines block Plasmodium falciparum malaria sporozoites through anticircumsporozoite protein antibodies. In animal models, intradermal (ID) sporozoites are more readily blocked than intravenous sporozoites. We hypothesized that this complicates human studies, where infectious mosquito bites deliver a mixture of ID and venous sporozoites. Here, to test whether vaccine efficacy varies by route of inoculation, we undertook a phase 2b open, randomized controlled trial, recruiting healthy volunteers in Kenya for randomization to the circumsporozoite protein-based R21/Matrix-M vaccine

BACKGROUND: Cerebral malaria (CM) is a serious complication of Plasmodium falciparum malaria that causes coma and, frequently, death. In malaria-endemic settings, a large percentage of the population presents with incidental P falciparum malaria parasitemia. In the absence of specific biomarkers for CM, when these individuals suffer from bacterial or viral infections causing coma, they are frequently misdiagnosed with CM.
METHODS: We have tested the specificity for CM of 2 candidate biomarkers for severe malaria, angiopoietin-like 4 (ANGPTL4) and inhibin-βE (INHBE), which are secreted by endo

Falciparum malaria and Vivax malaria are notorious for the development of multiple complications, including acute kidney injury, acute liver injury, severe hemolysis, bone marrow suppression, severe acidosis, hypoglycemia and CNS complications. We hereby present two cases of rare complications associated with Malaria, which were managed in a resource-constrained setting in Eastern Africa. Two individuals working in the malaria endemic region of South Sudan were detected with Mixed Plasmodium falciparum-Plasmodium vivax when they reported to the hospital with acute febrile illness for two days.

Cerebral malaria is a major complication of Plasmodium falciparum infection that occurs upon the sequestration of infected red blood cells (iRBCs) in brain capillaries, resulting in the loss of endothelial barrier integrity, brain swelling, and frequently long-term sequelae or death. P. falciparum-iRBCs cause the disruption of human brain microvascular endothelial cell barrier integrity in vitro, mimicking the microenvironment of cerebral malaria, yet the specific mechanisms mediating this process remain unknown. Upon infection of the host RBCs, P. falciparum produces hemozoin, a crystal form

Malaria, primarily caused by Plasmodium falciparum (Pf), remains a significant public health concern in Southeast Asia and sub-Saharan Africa, disproportionately affecting pregnant women, leading to adverse maternal and fetal outcomes. Placental malaria (PM) is characterized by sequestration of P. falciparum infected erythrocytes in intervillous space, which causes activation of immune response at maternal-fetal interface and altered expression of angiogenic regulators like VEGF, sVEGFR1, Angiopoietins (ANG), sEndoglin, and placental growth factor (PLGF). Emerging evidence suggests that the la

BACKGROUND: Very severe thrombocytopenia, characterized by a platelet count below 20,000/µL, in pregnant women with malaria, particularly Plasmodium falciparum infection, is a significant concern due to its association with increased mortality. While not always associated with bleeding, it can be a sign of severe disease and may require prompt intervention.
CASE PRESENTATION: The author presents a rare case of a successful vaginal delivery in a 22-year-old, Para II, woman from Western Ethiopia, who had very severe malarial thrombocytopenia. She started treatment with artesunate for acute mala

BACKGROUND: Acute acalculous cholecystitis (AAC) is an inflammatory gallbladder disease occurring in the absence of gallstones. It typically presents with right upper quadrant (RUQ) pain, fever and gastrointestinal symptoms, and is diagnosed by ultrasound evidence of cholecystitis without gallstones. AAC can have multiple causes, including infections, with malaria only rarely reported.
CASE PRESENTATIONS: We describe four Ugandan children who developed malaria-associated AAC. They were all referred from other centers and presented at a more advanced stage of disease. Apart from AAC, they also

Cerebral malaria (CM) is a severe complication of Plasmodium infection, classically attributed to parasite sequestration and neuroinflammation. Here, we uncover a spleen-centered humoral autoimmune circuit that drives CM pathology. Proteomic analyses identify CD36 as a dominant host-derived antigen enriched in infected red blood cells (iRBCs), triggering anti-CD36 autoantibody production in patients with falciparum malaria. Although contributing to iRBCs clearance, these autoantibodies also target other CD36-expressing cells, thereby driving thrombocytopenia, endothelial injury, and macrophage
