Maternal Fish Intake in the Year Prior to Conception and Birth Defects, National Birth Defects Prevention Study, 1997-2011.

BACKGROUND: Epidemiologic data on the association between maternal fish intake and birth defects are sparse. Our objective was to assess associations between maternal fish intake and 52 different birth defects, most of which have not been assessed previously. METHODS: Using logistic regression, data was analyzed for 29,242 mothers of infants with birth defects and 10,973 mothers of control infants who participated in the National Birth Defects Prevention Study (NBDPS) and delivered between 1997 and 2011. We focused on associations between mothers who reported high fish intake of 2 or more ser

Does Cardiopulmonary Bypass Influence Outcomes in Stage 2 Palliation? A meta-analysis and systematic review.

OBJECTIVES: The bidirectional Glenn (BDG) procedure, a key palliative intervention for single-ventricle physiology, is conventionally performed using cardiopulmonary bypass (CPB) despite its associated complications and costs. However, the comparative impact of CPB versus off-pump techniques on perioperative safety, neurological risk and long-term outcomes remains controversial. Therefore, this meta-analysis aimed to determine whether performing the BDG procedure without CPB improves perioperative outcomes (including morbidity, mortality, and neurological events), resource utilisation, and lon

Robot-Assisted Ileocecal Resection for Cecal Cancer with a Common Trunk of the Middle Colic and Ileocolic Arteries: A Case Report.

INTRODUCTION: Vascular variations around the superior mesenteric vessels increase the technical difficulty and risk of injury during right-sided colon surgery. We report a rare case of cecal cancer with a common arterial trunk formed by the middle colic artery (MCA) and ileocolic artery (ICA) treated by robot-assisted surgery. CASE PRESENTATION: A man in his 70s was diagnosed with cecal cancer during evaluation for anemia. Preoperative CT revealed that the MCA and ICA originated from a common arterial trunk. Robot-assisted ileocecal resection with lymphadenectomy was performed. After identifi

Successful Vaginal Delivery in a Young Female With Eisenmenger Syndrome and Double Outlet Right Ventricle: A Challenging Case Report.

BACKGROUND: Eisenmenger syndrome (ES), a severe complication of uncorrected congenital heart defects, is characterized by pulmonary hypertension and right-to-left intracardiac shunting resulting in systemic hypoxemia. Double outlet right ventricle (DORV), a rare congenital anomaly where both great arteries arise predominantly from the right ventricle, further exacerbates hemodynamic instability. Pregnancy in women with ES and DORV is associated with extremely high maternal and fetal mortality rates, requiring meticulous, multidisciplinary management. CASE PRESENTATION: A 20-year-old Gravida 1

Successful Management of a Neonate With Tricuspid Atresia, Double Outlet Right Ventricle, and L-malposed Great Arteries.

A neonate with prenatally suspected complex single ventricle physiology was found after birth to have tricuspid atresia with a large ventricular septal defect, double outlet right ventricle, L-malposed great arteries, and severe subpulmonary stenosis. The infant presented at term via cesarean delivery for breech presentation with cyanosis, poor tone, and absent spontaneous respirations, requiring immediate positive pressure ventilation and initiation of prostaglandin E₁ to support ductal patency during hemodynamic stabilization. Early transthoracic echocardiography confirmed complex single ven

Considering Multiple Acyl-CoA Dehydrogenase Deficiency (MADD) in Prenatal Genetic Assessment of Cystic Renal Anomalies.

OBJECTIVE: Multiple acyl-CoA dehydrogenase deficiency (MADD) is caused by variants in the ETFA, ETFB or ETFDH genes. MADD type 1, the most severe phenotype, presents antenatally with congenital anomalies and, commonly, renal cystic anomalies. We describe four prenatal cases with MADD-related genetic findings, detailed ultrasound findings, and outcomes. METHODS: This is a retrospective case series presenting the MADD cases detected with associated fetal renal anomalies. The spectrum of congenital anomalies associated with MADD in the brain, heart, and urinary system is documented. Disease-caus

Ultrasound-Guided Superficial Temporal Artery Cannulation for Proximal Arterial Pressure Monitoring During Coarctation Repair With an Aberrant Right Subclavian Artery in a Low-Birth-Weight Neonate.

Coarctation of the aorta (CoA) complex with an aberrant right subclavian artery (ARSA) poses a significant challenge for intraoperative cerebral perfusion assessment because arterial pressure from both the upper and lower extremities becomes unreliable during aortic arch reconstruction. We report a 14-day-old male neonate weighing 1.819 kg with CoA complex, including patent ductus arteriosus (PDA), right-ventricle-type single ventricle, double-outlet right ventricle, single atrium, total anomalous pulmonary venous return (type IIa), mitral atresia, and ARSA, who underwent left subclavian arter

Mid-Term Durability of Tissue-Engineered Vascular Graft Patches for Pulmonary Artery Reconstruction.

Autologous pericardium is commonly used for pulmonary artery (PA) reconstruction in congenital heart surgery but is not always available. Synthetic materials lack growth potential and may cause calcification or infection. Tissue-engineered vascular grafts (TEVGs), created by in-body tissue architecture, may overcome these limitations. This study evaluated mid-term outcomes of PA reconstruction using TEVG patches. From July 2014 to December 2024, TEVG molds were implanted in 19 patients; seven underwent PA reconstruction using 15 patches. Diagnoses included pulmonary atresia with ventricular se

Ablation of Intra-Atrial Re-Entrant Tachycardia in an Adult With Double-Inlet Ventricle Using Pulsed-Field Ablation.

BACKGROUND: Supraventricular tachycardia is common in patients with adult congenital heart disease and carries a high morbidity. Ablation of these arrhythmias is an effective treatment strategy, but difficulty accessing the cardiac chambers, variations in electroanatomical circuits, and concerns of damaging the surrounding structures can pose procedural challenges. CASE SUMMARY: A 45-year-old man with complex congenital heart disease and intra-atrial re-entrant tachycardia initially underwent ablation with radiofrequency energy but experienced clinical recurrence. He presented for a second pr

Postnatal Clinical Course in Fetuses Diagnosed with Tetralogy of Fallot and Double-Outlet Right Ventricle with Absent Ductus Arteriosus.

INTRODUCTION: Absent ductus arteriosus (ADA) in fetuses with tetralogy of Fallot (TOF) and TOF-type double-outlet right ventricle (DORV) remains poorly characterized. The absence of a patent ductus arteriosus precludes prostaglandin-mediated maintenance of ductal patency and may necessitate early postnatal intervention. METHODS: This retrospective study reviewed 31 consecutive fetuses diagnosed with TOF (n=23) or TOF-type DORV (n=8) between April 2018 and March 2025. Fetal echocardiographic measurements in the third trimester included pulmonary valve (PV), aortic valve (AoV), main pulmonary a

After the Lateral Tunnel Fontan, the Intra-Atrial Channel Grows and Remains Phasic, Expanding in Systole.

Although the extracardiac conduit (ECC) Fontan is now widely preferred, recent reports of improved early outcomes with the lateral tunnel (LT) have renewed interest in LT physiology. We evaluated long-term growth and phasic diameter changes of the intra-atrial channel after LT Fontan. We reviewed 67 patients who underwent LT Fontan at our center (1989-2009). Patients with transplantation, Fontan takedown, or no follow-up within the past 5 years were excluded. Forty-seven patients with follow-up within 5 years were included; 37 had transthoracic echocardiograms (TTE) of sufficient quality for a

A Unique Combination of Criss-Cross-Heart and Common Arterial Trunk: Case Report of Successful Management.

We present the unique combination of a criss-cross heart and common arterial trunk, our management strategy, and challenges of the treatment course. The child was born weighing 2.4 kg as one of twins at the 36th week of gestation. Bilateral pulmonary artery (PA) banding was performed initially as symptoms of congestive heart failure manifested. The decision to proceed with a 2-stage approach was made due to the low body weight (<2.5 kg) and distorted anatomy, suspected to affect the position of a right ventricle to pulmonary artery (RV-PA) conduit. During the interstage period, the patient dev

Recognizing Cyanotic Congenital Heart Disease Masked by Infant Pneumonia in a Resource-Limited Setting: A Somali Case Report of Tetralogy of Fallot.

BACKGROUND: In resource-limited settings lacking routine newborn screening, common lower respiratory tract infections like community-acquired pneumonia (CAP) can obscure or delay the diagnosis of underlying cyanotic congenital heart disease (CHD). Tetralogy of Fallot (TOF) is the most common cyanotic CHD, yet its co-occurrence with CAP creates significant diagnostic masking, as acute pulmonary signs often conceal cardiac abnormalities. CASE PRESENTATION: A 5-month-old male presented in Borama, Somalia, with severe respiratory distress, fever, cough, and bilateral crackles, leading to an initi

Ascending aortic dilation and aortic regurgitation - a neglected problem among single-ventricle patients after Fontan palliation.

BACKGROUND: The Fontan procedure revolutionized the management of univentricular heart physiology but is associated with long-term complications. Although pulmonary vascular disease, atrioventricular valve regurgitation, and ventricular dysfunction are well-recognized risks, aortic dilation and aortic regurgitation (AR) are emerging concerns. This study aimed to assess the prevalence and risk factors for aortic dilation and AR in adult post-Fontan patients. METHODS: We retrospectively analyzed 101 single-ventricle patients who underwent Fontan palliation and were hospitalized at NIKARD betwee

Case Report: 26-year follow-up after total cavopulmonary connection in crisscross heart-longitudinal assessment of anastomotic patency, biomarkers, and arrhythmia progression.

Crisscross heart (CCH) is an extremely rare congenital cardiac malformation, and literature on long-term outcomes after total cavopulmonary connection (TCPC) in CCH patients remains limited. We report a unique case demonstrating sustained anastomotic patency and objective postoperative evolution 26 years after TCPC in a patient with CCH. A 39-year-old female with crisscross heart and double-outlet right ventricle underwent TCPC at age 13 in 2000. Serial Doppler echocardiography (2013-2026) consistently demonstrated patent superior vena cava-right pulmonary artery anastomosis and inferior vena

Guillouet-Gordon Syndrome With Severe Microtia in an Infant With a Homozygous Variant in MED16.

Bi-allelic MED16 variants underlie the recently described MEDopathy termed Guillouet-Gordon syndrome (GGNS, MIM #621220), characterized by neurodevelopmental and structural anomalies. To date, 27 individuals with GGNS have been reported, collectively harboring 28 distinct compound heterozygous or homozygous MED16 variants. Here we present a male infant, born to unrelated parents, presenting with multiple congenital anomalies overlapping with the established GGNS phenotypic spectrum. Specifically, the clinical presentation included structural brain defects and neurodevelopmental deficits alongs
