Assessment of Double Outlet Left Ventricle in Pediatrics Using Transthoracic Echocardiography and Computed Tomographic Angiography.

Double outlet left ventricle (DOLV) is a rare congenital cardiac anomaly in which both great arteries originate entirely or predominantly from the morphologic left ventricle. The aim of this study is to explore its clinical presentations and compare the diagnostic accuracy of transthoracic echocardiography (TTE) with computed tomographic angiography (CTA) on DOLV. We took TTE as a first-line examination modality, considering CTA and surgical results as the gold standard. Ten suspected patients with DOLV were identified at the Children's Hospital of Chongqing Medical University from March 2009

A Retrospective Study on the Imaging Spectrum of Functional Single Ventricle and Its Associations.

Objective  The aim of the study was to identify the various spectrum of single ventricle (SV) physiology and to correlate the anatomical variants. Materials and Methods  All congenital heart disease cases with computed tomography/magnetic resonance imaging during the period from 2008 to 2023 were retrospectively assessed. Among these, we identified those corresponding to the definition of SV ( n  = 160). In each case, we described the situs, looping, atrioventricular (AV)/ventriculoarterial (VA) connections, and associated anomalies. Descriptive statistics of each parameter were obtained and c

Contemporary outcomes and health care costs associated with single-ventricle heart failure admissions in adults in the United States.

OBJECTIVE: Advances in surgical and medical therapies have enabled patients with single-ventricle (SV) physiology to survive into adulthood, leading to a growing population of adults with SV congenital heart disease (SV-ACHD). This study aims to characterize contemporary SV-ACHD health care utilization and outcomes in comparison to acquired heart failure (HF). METHODS: The National Inpatient Sample was queried for the years 2016-2021 to identify all nonelective hospital admissions for HF or cardiogenic shock in patients aged 18 years or older. Patients with a diagnosis of common ventricle, do

Coarctation Stenting When Origin of Aberrant Right Subclavian Artery Is Before Stenotic Segment.

BACKGROUND: Occasionally, coarctation of the aorta (CoA) may go unnoticed and present in adulthood with left ventricular dysfunction. Variations in anatomy of the neck vessels can add to the complexity in management. CASE SUMMARY: Our patient underwent surgical repair of a double-outlet right ventricle during which CoA was overlooked. She presented later with peripartum "cardiomyopathy" when CoA was diagnosed on 2-dimensional echocardiography. During transcatheter intervention, an aberrant right subclavian artery (ARSA) was seen arising before CoA opposite the origin of the left subclavian ar

KRAS4B is required for placental development.

Beyond its well-established role in cancer, KRAS is also crucial for embryogenesis, as its absence leads to embryonic lethality. However, the precise mechanisms underlying the developmental functions of KRAS, as well as the respective roles of its two splicing isoforms, KRAS4A and KRAS4B, remain incompletely characterized. To address these issues, we generated Kras4A knock-out (Kras4A-/-) and Kras4B-/- mouse models using CRISPR/Cas9 technology, and compared their phenotypes to those of a Kras-/- model, in which both isoforms are simultaneously inactivated. We observed that Kras-/- and Kras4B-/

Total anomalous pulmonary venous connection - prenatal echocardiography and neonatal follow-up.

Total anomalous pulmonary venous connection is a rare congenital heart defect that can be diagnosed prenatally and might be very important for perinatal management. In addition to other cardiac abnormalities (levocardia, situs inversus, small left ventricle, double outlet right ventricle, parallel great vessels, and hypoplastic aortic arch), total anomalous pulmonary venous connection of a subdiaphragmatic type was diagnosed during a prenatal echocardiography examination in the second half of pregnancy. Fetal echocardiography monitoring showed no signs of congestive heart failure. The neonate

The 24-Segment Global Sphericity Index Is Useful for Assessing Fetal Cardiac Function and as an Indicator of Fetal Well-Being.

BACKGROUND: Automated technologies have become increasingly vital in fetal cardiac assessment. Fetal heart quantification (HQ), which utilizes two-dimensional speckle tracking, provides quantitative measures of morphology and function. This study aims to identify the progression of many fetal cardiac function parameters during pregnancy in normal pregnant women and to determine which parameters are relevant to fetal well-being and worth considering. METHODS: We conducted a prospective observational study involving fetuses of 26 normal pregnant women to evaluate 27 fetal HQ parameters (includi

Common trunk branching from the renal artery to the diaphragm, adrenal gland, and testis: a case report with embryological hypotheses by observation.

The vascular systems of the kidneys and gonads are highly susceptible to variations because of the degeneration of the mesonephros and mesonephric artery and the progressive ascending development of the metanephros. In this report, we present a rare case of a common arterial trunk originating from the left renal artery and supplying the left diaphragm, adrenal gland, and testis. Anatomical dissection of a 95-year-old male was conducted during the 2023 academic year at Tokai University Medical School. The left renal artery had a common trunk that bifurcated to superior and inferior branches, fo

Acute stent placement for dynamic airway collapse after cardiac surgery in congenital heart disease with bronchial stenosis: A case report.

RATIONALE: Tracheomalacia, typically seen in relapsing polychondritis,[1] is rarely reported in association with congenital heart disease (CHD). In patients with pulmonary hypoperfusion-type CHD, surgical repair results in a rapid increase in pulmonary blood flow, predisposing them to mucus retention, airway obstruction, and respiratory distress. We describe acute airway collapse in a patient with double outlet right ventricle and congenital bronchial stenosis following cardiac repair. Surgically augmented pulmonary flow critically narrowed the stenotic airway, rapidly reversed by bronchoscopi

Significance of Papillary and Trabecular Muscular Volume in Right Ventricular Volumetry with Cardiac MR Imaging.

PURPOSE: Pulmonary valve regurgitation after repaired Tetralogy of Fallot (TOF) or double-outlet right ventricle (DORV) causes hypertrophy and papillary muscle enlargement. Cardiac magnetic resonance imaging (CMR) can evaluate the right ventricular (RV) dilatation, but the effect of trabecular and papillary muscle (TPM) exclusion on RV volume for TOF or DORV reoperation decision is unclear. METHODS: Twenty-three patients with repaired TOF or DORV, and 19 healthy controls aged ≥15, underwent CMR from 2012 to 2022. TPM volume is measured by artificial intelligence. Reoperation was considered wh
