Durability of the Arterial Switch Operation: Long-Term Survival and Structural Sequelae in a Multicentre Cohort.

BACKGROUND: The arterial switch operation (ASO) is the standard repair for complete transposition of the great arteries (TGA), but long-term data across large contemporary cohorts remain limited. In this study we evaluated 30-year survival, reintervention, and structural sequelae after ASO and identified factors that influenced late outcomes. METHODS: A total of 1125 patients, 652 with TGA with intact ventricular septum, 389 with TGA with ventricular septal defect, and 84 with double-outlet right ventricle with Taussig-Bing anomaly who underwent ASO between 1990 and 2015 at 10 tertiary centre

Enhancer of zeste homolog 2 (Ezh2) is required in mouse Isl1-expressing progenitors for proper development of cardiac and skeletal hindlimb structures.

Enhancer of zeste homolog 2 (EZH2), the catalytic subunit of Polycomb Repressive Complex 2, catalyses H3K27 trimethylation and directs lineage development programs, yet its function in early multipotent progenitors remains incompletely understood. ISL1 marks cardiopharyngeal and neural crest progenitors that contribute to second heart field derivatives and to craniofacial and limb morphogenesis. Here, Isl1-cre lineage tracing showed that lineage derivatives populated the cardiopharyngeal region and the posteromedial hindlimb bud during mid-gestation, and contributed extensively to the developi

Double-inlet double-outlet right ventricle: a case report and systematic review of the literature with cluster analysis.

Hearts with a double-inlet and double-outlet right ventricle are infrequent. Due to their infrequency, it is unclear whether there are natural patterns in hearts like this. A systematic review was performed to identify published cases of double-inlet double-outlet right ventricle. Characteristics for individual reported hearts were collated and entered into a cluster analysis. Hearts with double-inlet, double-outlet right ventricles tended to fall into two clusters largely based on aortic atresia and systemic venous connections.

In vitro construction of handmade polytetrafluoroethylene right ventricle-to-pulmonary artery valved conduit using a modification of the Ozaki technique.

OBJECTIVE: To develop a polytetrafluoroethylene (PTFE) valved conduit for right ventricle-to-pulmonary artery (RV-PA) reconstruction using a modified technique of the aortic neocuspidization described by Ozaki. METHODS: Between June 2024 and September 2025, 12 patients underwent the placement of handmade PTFE valved RV-PA conduits. An inverted PTFE graft was marked for commissures and cusp nadirs. Three leaflets were fashioned from 0.1-mm PTFE membrane using Ozaki's sizer and template. Leaflets were sutured to the inner wall following the technique of Ozaki. Graft was then re-everted and comm

Decades of single-center experience in managing 274 double-outlet right ventricle patients: Biventricular repair suitable for more complex forms?

OBJECTIVE: To investigate the long-term outcomes of double-outlet right ventricle (DORV), focusing on identifying specific subtypes capable of achieving successful biventricular repair (BVR) with favorable results. METHODS: A retrospective review was conducted of 274 DORV patients followed at a single center from 1999 to 2023. After exclusions, 238 patients were analyzed, including 97 who underwent single ventricle palliation (SVP) and 141 who underwent BVR. Outcomes were assessed using Kaplan-Meier survival and freedom from reintervention estimates, with competing risk analysis for survival,

How I Do It: Modified Senning Double Switch For Criss-Cross Heart With AV/VA Discordance.

The criss-cross heart with double outlet right ventricle (DORV) and combined atrioventricular (AV) and ventriculoarterial (VA) discordance presents one of the most formidable challenges in congenital cardiac surgery. The unique ventricular topology and complex inflow-outflow relationships often preclude standard approaches to biventricular repair, and single-ventricle palliation is frequently selected. However, in appropriately selected patients, anatomical repair through a double switch procedure may offer superior long-term outcomes by preserving systemic ventricular function. This report de

DNA Methylation and Fetal Programming of Cardiovascular Disease: From Congenital Heart Diseases to Adult Cardiovascular Dysfunction.

DNA methylation, the most extensively studied epigenetic mechanism, acts as a critical interface between maternal environmental influences and fetal cardiovascular development. During embryogenesis, tightly orchestrated methylation remodeling regulates the transcriptional networks required for cardiogenesis, including chamber septation, valve formation, and myocardial maturation. Disruption of these methylation patterns contributes to congenital heart disease (CHD), with distinct methylation signatures identified in tetralogy of Fallot, double-outlet right ventricle, bicuspid aortic valve, and

Heart failure and re-interventions in adult patients with residual right-sided congenital heart disease.

BACKGROUND: Heart failure (HF) is the leading cause of death among adults with congenital heart disease. However, specific data on right-sided adult congenital heart disease (RACHD) remain scarce. This study assessed (1) the prevalence and risk factors of HF; (2) its prognostic impact; and (3) the impact of re-intervention in adults with right-sided residual congenital heart disease and HF (RACHD-HF). METHODS: Data from RACHD patients since 2010 were retrospectively analysed. HF was defined as symptoms requiring therapy with objective cardiac dysfunction. Cox models identified predictors of t

Accuracy of fetal echocardiography in detecting lesions associated with, and predicting surgical plan for, dextro-transposition of the great arteries and double outlet right ventricle with subpulmonary ventricular septal defect and predicted transposition physiology.

OBJECTIVES: In dextro-transposition of the great arteries (D-TGA) and double outlet right ventricle with subpulmonary ventricular septal defect (DORV-SPV) and predicted transposition physiology, the presence of associated lesions, such as ventricular septal defect (VSD), coarctation of the aorta (CoA) or pulmonary stenosis (PS), impacts prenatal counseling and complicates neonatal surgery. This study aimed to evaluate the diagnostic accuracy of fetal echocardiography (FE) and its ability to predict the postnatal surgical plan in fetuses with D-TGA or DORV-SPV. METHODS: This was a single-cente

Impact of conotruncal anomalies on outcomes following total cavopulmonary connection.

OBJECTIVES: The impact of conotruncal anomalies on long-term outcomes after total cavopulmonary connection remains unclear, particularly regarding haemodynamic performance and dominant ventricular morphology. METHODS: All patients who underwent total cavopulmonary connection between 1994 and 2023 at a single centre were reviewed. Conotruncal anomalies were defined as transposition of the great arteries, double outlet right ventricle, tetralogy of Fallot, truncus arteriosus, and interrupted aortic arch type B. Four endpoints were analysed: transplant-free survival, Fontan failure, tachyarrhyth

A Retrospective Analysis of Cardiac Anatomy in Patients Undergoing the Fontan Operation.

Background Hearts suitable for Fontan palliation include those with an anatomically single ventricle (absence of a second ventricular mass), those with a single atrioventricular connection, and those with two ventricles in whom biventricular repair is not feasible due to anatomical or physiological complexity (functionally univentricular physiology). We describe the anatomical spectrum of such hearts over a decade at a single tertiary center. Methodology We encountered 420 patients undergoing the Fontan operation over a decade, between 2010 and 2019. Their detailed medical records were reviewe

Reoperative Aortic Root Surgery in Adults with Prior Conotruncal Repair.

Conotruncal anomalies comprise a heterogeneous group of congenital heart defects arising from abnormal embryologic development of the cardiac outflow tract and the great arteries. Aortic root dilatation represents a frequent yet comparatively underexplored finding in this population, and clear guidelines regarding surveillance and surgical treatment remain limited. This article reviews mechanisms of aortic root dilatation, diagnostic evaluation, and surgical indications and principles for aortic root surgery in adults with conotruncal anomalies, including lesion-specific considerations for Tet
