Hemodynamic Unmasking of Atrial Septal Defect After Pulmonary Arterial Hypertension Therapy: A Pathophysiologic Perspective.

Pulmonary arterial hypertension may mask underlying atrial septal defects by reducing left-to-right shunt flow through elevated pulmonary vascular resistance and impaired right ventricular compliance. Targeted therapy may unmask latent defects, emphasizing early multimodality imaging and sustained clinical suspicion for congenital shunts when invasive hemodynamic assessment suggests shunting despite inconclusive echocardiography.

Comparison of robot-assisted surgical and transcatheter closure of secundum atrial septal defect: A single-center retrospective study.

BACKGROUND: Atrial septal defect (ASD) closure can be performed using either transcatheter or robot-assisted surgical approaches. This study aimed to compare the clinical outcomes, procedural characteristics, and complications associated with these two techniques in a large patient cohort. METHODS: A retrospective analysis was conducted on 635 patients who underwent ASD closure via either robot-assisted surgery (Group I, n=290) or transcatheter closure (Group II, n=345). Preoperative, perioperative, and postoperative data, including procedural times, complications, and follow-up outcomes, wer

Early and Midterm Outcomes of Atrial Myxoma Resection Via Superior Transseptal Approach.

BACKGROUND: Atrial myxomas are the most common primary cardiac neoplasms, typically presenting with obstructive cardiac symptoms or embolic events. Surgical excision remains the definitive treatment; however, concerns persist regarding surgical exposure, postoperative arrhythmia, and recurrence. This study compared the early and mid-term outcomes of atrial myxoma excision performed exclusively using the superior transseptal technique with outcomes reported in the current literature. METHODS: This retrospective cohort study included 10 patients who underwent surgical resection of atrial myxoma

Transcatheter retrieval of a fully dislodged atrial septal defect occluder from the descending aorta using the "retained super-stiff guide wire" technique.

Device embolisation is one of the most feared early complications of percutaneous atrial septal defect closure. We report a 40-year-old woman in whom a 12-mm atrial septal defect occluder was accidentally displaced into the descending aorta 24 hours after an apparently successful procedure. The dislodged device was retrieved percutaneously through a 12-F long sheath that was continuously railroaded over a super-stiff guide wire. This "retained super-stiff guide wire" technique secured sheath stability, avoided aortic wall trauma, and allowed the occluder to be slipped into the sheath without c

Structural birth defects and exposure to oil and gas wells during pregnancy.

BACKGROUND: Exposure to unconventional hydraulically fractured oil and gas wells during pregnancy is associated with a higher risk of structural birth defects. These associations have not been examined in California, where most operators use conventional methods that do not use enhanced techniques. OBJECTIVES: Determine whether residing near oil and gas wells during early pregnancy was associated with birth defect risks among San Joaquin Valley, California, residents. METHODS: We conducted a case-control study with data from the California Center of the National Birth Defects Prevention Stud

Analytical interpretation of hemodynamic data in patients with intracardiac shunts: Role of mathematical modeling.

BACKGROUND: Shunt lesions are categorized into pre- and post-tricuspid. Although it is well recognized that these two entities have a different pathophysiology, hemodynamic variables involved are still poorly understood. This paper aims to analytically appraise shunt physiology exploiting a lumped parameters mathematical model. METHODS: Circulatory system was split into arterial and venous compartments, each of them being described by resistive, capacitive and inductive components. The model was modified including communication between atria and ventricles. Predicted changes in the ratio betw

Refractory hypoxemia following mitral valve edge-to-edge repair: why is it so refractory?

Transcatheter Edge-to-Edge Repair (TEER) is an established therapy for high-surgical-risk patients with severe mitral regurgitation (MR). Iatrogenic atrial septal defects (iASD) are a routine consequence of transseptal puncture, and significant right-to-left shunting (RLS) causing refractory hypoxemia is rare. This report presents a man in his 80s with multiple comorbidities who underwent successful surgery. Despite procedural success confirmed by transesophageal echocardiography (TEE), the patient developed refractory hypoxemia during the early recovery phase while still undergoing mechanical

Arterial switch operation with autologous sinus tube for transposition of great vessels with single coronary.

This video tutorial demonstrates an arterial switch operation utilizing an autologous coronary sinus tube in a 6-day-old female born with dextro-transposition of the great arteries, ventricular and atrial septal defects, and complex coronary anatomy, with a single coronary artery origin from left-facing running anterior to the aortic root. A standard button transfer was deliberately avoided to minimize the risk of distortion of the coronary arteries. Thus, native aortic sinus tissue was harvested as a contiguous flap and fashioned into a tubular conduit. The constructed tube was implanted into

Beyond the Aorta: Incidental Atrial Septal Defect in a Patient With Marfan Syndrome and Severe Aortic Dilation.

Marfan syndrome (MFS) is an autosomal dominant connective tissue disorder caused by mutations in the FBN1 gene, which carries a high risk of cardiovascular morbidity and mortality. We present the case of a 31-year-old man with a severe Marfanoid phenotype who was admitted with decompensated heart failure, septic shock, and renal failure. After stabilization, multimodal evaluation revealed a severely dilated aortic root and severe aortic regurgitation, along with an incidental 11 mm ostium secundum atrial septal defect (ASD). This unusual combination caused critical biventricular overload. The

Outcome of biventricular repair of congenital cardiac malformations with a borderline left ventricle.

INTRODUCTION: Biventricular repairs are occasionally done on hearts with a borderline-sized left ventricle to avoid univentricular palliation. We aim to assess the outcome. MATERIALS AND METHODS: Data of 27 patients with borderline left ventricle that had undergone biventricular repair in our centers in the past 20 years (median follow-up: 5 years) were analyzed retrospectively. RESULTS: Successful primary repair was performed in 17 (63%) patients. Four (15%) patients initially underwent primary repairs, which were converted to Norwood reconstructions, hoping to re-attempt biventricular repa

Late-onset hypercyanotic spells in duct-dominant Tetralogy of Fallot with pulmonary atresia from juxtaductal PDA-LPA stenosis: CT angiography for preoperative mapping.

Tetralogy of Fallot with pulmonary atresia (ToF-PA) requires precise delineation of extracardiac pulmonary blood supply to guide optimal palliation, especially in duct-dependent physiology. We report an 8-month-old infant with late-onset central cyanosis and recurrent cry-triggered hyper cyanotic spells. Computed tomography (CT) thoracic angiography showed classic ToF with short-segment pulmonary atresia, a malaligned perimembranous ventricular septal defect, and hypoplastic yet confluent branch pulmonary arteries. A long, tortuous patent ductus arteriosus (PDA) provided dominant pulmonary flo

Single Multi-snare® retrieval of a > 16 mm Occlutech Figulla II atrial septal defect occluder-a case series.

Transcatheter closure is the preferred treatment for type 2 atrial septal defects. Device retrieval for embolisation or malpositioning is rarely necessary and can often be performed percutaneously. However, transcatheter retrieval of Occlutech Figulla Flex II atrial septal defects occluders >16 mm can be very difficult, and it has even been stated to be impossible in the literature. Its right atrial hub is ball shaped so that snare devices could slip off when retrieving a large device into a sheath, necessitating the use of multiple snare or other retrieval devices. We present three paediatric

A scaffold-degradable single-rivet ASD occluder with amplatzer-like design: a multicenter randomized controlled trial.

Metallic atrial septal defect (ASD) occluders are associated with various mid- to long-term complications. To address this, our team developed a novel biodegradable Pansy ASD occluder, and this multicenter RCT aimed to evaluate its effectiveness and safety. A prospective, multicenter, randomized controlled trial was carried out on adult and pediatric patients using the Pansy occluder with a biodegradable polydioxanone (PDO) framework. The primary efficacy endpoint of this clinical trial is the closure success rate at six months post-operation. From May 2021 to March 2023, a total of 127 patien

Percutaneous Closure of Coronary Sinus Atrial Septal Defect across Two Spectrums: A Case Series.

Coronary Sinus ASD or Unroofed Coronary Sinus defect is the rarest form of atrial septal defect and is seldom reported due to its non specific manifestations and difficult diagnosis on transthoracic echocardiography. Multimodality imaging is required for accurate diagnosis of this defect. The standard treatment is surgical and it has been incidentally diagnosed during other cardiac surgeries. Here we present two such patients who presented to us with non-specific complaints and were incidentally detected to have RA, RV dilatation. On detailed transthoracic and transoesophageal echocardiography

The Paradox of Support: Mechanical Ventilation-Induced Hypoxemia Through a Patent Foramen Ovale.

Patent foramen ovale (PFO) is common and usually harmless, but can cause significant positional and refractory hypoxemia in situations when there is a significant right-to-left shunt across it, and this phenomenon often goes unnoticed, especially in patients on ventilators, in whom it can be fatal. Platypnea-orthodeoxia syndrome (POS) is a condition characterized by positional dyspnea (platypnea) and hypoxemia (orthodeoxia), usually associated with right-to-left interatrial shunting across a PFO or an atrial septal defect (ASD). The positional variation is believed to be due to interatrial str

The Pivotal Role of Advanced Echocardiography in Transcatheter Closure of Challenging Secundum Atrial Septal Defect Anatomies: An Expert-Based Review.

Transcatheter secundum atrial septal defect (ASD) closure is the preferred approach in the majority of cases. Building skill sets through understanding complex ASD anatomies is essential. Such anatomies include ASD associated with aneurysmal septum, multiple defects, absence of anterosuperior or posteroinferior rim and malaligned septum This expert-based review will focus on the key role of advanced TOE imaging during transcatheter ASD closure in challenging anatomies. We describe our institutional experience and provide a practical approach of how to plan and navigate device choice and its de

Percutaneous Closure of a Giant Secundum Atrial Septal Defect Using 2 Sutured Occluders.

BACKGROUND: Giant secundum atrial septal defects (ASDs) are generally considered unsuitable for transcatheter closure and often require surgical repair. Innovative percutaneous strategies may provide an alternative in prohibitive-risk patients. CASE SUMMARY: A 75-year-old man with a giant secundum ASD (64.5 × 38.8 mm), severe mitral and tricuspid regurgitation, pulmonary hypertension, atrial fibrillation, cirrhosis, and chronic kidney disease was deemed inoperable. After staged mitral and tricuspid transcatheter edge-to-edge repair, the ASD was successfully closed using two 38-mm septal occlu

Small Secundum Atrial Septal Defects Are Associated with Paradoxical Embolism: A Retrospective Study Using Intraoperative Transesophageal Echocardiography.

INTRODUCTION: Paradoxical embolism is a clinically important complication of secundum atrial septal defect (ASD), yet anatomical characteristics predisposing to embolic events remain incompletely defined. We investigated clinical and morphological factors associated with paradoxical embolism among adults undergoing transcatheter ASD closure, with particular focus on ASD size. METHODS: We retrospectively analyzed 270 adults who underwent transcatheter closure of secundum ASD between 2013 and 2024. Patients were classified according to history of paradoxical embolism.
