Renal Hemosiderosis Associated with Mitral Regurgitation Detected by Severe Proteinuria Due to Congestion.

A 53-year-old woman with an atrioventricular septal defect who had undergone 3 valvuloplasties by 7 years old underwent a kidney biopsy because of lower extremity edema and proteinuria. The glomeruli were intact; however, the proximal tubules showed marked hemosiderin deposition, which was assumed to be related to a fissure in the calcified cardiac patch. The fissure acts as a collateral blood channel from the left ventricle to the left atrium, and the resulting mitral regurgitation and backflow of blood cause turbulent flow phenomena and chronic hemolysis. After hospitalization and dietary gu

Re-operative minimally invasive endoscopic mitral valve repair after partial atrioventricular canal repair.

Re-operation following previous congenital heart repair can be challenging. We present a 38-year-old female with a history of partial atrioventricular septal defect repair in infancy who developed severe mitral regurgitation due to a cleft anterior mitral leaflet. Given her anatomy and prior sternotomy, we performed a redo minimally invasive endoscopic mitral valve repair via right anterolateral minithoracotomy access. We meticulously closed the cleft using precise suture placement to restore leaflet integrity and function and performed an annuloplasty to reinforce the annulus and optimize lea

Intraoperative predictors of left atrioventricular valve reoperation after repair of complete atrioventricular septal defect.

OBJECTIVE: Although outcomes of repair of complete atrioventricular septal defect have improved in recent years, the burden of reoperation on the left atrioventricular valve (LAVV) remains high. We investigated predictors of LAVV reoperation on the intraoperative echocardiogram that may help guide surgical decision making. METHODS: All patients who underwent repair of complete atrioventricular septal defect at the Royal Children's Hospital from 2010 to 2020 were included in the study. Intraoperative postbypass echocardiograms were individually reviewed. Long-term data were obtained from revie

Partial Atrioventricular Septal Defect Repair through Right Thoracotomy in A 73-Year-Old Woman.

INTRODUCTION: While surgical treatment for partial atrioventricular septal defect (AVSD) is commonly performed in pediatric patients, its application is relatively rare in older patients. CASE PRESENTATION: A 73-year-old woman was referred to our hospital for evaluation of shunt blood flow caused by a partial atrioventricular septal defect and left atrioventricular valve regurgitation that caused right heart overload and cardiomegaly. She underwent minimally invasive atrial septal defect closure and left atrioventricular valve repair through right-sided thoracotomy. CONCLUSIONS: We report a

Case Report: Concurrent atrial and ventricular septal defect in a young Sphynx cat.

Concurrent atrial septal defect (ASD) and ventricular septal defect (VSD) are rare in cats. A 7-month-old intact female Sphynx cat was presented for evaluation of respiratory distress and exercise intolerance. Thoracic radiographs showed generalized cardiomegaly and pulmonary infiltrates, consistent with cardiogenic pulmonary edema. Transthoracic echocardiography revealed an ostium secundum-type ASD and a perimembranous VSD, with structurally normal atrioventricular valves and annuli. These features supported the diagnosis of concurrent ASD and VSD, rather than an atrioventricular septal defec

Concurrent incomplete atrioventricular septal defect and supravalvular mitral stenosis causing heart failure in a 13-year-old cat: a case report.

A 13-year-old female Persian cat with a heart murmur presented with dyspnea. Thoracic radiography revealed pleural effusion and dilation of peripheral pulmonary vessels. Echocardiography showed significant enlargement of the right atrium and ventricle and left atrial appendage; an atrial septal defect (ASD) near the atrioventricular valves; and a septum-like structure below the left atrial appendage. Left-to-right shunting through the ASD was noted, with a pulmonary-to-systemic flow ratio of 2.72. The cat was diagnosed with incomplete atrioventricular septal defect (IAVSD) and supravalvular mi

Diastolic Mitral Regurgitation With Interatrial Left-to-Right Shunt.

BACKGROUND: Diastolic mitral regurgitation (MR) occurs when left ventricular pressure exceeds left atrial pressure during diastole, resulting from a reverse atrioventricular gradient. Diastolic MR is occasionally detected in patients with structural mitral valve abnormalities, left ventricular dysfunction, atrioventricular block, or severe aortic regurgitation. However, there are no reports on diastolic MR associated with interatrial left-to-right shunts. CASE SUMMARY: Case 1 was a 66-year-old man diagnosed with an atrial septal defect and diastolic MR between the E wave and A wave. Case 2 wa

Higher Incidence of Moderate and Severe Congenital Heart Disease in Patients With Neonatal Abstinence Syndrome.

Studies suggest that in utero opioid exposure may be associated with congenital heart disease (CHD). We sought to assess the incidence of CHD in infants with neonatal abstinence syndrome (NAS). A review of a national, administrative database from January 2019 to December 2022 was conducted for neonates with an ICD-10 code for NAS and moderate or severe CHD. The incidence of CHD in NAS (the main outcome variable) was compared to those without NAS using χ2 analysis and odds ratios for CHD with significant differences. There were 4,994,919 neonatal admissions during the study period: 26,284 with

Left ventricular outflow tract obstruction repair after atrioventricular septal defect correction.

This video tutorial presents the case of surgical correction of left outflow tract obstruction after atrioventricular septal defect repair. The left outflow tract obstruction was presented as a fixed fibromuscular membrane combined with accessory tissues from the left atrioventricular valve. The surgical correction included fibromuscular membrane resection combined with myotomy and resection of additional tissues arising from the atrioventricular valve. The repair was conducted via a transaortic approach. Post-operative echocardiography demonstrated a good result of the surgery without flow ac

Berlin Heart EXCOR sVAD implantation technique for neonates and infants with functionally univentricular ductal-dependent systemic circulation.

The Berlin Heart EXCOR is a pulsatile paracorporeal ventricular assist device (VAD) for neonates, infants, children and adults with congenital or acquired severe ventricular dysfunction. Berlin Heart EXCOR VADs are routinely used as either a bridge to a cardiac transplantation, or occasionally as a bridge to ventricular recovery. Our programmatic philosophy is to bridge neonates and infants with functionally univentricular ductal-dependent systemic circulation or functionally univentricular ductal-dependent pulmonary circulation who are at high risk for staged palliation because of important c
