Sudden-onset diplopia warrants an emergency management in ophthalmology. A binocular, vertical and persistent diplopia, should raise suspicion of a central lesion, particularly within the brainstem. We reporte the case of a 53-year-old man, who presented with acute vertical binocular diplopia. Ophthalmological examination revealed a bilateral elevation limitation, characteristic of Parinaud's syndrome. Brain magnetic resonance imaging identified a right-sided ischemic midbrain infarction, and further cardiovascular work-up demonstrated a patent foramen ovale causing a right-to-left shunt.

BACKGROUND: Holt-Oram syndrome (HOS) is a rare autosomal dominant disorder caused by pathogenic variants in the TBX5 gene and characterized by radial ray malformations associated with congenital heart defects and/or cardiac conduction abnormalities. Although atrial and ventricular septal defects are the most common cardiac manifestations, other congenital cardiac lesions may occur. Early recognition of cardiac involvement is essential to prevent complications and improve outcomes.
CASE PRESENTATION: We report the case of an 8-month-old female infant presenting with failure to thrive, feeding

BACKGROUND: To date, >10 million children have been conceived via assisted reproductive technology (ART) globally. However, there are some concerns about potential adverse outcomes associated with ART-conceived pregnancies, including the risk of congenital heart defects (CHDs). This meta-analysis aimed to evaluate the association between ART and the risk of major CHDs as well as their specific subtypes.
METHODS: Major electronic databases were screened for studies comparing the incidence of CHD following ART-conceived pregnancies versus spontaneous conception. The primary end point was study-

Oculo-facio-cardio-dental (OFCD) syndrome is a rare X-linked dominant disorder caused by pathogenic variants in the BCOR gene and is characterized by distinctive craniofacial, ocular, cardiac, and dental abnormalities. Although OFCD syndrome is a well-defined developmental disorder, its association with endocrine tumors has not been clearly established. We report a female patient with genetically confirmed OFCD syndrome harboring a novel heterozygous deletion in BCOR who developed metachronous multiple insulinomas. The patient presented with recurrent hypoglycemic episodes, and pancreatic neur

INTRODUCTION: Percutaneous atrial septal defects (ASD) closure with fluoroscopy guidance is the standard procedure. However, fluoroscopy poses stochastic and deterministic risks for small infants and children. Zero fluoroscopy ASD closure is an alternative, yet its feasibility and safety compared to fluoroscopy remain unclear. Therefore, this study compares outcomes using standardized fluoroscopy and zero fluoroscopy methods for transcatheter ASD closure.
METHODS: Four databases (PubMed, ProQuest, Google Scholar, Wiley) were used to search literature published before July 2023. The main resul

Transcatheter closure is the preferred treatment for type 2 atrial septal defects. Device retrieval for embolisation or malpositioning is rarely necessary and can often be performed percutaneously. However, transcatheter retrieval of Occlutech Figulla Flex II atrial septal defects occluders >16 mm can be very difficult, and it has even been stated to be impossible in the literature. Its right atrial hub is ball shaped so that snare devices could slip off when retrieving a large device into a sheath, necessitating the use of multiple snare or other retrieval devices. We present three paediatric

OBJECTIVE: Dextrocardia refers to the right-sided positioning of the heart during embryonic development and may occur in isolation or in association with visceral malposition or other malformations. However, few studies have investigated this condition. This study aimed to determine the incidence of cardiac and non-cardiac malformations, as well as to analyze long-term follow-up and survival outcomes in patients with dextrocardia.
METHODS: This was a retrospective chart review of dextrocardia cases at King Faisal Specialist Hospital & Research Centre in Riyadh, Saudi Arabia from April 22, 197

While atrial septal defects (ASDs) and atrial fibrillation (AF) present differently, there is evidence that they share some genetic basis. Here, we have used directed differentiation of human induced pluripotent stem cells into atrial or ventricular cardiomyocytes (CMs) to delineate gene regulatory networks (GRNs) that define each identity. We uncovered accessible chromatin regions, transcription factor motifs and key regulatory nodes specific to, or shared by, both CM types, including the transcription factor TBX5, which is linked to genetic susceptibility of ASDs and AF in humans. PURPOSE: The septal pouch is an anatomical variant resulting from incomplete fusion between the septum primum and the septum secundum at the interatrial septum. This partial fusion creates a conical pouch, which is potentially predisposed to thrombus formation, increasing the degree of thromboembolic risk. This cadaveric study aimed to evaluate the prevalence, morphology, and morphometry of septal pouches and their associations with other septal defects.
METHODOLOGY: The interatrial septa of 60 human hearts were examined for septal pouches. Characteristics such as pouch type, apex and base or

BACKGROUND: Congenital heart defects (CHD) are the leading cause of infant mortality associated with birth defects in Brazil. Access to prenatal diagnosis is still considered low, and treatment is limited to referral centers, especially in large urban centers.
OBJECTIVES: To characterize the profile of live births diagnosed with CHD in three university hospitals in Rio Grande do Sul, a state in southern Brazil.
METHODOLOGY: This study was based on an active surveillance system, and data were collected between November 2021 and February 2024 through medical record searches and maternal interv

BACKGROUND: Atrial septal defect (ASD) is a simple defect but carries considerable morbidity, especially arrhythmias.
OBJECTIVES: Data on the impact and timing of ASD closure and the risk of (recurrent) arrhythmia remain scarce and will be the focus of this study.
METHODS: This Danish nationwide cohort study included all patients diagnosed with secundum ASD (1977-2024), followed until arrhythmia, death, emigration, or study end (January 2024). The risk of (recurrent) arrhythmias was assessed based on ASD closure status and closure technique.
RESULTS: Among 6469 patients with ASD (43.3% men)

IMPORTANCE: Patent foramen ovale (PFO) closure decreases recurrent stroke but increases atrial fibrillation (AF). Careful selection of patients in whom PFO is more likely to be the cause of stroke may improve outcomes by avoiding closure in patients unlikely to benefit.
OBJECTIVE: To determine whether the PFO-Associated Stroke Causal Likelihood (PASCAL) classification system identifies who will experience net benefit and net harm from PFO closure.
DESIGN, SETTING, AND PARTICIPANTS: This meta-analysis was a secondary analysis of individual participant-level data from the Systematic, Collabora

OBJECTIVE: To quantify the impact of nonhemodynamically significant patent ductus arteriosus (non-hsPDA) on pulmonary function in neonates.
STUDY DESIGN: We analyzed a retrospective cohort of very low birthweight (<1500 g) infants born <32 weeks' gestation admitted to a single tertiary neonatal intensive care unit between 2019 and 2023 who underwent both echocardiography and pulmonary function testing at approximately 34 weeks postmenstrual age. Infants with ventricular or atrial septal defects were excluded. Passive respiratory compliance (Crs) was compared among infants with hsPDA, non-hsPD

OBJECTIVE: We aimed to evaluate the outcomes of beating-heart atrial septal defect (ASD) repair through a right subaxillary mini-incision in children of different ages and identify the preferred surgical age. METHODS: This retrospective study included 215 children who underwent beating-heart ASD repair via a right subaxillary mini-incision between January 2022 and December 2024. Patients were divided into three age groups: Group 1 (≤ 12 months, n = 47), Group 2 (12–60 months, n = 137), and Group 3 (> 60 months, n = 31). Perioperative variables including cardiopulmonary bypass (CPB) time, opera

BACKGROUND: The role of right ventricular (RV) incision during tetralogy of Fallot (TOF) repair remains controversial. Although RV incisions facilitate the closure of ventricular septal defects (VSDs) and relieve right ventricular outflow tract (RVOT) obstruction, concerns remain regarding late ventricular dysfunction. Alternative approaches that limit or avoid RV incision have been advocated; however, most evidence derives from single-center retrospective reports, leaving the clinical impact uncertain.
METHOD: We retrospectively analyzed 237 patients who underwent repair of TOF at two tertia

BACKGROUND: Atrial septal defects (ASDs) are associated with an increased risk of atrial arrhythmias due to right atrial dilation, electrical remodeling, and conduction abnormalities. In addition to arrhythmias, autonomic dysfunction may also occur. Although several studies have investigated the impact of transcatheter ASD closure on arrhythmia risk in pediatric patients, direct comparative analyses between surgical and transcatheter closure techniques remain limited in the current literature.
METHODS: This study included patients who underwent ASD closure via surgical or transcatheter method

BACKGROUND: Atrial septal defects (ASDs) are a prevalent type of congenital heart disease. Previous GWAS (Genome-Wide Association Studies) have identified common variants associated with ASDs, though their mechanisms remain unknown. We aimed to expand insights into the architecture of common variants associated with ASD risk and elucidate functional mechanisms.
METHODS: We conducted a GWAS using isolated ASD cases and healthy controls and replicated findings in an independent cohort. We examined epigenetic marks within this ASD locus in human induced pluripotent stem cell-derived cardiomyocyt

Patent foramen ovale (PFO) is a common congenital cardiac anomaly, and ultra-long tunnel PFO (> 20 mm) presents a significant interventional challenge despite routine standard closure. A 68-year-old female with recurrent headaches, chest pain history, and prior right coronary artery stenting was confirmed to have a 37-mm ultra-long tunnel PFO via transesophageal echocardiography (TEE) and digital subtraction angiography (DSA). After initial percutaneous closure failure, the patient was successfully treated with a modified trans-tunnel puncture technique and deployment of an Abbott 18/25 mm Amp
