Timely surgical intervention and risk stratification in patients with Ebstein anomaly: a 20-year retrospective cohort study.

BACKGROUND: The optimal timing and choice of surgery for Ebstein anomaly (EA), a complex congenital heart defect, remain challenging due to the heterogeneity and lack of robust long-term data on EA. In this study, we aimed to evaluate the long-term outcomes of surgical management in patients with EA, identify the prognostic risk factors, and develop a predictive model. MATERIALS AND METHODS: We conducted a retrospective analysis of data from 332 patients with EA who were treated at a tertiary center between January 2000 and December 2021. Among them, 269 underwent surgery: tricuspid valve rep

Use of right atrial wall to repair severely dysplastic tricuspid valve in an infant with Ebstein's anomaly.

The management of infants with Ebstein's anomaly is challenging and requires complex involvement of the multidisciplinary team. Surgical repair technique is dependent on the degree of tricuspid leaflet dysfunction and the decision to undergo univentricular or biventricular repair. Patients with a severely dysplastic tricuspid valve are less suited to a cone repair alone and require leaflet reconstruction. Currently used patch materials include autologous untreated pericardium and treated pericardium, with limitations such as unpredictable shrinkage and no growth potential, respectively. To ove

Changing Course-Starnes Takedown in High-Risk Children and Review of the Current Literature.

Neonates with Ebstein anomaly or severe tricuspid valve dysplasia represent a high-risk patient cohort. The modified Starnes procedure was historically performed for cases incompatible with biventricular (BV) repair, leading them towards single-ventricle palliation. However, in recent years, a paradigm shift has emerged in which the Starnes procedure in the acute setting can provide a means for right ventricular (RV) rehabilitation, by providing relief of RV volume overload and time for recovery of RV function, thus serving as a bridge to subsequent one and a half ventricle or BV repair. Herei

Ablation of manifest accessory pathways in pediatric patients: comparison between conventional and non‑radiological navigation techniques.

The purpose was to evaluate clinical characteristics and outcomes of patients under 18 years of age submitted for an electrophysiological study (EPS) for the presence of a manifest accessory pathway (AP) and to compare conventional versus non‑radiological navigation system (NRNS) technics. An observational, multicenter and analytical study was conducted using Latin American Heart Rhythm Society´s pediatric registry. A total of 746 patients underwent EPS due to manifest AP and were all included. The mean age was 10,5 ± 4,4 years, and 471 (63.1%) were male. There were 68 (9,1%) with a structural

Surviving Adulthood with Rare Combined Congenital Heart Defects: Complete AV Canal Defect, Ebstein's Anomaly, and Right Ventricular Hypoplasia.

Background/Objectives. Ebstein's anomaly (EA), which accounts for fewer than 1% of congenital heart diseases, and atrioventricular canal defect (AVCD), present in approximately 4-5% of cases, exceptionally coexist, with this combination observed in fewer than 0.5% of patients with AVCD. We aim to report the oldest documented case of a 45-year-old female with the exceptionally rare combination of complete AVCD, EA, and right ventricular hypoplasia and to provide a concise review of these anomalies. Case presentation. Diagnosed in early childhood with a complete AVCD, pulmonary stenosis, and rig

Ebstein Anomaly Fetal Echocardiogram Parameters are Associated with Illness Severity on Admission to the Cardiac Intensive Care Unit.

BACKGROUND: Ebstein anomaly is associated with a broad spectrum of presentations. Many fetal factors have been associated with intrauterine fetal demise and early neonatal mortality. Little is described about how fetal factors may influence illness severity in the immediate postnatal period. OBJECTIVES: To identify the relationship between fetal echocardiogram measures and severity of illness on admission utilizing pSOFA scores. METHODS: Retrospective chart review of 32 neonates with prenatally diagnosed Ebstein anomaly. Fetal echocardiographic factors assessed included right ventricular press

Risk factors for morbidity and mortality in Ebstein's anomaly: a registry-based study of 398 patients.

OBJECTIVES: To investigate the long-term outcomes and risk factors for morbidity and mortality in patients with Ebstein's anomaly, including the effects of type and timing of valve surgery. METHODS: For this retrospective, record-based study, all patients with Ebstein's anomaly enrolled in the German National Register for Congenital Heart Defects up to June 2021 were eligible for inclusion. RESULTS: Non-surgical patients (n = 194/49% of 398 patients) had less tricuspid valve regurgitation (p < 0.001) and heart failure symptoms (p < 0.001) than surgical patients (n = 204/51%). Role of speckle tracking imaging in risk stratification in Ebstein anomaly.

BACKGROUND: Certain factors such as left ventricular (LV) geometry and loading conditions affect the validity and reliability of LV ejection fraction (LVEF) as a true measure of LV contractility. LV global longitudinal strain (LVGLS) is less sensitive to these factors, and it has superior prognostic performance in patients with acquired heart disease. We hypothesized that LVGLS had a stronger correlation with heart failure indices and transplant-free survival compared to LVEF. METHODS: Retrospective cohort study of 673 adults with Ebstein anomaly (2003-2018). RESULTS: Compared to LVEF, LVGLS

Total 3d endoscopic approach to complete repair of Ebstein's anomaly: a case series.

BACKGROUND: Ebstein anomaly is a rare congenital malformation of the tricuspid valve. Although robotic-assisted minimally invasive repair has shown promising results, its high cost limits accessibility in developing countries. We report our initial experience with a totally endoscopic cone repair using three-dimensional (3D) visualization and standard thoracoscopic instruments, performed without robotic assistance, for the treatment of Ebstein anomaly. METHODS: This retrospective case series included five adult patients who underwent total 3D endoscopic tricuspid valve cone reconstruction at E

Left ventricular outflow tract obstruction after Ebstein tricuspid valve surgery: a case report.

INTRODUCTION AND IMPORTANCE: Left ventricular outflow tract obstruction (LVOTO) is a rare complication after surgery for Ebstein's anomaly. Early detection of this complication is crucial. CASE REPORT: We report a case of an elderly woman with Ebstein's anomaly who developed LVOTO after cone reconstruction. She was discovered in time and underwent tricuspid valve replacement, saving her life. CLINICAL DISCUSSION: This rare complication of cone-plasty has taught us lessons. Early diagnosis and clinical monitoring of ultrasound during the clinical process and reduction of multiple injuries dur

Ebstein's Enigma: Managing Unforeseen Interventricular Septal Hematoma After One and Half Ventricular Repair for Ebstein's Anomaly.

Interventricular septal hematoma (IVSH) is an extremely rare complication reported after the congenital heart surgery. An IVSH is defined as a hemorrhagic dissection between muscle bundles of the interventricular septum. Herein, we discuss a case of IVSH, which developed following one and half ventricular repair for Ebstein's anomaly in a 47-year-old lady. It appeared initially as an intraventricular mass on postoperative echocardiography creating a diagnostic dilemma. It was confirmed to be an IVSH on intraoperative transesophageal echocardiography during surgical re-exploration indicated for

Advanced Echocardiographic Characterization of Neonatal Ebstein's Anomaly Using Myocardial Deformation Imaging: A Single-Center Study.

BACKGROUND: Neonatal Ebstein's anomaly (EA) is a severe condition with significant hemodynamic instability and early myocardial dysfunction, where abnormal right-heart geometry limits conventional echocardiography and highlights the value of myocardial deformation imaging. METHODS: We conducted a single-center retrospective observational study including 16 neonates with EA and 26 healthy neonates. All subjects underwent comprehensive transthoracic echocardiography during the neonatal period. Conventional two-dimensional imaging and speckle-tracking echocardiography (STE) were used to assess b
