Damus-Kaye-Stansel anastomosis with pulmonary arterial trunk reconstruction using valved conduit 20 years after Fontan procedure.

We report a case of 24-year-old man with single ventricle congenital heart disease and concomitant familial Fabry disease who developed progressive subaortic stenosis 22 years after Fontan procedure. Despite stable ventricular volumes, subaortic stenosis developed due to Fabry disease-related myocardial hypertrophy. He underwent pulmonary arterial trunk reconstruction with valved conduit and Damus-Kaye-Stansel (DKS) anastomosis. Postoperatively, smooth outflow tracts and stable valve function were observed. This case highlights the impact of Fabry disease on ventricular outflow tract obstructi

Impact of cardiovascular comorbidities on echocardiographic parameters in aortic stenosis.

INTRODUCTION: Comorbidities in aortic stenosis (AS) significantly impact treatment outcomes by influencing intervention timing, choice, and prognosis. Diseases affecting cardiac hemodynamics independently of AS progression may distort echocardiographic interpretation, risking misclassification of AS severity. Understanding how comorbidities alter key echocardiographic parameters may facilitate more precise evaluation. AIM: To assess the impact of common cardiovascular comorbidities on echocardiographic evaluation of AS. MATERIAL AND METHODS: Medical records of 234 hospitalized patients with

Vitamin K-Dependent Inhibition Mechanisms in Human Aortic Valve Calcification.

Introduction: Stenosis of the aortic valve is a leading cause of severe cardiovascular lesions. Progressive calcification, rheumatic modifications and also congenital events are the main etiopathogenetic factors. Extended fibrotic changes and aberrant ectopic calcification of the specific aortic valve interstitial cells are the most recognizable histopathological features. In fact, the previously referred cells are transformed from their initial myofibroblast phenotype to an osteoblast-like cell formation mediated by an inflammatory process. Concerning the potentially effective anti-calcificat

Alternative Transaxillary Access for Transcatheter Aortic Valve Implantation.

Background/Objectives: Currently, the transfemoral approach is recognized as the primary method for accessing transcatheter aortic valve implantation (TAVI). However, alternative techniques are needed when the transfemoral access is not suitable. We proposed that a modified transaxillary approach through the distal left axillary artery is both viable and safe for conducting TAVI, potentially offering benefits for patients. Methods: From December 2018 to February 2024, a total of 24 patients (7 women, average age 77.9 ± 8 years) received TAVI using transaxillary access via the left axillary art

Characterizing Periprocedural Care for Pediatric Patients With Williams Syndrome Undergoing General Anesthesia at a Tertiary Pediatric Hospital.

Williams syndrome is a rare congenital disorder affecting connective tissue and the cardiovascular and central nervous systems. Pediatric patients diagnosed with Williams syndrome face significant risk for cardiac collapse and death when undergoing anesthesia. We sought to evaluate our institution's historical practices, evaluate individual risk stratification, and create detailed standardized perianesthesia guidelines for management of this population, particularly during noncardiac procedures. The study included a retrospective chart review of pediatric patients with Williams syndrome who re

Staged biventricular repair in aortic hypoplasia: the value of intraoperative myocardial perfusion.

The patient had a hypoplastic aortic arch with coarctation, a right aberrant subclavian artery, a bicuspid aortic valve, a ventricular septal defect and severe long-segment left ventricular outflow tract obstruction. The patient underwent initial univentricular palliation with subsequent biventricular conversion. Initially the patient had a Norwood operation with a Sano conduit at 23 days of age. The operation was performed on a beating heart with full-body perfusion for optimal preservation of heart cardiac function. At 9 months of age, the patient had a biventricular conversion that required

Disruption of Notch1 and Gata5 in Mice Leads to Congenital Aortic Valve Disease.

Here, we describe Notch1;Gata5 compound mutant mice as a novel mouse model of highly penetrant congenital aortic valve disease displaying bicuspid aortic valve and progressive aortic valve stenosis. Further, we find downregulation of smooth muscle genes in the neonatal aortic valves in Notch1;Gata5 compound mice consistent with an immature valve phenotype. Our findings demonstrate a novel genetic interaction between Notch1 and Gata5 in mice that is critical for proper aortic valve development. This novel model is an important tool to define dysregulated signaling pathways for congenital aortic

Adult Transcatheter Pulmonary Valve Replacement.

Transcatheter pulmonary valve replacement (TPVR) is one of the structural interventions developed over the past 25 years. It is being used for patients with moderate to severe pulmonary stenosis and regurgitation. It is an alternative and preferred option to surgical pulmonary valve replacement (SPVR). Common indications include isolated congenital native valve pulmonary stenosis, right ventricular outflow tract obstruction with or without right ventricular conduit stenosis, severe pulmonary regurgitation with right ventricular dysfunctions, and lastly, bioprosthetic pulmonary valve dysfunctio

Assessing the Utility of Targeted Neonatal Echocardiography for Congenital Heart Disease Detection: Retrospective Cohort Analysis.

PURPOSE: Targeted neonatal echocardiography (TNE) is increasingly utilized by neonatologists to assess hemodynamics, particularly when cardiology-performed echocardiography (CPE) is unavailable. It is crucial that TNE screens and identifies structural abnormalities. This study evaluated the agreement between TNE and CPE in detecting significant congenital heart disease (CHD). METHODS: This retrospective, single-center cohort study included infants who underwent TNE between 2015 and 2019 and had at least one complete CPE before discharge. Infants with a known CHD diagnosis prior to TNE were ex

Elusive Barriers: The Challenges of Diagnosing Subaortic Membranes.

Subvalvular aortic stenosis (SAS) is a relatively uncommon cause of left ventricular outflow tract (LVOT) obstruction, constituting only 8-20% of cases. Among the etiologies, subaortic membranes (SAoM) are the most prevalent, manifesting in various anatomical forms, including thin discrete membranes, fibromuscular ridges, and diffuse tunnel-like narrowings. While transthoracic echocardiography (TTE) is the primary diagnostic tool, it often presents challenges, particularly in cases where the membrane is not readily visible, and needs further imaging with transesophageal echocardiogram (TEE) or

Long-term clinical outcomes in adult congenital aortic stenosis: results from a national study.

BACKGROUND: Previous research on congenital aortic stenosis (AS) mainly focused on children, while data on long-term outcomes in adults is scarce. Therefore, this study aims to evaluate outcomes in adult patients with congenital AS and identify prognostic markers for aortic valve replacement (AVR). METHODS: In this multicentre study, patients aged 18-55 years with congenital AS (peak velocity ≥2.5 m/s) registered in the Dutch CONCOR (Congential Cor Vitia) registry from expert centres for congenital heart disease between 2001 and 2019 were included. Exclusion criteria were severe aortic regurg

Hypercontractile phenotype in hypertrophic cardiomyopathy indicates unfavorable hemodynamics, coronary flow and prognosis.

AIMS: Our aim was to assess the clinical and prognostic significance of the left ventricular (LV) hypercontractile phenotype (HP) in hypertrophic cardiomyopathy (HCM), which until now remains unclear. METHODS AND RESULTS: We enrolled 1533 HCM patients (age 51 ± 15 years, 965 males, 63%) with ejection fraction (EF) ≥ 50%, referred for rest transthoracic echocardiography (TTE) in 27 laboratories from 13 countries. Two-dimensional volumetric TTE assessment included LV outflow tract gradient (LVOTG), LV EF, and LV force (systolic blood pressure + LVOTG/ESV, mmHg/mL). HP was defined as the highest

Surgical Aortic Valve Replacement Combined With Coronary Artery Bypass Grafting in a Patient With Progeria.

BACKGROUND: Calcific aortic stenosis is increasingly recognized as a major determinant of mortality in the aging subset of patients with the ultra-rare Hutchinson-Gilford progeria syndrome (HGPS), particularly as survival improves with lonafarnib therapy. However, the optimal treatment strategy for severe aortic stenosis in this population remains undefined. CASE SUMMARY: We report the first successful case of combined surgical aortic valve replacement and coronary artery bypass grafting in a severely symptomatic 21-year-old man with HGPS, severe calcific aortic stenosis, and coronary artery

Subaortic Membrane With a Patent Ductus Arteriosus in an Adult Patient: A Challenging Diagnostic Dilemma.

Subaortic membrane (SAM) is a subtype of left ventricular outflow obstruction, rarely seen in adults. In some cases, SAM may be associated with other congenital defects. The association of patent ductus arteriosus (PDA) and SAM is the rarest, especially in adult patients. The management of these patients is delicate in terms of diagnosis, timing, and the appropriate surgical technique. A 34-year-old patient with a known aortic heart murmur presented with exertional syncope. She was referred to our hospital for aortic valve replacement due to symptomatic aortic stenosis. Transthoracic and trans

Balloon vs. self-expanding valves for transcatheter aortic valve implantation in bicuspid aortic stenosis: a meta-analysis.

AIMS: Bicuspid aortic valve (BAV) is a common congenital heart disease. However, pivotal randomized trials of transcatheter aortic valve replacement (TAVR) have excluded this population. There remains a lack of consensus on the optimal choice between balloon-expandible valves (BEVs) and self-expanding valves (SEVs) in this setting. This study aimed to compare the efficacy and safety of BEVs vs. SEVs in patients with BAV stenosis. METHODS: A systematic search using four databases, including PubMed, Embase, Web of Science, and Cochrane CENTRAL, was conducted from inception to 26 November, 2024.

An Expanding Perspective: Extreme Aortic Dilation in a Patient With Single Ventricle Palliation.

Aortic dilation following single ventricle palliation is increasingly recognized, although its long-term clinical implications remain unclear. We present the case of a 38-year-old male born with a bicuspid aortic valve, d-transposition of the great arteries, ventricular septal defect, right ventricular hypoplasia with a straddling right atrioventricular valve. He was palliated with Fontan circulation and developed severe neo-aortic root and arch dilation with native aortic valve stenosis, requiring surgical intervention. His neo-aortic root measured 81 × 65 mm, necessitating a Bentall procedur

Iatrogenic Reopening of a Congenital VSD After TAVR.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) has become an increasingly used technique for patients with severe aortic stenosis who are at high risk of perioperative mortality during surgical valve replacement. CASE SUMMARY: Here we describe the first case to our knowledge of a patient with a congenital perimembranous ventricular septal defect with prior spontaneous closure who experienced a post-TAVR acquired intracardiac shunt (AICS) due to the reopening of her ventricular septal defect, which caused new-onset heart failure and required surgical closure. DISCUSSION: AICS is a
