BACKGROUND: Women with left ventricular outflow tract obstruction (LVOTO) are at risk of adverse pregnancy outcomes, which have been related to the severity of the LVOTO. However, the physiological adaptations of pregnancy increase the gradient across an LVOTO, often altering the clinical perception of risk. Here, we examined how the LVOTO gradient changes during pregnancy, investigating whether a greater increase in the gradient is associated with worse pregnancy outcome and whether any increase in the gradient is transient, reversing after the end of the pregnancy.
METHODS: The study was co

Aortic stenosis (AS) represents a prevalent valvular heart disease, particularly affecting the elderly population, with substantial implications for morbidity and mortality. This review examines the epidemiology, pathophysiology, management strategies, complications, and future implications of AS and its treatment modalities, focusing primarily on transcatheter aortic valve replacement (TAVR). The prevalence of AS is rising as the population ages, with notable differences observed across various demographic groups and regions. Additionally, AS can also occur in younger adults, typically due to

Inferior vena cava (IVC) agenesis is a rare congenital vascular anomaly that may predispose young individuals to venous thromboembolism in the absence of classical risk factors. We report the case of a 35-year-old man, an active smoker with no significant medical history, who presented with a 10-day history of bilateral lower limb swelling and severe back pain radiating to the lower limbs. Doppler ultrasonography demonstrated extensive bilateral deep vein thrombosis (DVT), involving the right external iliac, common femoral, and femoral veins, with complete thrombosis of the left lower limb ven

BACKGROUND: A right aortic arch withmirror-image branching is a rare congenital anomaly resulting from the abnormal regression ofthe embryonic left aortic arch. This variant, in which the aortic arch courses to the right of the trachea and gives rise to its branches in a mirrored configuration compared to the standard left-sided arch, can present unique challenges during cardiovascular interventions such as transcatheter aortic valve replacement (TAVR). CASE REPORT: We describe a 75-year-old male undergoing evaluation for TAVR who was incidentally found to have a right aortic arch with mirror-

Unicuspid aortic valves (UAVs) are rare congenital anomalies associated with severe aortic stenosis (AS) in infancy or later adulthood. This is a case of a 22-year-old woman who presented with acute hypoxemic respiratory failure, which was initially attributed to multilobular pneumonia with acute respiratory distress syndrome. However, echocardiography revealed UAV with severe stenosis, suggesting cardiogenic pulmonary edema. The patient underwent successful surgical aortic valve replacement. This case highlights that the diagnosis and management of UAVs is tenuous: cases are prone to rapid de

INTRODUCTION: In aortic valve disease, several interventions are available. In young people who are still growing, or considering the risks of long-term anticoagulation, the Ross procedure remains an alternative for aortic valve replacement. This procedure entails the transposition of the patient's pulmonary valve to the aortic position, with placement of a homograft in the pulmonary position. However, long-term prognosis remains largely unknown.
METHODS: The Swedish national registry of congenital heart disease was searched for adult patients with a history of Ross operation.
RESULTS: 82 pa

Bicuspid aortic valve (AV) is the most common congenital cardiac valvular anomaly and often progresses to severe calcific aortic stenosis (AS). Although surgical AV replacement has been the conventional treatment, transcatheter AV implantation (TAVI) is emerging as a viable alternative, especially in patients with high surgical risk or complex anatomy. This case report demonstrates the crucial role of echocardiography in the management of TAVI in a patient with bicuspid AS. A 66-year-old male with a 3-year history of palpitations and recurrent presyncope underwent a comprehensive evaluation fo

BackgroundCongenital aortic valve stenosis (AVS) is the most common cause of ventricular outflow tract obstruction. Surgical aortic valvuloplasty (SAV) and balloon aortic valvuloplasty (BAV) are the most common primary interventions, with conflicting data regarding their long-term outcomes. We aimed to study the long-term clinical outcomes of patients allocated to BAV or SAV.MethodsWe searched PubMed, Scopus, Web of Science, and Cochrane CENTRAL through August 2025 for relevant studies reporting long-term outcomes of SAV versus BAV in patients with congenital AVS. The primary outcome was the 1

BACKGROUND: Accessory tricuspid valve (TV) is a rare congenital anomaly usually associated with other cardiac anomalies. It is typically classified as mobile or fixed type; both are previously described as arising exclusively from the right ventricular septum. We report the first known case of a straddling accessory TV anchoring to the left ventricular outflow tract (LVOT) wall as well, causing severe LVOT obstruction in a neonate with coarctation complex.
CASE SUMMARY: A female neonate was diagnosed with coarctation of the aorta, ventricular septal defect (VSD), persistent left superior vena

BACKGROUND: Quadricuspid aortic valve (QAV) is a rare congenital anomaly with an incidence of 0.006% to 0.05%. It often leads to aortic stenosis or regurgitation due to progressive fibrosis and calcification. While surgical aortic valve replacement has been the standard treatment, transcatheter aortic valve implantation (TAVI) is emerging as a less invasive alternative for high-risk patients.
CASE SUMMARY: An 84-year-old woman with ischemic cardiomyopathy and heart failure presented with severe aortic stenosis on a calcified QAV. Given her high surgical risk (EuroSCORE II: 9.69), she underwen

Calcific aortic valve stenosis (CAVS) is the most common valvular heart disease requiring intervention. Progressive fibrocalcific thickening of the valve leaflets results in left ventricular outflow tract obstruction. Once symptoms emerge, CAVS is associated with high morbidity and mortality, contributing to over 100,000 deaths globally each year. The worldwide prevalence of CAVS is rising, affecting a 9.6 to 12.6 million people according to Global Disease Burden estimates, thereby becoming an increasingly public health burden. This narrative review summarizes current knowledge on the epidemio

BACKGROUND: While athletic patients may exhibit bradycardia, significant exertional symptoms in the setting of a bicuspid aortic valve (BAV) warrant further investigation.
CASE SUMMARY: A 36-year-old male triathlete presented with progressive exercise intolerance, fatigue, and bradycardia. Electrocardiogram demonstrated complete atrioventricular block (junctional ventricular escape: 52 beats/min). Echocardiography revealed a moderately calcified BAV with mild to moderate stenosis and preserved ventricular function. Cardiac imaging demonstrated a Sievers type 0 valve with asymmetric extravalvu

The coarctation of the aorta is a congenital anomaly characterized by a local narrowing of the aortic lumen localized near the ductus arteriosus. Typically diagnosed in childhood, but it can remain until symptoms become evident. This aortic anomaly can also coexist with aortic valve stenosis. In our case report, we present a 46-year-old male with chest pain, dyspnea, and a significant blood pressure gradient between upper and lower extremities. Diagnostic examination included transthoracic echocardiography and computerized tomography. This diagnostic imaging showed narrowing of the aortic lume

BACKGROUND: The optimal timing of surgery for discrete subaortic stenosis (DSS) is controversial due to limited evidence from large-scale trials. This study aimed to identify prognostic factors and reassess current surgical thresholds.
METHODS: A total of 508 patients with DSS, including 375 pediatric patients, who underwent surgery at a single tertiary center between May 2018 and March 2025 were retrospectively analyzed. The primary endpoint was a composite of DSS-related adverse events, and the secondary endpoint was aortic valve (AV) dysfunction. Multivariable Cox regression and receiver o

Discrete subaortic stenosis (DSS) is a congenital heart disease in which a fibrotic membrane forms below the aortic valve; the underlying cellular mechanisms are currently unknown. Since an elevated pressure gradient in the left ventricular outflow tract (LVOT) is a distinguishing feature of DSS, it is hypothesized that the membrane formation is caused by elevated wall shear stress applied to the endocardial endothelial cells (EECs) that line the LVOT, triggering fibrosis. To correlate shear stress to an EEC fibrotic phenotype, we applied fluid shear stress to EECs at physiological and patholo

Noonan syndrome (NS) is a rare multisystemic condition among the RASopathy group, characterized by a broad phenotypic spectrum and genetic variability. It results from pathogenic variants in genes regulating the RAS/MAPK pathway, affecting cell proliferation and differentiation. While the PTPN11 gene accounts for approximately 50% of cases, other genes, including MRAS, have been implicated. NS presents with features such as facial dysmorphism, short stature, and cardiac anomalies. Hypertrophic cardiomyopathy (HCM) is a major contributor to mortality, with specific variants conferring higher ri

Background and Clinical Significance: Subvalvular aortic stenosis (SAS) is the second most common form of aortic stenosis after valvular disease and predominantly affects male patients. It is frequently associated with other congenital cardiac anomalies, such as ventricular septal defect, and is rarely diagnosed during infancy. Instead, SAS typically manifests during childhood or adulthood as a progressive left ventricular outflow tract obstruction, leading to left ventricular hypertrophy and, in many cases, aortic regurgitation. Case Presentation: The first patient was a 61-year-old man prese

AIMS: Hypertrophic cardiomyopathy (HCM) shows variable left ventricular hypertrophy, yet current diagnostic criteria rely on absolute wall thickness (WT) cut-offs that may miss early or subtle phenotypes, particularly in sarcomere mutation carriers.
OBJECTIVES: To determine whether wall thickness standard deviation (WTSD), reflecting WT heterogeneity, improves identification of HCM and mutation carriers.
METHODS AND RESULTS: Cardiac magnetic resonance was performed in 382 healthy controls, 297 patients with HCM, 82 sarcomere mutation carriers without overt hypertrophy, and 180 patients with

BACKGROUND: Congenital aortic stenosis (ConAoS) accounts for approximately 4%-8% of all congenital heart diseases. Chronic pressure overload may result in myocardial stiffening, leading to impaired cardiac filling, reduced contractility and ultimately contributing to the onset of symptomatic heart failure. Shear wave elastography (SWE), using high-frame-rate echocardiography, allows visualisation of myocardial shear waves. By measuring shear wave propagation velocity (SWV), this technique may enable non-invasive assessment of myocardial stiffness. This study aimed to explore the feasibility of
