Transcatheter Mitral and Aortic Paravalvular Leak Closure: Innovations, Challenges, and Future Directions.

Paravalvular leak (PVL) is a well-known complication of valve replacement, occurring in up to 10% of aortic and 17% of mitral prosthetic valves. Although many leaks are small and clinically insignificant, ≈1% to 5% of cases are associated with clinically relevant complications, such as heart failure symptoms and hemolytic anemia. Surgical reoperation is currently considered the first-line option; however, it encompasses high mortality and morbidity risks. In the past years, transcatheter PVL closure has proven to be highly effective when performed in experienced centers. Transcatheter PVL clos

The pivotal role of echocardiography in structural heart interventions: from planning to success.

Structural heart disease (SHD) encompasses a wide range of congenital and acquired cardiac abnormalities, increasingly treated with transcatheter interventions. Echocardiography plays a crucial role in optimising these procedures, providing high-resolution imaging for pre-procedural planning, real-time guidance and post-intervention assessment. The shift from two-dimensional to advanced three/four-dimensional (3D/4D) echocardiography has significantly enhanced the visualisation of complex cardiac structures, improving procedural success and patient safety. The purpose of this review article is

Genotype-Guided Risk Stratification of Mitral Valve Surgery in Marfan Syndrome.

BACKGROUND: Although genotype-based risk stratification for aortic disease has been extensively studied in Marfan syndrome (MFS), mitral valve disease has received less attention despite being a major cardiovascular complication requiring surgery in up to 16% of cases. OBJECTIVES: The authors aimed to evaluate genotype-specific differences in mitral valve disease progression and surgical intervention to inform precision medicine approaches in MFS. METHODS: This retrospective cohort study included 437 MFS patients with pathogenic FBN1 variants (2006-2024). Variants were classified by molecula

Microfibril Denudation Marks Early Aortic Vulnerability in SMAD3 Variant Carriers: A Multigenerational Study.

BACKGROUND: Individuals with Loeys-Dietz syndrome 3 are at risk for thoracic aortic dissection even if their aortas are not substantially dilated. A pathogenic variant of SMAD3 is a genetic trigger, but the cellular programs that underlie the heightened aortic risk remain poorly understood. METHODS: We ascertained the cardiovascular profile of 57 individuals in a 4-generation family with a missense variant in SMAD3 (p.Arg287Gln). Aortic medial architectures were quantified from aortas harvested during thoracic aortic replacement surgery, with 2-dimensional and 3-dimensional profiling of the e

Bailout valve-in-valve therapy during transcatheter aortic valve replacement in bicuspid and tricuspid aortic stenosis: an observational single-centre study in China.

BACKGROUND: Bailout valve-in-valve (ViV) transcatheter aortic valve replacement (TAVR) is a critical rescue strategy for procedural failure, yet evidence regarding its outcomes in bicuspid aortic valve (BAV) anatomy remains limited. METHODS: This retrospective, single-centre study analysed 1597 patients (48.3% BAV) undergoing TAVR. Patients were stratified by the requirement for bailout ViV, which was conducted for significant residual aortic regurgitation (AR) or valve embolisation. Predictors were identified via multivariate logistic regression. Early- and mid-term survival outcomes were co

Accessory mitral valve tissue causing left ventricular outflow tract obstruction associated with subaortic membrane and aortic sinus aneurysm: A case report and literature review.

Accessory mitral valve tissue is a rare congenital cardiac anomaly that can lead to left ventricular outflow tract obstruction. We present the case of an older female patient with accessory mitral valve tissue, a subaortic membrane, an unruptured aneurysm of the aortic sinus, and left ventricular outflow tract obstruction. Successful excision of the accessory mitral valve tissue and repair of the aortic sinus were performed. Postoperative echocardiography showed complete removal of the subaortic membrane, with a residual accessory mitral valve tissue (5 mm). Color Doppler imaging revealed a si

Understanding the hemodynamic changes in fetuses with coarctation of the aorta using a lumped model of fetal circulation.

Coarctation of the aorta (CoA) is a common congenital heart defect characterized by aortic narrowing. Prenatally, it has mild hemodynamic effects as right ventricular disproportion and ductus arteriosus (DA) dilation occur as adaptive mechanisms, but their impact on CoA hemodynamics remains poorly understood. To investigate this, we built a closed 0D computational model of fetal circulation and simulated different CoA cardiovascular remodeling patterns, including aortic isthmus (AoI) narrowing, ventricular disproportion, and DA dilation. Our results showed mild AoI narrowing (80% of reference

Case Report: A novel compound heterozygosity of the EVC2 gene identified in a Chinese pedigree with congenital heart defect.

BACKGROUND: Congenital heart defects (CHDs) represent the leading cause of neonatal mortality among congenital abnormalities. Genetic factors, such as EVC2 gene mutations and other genetic alterations, constitute a major cause of CHD. Thus, determining the genetic etiology of fetal CHDs is crucial for optimizing pregnancy management and informing future reproductive decisions. CASE PRESENTATION: Here, we describe a male fetus with complex CHD who was diagnosed at 25 weeks of gestation, delivered at full term, and died prematurely within a month due to heart failure. The cardiac abnormalities

Aortic stiffness assessed by blood pressure and echocardiography in young and normotensive patients with isolated aortic coarctation versus those with aortic coarctation and ventricular septal defect.

BACKGROUND: Aortic coarctation can occur isolated or associated with ventricular septal defect. This study evaluated aortic stiffness in normotensive patients surgically treated for aortic coarctation and ventricular septal defect and in those who underwent simple aortic coarctation repair. Both groups were compared with healthy controls. Again, the two pathological groups were compared with each other regarding aortic stiffness and left ventricular diastolic function. A possible relationship between aortic stiffness and left ventricular diastolic function was investigated. METHODS: Twenty-tw

Automated, Standardized, Quantitative Analysis of Cardiovascular Borders on Chest X-Rays Using Deep Learning.

BACKGROUND: The analysis of cardiovascular borders (CVBs) in chest x-rays (CXRs) traditionally relied on subjective assessment and does not have established normal ranges. OBJECTIVES: The authors aimed to develop a deep learning-based method for quantifying CVBs on CXRs and to explore its clinical utility. METHODS: This study used a prevalidated deep learning to analyze CVBs. A total of 96,129 normal CXRs from 4 sites were used to establish age- and sex-specific normal ranges of CVBs. The quantified CVBs were standardized into z-scores for newly inputted CXRs. The clinical utility of the z-s

Automated detection of pediatric congenital heart disease from phonocardiograms using deep and handcrafted feature fusion.

Congenital heart disease (CHD) is the most common type of birth defect, impacting about 1% of live births worldwide. Echocardiography, the gold-standard diagnostic method, is costly and inaccessible in low-resource settings. Diagnosis is delayed due to limited skilled experts, whose ability to interpret pathological patterns varies significantly, causing inter- and intra-clinician variability. Therefore, we present a new method for a more accessible diagnostic modality, the digital stethoscope, to detect CHDs. Our method is based on deep feature fusion, integrating deep and handcrafted feature

Human Genetics of Hypoplastic Left Heart Syndrome.

Hypoplastic left heart syndrome (HLHS) is a severe congenital cardiovascular malformation characterized by hypoplasia of the left ventricle, aorta, and other structures on the left side of the heart. The pathologic definition includes atresia or stenosis of both the aortic and mitral valves. Despite considerable progress in clinical and surgical management of HLHS, mortality and morbidity remain concerns. One barrier to progress in HLHS management is poor understanding of its cause. Several lines of evidence point to genetic origins of HLHS. First, some HLHS cases have been associated with cyt
