CCDC39 Mutation-Related Primary Ciliary Dyskinesia with Congenitally Corrected Transposition of the Great Arteries: A Case Report.

BACKGROUND Primary ciliary dyskinesia (PCD) is an uncommon autosomal recessive disease resulting from dysfunction of motile cilia that causes impaired mucociliary clearance and abnormal embryonic left-right axis differentiation. The CCDC39 gene is a known cause of PCD, which is most commonly associated with inner dynein arm defects and microtubular disorganization. Although heterotaxy-related congenital heart defects are well described in PCD, their presence in patients with CCDC39 mutations, particularly those with congenitally corrected transposition of the great arteries (ccTGA), has not be

A Study of the Frequency of Lead Reversal at a Tertiary Care Institution.

BACKGROUND: Electrocardiography (ECG) remains an important point-of-care investigative modality in the screening and diagnosis of cardiac diseases due to rapidity, cost-efficiency, and a gentle learning curve for most emergent patterns. Mechanistical problems in recording ECGs can lead to distorted graphical representations of cardiac electrophysiology, which can lead to incorrect or obstructed care for the patient. AIM: To identify the prevalence of ECG lead reversal in a tertiary care hospital over two months and to discuss the various patterns of ECG lead misplacement. METHODS: A total of

Valve-In-Valve TAVI in a Patient With Dextrocardia, Situs Inversus, and Challenging Commissural Alignment.

Valve-in-valve transcatheter aortic valve implantation (ViV-TAVI) for dextrocardia is challenging. An 81-year-old man with dextrocardia and situs inversus developed prosthetic aortic stenosis. Loading Evolut FX+ C-tab 180° upside down in a delivery catheter facilitates commissural alignment during ViV-TAVI in dextrocardia. To accommodate anatomical variations, the flush-port position can be adjusted. In this case, the flush port was positioned 5-o'clock (60° from the usual 3-o'clock position) to align the TAVI valve with former prosthesis, as the former prosthetic valve commissure was oriented

Incidental Diagnosis of Situs Inversus Totalis in a 45-Year-Old Male Who Presented With Acute Asthma Exacerbation: A Case Report and Brief Literature Review.

Incidental situs inversus totalis (SIT) requires thorough anatomical mapping to exclude associated syndromes (e.g., Kartagener) and congenital anomalies. Early identification, patient education on mirrored anatomy, and multidisciplinary coordination are essential to prevent iatrogenic errors during future interventions, even in asymptomatic adults.

The role of multidisciplinary team discussions in the management of thoraco-omphalopagus conjoined twins: A case report from a low-resource setting.

INTRODUCTION AND IMPORTANCE: Conjoined twins represent a rare medical phenomenon that poses significant clinical and ethical challenges, particularly in resource-limited settings. CASE PRESENTATION: We present a case of conjoined twins delivered at 39 weeks gestation to a 27-year-old refugee mother. The delivery was by emergency cesarean section due to footling breech presentation. Initial assessments revealed shared cardiac anatomy, with Twin A exhibiting a more favorable anatomical structure compared to Twin B, who presented with severe cardiac malformations. Follow-up imaging, including CT

Pulsed field ablation of atrial fibrillation via jugular transseptal puncture in a patient with dextrocardia and interrupted inferior vena cava: a case report.

BACKGROUND: Situs viscerum inversus with dextrocardia and interrupted inferior vena cava (I-IVC) are rare congenital diseases that might challenge interventional procedures, such as transcatheter ablation for atrial fibrillation (AF), when transseptal puncture (TSP) is required for left atrial access. Recently, one-shot pulsed field ablation (PFA) has emerged as a safe and effective energy modality for AF ablation. We present a case report on jugular TSP guiding one-shot PFA in a patient presenting dextrocardia and an I-IVC, addressing the unique anatomical challenges. CASE SUMMARY: A 52-year

Successful Right Sided Subcutaneous Implantable Cardioverter-Defibrillator Implantation in Situs Inversus Dextrocardia: A Case Report and Literature Review.

This case report details the successful implantation of a subcutaneous implantable cardioverter-defibrillator (S-ICD) in a 70-year-old male with situs inversus dextrocardia, a rare congenital condition where the heart and visceral organs are reversed. The patient, with a history of ischemic heart disease and severe left ventricular dysfunction (ejection fraction 25%), required primary prevention of sudden cardiac death. Due to anatomical challenges with transvenous ICD placement and no need for pacing or resynchronization, an S-ICD was selected. Pre-procedural screening was modified by reversi

An incidental situs Inversus Totalis in a Pediatric case of granulomatous Panuveitis. A case report.

Situs inversus totalis (SIT) is a rare congenital abnormality characterized by a mirror-image transposition of thoracic and abdominal organs. Although SIT is a recognized congenital anomaly, it is often unfamiliar to physicians because it is typically asymptomatic and discovered incidentally. We report the case of a 7-year-old girl from Central Honduras who presented with decreased visual acuity in the right eye, and was diagnosed with granulomatous panuveitis. Clinical evaluation revealed a right-sided apex beat, systolic murmur, and left-sided hepatic dullness. Electrocardiogram showed right

Successful left lateral AP ablation with transseptal puncture in a patient with situs inversus totalis and dextrocardia.

Wolff-Parkinson-White syndrome, characterised by accessory pathways, is rarely seen with dextrocardia. We present a case of situs inversus-dextrocardia with Wolff-Parkinson-White syndrome successfully treated via catheter ablation using 3D mapping (EnSite Precision®). Adjustments included reversed electrocardiogram (ECG) electrode placement, EnSite patch positioning, and fluoroscopic views. Coronary sinus access required counterclockwise manoeuvers. Mapping identified a left-sided accessory pathway, necessitating transseptal puncture with mirror-image adjustments. Ablation at the optimal site

A Mirror-Image Heartache: Reflections on a False Diagnosis of Acute Coronary Syndrome in a Patient With Situs Inversus Totalis.

Situs inversus totalis (SIT) is a rare congenital anomaly characterized by complete mirror-image transposition of the thoracic and abdominal organs. Unrecognized SIT can create diagnostic confusion in patients presenting with chest pain, as standard electrocardiographic (ECG) lead placement may generate waveforms that mimic acute coronary syndromes (ACS). We report the case of a 30-year-old physically fit man who presented with acute central chest pain following exertion. Initial investigations revealed nonspecific ECG abnormalities and a mildly elevated high-sensitivity troponin level, leadin

Mesocardia With Upward and Leftward Apex Secondary to a Large Hiatal Hernia: A Case Report.

We report the case of an 82-year-old woman with a known history of hiatal hernia who presented with intermittent chest discomfort and exertional dyspnea. Electrocardiography revealed sinus rhythm with axis deviation and a prominent R wave in lead aVR, initially raising concern for limb lead reversal, conduction abnormality, or myocardial ischemia. Chest radiography demonstrated a retrocardiac air-fluid level, and computed tomography (CT) confirmed a large type III hiatal hernia displacing the heart centrally, with the apex directed upward and leftward. No congenital structural cardiac abnormal
