Reinterventions and medical costs after tetralogy of Fallot repair: a retrospective cohort study using health insurance claims in Japan.

BACKGROUND: Reinterventions after congenital heart disease surgery include not only reoperations but also medical catheter interventions, and the details of these treatment realities are often unclear. This study aimed to elucidate the medical and surgical reinterventions and associated medical costs after the tetralogy of Fallot (TOF) repair using Japanese health insurance claims data. METHODS AND RESULTS: We analyzed reinterventions and medical costs from insurance claims data of patients who underwent TOF repair between 2005 and 2021. Of 174 patients who underwent TOF repair, 23 (13.2%) re

Valeo Stent Use for Ductal-Dependent Systemic Circulations.

Our aim is to describe the experience of ductal stenting using the balloon-expandable Valeo stent in ductal-dependent systemic circulation. Outcomes assessed were procedural success, procedural survival, stent related complications, and re-intervention rates. Hybrid palliation, consisting of ductal stenting and bilateral pulmonary artery band placement, is a widely accepted alternative for stage I palliative surgery. Off-label use of stents is the norm for this procedure. The Valeo stent is one such example with minimal data on its use for ductal stenting. Retrospective data was collected for

Benefits of Coiling Aorto-Pulmonary Collaterals in Children with Complex Congenital Heart Diseases.

This study investigates the benefits of coiling aortopulmonary collaterals (APCs) before Fontan completion and prior to heart transplantation due to failed Fontan. The advantages of APC coiling in these situations remain unclear. Outcomes were compared between those undergoing the Fontan operation between June 2013 and December 2015, who did not undergo coiling of APCs, and those between January 2016 and May 2022, when aggressive coiling of APCs was performed. The 1-year post-transplant survival was compared for patients from Memphis, TN, where aggressive APC coiling was performed before trans

A retrospective observational study of tetralogy of fallot with pulmonary atresia and ductal-dependent pulmonary circulation focusing on the staged transannular patch procedure: a single-center study.

BACKGROUND: Management of pulmonary atresia with ventricular septal defect and ductal-dependent pulmonary circulation (PA/VSD/PDA) varies according to pulmonary artery morphology and institutional surgical strategy. We adopted a range of initial palliative surgical options for patients with PA/VSD/PDA and evaluated the effectiveness of our management strategy. METHODS: Twenty-five patients with PA/VSD/PDA were enrolled between May 2015 and July 2023. Patients with major aortopulmonary collateral arteries were excluded. The mean age at initial surgery was 14.9 ± 13.7 days, and the mean weight

A Rare Case of Undiagnosed Anomalous Right Coronary Artery From the Pulmonary Artery in an Asymptomatic Seven-Year-Old.

Anomalous origin of the right coronary artery from the pulmonary artery (ARCAPA) is an extremely rare congenital coronary artery anomaly. Diagnosis is often made incidentally when evaluating for a murmur or other congenital heart disease. The ARCAPA may present with right heart failure and/or sudden cardiac death and surgical repair is recommended once the diagnosis is made.A seven-year-old child previously diagnosed with small ventricular septal defects presented to our cardiology clinic as a new patient. Echocardiogram revealed suspicion of a coronary anomaly and computed tomography angiogra

Right Pulmonary Artery Agenesis in an Adult: A Case Report of Silent Pulmonary Hypoplasia.

Pulmonary artery agenesis is a rare congenital malformation and most often affects the right pulmonary artery. It is associated with other congenital anomalies such as tetralogy of Fallot, or it can be isolated and revealed in a less dramatic presentation in adulthood. We report in this work a case in an adult. This is a 40-year-old patient admitted for chest tightness and pain. The clinical examination was normal. Chest CT angiography and echocardiography confirmed the diagnosis of right pulmonary artery agenesis without other associated malformations. The patient has been followed for one ye

New approaches to preventing venous thromboembolism, prognostication in stroke and myocardial infarction, and antiplatelet therapy after spontaneous coronary artery dissection.

Immobility following orthopedic surgery is a risk factor for venous thromboembolism (VTE), but the optimal duration of anticoagulant thromboprophylaxis remains controversial. The protocol of the "Enhanced Recovery and Abbreviated Length of Anticoagulation for Thromboprophylaxis After Primary Hip Arthroplasty" (ENABLE-Hip) is presented, an investigator-initiated randomized double-blind trial, comparing shorter anticoagulant duration with standard-of-care. To improve outcomes after complex thrombotic events, a novel multidisciplinary approach is presented, based loosely on the tumor board model

Minimally invasive surgical repair of an aneurysmal coronary-pulmonary artery fistula.

We report a case of a 77-year-old woman with an aneurysmal coronary-pulmonary artery fistula, which was incidentally identified on contrast-enhanced computed tomography. To prevent rupture and other potential complications, surgical repair was indicated. The procedure was performed via bilateral minithoracotomy, providing sufficient exposure while minimizing invasiveness. The aneurysmal segment was successfully excluded without intra-operative complications. Post-operative contrast-enhanced computed tomography confirmed complete exclusion of the lesion with no residual opacification. The patie

Navigating Complex Cardiac Defects: A Neonatal Case of Pentalogy of Fallot With Vascular Anomalies Identified via Cardiac CT Angiography.

The pentalogy of Fallot stands out as one of the rarest and most complex congenital heart defects. Pulmonary artery stenosis, a ventricular septal defect, an overriding aorta, right ventricular hypertrophy, and atrial septal communication characterize it. Due to its complexity and associated malformations, advanced imaging methods, such as cardiac CT angiography, have become essential for proper characterization. In this study, we report the case of a 35-week gestation newborn diagnosed with pentalogy of Fallot and additional vascular malformations, identified through cardiac CT angiography.

SOX17 in pulmonary arterial hypertension: from development to clinical phenotype.

Pulmonary arterial hypertension (PAH) is a severe disease characterised by progressive remodelling and loss of pulmonary microvessels, driven by endothelial cell dysfunction, smooth muscle cell abnormalities, inflammation and immune system dysregulation. Recent research advancements have uncovered pathogenic rare loss-of-function variants of SOX17 (SRY-box transcription factor 17) associated with PAH onset. SOX17, a member of the Sry-related high-mobility group box gene family, encodes a crucial transcription factor in embryogenesis, implicated in the formation and maintenance of endoderm, for

Ultrasonographic diagnosis of secondary cardiac position alterations led by fetal thoraco-abdominal anomalies: a report of three cases.

BACKGROUND: Congenital diaphragmatic hernia (CDH) and congenital diaphragmatic eventration (CDE) are rare congenital anomalies. They occur in approximately one in 2,000-3,000 and 2,500-5,000 children respectively. It is difficult to distinguish between CDE and right diaphragmatic hernia in fetal period, which can cause changes in the position of the heart and lead to dysplasia of the right lung; absence of the right lung can lead to secondary dextrocardia. These are common in dextrocardia and lung agenesis, which are the main obstacles for diagnosis. CASE DESCRIPTION: This paper reports on th

Pulmonary arterial compliance as a long-term prognostic indicator in pulmonary arterial hypertension associated with adult congenital heart disease: results from a National multicenter prospective registry.

BACKGROUND: Pulmonary arterial compliance (PAC) reflects the pulsatile load and predicts outcome in pulmonary arterial hypertension (PAH). The prognostic role of PAC in the heterogeneous patient population of PAH associated with congenital heart disease (PAH-CHD) is poorly defined. This study aimed to explore the prognostic value of PAC in patients with PAH-CHD. METHODS: Adult patients diagnosed with PAH-CHD were collected from a PAH multicenter prospective registry between August 2009 and December 2019. The primary endpoint was all-cause mortality. Multivariable Cox regression and restricted

Long-term outcomes of Eisenmenger syndrome in the era of targeted pulmonary vasodilator therapy.

Background There is a paucity of newer studies focusing on survival patterns in the era of targeted pulmonary vasodilators in Eisenmenger syndrome (ES). We aimed to study the survival pattern, mortality predictors and outcome of targeted pulmonary vasodilator therapy in patients with ES. Methods This was a single-centre retrospective observational study of patients treated between January 2000 and January 2020. Consecutive patients with clinical and echo- cardiographic and/or cardiac catheterization diagnoses of ES were included in the study. Shunt types were classified into pre-tricuspid shun

From Scar to Shunt: Incidental Discovery of Pulmonary Arteriovenous Malformation in a Healthy Adult Trauma Patient.

Pulmonary arteriovenous malformations (PAVMs) are rare vascular anomalies that create right-to-left shunts, predisposing patients to hypoxemia, paradoxical emboli, and cerebral abscess. While often congenital and associated with hereditary hemorrhagic telangiectasia (HHT), they may also be acquired. We describe the case of a 59-year-old Hispanic man who was incidentally found to have a large left lower lobe PAVM with a 1.1 cm feeding artery during evaluation for traumatic injuries. Despite severe hypoxemic respiratory failure, he had no respiratory symptoms or distress. Initial embolization re

Feasibility of fetal cardiac output measurement by phase-contrast magnetic resonance imaging using Doppler ultrasound gating increases with gestational age.

BACKGROUND: Fetal cardiovascular magnetic resonance (CMR) is a valuable tool for assessing fetal blood flow; however, its use has primarily been focused on near-term pregnancies. This study aimed to evaluate the feasibility of Doppler ultrasound-gated two-dimensional (2D) phase-contrast CMR of the human fetus in the early, mid, and late third trimester. METHODS: A total of 100 fetal MRI scans were performed at gestational age (GA) 28, 32, and 38 weeks in 38 fetuses with (n = 13) and without (n = 25) congenital heart defects. Combined ventricular output was measured by Doppler ultrasound-gated

Paediatric coronary artery bypass grafting in a middle-income country: a case series and literature review.

BACKGROUND: Coronary artery abnormalities in children that require bypass grafting are infrequent but represent a well-recognised entity with a broad spectrum of indications beyond Kawasaki disease. Although myocardial revascularisation in children is uncommon, studies have shown that it can yield favourable short- and long-term outcomes, allowing affected children to regain health and grow up to live normal lives.Myocardial revascularisation in children is an extremely rare intervention in Western countries, accounting for less than 1% of all paediatric cardiac surgeries in this region.
