Surgical Ligation for Partial Anomalous Pulmonary Venous Connection With Dual Drainage in an Elderly Patient.

BACKGROUND: Partial anomalous pulmonary venous connection with dual drainage into both the superior vena cava (SVC) and left atrium (LA) is a rare congenital anomaly, with few reported cases. CASE SUMMARY: A 78-year-old woman presented with exertional dyspnea. Cardiac computed tomography revealed that the right pulmonary vein drained into both the SVC and LA, resulting in a significant left-to-right shunt. We ligated the anomalous connection with a median sternotomy. This approach allowed avoidance of complex reconstructions and cardioplegic arrest. DISCUSSION: Simple ligation may represent

Anaesthetic Management of a Case of Minimally Invasive Surgical Repair of Sinus Venosus Atrial Septal Defect (SVASD) and Persistent Left Superior Vena Cava (LSVC), Assisted by Unique Bilateral Percutaneous Superior Vena Cava Cannulation.

The most common venous abnormality of the thorax is persistent left superior vena cava (LSVC), incidence being less than 0.5%. Of all the congenital heart anomalies, the sinus venosus type of atrial septal defect (SVASD) is most commonly associated with LSVC and accounts for 4-10% of all ASD. We report a 52-year-old male patient with a SVASD with LSVC and PAPVC, who underwent minimally invasive cardiac surgical (MICS) repair of the same, assisted by unique bilateral percutaneous SVC cannulation and had an uneventful course and recovery. Presence of LSVC usually requires sternotomy and its drai

Distal coronary sinus-left atrial fistulous communication uncovered during atrial fibrillation ablation.

BACKGROUND: Congenital anomalies of the coronary sinus (CS) are rare and often overlooked on standard imaging. Among these, the unroofed coronary sinus (URCS) is the most recognized, typically involving the proximal CS and frequently associated with a persistent left superior vena cava. In contrast, fistulous communications between the distal CS and the left atrium (LA) are exceptionally uncommon. Their recognition is important during atrial fibrillation (AF) ablation, where unanticipated channels may complicate catheter manipulation and lesion delivery. CASE SUMMARY: A 76-year-old woman with

Lung cancer surgery with partial anomalous pulmonary venous connection presenting an inverted Scimitar sign.

BACKGROUND: Partial anomalous pulmonary venous connection (PAPVC) is a congenital anomaly in which a part of the pulmonary veins drain into the right heart system. Many cases of PAPVC remain asymptomatic and often do not present clinical problems. However, when performing a lung resection, it becomes important in terms of surgical techniques and complications. CASE PRESENTATION: A 70-year-old woman was referred to our hospital for evaluation of an abnomal shadow on chest radiography performed during medical check-up. A contrast-enhanced chest computed tomography (CT) revealed a part-solid nodu

Isolated anomalous left upper pulmonary vein connection in a child: a case report.

INTRODUCTION: Partial anomalous pulmonary venous connection (PAPVC) is a rare congenital heart defect where one or more pulmonary veins fail to connect to the left atrium. Left-sided PAPVC accounts for only 10% of cases, often involving the left upper pulmonary vein (LUPV). While many patients remain asymptomatic, prolonged volume overload in the right heart can lead to symptoms like dyspnea. Non-invasive imaging (TTE/CTA) is critical for diagnosis. CASE PRESENTATION: An 11-year-old female presented with dyspnea and recurrent respiratory infections. Imaging revealed isolated LUPV drainage int

Case Report: Analysis of the missed diagnosis of atrial septal defect with partial pulmonary vein dual drainage.

OBJECTIVE: This study aimed to investigate the reasons for the misdiagnosis of a case of atrial septal defect (ASD) complicated by partial anomalous pulmonary venous connection (PAPVC) to improve clinical diagnosis accuracy in similar cases. METHODS: The clinical data, imaging findings, and surgical records of a patient with ASD complicated by PAPVC in our department were retrospectively analyzed, and the reasons for the missed diagnosis were comprehensively analyzed in combination with the literature. RESULTS: The patient was diagnosed with PAPVC at another hospital; however, neither PAPVC

Transcatheter Repair of Sinus Venosus Defect Using a 12-zig Covered Cheatham-Platinum Stent in Large Superior Vena Cava-Right Atrium Junctions: Early Multicenter Experience.

Sinus venosus defect (SVD) is a rare congenital heart anomaly characterised by an abnormal communication between the atria, often associated with partial anomalous pulmonary venous connection (PAPVC). Traditional surgical repair carries significant risks, prompting exploration into less invasive transcatheter approaches. This study aims to evaluate the efficacy and safety of a 12-zig covered Cheatham-platinum stent for the correction of SVD, particularly in patients with enlarged superior vena caval-right atrial (SVC-RA) junctions unsuitable for standard stenting techniques. A retrospective an

Partial Anomalous Pulmonary Venous Return in Adults: Beyond Anatomy: A Cardiologist's Guide.

Partial anomalous pulmonary venous return (PAPVR) is a congenital cardiovascular malformation in which one or more pulmonary veins drain abnormally into the right atrium or systemic venous circulation, producing a pretricuspid left-to-right shunt. In anatomic terms, this condition is more precisely described as a partial anomalous pulmonary venous connection, whereas PAPVR refers to the physiological consequence of anomalous venous drainage. Historically, these anomalies were most often diagnosed during childhood; however, with increasing use of cross-sectional imaging modalities such as cardi

Efficacy of Right Ventricle-to-Pulmonary Artery Conduit as a Pulmonary Blood Source at Unifocalization of Major Aortopulmonary Collateral Arteries.

Multimodality treatment with catheter-based intervention is still commonly used for pulmonary atresia with ventricular septal defect (PA/VSD) or hemi-truncus with major aortopulmonary artery collateral arteries (MAPCAs) requiring unifocalization. A total of 12 patients with PA/VSD with MAPCAs (n = 10) or hemi-truncus (n = 2) who had undergone unifocalization since 1994 were enrolled. The development of the central pulmonary artery (cPA) was absent in 2 patients, diminutive (< 2 mm) in 3, sizable (> 2 mm) in 5, and unilateral (hemi-truncus) in 2. Treatment strategy was determined according to t

Adult cor-triatriatum sinistrum-a sinistral lesion with varied presentations: a case series.

BACKGROUND: Cor-triatriatum sinistrum (CTS) is a rare congenital heart disease, characterized by a membrane dividing the left atrium into two chambers. Echocardiography is the primary imaging modality. However, computed tomography and magnetic resonance imaging both play important roles in delineating the anatomy of the condition and also determining the associated anomalies. Although thought to be a disease of childhood, CTS can also present in adulthood with varied clinical manifestations like dyspnea, palpitations, and syncopal attacks. In CTS, the left atrial membrane causes pulmonary veno

Single-stage repair of Scimitar variant with Swiss cheese ventricular septal defects without deep hypothermic circulatory arrest.

Scimitar variant is a rare partial anomalous pulmonary venous connection that lacks right lung hypoplasia and cardiac dextroposition. Its coexistence with Swiss cheese-type ventricular septal defects (VSDs) is extremely rare and can result in complex hemodynamic changes. A 23-month-old boy with Scimitar variant and multiple Swiss cheese-type VSDs underwent single-stage repair. Preoperative imaging revealed anomalous drainage of the entire right pulmonary venous return into the inferior vena cava and a persistent left superior vena cava. The dominant perimembranous defect was patched, while sma

Closure of Sinus Venosus Atrial Septal Defects with Transcatheter Balloon-Expandable Stents: A Single-Center Case Series.

BACKGROUND: Sinus venosus atrial septal defect (SVASD) is a congenital heart defect involving an opening between the superior vena cava (SVC) and the right upper pulmonary vein (RUPV). Surgical closure is the standard treatment. Nonetheless, it is more complex than secundum ASD reconstruction and is associated with higher complication rates. Transcatheter closure is an emerging alternative. METHODS: We report 5 cases of transcatheter SVASD closure and 1 case of failure requiring surgical intervention. All patients underwent preprocedural computed tomography angiography to assess defect size a

Conservative management of iatrogenic pulmonary vein obstruction following transcatheter sinus venosus ASD repair: a case insight and literature review.

BACKGROUND: Iatrogenic pulmonary vein obstruction (PVO) is a rare but serious complication that may occur following transcatheter correction of sinus venosus atrial septal defect (SVASD) with partial anomalous pulmonary venous connection (PAPVC) using covered stents. This procedure has gained increasing acceptance in recent years. The optimal management strategy, particularly the role of conservative therapy, remains poorly defined. A 42-year-old man with a 1.5 × 1.0 cm superior vena cava (SVC)-type SVASD, PAPVC, and severe pulmonary arterial hypertension (PAH) underwent transcatheter repair a
