Uses of the nasolabial flap in the correction of cleft lip nasal deformities.

A review of 127 operations for the secondary correction of cleft lip nasal deformities among 65 patients emphasizes the need for a wide repertoire of surgical techniques to achieve a natural-appearing nose with symmetry. The use of nasolabial flaps, pedicled or island, may allow the correction of several cleft lip nasal deformities which are not really reversed by other techniques. These are described, with the suggested repairs.

[Correcture of congenital defects of the wing and the tip of the nose (author's transl)].

Congenital hypoplasia of the nasal wing can appear as a defect (congenital notch) or as a flattening and deficiency of the cartilagineous support. The combination with other deformities is seen frequently, 2 own cases: 1. Congenital notch of the left ala nasi. Operation: forming a window in the wing by incision, implantation of a composite graft from the ear, undisturbed and complete healing. 2. Hypoplasia of both cartilages of the ala nasi, flatness and dropping of the nasal tip, combined with deformities in both middle ears. Operation: Lifting the nasal tip, median suture between the cartila

Inspiratory nasal obstruction.

Inspiratory nasal obstruction or collapsing alae is caused by a sucking in of the candal cartilaginous parts of the nose; this is followed by a partial or complete stoppage of the inspiratory air. The nose usually assumes a pinched appearance together with a widening of the nostrils during the inspiratory phase, in cases where the collapse is confined to the upper laterals. Weakening of the upper laterals can results from development anomalies, or atrophy of the tissue with aging. Fractures of the nasal framework may cause displacement or buckling, with subsequent thickening or scar formation.

[The neurovascular island pedicle: basic considerations and its application in the face (author's transl)].

The island pedicle flap has in comparison with other skin flaps a number of important advantages which include elasticity, minimal shrinkage, absence of dog ears, success in irradiated areas and innecessity of a second stage procedure. On the head the most common donor areas for an island flap are postauricular, nasolabial, submental, and less cosmetically satisfying frontal and temporal regions. For large reconstrictions, however, only those pedicles which contain a branch of the superficial temporal or frontal arteries suffice. The major indications for an island pedicle flap include defects

[A new (brachymelic) type of primordial dwarfism (author's transl)].

Second report of an apparently new type of primordial dwarfism characterized by severe intrauterine and postnatal growth retardation, striking craniofacial deformities and a peculiar osteochondrodysplasia. The major clinical findings are microcephaly, hypotrichosis, bulging eyes, prominent nose, micrognathia and short extremities. The osteochondrodysplasia is characterized by short and bowed humeri and femora with absent ossification of the femoral necks, small and dysplastic iliac wings, strikingly retarded ossification of the epiphyses and shortened metacarpal I and middle phalanges II-V. [Spinocerebellar ataxia with monilethrix, typical facial features, and dental anomalies].

The authors present a form of heredodegenerative spinocerebellar ataxia with concomitant involvement of ectodermal structures, different from other variants. The disease was characterized by spinocerebellar ataxia (the complete form); curly, thin, short and dense hair, mikroskopically monilethrix; a characteristic face (hypertelorism, flattened and wide base of the nose, prominent nostrils, large mouth, thick lips); and abnormal teeth which were reduced in number and had gaps inbetween. The psycho intellectual development and the voice of the patient were normal. The first neurological symptom

Further studies on the genetic heterogeneity of cebocephaly.

The detailed morphological description of 4 cases with cebocephaly, 3 of which were karotyped (one with D trisomy and 2 with normal karyotypes), are presented. Analysis of all cytogenetically studied cases with this malformation reveals that cebocephaly with a normal karyotype may result from more than one mutant gene, and so it may be accompanied by different extracranial abnormalities. On the other hand an absence of visceral malformations does not exclude chromosomal aberrations; thus 18p- syndrome, where cebocephaly is frequent, may have no visceral abnormalities.

The syndrome of partial trisomy 14q.

The case of a 4-month-old male with de novo partial trisomy for chromosome 14 involving the p13 leads to q24 portion is reported. He presented with growth and psychomotor retardation, peculiar facies due to nose-mouth anomalies, monolateral microphtalmia, high arched palate, and anomalies of hands and feet. These symptoms are found also in the other 8 cases of partial trisomy 14 reported in the literature. This confirms a characteristic chromosomal syndrome although the breaking points on the extra chromosome 14 are not the same in the 9 cases. The clinical picture of our case calls for carefu

Construction of the upper lip, columella and orbicularis muscle in bilateral clefts.

A concept for repair of double clefts is described. In the primary operations at the age of six months on the first side, and six weeks later on the other, it is important to supply the prolabial mucosa by muscular vermilion flaps from the lateral sides, crossing the midline. Thus a muscular ring in the vermilion is obtained. At the age of 3-4 years both remaining muscular halves of the lip are united. This is achieved togeather with lengthening of the columella, using broadened prolabium to a natural looking philtrum. Lips repaired this way are satisfying with regard to both appearance and fu

The bifid uvula in ear, nose and throat practice.

Out of 2,000 consecutive ear, nose and throat patients, 44 were found with bifid uvulas. Five of the cleft uvulas were associated with submucous cleft palate, and eight were associated with short palates; however, dyslalia was evident in only one case. Compensation for the wide palato-pharyngeal space can occur by hyperplasia of the nasopharyngeal tonsil and hypertrophy and increased mobility of the pharyngeal constrictors and Passavant's ridge. On the other hand, otologic manifestations were complained of in 13 cases. The asymptomatic cases were examined under magnification and audiometricall

[Increase of the LDH-B activity in a boy with 12p trisomy by malsegregation of a maternal translocation t(12;14) (q12;p11)].

A newborn male trisomic for 12p is compared with three other 12p trisomics already reported in the literature, as well as with three patients monosomic for 12p. A "type and countertype" opposition is observed for five characters: in the trisomy, turricephaly, shortness of the nose, protruding anthelix, wide palms, and increased LDH-B activity; in the monosomy, protruding occiput, large nose, hypoplasia of the anthelix, narrow palms, and decreased LDH-B activity.

The trisomy 9p syndrome.

Since the first description of trisomy 9p in 1970, there has been a rapidly increasing recognition and reporting of new cases. The physical and mental features of retarded growth and development, down-turned corners of the mouth, mildly globular nose, slightly wide-set and deep-set eyes with anti-Mongoloid slant, and unusual dermatoglyphics are distinctive enough to establish trisomy 9p as a clinical chromosomal entity--one which may prove to be the fourth most common autosomal syndrome (after trisomies 21, 13, and 18). This paper includes a review and tabulation of features seen in the 20 rep

New guiding principles in the treatment of clefts.

Breathing through the nose with the mouth shut is essential for the normal development of the facial region related to the mouth and nasal cavities and for the normal development of the two cavities named. Surgical treatment of unilateral total cleft practised hitherto calls for the closure of a cleft in the lip and a cleft in the alveolar ridge at the same surgical intervention. In the great majority of cases a combined intervention closes the respiratory pathway, which was established through the cleft nasal floor and alveolar ridge after birth. From the first surgical intervention on, the c

Familial partial trisomy of the long arm of chromosome 10 (q24-26).

Two fourth cousins with a strikingly similar pattern of malformation and who have an unbalanced translocation (46, XY, -17, +t (17p; 10q) are described. From an analysis of the phenotypes of these patients and others reported with 10q trisomy, we propose that the trisomy 10q 24-26 syndrome includes: growth and mental retardation, a characteristic facies (microcephaly, flat face with spacious forehead, small nose, depressed nasal bridge, arched wide-spaced eyebrows, blepharophimosis, microphthalmia, low-set ears, bow-shaped mouth with prominent upper lip, micrognathia), palate anomalies (high-a

Nasal emission as a component of the misarticulation of sibilants and affricates.

Nasal emission confined solely to the sibilants or sibilants and affricates may be encountered as an articulatory phenomenon in speakers who do not demonstrate velopharyngeal incompetency. This phenomenon may occur both in patients who previously had an incompetent mechanism and in patients in whom such incompetency has never been documented. In the latter case, the patient may be referred to a cleft palate clinic on suspicion of a submucous cleft or other form of palatopharyngeal incompetency. Typically the nasal emission is combined with incorrect tongue placement. The persistent oral-nasal

Trisomy 13 and Rubinstein-Taybi syndrome.

Initial diagnosis of Rubinstein-Taybi syndrome was made in an infant with a prominent nose and broad thumbs and first toes. However, due to the presence of other anomalies such as low-set, malformed ears, anti-mongoloid slant of the eyes, colobomata of the iris, and cleft palate, cytogenetic studies were carried out and the diagnosis of trisomy 13 was confirmed. Since, occasionally, trisomy 13 syndrome may mimic the Rubinstein-Taybi syndrome, cytogenetic studies should be considered in all patients with clinical diagnosis of Rubinstein-Taybi syndrome.
