Laryngotracheal stenosis (LTS) may afflict patients in all stages of life. Congenital subglottic stenosis is the third most common upper airway anomaly encountered in infants and neonates after laryngomalacia and vocal cord paralysis or fixation. It is a complex condition that demands arduous, often frustrating, efforts at all stages of management. Prevention of LTS is the most effective management of this problem. This paper will focus on the role of diagnostic radiology in the management of LTS.

Eight patients (ages 10 months to 5 years) underwent laryngotracheal reconstruction (LTR) without intraluminal stenting or tracheostomy. All patients had moderate to severe acquired or congenital subglottic stenosis. Six patients were tracheostomy-dependent prior to reconstruction. The other two children were intubated and would have required tracheostomy for airway control. LTR and decannulation/extubation were accomplished as a one-stage procedure. Autologous rib cartilage was used (anterior graft only--6 patients, anterior and posterior graft--3 patients) and patients were intubated for 7-1

Laryngoceles and saccular cysts are an unusual cause of respiratory obstruction in infants and children. Laryngoceles in particular are less common in the pediatric population than in adults. They typically produce intermittent upper airway obstruction and hoarseness (due to episodic filling with air), and are, therefore, difficult to diagnose. Saccular cysts produce similar symptoms but are not episodic. While smaller anterior saccular cysts are readily managed by endoscopic excision, larger saccular cysts tend to recur repeatedly. Review of experience with 20 infants and children having sacc

BACKGROUND: Conditions that completely occlude the fetal larynx are rare. We describe the perinatal course of an infant with severe congenital subglottic stenosis, for whom the prenatal recognition of airway occlusion led to enhanced neonatal management.
CASE: Serial ultrasound examinations from 23-37 weeks' gestation led to the diagnosis of upper airway obstruction in the fetus of a 29-year-old woman. Significant findings included fetal ascites, echogenic enlarged lungs, and a dilated fluid-filled trachea. The infant was delivered in a level III hospital with a neonatologist and anesthesiolo

Twenty-five children with acquired and congenital subglottic stenosis (SGS) were managed with the anterior cricoid split (ACS) operation at the authors' institution from September 1987 to January 1990. Ages ranged from 2.5 months to 5.5 years. Twenty-one (84%) of the children were extubated after 5 to 14 days of nasotracheal intubation and have remained stable after an average follow-up of 10 months. Atelectasis was a common postoperative problem, encountered in 12 (48%) of the patients. Other complications included a tracheocutaneous fistula, prolapse of soft tissue into the tracheal lumen vi

The authors report about the cases of two infants less than 5 weeks old, who presented with congenital subglottic stenosis and in whom a cricoid split produced good results. This procedure, which is classically reserved for difficult extubation of infants due to acquired subglottic stenosis, is compared with the other surgical techniques used in the congenital condition. It may be useful in the latter case owing to its simplicity and to the possibility of performing laryngoplasty later in case of failure.

Congenital anomalies of the airway are generally uncommon, but a vast array of possibilities exists. Some present life-threatening emergencies at birth, and others go undiagnosed for years. Clinical symptoms are often nonspecific, and radiographic evaluation is frequently requested to localize and characterize the lesion before endoscopy, surgery, or medical management. The most common intrinsic congenital anomalies causing airway compromise in infants include choanal atresia, mandibular hypoplasia, laryngomalacia, vocal cord paralysis, and congenital subglottic stenosis. The radiologist must

Anterior cricoid resection is an effective procedure to relieve subglottic stenosis. This is well documented in adults, although reports of the procedure in growing airways are limited. Over an 11-year period, seven pediatric patients underwent anterior cricoid resection for recalcitrant subglottic stenosis. In four patients, the stricture was secondary to prolonged intubation, one developed subglottic stenosis following a high placement of tracheostomy for epiglottitis and another had congenital subglottic stenosis. One child had subglottic stenosis combined with laryngotracheoesophageal clef

Strictures of the trachea traditionally have been dilated by antegrade bouginage. This method not only dilates but imparts a shearing force to the wall of the trachea. The resultant trauma may contribute to further scarring and aggravation of the stricture. We report a 15-year experience with 37 patients and 158 tracheal dilatations using balloon catheters that impart only radially directed forces and can be precisely placed and gradually inflated. Various adjunctive techniques have been performed, including: (1) electrocautery of granulation tissue; (2) parenteral and local use of steroids; a

We report our experience with laryngotracheal stenosis (LTS) in children during the last 12 years. Documentation and follow-up were available for 115 patients who underwent surgery for acquired or congenital LTS. Most were severe cases according to Cotton's classification. Forty-six weighed less than 10 kg at the time of surgery; 45 had pure congenital subglottic stenosis; 70 had acquired subglottic stenosis, mainly due to endotracheal intubation. The surgical techniques used have been various. The three main types of procedure were castellated laryngotracheoplasty, anterior cartilage rib graf

Various malformations of the cricoid cartilage produce congenital subglottic stenosis. The elliptical cricoid cartilage first was demonstrated histopathologically in horizontal sections of six postmortem specimens and now has been identified clinically in ten patients. The clinical diagnosis of the elliptical cricoid cartilage, suggested by anteroposterior soft tissue neck films, is confirmed at direct laryngoscopy. Delineation of the precise location, extent, and histopathology of subglottic stenosis provides the basis for rational management.

The experience with laryngotracheal reconstruction in 20 children in Chicago is reviewed. Nine of the 20 children (45%) operated upon had congenital subglottic stenosis. Ten (50%) had acquired stenosis. One child was classified as having combined types. All patients underwent laryngotracheal reconstruction with autogenous costal cartilage grafts. Eight patients had both anterior and posterior costal cartilage grafts with stent insertion. One had a posterior costal cartilage graft only. Sixteen of the 20 children (80%) have been decannulated. An elliptically shaped cricoid cartilage, a specific type of congenital subglottic stenosis, is described, and the laryngoscopic and radiographic findings in three cases are discussed. An "hourglass" deformity of the subglottic laryngeal airway on an anteroposterior airway radiograph is suggestive of this diagnosis.

The authors report a case of a 3 kg newborn with oesophageal atresia and congenital subglottic stenosis. The literature is reviewed and the management of our case reported. As in many cases the subglottic stenosis outgrows within a year of age, though tracheostomy is not justified even if general anesthesia is needed in case of major surgery.

Anterior cricoidotomy or "cricoid split" was described in 1980 as an open incisional procedure for failed extubation in neonates. In the past year, we modified the technique to an endoscopic-percutaneous cricoidotomy in 22 patients. We broadened the eligible population to include children with a mature membranous stenosis or a congenital subglottic stenosis. Our overall success rate of 80% (73% in the premature group) demonstrates that a cricoidotomy can be used in children older than the neonatal group and suggests that a cricoidotomy does not need to be an open operative procedure, and shoul

Severe laryngotracheal stenosis (LTS) in children is a problem of increasing incidence in the past 15 years, following the widespread adoption of prolonged endotracheal intubation for respiratory support. Rarer cases of stenosis secondary to external trauma, high tracheotomy, thermal and chemical burns, and dystrophic cartilage are seen. In contrast to congenital subglottic stenosis, where conservative therapy is generally indicated, severe, mature LTS often requires surgical correction--either endoscopic or external reconstruction. The prevailing attitude has been to perform a tracheotomy and

In contrast to the generally accepted concept, I have classified congenital subglottic stenosis in a different way. During the first months of life, the abnormality may be recognized by its frequent association with acute respiratory infections, providing the opportunity to recognize it once the infection has been controlled. I emphasize the role of the radiologic examination of the larynx for diagnosis and follow-up of this disorder and its different evolution from that of pure subglottic laryngitis. A comparison with subglottic laryngitis and laryngotracheobronchitis emphasizes the seriousne

Congenital laryngeal web is not as common as has been previously believed, and thorough clinical, radiographic, and endoscopic assessment is needed before any form of surgical treatment is undertaken. It is important to detect associated anomalies in the larynx, the respiratory tract, or in other organ systems. The results of treatment with regard to voice quality are good only in thin, membranous, uncomplicated webs. Treatment for thick webs with or without associated congenital subglottic stenosis remains unsatisfactory.

In the newborn period and early infancy upper respiratory tract obstruction is commonly caused by congenital and neoplastic disorders, such as laryngomalacia, congenital subglottic stenosis, or brainstem anomalies. However, inflammatory causes of airway obstruction predominate in infancy and childhood. Laryngotracheal foreign bodies and trauma, on the other hand, have no age predilection. The diagnosis and therapy of airway obstruction in the pediatric patient are discussed.
