Conjoined Parasitic Twins (Cephalo-Thoracopagus Parasiticus) in a Free Ranging Northern Bat (Eptesicus nilssonii): A Micro-CT Anatomic and Genetic Survey.

The detailed anatomical situation of male stillborn cephalo-thoracopagus twins in a wild ranging northern bat (Eptesicus nilssonii) is described by means of full body micro-CT scans in high resolution with three-dimensional computational reconstruction. The external anatomy was characterised by supernumerary wings and legs and marked cranial disfiguration. The most striking features of the internal anatomy were the presence of two hearts with different characteristics, three lungs with two tracheas, three kidneys as well as a doubled liver. Vascular aberrations and other organ deformities are

Primary pulmonary hypoplasia masquerading as pneumonia in a preterm neonate and creating a diagnostic challenge: a case report.

BACKGROUND: Pulmonary hypoplasia is a rare congenital lung disorder characterized by the incomplete development of lung tissue. This condition can be classified as primary or secondary to other congenital anomalies, with primary pulmonary hypoplasia being considerably rarer than the secondary form. Severe cases often manifest as respiratory failure in the immediate newborn period. While secondary pulmonary hypoplasia is often antenatally detected, primary cases-such as ours-may evade prenatal imaging, presenting as refractory neonatal respiratory distress. Secondary causes, such as oligohydram

The clinical value of invasive prenatal diagnosis in fetuses with isolated aberrant right subclavian artery: a retrospective study.

BACKGROUND: Aberrant right subclavian artery is a common anatomical variant of the embryonic aortic arch, with a prevalence ranging from 0.4 to 2.0%. Although frequently associated with vascular rings or congenital cardiac defects, prenatal assessment primarily relies on the three-vessel and trachea view in ultrasonography. Currently, there is no consensus regarding whether isolated ARSA necessitates invasive diagnostic procedures. This study aimed to evaluate the necessity of routine invasive prenatal diagnosis for fetuses with sonographically isolated ARSA. By conducting a long-term postnata

Congenital esophageal stenosis related to tracheobronchial remnant: Balloon dilatation versus resection and anastomosis - A systematic review.

INTRODUCTION: Congenital esophageal stenosis (CES) related to tracheobronchial remnant (TBR) is an extremely rare malformation, typically diagnosed in infants and young children. Endoscopic dilatation or resection and anastomosis are both accepted therapeutic options, but the superiority of one versus the other remains controversial. METHODS: We performed a comprehensive literature search in PubMed and MEDLINE using standardized search terms and subject headings from 1969 onwards. Inclusion criteria were patient age 0-18 years, confirmed diagnosis of CES due to TBR, English language, interven

Co-Occurring Non-Cardiac Congenital Anomalies Among Cases With Congenital Heart Defects.

Cases with congenital heart defects (CHD) often have other associated anomalies. The aim of this investigation was to assess the prevalence and the types of co-occurring anomalies in CHD in a well-defined population. The anomalies co-occurring with CHD were ascertained in all live births, stillbirths and terminations of pregnancy for fetal anomaly during 29 years in 387,067 consecutive pregnancies of known outcome in the area covered by our population-based registry of congenital anomalies. Of the 4640 cases with CHD ascertained during this period (prevalence of 119.9 per 10,000), 1228 (26.5%)

Distinguishing Tracheobronchomalacia and Excessive Dynamic Airway Collapse on Dynamic CT Images.

BACKGROUND: Expiratory central airway collapse (ECAC) is caused by anterior displacement of the posterior membrane (EDAC) or weakening of the cartilaginous walls (TBM). Differentiating the 2 is challenging, resulting in misdiagnosis and missed opportunities for patients. We investigated how accurately providers identify each pathology on computed tomographic (CT) images. METHODS: Images from 100 patients who underwent tracheobronchoplasty (TBP) were reviewed by 4 airway experts and categorized into TBM, EDAC, normal airway, or unknown by majority consensus. Selected images of patients with TB

Perioperative risk factors for short- and long-term respiratory morbidity in patients with esophageal atresia.

PURPOSE: Respiratory complications are a major source of morbidity in esophageal atresia (EA), yet predictors of short- and long-term respiratory outcomes are not well defined. This study aimed to identify perioperative factors associated with respiratory morbidity in EA/TEF patients. METHODS: We conducted a retrospective cohort study of 242 EA patients treated at a single tertiary center from 2005 to present. Short-term respiratory morbidity was defined as the need for supplemental respiratory support at 30 days postoperatively from primary EA repair. Long-term morbidity was determined based

A translational approach to airway reconstruction leveraging decellularized meniscus and cartilage progenitor cells.

Severe subglottic stenosis develops in over 20,000 infants per year and requires laryngotracheal reconstruction to enlarge the airway by implanting autologous cartilage from a rib graft. However, young children often lack sufficiently sized costal cartilage for this procedure, resulting in increased donor site morbidity and operative time, as well as an elevated risk for airway restenosis, necessitating revision surgery. To overcome these limitations, we create a scaffold based on porcine meniscal cartilage decellularization - which we term MEND - by selectively digesting away the elastin and

Non-invasive removal of the Smart tracheal occlusion device for fetal congenital diaphragmatic hernia: a single-arm, open-label, phase 1 study.

BACKGROUND: One of the drawbacks of fetoscopic endoluminal tracheal occlusion (FETO) for congenital diaphragmatic hernia (CDH) is the need for a second intervention to establish airway patency. In this first-in-human study, we aimed to evaluate a novel tracheal occlusion device (Smart-TO) that spontaneously deflates when in close proximity to a strong magnetic field. METHODS: We conducted two parallel single-centre, single-arm, open-label trials in Paris, France, and Leuven, Belgium. Eligibility criteria included patient age 18 years or more, singleton pregnancy with CDH, and severe or modera

Perioperative Management of Presumed Schwartz-Jampel Syndrome During Complex Pediatric Spinal Fusion: A Case Report.

Schwartz-Jampel syndrome (SJS) is associated with neuromuscular and structural abnormalities that can complicate perioperative and anesthetic management. The syndrome has been noted to create increased difficulty in anesthetic airway management with age. We report the perioperative course of a 13-year-old female with presumed SJS undergoing combined anterior and posterior spinal fusion for severe progressive congenital scoliosis. Preoperative evaluation revealed features predictive of a difficult airway, including limited mouth opening and restricted neck mobility, requiring video laryngoscopy

Unusual radiological presentation of extralobar pulmonary sequestration: a preoperative diagnostic challenge.

BACKGROUND: Extralobar pulmonary sequestration is a rare disease and is frequently associated with congenital malformations. Herein, we report an adult case of an unusual extralobar sequestration accompanied by a bronchogenic cyst and the concurrent absence of the left pericardium. CASE PRESENTATION: A 57-year-old female patient presented with an abnormal shadow on a chest radiograph. Contrast-enhanced computed tomography revealed a partially enhanced cystic mass in the left anterior mediastinum. Another cystic lesion was detected on the left side of the trachea in the upper mediastinum. The a

In Vitro Fertilization and VACTERL Birth Defects.

IMPORTANCE: The VACTERL (vertebral defects, anal atresia, cardiac defects, tracheo-esophageal fistula, renal defects, and limb defects) pattern of birth defects is a complex congenital condition with largely unknown causes. Studies suggest a link between in vitro fertilization (IVF) and birth defects, including VACTERL, but it remains unknown whether IVF has a unique association with VACTERL. OBJECTIVE: To characterize the association between IVF and VACTERL. DESIGN, SETTING, AND PARTICIPANTS: This cohort study included 1 555 936 live births in 4 US States (Massachusetts, New York, North Car

Finnish-Enriched SLC26A7 Variant in Congenital Hypothyroidism: Clinical Spectrum, Thyroid Histopathology, and Expression Analysis.

BACKGROUND: Defects in thyroid hormone synthesis at birth lead to congenital hypothyroidism (CH). Recently, pathogenic variants in the SLC26A7 gene have been linked to dyshormonogenetic goitrous CH. This anion transporter is highly expressed in the thyroid and is involved in thyroid hormone synthesis; however, its exact function and cellular localization remain unclear. In this study, we investigated SLC26A7 variants in Finnish patients with CH, characterized the phenotypes, and analyzed thyroid-specific gene expression. METHODS: SLC26A7 variants were identified from a clinical CH cohort (n =

Dynamic single-cell transcriptomic reveals the cellular heterogeneity and a novel fibroblast subpopulation in laryngotracheal stenosis.

BACKGROUND: Laryngotracheal stenosis (LTS), a pathological narrowing of the upper airway caused by excessive extracellular matrix (ECM) deposition, often leads to dyspnea and even respiratory failure. However, systematic studies addressing the specific subpopulations and their contribution to LTS development still remain underexplored. RESULTS: We collected laryngotracheal tissue at multiple time points of LTS rat model, established by injuring their laryngotracheal lining, and performed dynamic single-cell RNA sequencing (scRNA-seq) to elucidate the transcriptomic atlas of LTS development. [Expert consensus on auditory intervention and language rehabilitation of CHARGE syndrome].

Objective:CHARGE syndrome(CS) is an autosomal dominant congenital malformation involving multiple organs and sensory systems. The incidence of hearing impairment and developmental retardation in CS patients is 87% and 84%, respectively, which is often accompanied by malformations such as abnormal development of temporal bone, venous sinus, and facial nerve that directly affect cochlear implant surgery. Other systemic malformations increase perioperative risks, such as heart defects, micrognathia, laryngeal and trachea malformations. Cochlear implants are significant in promoting hearing and la

Wnt5a and Notum influence the temporal dynamics of cartilaginous mesenchymal condensations in developing trachea.

INTRODUCTION: The trachea is essential for proper airflow to the lungs for gas exchange. Frequent congenital tracheal malformations affect the cartilage, causing the collapse of the central airway during the respiratory cycle. We have shown that Notum, a Wnt ligand de-acylase that attenuates the canonical branch of the Wnt signaling pathway, is necessary for cartilaginous mesenchymal condensations. In Notum deficient tracheas, chondrogenesis is delayed, and the tracheal lumen is narrowed. It is unknown if Notum attenuates non-canonical Wnt signaling. We observed premature tracheal chondrogenes

Esophageal Atresia and Tracheoesophageal Fistula: Diagnosis, Management, and Outcomes.

Esophageal atresia is a congenital disease that results in discontinuity of the esophagus and can occur with or without fistulous connections to the trachea. It is often associated with other anomalies and has multiple long-term health implications. Because of the complexity of care involved in affected children, it is important to diagnose this lesion as early as possible. Surgical repair via either anastomosis of the native esophagus or reconstruction using either the stomach or intestine if this is not possible remain the mainstays of therapy, although variations in technique prevail.
