Interventions for adult congenital heart disease.

Advances in imaging diagnostics, surgical techniques and transcatheter interventions for paediatric patients with severe congenital heart disease (CHD) have substantially reduced mortality, thereby extending the lifespan of these individuals and increasing the number of adults with complex CHD. Transcatheter interventions have emerged as an alternative to traditional open-heart surgery to mitigate congenital defects. The evolution of techniques, the introduction of new devices and the growing experience of operators have enabled the treatment of patients with progressively more complex conditi

Isolated congenital diaphragmatic hernia and three-year neurodevelopmental outcomes.

BACKGROUND: To retrospectively investigate the developmental outcomes at 3 years of age in patients with congenital diaphragmatic hernia (CDH) using a multicenter collaborative research approach. METHODS: We evaluated patients with CDH and no other malformations born between 2010 and 2016 in seven facilities in the Japanese CDH Research Group. The developmental quotient (DQ) at 3 years of age was evaluated using the Kyoto Scale of Psychological Development 2001, the most standardized scale in Japan. Factors associated with a DQ score < 85 were also analyzed. RESULTS: Of 196 patients, develop

Pathohistological Changes in the Lungs of Very Preterm Infants with Bronchopulmonary Dysplasia Depending on the Clinical Features.

Establishing clinical factors associated with histological changes in the lungs of very preterm infants with evolving or established bronchopulmonary dysplasia (BPD) is essential for the development of more effective preventive interventions.Thirty-two infants with a gestational age (GA) of <32 weeks who died of BPD or had BPD but died due to other causes were included in the study. The associations of clinical data with histopathological changes in the lungs were assessed. The mean (standard deviation) GA of infants was 26.7 (1.9) weeks, and the mean birth weight was 919.7 (242.9) g. We reveal

Respiratory support in patients with Down syndrome: a systematic review.

BACKGROUND: The population with Down syndrome (DS) makes up a significant portion of patients using short- and long-term noninvasive ventilation (NIV). However, limited data are available concerning the use and outcomes for patients with DS compared with other populations. We systematically reviewed articles providing indications, benefits, adherence and limits of long-term NIV use in the paediatric and adult population with DS. METHODS: A systematic review of the literature for articles published between January 2013 and November 2023 was conducted using a protocol registered at PROSPERO (CR

Days Alive Out of Health Care: A Novel Measure of Health Status After Congenital Heart Surgery.

BACKGROUND: As pediatric and adult operative mortality improves, longitudinal outcomes beyond survival become increasingly important. "Days alive and out of hospital" is well-studied as a longitudinal, patient-centered outcome in adult populations. However, it has yet to be validated for children, whose health care use and survival differ from that of adults. OBJECTIVES: This study sought to evaluate days alive and out of health care/hospital as outcomes for neonatal and infant cardiac surgery. METHODS: Using linked, locally held, clinical registry data, the National Death Index, and Medicai

Overlapping Clinical Phenotypes in Patients with Primary Ciliary Dyskinesia or Activated Phosphoinositide 3-Kinase Delta Syndrome.

Primary ciliary dyskinesia and activated phosphoinositide 3-kinase delta syndrome type 1 present similarly, with recurrent respiratory infections and reduced nasal nitric oxide levels. When diagnostic confirmation of primary ciliary dyskinesia with genetic testing and/or ciliary electron microscopy is inconclusive, activated phosphoinositide 3-kinase delta syndrome type 1 and other inborn errors of immunity must be investigated.

Recessive genetic contribution to congenital heart disease in 5,424 probands.

Variants with large effect contribute to congenital heart disease (CHD). To date, recessive genotypes (RGs) have commonly been implicated through anecdotal ascertainment of consanguineous families and candidate gene-based analysis; the recessive contribution to the broad range of CHD phenotypes has been limited. We analyzed whole exome sequences of 5,424 CHD probands. Rare damaging RGs were estimated to contribute to at least 2.2% of CHD, with greater enrichment among laterality phenotypes (5.4%) versus other subsets (1.4%). Among 108 curated human recessive CHD genes, there were 66 RGs, with

[Integrative transcriptomics-metabolomics approach to identify metabolic pathways regulated by glutamine synthetase activity].

Glutamine synthetase (GS), the only enzyme responsible for de novo glutamine synthesis, plays a significant role in cancer progression. As an example of the consequences of GS mutations, the R324C variant causes congenital glutamine deficiency, which results in brain abnormalities and neonatal death. However, the influence of GS-deficient mutations on cancer cells remains relatively unexplored. In this study, we investigated the effects of GS and GS-deficient mutations, including R324C and previously unreported K241R, which serve as models for GS inactivation. This study provided intriguing in

Hemodynamics during noninvasive ventilation in neonates.

Noninvasive ventilation (NIV) is increasingly used in neonatal care to reduce lung injury and improve respiratory outcomes. However, its effects on cardiovascular function and cardiopulmonary interactions, particularly in preterm neonates, remain unclear. This paper reviews the hemodynamic changes associated with NIV and provides an overview of how NIV influences cardiac preload, pulmonary vascular resistance, ventricular output, and systemic blood flow. NIV also affects these parameters in different ways in neonates with compromised cardiovascular stability, such as those with persistent pulm

Lung Density Measurement in Fontan Patients Using Zero Echo Time Sequences.

Pulmonary complications are known to occur in patients after Fontan palliation. Cardiac MRI is performed in the follow-up of Fontan patients to assess single ventricular function, hemodynamics and potential collateral flow. To date, pulmonary function tests have been used to detect functional lung impairment, but lung MRI has not been integrated into imaging follow-up. In this study, we measured lung-to-background ratio (LBR) on zero echo time (ZTE) MRI sequences in 32 children with Fontan palliation. Patients were divided into 2 groups: LBR > 1.5 and < 1.5 and assessed for associations betwee

Management of patients with congenital bleeding disorders and cardiac indications for antithrombotic therapy.

AIMS: Cardiologists have only had rare exposure to haemophilia patients and patients with other congenital bleeding disorders during the last decades, as these patients had a reduced life expectancy and were partly protected against thrombosis due to the bleeding disorder. With the availability of effective and safe replacement therapies of clotting factors, the average life expectancy in these populations of patients has significantly increased, and thrombotic complications may occur. METHODS AND RESULTS: The European Society of Cardiology Working Group on Thrombosis has taken the initiative

Three-dimensional echocardiography in adults with congenital heart disease: a scientific statement of the European Society of Cardiology Working Group on Adult Congenital Heart Disease and the European Association of Cardiovascular Imaging of the European Society of Cardiology.

Congenital heart disease (CHD) is the most common inborn defect affecting up to 1% of newborns. The prevalence of CHD has shifted from childhood to adulthood, and the number of adult patients living with CHD continues to increase. This patient population presents unique challenges in diagnostic imaging and management due to complex underlying cardiac morphology, previous operations and interventions, and haemodynamic conditions. 3D echocardiography (3DE) has significantly improved our understanding of complex anatomic and haemodynamic substrates and emerged as a clinically useful tool that pro

[Physiotherapy in Primary Ciliary Dyskinesia].

Primary ciliary dyskinesia (PCD) is a multisystem disease caused by dysfunction of motile cilia. It is characterized by chronic mucus retention of the upper and lower airways. This results in a destructive lung disease with the development of bronchiectasis. There are a very few evidence-based therapies for patients with PCD. Most treatment concepts are based on other respiratory diseases or expert opinion. This article is about the physiotherapy care of patients with PCD and provides an overview of recommendations from the current literature as well as many years of experience in the physioth

Mutations in the small nuclear RNA gene RNU2-2 cause a severe neurodevelopmental disorder with prominent epilepsy.

The major spliceosome includes five small nuclear RNA (snRNAs), U1, U2, U4, U5 and U6, each of which is encoded by multiple genes. We recently showed that mutations in RNU4-2, the gene that encodes the U4-2 snRNA, cause one of the most prevalent monogenic neurodevelopmental disorders. Here, we report that recurrent germline mutations in RNU2-2 (previously known as pseudogene RNU2-2P), a 191-bp gene that encodes the U2-2 snRNA, are responsible for a related disorder. By genetic association, we identified recurrent de novo single-nucleotide mutations at nucleotide positions 4 and 35 of RNU2-2 in

Primary ciliary dyskinesia: Aetiology, diagnosis and clinical management.

Primary ciliary dyskinesia (PCD) is a rare genetic disorder characterised by abnormal function of motile cilia. The condition usually manifests in early life with neonatal distress, chronic sinopulmonary disease and organ laterality disorders. In adults, it is an underdiagnosed cause of bronchiectasis as well as subfertility. This review provides an overview of PCD for clinicians. We discuss its aetiology, its presentation, how it is diagnosed and its multidisciplinary clinical management.
