BACKGROUND: Congenital pulmonary airway malformation (CPAM) is a rare developmental anomaly of the lower respiratory tract, with an estimated incidence ranging from 1 in 10,000 to 1 in 35,000 live births. CPAM may present during infancy with severe respiratory distress and complications such as pneumothorax or recurrent pulmonary infection.
CASE PRESENTATION: We report two female infants aged 7 months and 2 months who presented with severe respiratory distress and radiological findings initially suggestive of pneumothorax. In Case 1, chest computed tomography (CT) demonstrated a large solitar

Particulate matter (PM) aggravates respiratory diseases, including asthma, and is associated with bronchus-associated lymphoid tissue (BALT) formation. However, the time-course effects of different PM types on pulmonary inflammation and BALT development remain unknown. Herein, we investigated the time-dependent effects of 3 PM types-titanium dioxide nanoparticles (TiO₂), diesel exhaust particles, and Asian sand dust-on lung inflammation and ectopic lymphoid tissue development over time, in the presence/absence of allergen ovalbumin. All PM types supported BALT formation, with stronger developm

Chemotactic receptors involved in generation of ectopic pulmonary germinal centers (GCs) within inducible bronchus-associated lymphoid tissue (iBALT) are poorly defined. Here, using CIBER Cxcr3-reporter mice, we demonstrate that the prototypical type 1 inflammatory chemokine receptor CXCR3 is highly induced in influenza A virus (IAV)-reactive B cells in the mediastinal lymph node, spleen, lung, peripheral blood, and airways following intranasal infection. Notably, elevated Cxcr3 was observed in ectopic pulmonary germinal center B (GCB) cells in iBALT relative to their contemporaneous counterpa

Pulmonary arteriovenous malformation (PAVM) is a congenital vascular anomaly that forms a shunt between the pulmonary artery and vein. It is frequently detected incidentally in asymptomatic patients. We encountered a 69-year-old female patient with bilateral PAVMs that were incidentally detected on abdominal computed tomography( CT) performed for a detailed examination of appendicitis. Abdominal CT revealed nodular opacities with contrast enhancement in the middle lobe of the right lung and the lingular segment of the left lung. They were further examined, and a diagnosis of PAVMs measuring 14

BACKGROUND: This study evaluated the ability of the ABC Bicuspid Sizing Algorithm to predict aortic root rupture in patients treated with transcatheter aortic valve replacement using SAPIEN 3 valves.
METHODS: We conducted a retrospective multicenter study among 15 centers in 7 countries of aortic root rupture cases and controls without root complications. Computed tomography analysis was performed blinded to rupture status. The ABC algorithm was applied to classify risk (high-risk criteria): (1) annular oversizing >10%, (2) intercommissural distance - valve diameter ≤1 mm, (3) maximum sinus d

OBJECTIVES: We wanted, as part of a wider study, to elicit perspectives of patients with congenital heart disease (CHD) and/or their parents/carers about which outcomes of CHD and/or cardiac surgery matter and what should be measured and reported.
DESIGN: Qualitative, closed, asynchronous, online discussion forums underpinned by an interpretivist framework.
SETTING AND PARTICIPANTS: Three patient charities in the UK set up and moderated separate online forums for adult patients with CHD/carers, adolescent patients with CHD and parents/carers of children and young people with CHD.
ANALYSIS:

BACKGROUND: Stachybotrys chartarum is a damp building mould and a potent toxin producer that has been related to serious cases of respiratory health problems. However, the direct link between exposure and health symptoms has not been established.
OBJECTIVE: To examine the mechanism by which exposure to spores of satratoxin producing and non-producing S. chartarum strains induce inflammatory responses in murine lungs.
METHODS: BALB/c mice were intranasally exposed for 3 weeks to spores of a satratoxin-producing and a non-producing S. chartarum strain. Inflammatory cell infiltration was charac

Paravalvular leak (PVL) is a well-known complication of valve replacement, occurring in up to 10% of aortic and 17% of mitral prosthetic valves. Although many leaks are small and clinically insignificant, ≈1% to 5% of cases are associated with clinically relevant complications, such as heart failure symptoms and hemolytic anemia. Surgical reoperation is currently considered the first-line option; however, it encompasses high mortality and morbidity risks. In the past years, transcatheter PVL closure has proven to be highly effective when performed in experienced centers. Transcatheter PVL clos

Metabolic syndrome (MetS) is associated with ectopic fat deposition, chronic systemic inflammation, oxidative stress, and increased susceptibility to organ dysfunction. While its cardiovascular consequences have been extensively studied, pulmonary al-terations remain less well characterized, particularly with respect to ferropto-sis-regulatory pathways. In this study, we evaluated ectopic fat deposition, TLR4/NF-κB signaling pathway, and ferroptosis-regulatory markers in a MetS rat model and analyzed the therapeutic potential of combined resveratrol plus quercetin (combined R + Q) supplementat

Achondroplasia is the most common skeletal dysplasia, characterized by disproportionate short stature and associated craniofacial, musculoskeletal, and neurological abnormalities. Respiratory complications are frequent and represent a major contributor to morbidity and mortality. Sleep-disordered breathing (SDB), encompassing obstructive (OSA) and central sleep apnea (CSA), is the most prevalent respiratory complication with pediatric prevalence reported between 22% and 93%. OSA results primarily from multilevel upper-airway obstruction related to craniofacial abnormalities and adenotonsillar

INTRODUCTION: Hammock mitral valve (HMV), or mitral arcade (MA), is an extremely rare congenital mitral anomaly characterized by absent fibrous chordae and hypertrophic papillary muscles directly inserting into the leaflets. This rare valve structure can trigger severe mitral regurgitation and overwhelm a newborn's heart and lungs within days. Because symptoms often resemble those of more common respiratory or cardiac issues, early diagnosis is difficult - but vital. Recognizing the anomaly in time and referring the infant to a specialized center can make the difference between life and death.

BACKGROUND: Congenital aerodigestive fistulae (ADF) represent extremely rare developmental malformations derived from disrupted embryological separation of the primitive foregut, which can be classified by lesion location into tracheoesophageal fistulae, bronchoesophageal fistulae (BEF), and other subtypes. While typically diagnosed in neonates, isolated BEF without esophageal atresia ("H-type") may remain undetected for decades, presenting in adulthood with chronic, stereotyped symptoms. Endoscopic interventions for congenital BEF remain poorly characterized, with limited evidence regarding p

A 79-year-old woman presented with progressive hemoptysis. Computed tomography revealed a systemic artery-to-pulmonary artery fistula (SA-PAF) extending from the left upper lobe to the anterior mediastinum. The SA-PAF was treated with transarterial embolization followed by surgical resection, resulting in resolution of the hemoptysis. Pathological examination of the resected specimen incidentally revealed ectopic pancreas (EP) adjacent to the SA-PAF, along with chronic inflammation of the surrounding lung parenchyma and pleura. These findings suggest that mediastinal EP may be associated with

OBJECTIVE: Syndromic conditions are frequently associated with congenital heart disease (CHD). Their distribution, recognition and prognostic impact in adults with CHD (ACHD) remain poorly characterised due to limited assessment beyond childhood.
METHODS: We retrospectively analysed 9826 patients with ACHD from a tertiary ACHD centre. Syndromic conditions were identified from clinical records and categorised as chromosomal, single-gene or other syndromes. Clinical characteristics, comorbidities and CHD complexity were compared between syndromic and non-syndromic patients. Multivariable Cox re

BACKGROUND: Patent foramen ovale (PFO) and pulmonary arteriovenous malformations (PAVMs) are congenital vascular abnormalities that can lead to right-to-left shunting, potentially causing paradoxical embolism.
CASE SUMMARY: We describe a case of a 39-year-old woman who presented with syncope and left-sided facial numbness; her brain imaging confirmed a subacute lacunar infarct in the left thalamus. A transthoracic echocardiogram showed a PFO with moderate to severe right-to-left shunting. The patient underwent successful PFO closure. Postprocedurally, she developed transient hypotension. Comp

Congenital lung malformations (CLMs) are developmental conditions characterized by overlapping clinical and imaging features, with hybrid lesions that combine congenital pulmonary airway malformation (CPAM) and bronchopulmonary sequestration (BPS) becoming increasingly recognized by multidetector CT (MDCT). We present the case of a six-year-old girl with a one-year history of recurrent low-volume hemoptysis. Her chest radiograph indicated a subtle opacity in the right basal area. Thoracic CT angiography revealed parenchymal consolidation in the right lower lobe, characterized by an air broncho

Ectopic hormone secretion from neuroendocrine neoplasms in the lung (lung-NENs) is rare and poorly explored. Emerging evidence indicates that hormone-secreting NENs carry a worse prognosis. We hypothesised that ectopic hormone production and secretion in lung-NENs is a negative prognostic marker. A retrospective cohort of 167 patients with lung-NENs (121 females, median age: 68 (min-max: 15-85) years), excluding small cell lung cancer, was analysed for clinical and biochemical data and outcome. Tumour tissue was evaluated for serotonin and calcitonin expression. Among 127 patients with hormone

Primary ciliary dyskinesia (PCD) is a rare hereditary disorder characterized by defective motile cilia and impaired mucociliary clearance in the respiratory tract. In recent years, significant advances have been made in both clinical and basic research on PCD, especially regarding the identification of newly discovered pathogenic genes. However, early diagnosis and treatment of PCD in China still face considerable challenges. In response, a new expert consensus on the diagnosis and treatment of PCD in China has been developed. This consensus was initiated by the Genetic and Rare Lung Disease G

RATIONALE: A cross-sectional study employing magnetic resonance imaging (MRI) of paranasal sinuses recently showed a high prevalence and extent of chronic rhinosinusitis (CRS) in children with primary ciliary dyskinesia (PCD). However, longitudinal imaging data on CRS onset and progression are lacking.
OBJECTIVE: To longitudinally evaluate CRS in PCD from infancy through adolescence with MRI.
METHODS: 22 children with PCD (median baseline age 8.5 year, range 0.0-18.0 year) underwent a median of 3 (range 1-9) annual standardized paranasal sinus MRI examinations. MRI were assessed by 2 indepen

INTRODUCTION: Eisenmenger syndrome and pulmonary arterial hypertension (PAH) due to unrepaired congenital shunts, including atrial septal defect (ASD), ventricular septal defect (VSD) and patent ductus arteriosus (PDA), remain life-threatening conditions despite advances in congenital heart disease (CHD) care. In this population, vasodilator-based therapies effective in other forms of PAH have shown limited benefit, and no disease-modifying treatment has been established. Sotatercept, an activin-signalling inhibitor, improved exercise capacity and haemodynamics in phase 2/3 PAH trials; however
