Risk of Aortic Root Rupture in Bicuspid Aortic Stenosis: The ABC Sizing Algorithm for SAPIEN 3 Valves.

BACKGROUND: This study evaluated the ability of the ABC Bicuspid Sizing Algorithm to predict aortic root rupture in patients treated with transcatheter aortic valve replacement using SAPIEN 3 valves. METHODS: We conducted a retrospective multicenter study among 15 centers in 7 countries of aortic root rupture cases and controls without root complications. Computed tomography analysis was performed blinded to rupture status. The ABC algorithm was applied to classify risk (high-risk criteria): (1) annular oversizing >10%, (2) intercommissural distance - valve diameter ≤1 mm, (3) maximum sinus d

Data that matter: what outcomes matter to patients with congenital heart disease and their carers and which should be routinely measured? A qualitative asynchronous online discussion forum.

OBJECTIVES: We wanted, as part of a wider study, to elicit perspectives of patients with congenital heart disease (CHD) and/or their parents/carers about which outcomes of CHD and/or cardiac surgery matter and what should be measured and reported. DESIGN: Qualitative, closed, asynchronous, online discussion forums underpinned by an interpretivist framework. SETTING AND PARTICIPANTS: Three patient charities in the UK set up and moderated separate online forums for adult patients with CHD/carers, adolescent patients with CHD and parents/carers of children and young people with CHD. ANALYSIS:

Occurrence and Determinants of Periprosthetic Fractures of the Femur During Intraoperative and Post-operative Period in Patients Undergoing Hip Arthroplasty: A Prospective Cohort Study.

BACKGROUND: Stachybotrys chartarum is a damp building mould and a potent toxin producer that has been related to serious cases of respiratory health problems. However, the direct link between exposure and health symptoms has not been established. OBJECTIVE: To examine the mechanism by which exposure to spores of satratoxin producing and non-producing S. chartarum strains induce inflammatory responses in murine lungs. METHODS: BALB/c mice were intranasally exposed for 3 weeks to spores of a satratoxin-producing and a non-producing S. chartarum strain. Inflammatory cell infiltration was charac

Transcatheter Mitral and Aortic Paravalvular Leak Closure: Innovations, Challenges, and Future Directions.

Paravalvular leak (PVL) is a well-known complication of valve replacement, occurring in up to 10% of aortic and 17% of mitral prosthetic valves. Although many leaks are small and clinically insignificant, ≈1% to 5% of cases are associated with clinically relevant complications, such as heart failure symptoms and hemolytic anemia. Surgical reoperation is currently considered the first-line option; however, it encompasses high mortality and morbidity risks. In the past years, transcatheter PVL closure has proven to be highly effective when performed in experienced centers. Transcatheter PVL clos

Hammock mitral valve in a 12-day-old female: pathophysiology, management and legal issues.

INTRODUCTION: Hammock mitral valve (HMV), or mitral arcade (MA), is an extremely rare congenital mitral anomaly characterized by absent fibrous chordae and hypertrophic papillary muscles directly inserting into the leaflets. This rare valve structure can trigger severe mitral regurgitation and overwhelm a newborn's heart and lungs within days. Because symptoms often resemble those of more common respiratory or cardiac issues, early diagnosis is difficult - but vital. Recognizing the anomaly in time and referring the infant to a specialized center can make the difference between life and death.

Congenital aerodigestive fistula with extraordinarily delayed adult presentation: a case report and review of endoscopic closure for bronchoesophageal fistula.

BACKGROUND: Congenital aerodigestive fistulae (ADF) represent extremely rare developmental malformations derived from disrupted embryological separation of the primitive foregut, which can be classified by lesion location into tracheoesophageal fistulae, bronchoesophageal fistulae (BEF), and other subtypes. While typically diagnosed in neonates, isolated BEF without esophageal atresia ("H-type") may remain undetected for decades, presenting in adulthood with chronic, stereotyped symptoms. Endoscopic interventions for congenital BEF remain poorly characterized, with limited evidence regarding p

Distribution, recognition and clinical implications of syndromic conditions in adults with congenital heart disease.

OBJECTIVE: Syndromic conditions are frequently associated with congenital heart disease (CHD). Their distribution, recognition and prognostic impact in adults with CHD (ACHD) remain poorly characterised due to limited assessment beyond childhood. METHODS: We retrospectively analysed 9826 patients with ACHD from a tertiary ACHD centre. Syndromic conditions were identified from clinical records and categorised as chromosomal, single-gene or other syndromes. Clinical characteristics, comorbidities and CHD complexity were compared between syndromic and non-syndromic patients. Multivariable Cox re

Concurrent Patent Foramen Ovale and Pulmonary Arteriovenous Malformation Etiology: Complex Case of Cryptogenic Stroke.

BACKGROUND: Patent foramen ovale (PFO) and pulmonary arteriovenous malformations (PAVMs) are congenital vascular abnormalities that can lead to right-to-left shunting, potentially causing paradoxical embolism. CASE SUMMARY: We describe a case of a 39-year-old woman who presented with syncope and left-sided facial numbness; her brain imaging confirmed a subacute lacunar infarct in the left thalamus. A transthoracic echocardiogram showed a PFO with moderate to severe right-to-left shunting. The patient underwent successful PFO closure. Postprocedurally, she developed transient hypotension. Comp

Hybrid Congenital Pulmonary Malformation Revealed by Chronic Hemoptysis in a Six-Year-Old Girl: A Case Report.

Congenital lung malformations (CLMs) are developmental conditions characterized by overlapping clinical and imaging features, with hybrid lesions that combine congenital pulmonary airway malformation (CPAM) and bronchopulmonary sequestration (BPS) becoming increasingly recognized by multidetector CT (MDCT). We present the case of a six-year-old girl with a one-year history of recurrent low-volume hemoptysis. Her chest radiograph indicated a subtle opacity in the right basal area. Thoracic CT angiography revealed parenchymal consolidation in the right lower lobe, characterized by an air broncho

[Expert consensus on diagnosis and treatment of adult primary ciliary dyskinesia (2025 edition)].

Primary ciliary dyskinesia (PCD) is a rare hereditary disorder characterized by defective motile cilia and impaired mucociliary clearance in the respiratory tract. In recent years, significant advances have been made in both clinical and basic research on PCD, especially regarding the identification of newly discovered pathogenic genes. However, early diagnosis and treatment of PCD in China still face considerable challenges. In response, a new expert consensus on the diagnosis and treatment of PCD in China has been developed. This consensus was initiated by the Genetic and Rare Lung Disease G

Onset and development of chronic rhinosinusitis from infancy through adolescence in primary ciliary dyskinesia detected by longitudinal magnetic resonance imaging.

RATIONALE: A cross-sectional study employing magnetic resonance imaging (MRI) of paranasal sinuses recently showed a high prevalence and extent of chronic rhinosinusitis (CRS) in children with primary ciliary dyskinesia (PCD). However, longitudinal imaging data on CRS onset and progression are lacking. OBJECTIVE: To longitudinally evaluate CRS in PCD from infancy through adolescence with MRI. METHODS: 22 children with PCD (median baseline age 8.5 year, range 0.0-18.0 year) underwent a median of 3 (range 1-9) annual standardized paranasal sinus MRI examinations. MRI were assessed by 2 indepen

Protocol for an open-label, randomised, controlled trial to evaluate the efficacy and safety of sotatercept add-on therapy compared with pulmonary vasodilator-based standard of care for pulmonary vasodilator-resistant pulmonary arterial hypertension associated with unrepaired congenital shunts (atrial septal defect, ventricular septal defect or patent ductus arteriosus), including Eisenmenger syndrome: the SuMILE trial.

INTRODUCTION: Eisenmenger syndrome and pulmonary arterial hypertension (PAH) due to unrepaired congenital shunts, including atrial septal defect (ASD), ventricular septal defect (VSD) and patent ductus arteriosus (PDA), remain life-threatening conditions despite advances in congenital heart disease (CHD) care. In this population, vasodilator-based therapies effective in other forms of PAH have shown limited benefit, and no disease-modifying treatment has been established. Sotatercept, an activin-signalling inhibitor, improved exercise capacity and haemodynamics in phase 2/3 PAH trials; however

Association of social determinants of health and all-cause mortality among individuals across cardiovascular-kidney-metabolic stages 0-4: a nationwide prospective cohort study.

Clozapine remains the antipsychotic of choice for refractory schizophrenia despite its propensity for serious blood disorders. When neutropenia or agranulocytosis occur in people taking clozapine, cessation of treatment is mandated and relapse often results. Because such patients are usually unresponsive to other antipsychotics, many clinicians consider restarting clozapine, despite the risks involved. However, the risks of clozapine rechallenge vary according to the cause and nature of the blood dyscrasia. Neutropenia can arise because of factors unrelated or indirectly related to clozapine t

Imaging findings in pediatric interferonopathies.

Type I interferon (IFN-I) is a proinflammatory cytokine that activates signalling pathways crucial for antiviral defence in innate immunity. The IFN-I system is a self-preserving mechanism that normally responds to exogenous nucleic acids derived from pathogens, but not to endogenous nucleic acids from host cells. However, genetic defects that constitutively activate this pathway can lead to aberrant IFN-I production (the "IFN signature") and sterile inflammation (autoinflammation), giving rise to a group of monogenic disorders collectively termed "autoinflammatory interferonopathies." This em

Racial and Ethnic Differences in Transcatheter Coarctation Repair Among Adolescents and Adults.

Racial and ethnic (RAE) differences in the presentation and outcomes of coarctation of the aorta (CoA) among adolescents and adults undergoing transcatheter coarctation repair (TCR) remain poorly defined. We retrospectively analyzed patients aged ≥16 years who underwent TCR at US centers participating in the NCDR IMPACT registry between 2011 and 2023. Among 2,267 patients, 70.9% were Non-Hispanic White (NHW), 18.5% Hispanic (H), 7.4% Non-Hispanic Black (NHB), and 3.3% Non-Hispanic Other (NHO), demonstrating underrepresentation of Hispanic and NHB patients. Late CoA presentation occurred in 53.

Clinical Outcomes of Chronic Airway Infection in Primary Ciliary Dyskinesia.

INTRODUCTION: Primary ciliary dyskinesia (PCD) is a rare disorder of impaired respiratory mucociliary clearance and chronic pulmonary infections. The clinical impact of chronic infection is poorly understood. The objective of this single-center, retrospective cohort study was to assess clinical outcomes of people with PCD and chronic airway infection. We hypothesized that chronic airway infection would be associated with greater declines in lung function and higher pulmonary exacerbation rates. METHODS: Linear mixed effects models were used to analyze the association of chronic infection with

Alveolar Capillary Dysplasia With Misaligned Pulmonary Veins (ACDMPV): Description of Two Cases and Literature Review.

INTRODUCTION: Alveolar capillary dysplasia with misaligned pulmonary veins (ACDMPV) is one of the major causes of neonatal interstitial lung disease, causing persistent pulmonary hypertension (PH) of the newborn, although symptoms can present later in infancy. Two patients were recently diagnosed with ACD at our Institution and prompted us to perform a structured review of the condition. METHODS: We considered published articles from 2018 to 2025. The final population analyzed was composed of 165 patients, including our cases. From each publication, we extrapolated: clinical presentation, rad

Liposomal amphotericin B (AmBisome) is a lipid-associated formulation of the broad-spectrum polyene antifungal agent amphotericin B. It is active against clinically relevant yeasts and moulds, including Candida spp., Aspergillus spp. and filamentous moulds such as Zygomycetes, and is approved for the treatment of invasive fungal infections in many countries worldwide. It was developed to improve the tolerability profile of amphotericin B deoxycholate, which was for many decades considered the gold standard of antifungal treatment, despite being associated with infusion-related events and nephr
