Systematic Reanalysis of Whole-Exome Sequencing in Genetically Unsolved Pediatric Primary Ciliary Dyskinesia.

BACKGROUND: Primary ciliary dyskinesia (PCD) is a genetically heterogeneous disorder, and despite advances in next-generation sequencing, a substantial proportion of clinically suspected pediatric cases remain without a molecular diagnosis. This persistent diagnostic gap reflects limitations of earlier analysis pipelines and incomplete knowledge of PCD-related genes. Reanalysis of existing whole-exome sequencing (WES) data using updated resources has become a practical strategy to improve diagnostic yield in pediatric populations. METHODS: We retrospectively reanalyzed WES data from 34 Turkis

Comparison between segmentectomy and lobectomy for treating congenital lung malformation: A NSQIP database study.

BACKGROUND: Congenital pulmonary airway malformation (CPAM) and bronchopulmonary sequestration (BPS) are diagnosed more often now than in past decades, largely because prenatal imaging has improved substantially. Thoracoscopic lobectomy is still the default operation, though anatomic segmentectomy has gained traction to spare healthy lung tissue. We used a national pediatric surgical database to compare the two approaches during the first 30 days after surgery. METHODS: Patients under 19 years of age with CPAM or BPS who underwent thoracoscopic lobectomy or segmentectomy were identified in th

A Novel Frameshift Variant in SPAG1 Likely Causes Primary Ciliary Dyskinesia in Cocker Spaniels in Australia.

Primary ciliary dyskinesia (PCD) is a clinical syndrome that in dogs primarily manifests as chronic respiratory disease associated with cilial malfunction. The current study employs whole-genome sequencing and a candidate gene approach to uncover the genetic basis of PCD in three Cocker Spaniel siblings following diagnosis of their respiratory cilia by scanning electron microscopy and high-speed video microscopy. Absence of the disorder in the parents suggested autosomal recessive inheritance. A 29 bp frameshift insertion in the eleventh exon of the candidate gene sperm-associated antigen 1 (S

Pulmonary-to-systemic flow ratio: lung cancer with partial anomalous pulmonary venous return.

We investigated the usefulness of pulmonary-to-systemic flow ratio as an indicator for vascular reconstruction for pulmonary resection in two patients with primary lung cancer complicated by partial anomalous pulmonary venous return. The lung cancer and partial anomalous pulmonary venous return were in different lobes; therefore, intraoperative pulmonary-to-systemic flow ratio was measured. In one case, the pulmonary-to-systemic flow ratio was 1.06, and surgical repair was deemed unnecessary. In the other case, the pulmonary-to-systemic flow ratio was 2.20; because of the high risk of postoper

Airway Involvement in Conradi-Hünermann-Happle Syndrome: A Novel Clinical Manifestation.

We report the first documented case of airway involvement in Conradi-Hünermann-Happle syndrome (CDPX2), an X-linked dominant form of chondrodysplasia punctata caused by pathogenic variants in EBP. A 2-month-old female with genetically confirmed CDPX2 developed severe subglottic stenosis and persistent respiratory distress requiring CPAP; cross-sectional imaging demonstrated extensive laryngotracheal calcifications. Serial endoscopic balloon dilations successfully restored airway patency, enabled weaning from respiratory support, and avoided tracheostomy, expanding the recognized clinical spect

Computational Predictive Modeling of Surgical Outcomes in Total Anomalous Pulmonary Venous Connection: Assessing the Impact of Pulmonary Venous Confluence Size on Preoperative Planning.

To develop a predictive model for optimal anastomosis sizing in TAPVC surgery, focusing on the role of pulmonary venous confluence (PVC) size. A patient-specific fluid-structure interaction (FSI) model simulated blood flow through various anastomosis sizes. Key variables included body weight, anastomosis length, and PVC size. The model's predictions were validated against postoperative echocardiographic measurements from nine TAPVC cases. A strong positive correlation was found between flow velocity and the ratio of body weight to anastomosis length and PVC circumference. Including PVC size si

[Double-decker Repair for Partial Anomalous Pulmonary Venous Return in an Adult:Report of a Case].

We report the case of a 19-year-old woman with partial anomaly of pulmonary venous return (PAPVR) associated with a secundum atrial septal defect( ASD). She initially presented with dyspnea during a febrile episode and was diagnosed with ASD but was lost to follow-up. One year later, she developed progressive exertional dyspnea. Contrast-enhanced computed tomography (CT) demonstrated anomalous drainage of the right upper pulmonary vein into the superior vena cava, confirming the diagnosis of PAPVR. Elective surgical repair using the double-decker technique was performed. Postoperative recovery

Rare presentation of congenital cystic adenomatoid malformation type II detected in adulthood - a case report.

Congenital cystic adenomatoid malformation (CCAM) is a very rare innate pulmonary disorder characterized by the formation of cysts and abnormal dilatation of the respiratory bronchioles. The etiology of the disease remains unknown. CCAM is most commonly diagnosed prenatally or during the neonatal period, and diagnosis in adulthood is extremely rare. The condition is classified into subtypes based on the morphology of the cysts and the extent of pulmonary parenchymal involvement. Clinical symptoms are nonspecific. The aim of this study is to present a case of CCAM type II diagnosed de novo in a

Isoprothiolane disrupts mitochondrial and endoplasmic reticulum homeostasis and impairs development of the embryonic nervous system in zebrafish.

Isoprothiolane (IPT) is a widely used fungicide in Asian rice cultivation, yet its toxicological profile remains incompletely characterized, particularly regarding dermal exposure relevant to occupational settings. This study evaluated the cytotoxic mechanisms and developmental consequences of IPT exposure using human keratinocytes (HaCaT), dermal fibroblasts (HDF), and zebrafish (Danio rerio) embryos. In human skin cells, IPT produced predominantly cytostatic effects associated with severe mitochondrial dysfunction, including membrane depolarization, impaired oxidative phosphorylation, and a

An Unusual Cause of Common Chamber to LA Anastomosis Obstruction in Supracardiac TAPVC.

This report details a rare cause of early postoperative pulmonary venous obstruction following supracardiac total anomalous pulmonary venous connection repair in a 4-week-old infant. Despite a successful sutureless anastomosis, intraoperative transesophageal echocardiography revealed significant flow acceleration (1.69 m/s and a mean gradient of 5 mm Hg). Surgical inspection identified that the left atrial appendage (LAA) was sagging over the common chamber-to-left atrial anastomosis, causing an obstruction. The mechanical obstruction was relieved by suturing the LAA to the undersurface of the

Thirty-year nationwide research on epidemiology, treatment, and outcomes of congenital lung malformations.

Congenital lung malformations (CLMs) may result in inflammation, malignancy, lung-dependent heart disease, and increased mortality. This study aimed to analyze the epidemiology of congenital lung malformations, changes in the prevalence, treatment patterns, and outcomes, including mortality. This nationwide retrospective research included all patients diagnosed with CLMs during 1986-2015. Histological findings, treatments, and primary outcomes were reviewed. Mortality due to CLMs was analyzed using official death certificates. The detection of CLMs increased from 1.9/100 000 live births (1990)

Supracardiac Total Anomalous Pulmonary Venous Connection with Left Main Bronchial Compression in an Infant.

Total anomalous pulmonary venous connection (TAPVC) is a rare congenital cardiac malformation, and cases complicated with airway stenosis are even less common, posing substantial diagnostic and therapeutic challenges. We report a 4-month-old male infant with supracardiac TAPVC, accompanied by left main bronchial stenosis, a central atrial septal defect (ASD), and severe pulmonary hypertension. Following supportive management, the patient underwent TAPVC repair and atrial septal defect patch repair. Intraoperatively, weaning from cardiopulmonary bypass initially failed due to severe pulmonary h

A rare case of non-obstructed total anomalous pulmonary venous connection with a giant superior vena cava aneurysm complicated by acute young ischemic stroke.

BACKGROUND: Non-obstructed total anomalous pulmonary venous connection (TAPVC) with a giant superior vena cava aneurysm (SVCA) is rare. This combination in a young patient presenting with acute ischemic stroke has not been previously reported. CASE PRESENTATION: We report a 32-year-old male who presented with acute ischemic stroke. He also had TAPVC, SVCA and atrial septal defect with a bidirectional shunt (primarily right to left), which may represent a potential mechanism for the index stroke. And we discussed the possible mechanisms in detail. CONCLUSIONS: This case underscores the need for

Case Report: Bronchogenic cysts with pericardial hypoplasia are directly supplied by the descending aorta.

Bronchogenic cysts (BCs) are a type of congenital mediastinal tumor. This article reports a case that a BC associated with pericardial hypoplasia receives direct arterial supply from the descending aorta, forming a structure resembling pulmonary sequestration. A 17-year-old female was accidentally discovered to have a mediastinal cyst during a routine physical examination. The patient was asymptomatic. Computed tomography (CT) imaging revealed that there was a thick blood vessel on the surface of the cyst, which directly connected to the descending aorta. Following comprehensive preoperative v

Keeping up the beat of Kleefstra syndrome.

Long QT syndrome (LQTS) is a rare and potentially life-threatening arrhythmia characterized by delayed repolarization on electrocardiogram (EKG) evaluation. Although LQTS is primarily associated with defects in ion channels, in approximately 20% of cases the genetic cause remains unknown. Kleefstra syndrome (KS), a rare neurodevelopmental disorder caused by alterations in the EHMT1 gene, has been associated with various cardiac abnormalities, including structural defects and arrhythmias. Here we report the second case of LQT in a patient with KS, strengthening the association between these two
