Pericardial Cyst at the Left Cardiophrenic Angle Mimicking Localized Effusion: A Diagnostic Dilemma.

Pericardial cysts are rare, benign, congenital anomalies of the pericardium, usually located within the right cardiophrenic space, often discovered incidentally during imaging performed for unrelated reasons. They usually do not cause symptoms and are detected by chance. Chest X-ray, echocardiography, and chest computed tomography (CT) or magnetic resonance imaging (MRI) are useful diagnostic tools. Nevertheless, after safe diagnostic, management options include conservative management with follow-up, percutaneous aspiration of the cyst, and excision of the cyst (direct invasive therapeutic ap

Intralobar pulmonary sequestration with bronchogenic cyst: a case report.

Intralobar pulmonary sequestration (ILS) with concurrent bronchogenic cyst represents a rare congenital malformation often diagnosed incidentally. We present a 48-year-old asymptomatic female whose routine Computed Tomography (CT) revealed a left lower lobe mass with aberrant systemic arterial supply (thoracic aortic branch) and a separate mediastinal cyst. Thoracoscopic resection confirmed ILS (48.11 mm × 37.45 mm solid-cystic lesion with vascular anomalies) and bronchogenic cyst (27.91mm × 24.16 mm). This case demonstrates three critical findings: (i) Contrast-enhanced CT with vascular recon

Incidental finding of supra cardiac total anomalous pulmonary venous connection with Tetralogy of Fallot: A case report.

Tetralogy of Fallot (TOF) is the most common cyanotic congenital heart disorder (CHD) worldwide and is known to be associated with a variety of other CHDs. This case report explores a rare incidental combination of TOF and supracardiac total anomalous pulmonary venous connection (TAPVC). Such a coexistence is exceedingly uncommon with only a handful of reported cases which prompts the need for documentation, especially its management in a developing country. Prompt diagnosis is challenging due to atypical symptoms which emphasises the importance of comprehensive imaging. Surgical intervention,

A novel CTNND1 genotype-phenotype association: total anomalous pulmonary venous connection in a fetus.

We present the first case of CTNND1 variant with a prenatal clinical phenotype of total anomalous pulmonary venous connection. The genetic analysis identified a heterozygous pathogenic variant NM_001085458.2: c.2098C>T (p.R700*) in the CTNND1 gene of the fetus. This case expands the phenotypic spectrum of CTNND1 gene, provides a novel clinical reference for prenatal diagnosis, and suggests that the CTNND1 gene plays a crucial role in early cardiac development.

Open Cyst Drainage for Reliable Decompression in a Critically Ill Neonate with Congenital Pulmonary Airway Malformation.

INTRODUCTION: Congenital pulmonary airway malformation (CPAM) is a rare congenital lung lesion that is often diagnosed prenatally. Large macrocystic lesions can cause severe respiratory compromise after birth, requiring urgent decompression. Although staged management with cyst drainage followed by delayed resection has been reported, the optimal method for achieving reliable decompression in critically unstable neonates remains unclear. We report a case in which emergency open cyst drainage under direct visualization enabled secure catheter placement and reliable decompression before definiti

Mediastinal thyroglossal duct cyst after thyroidectomy mimicking recurrent thyroid disease: a case report.

Thyroglossal duct cysts (TGDCs) are common congenital midline neck lesions, but mediastinal extension in adults is exceptional and may mimic recurrent thyroid disease after thyroidectomy. We report a 55-year-old woman with dysphagia and exertional dyspnoea one year after near-total thyroidectomy for benign multinodular hyperplasia. Computed tomography demonstrated a 76-mm heterogeneous cervico-mediastinal mass extending to the aortic arch with tracheal displacement. Scintigraphy showed no uptake, and fine-needle aspiration revealed squamous epithelial elements with atypia, creating concern for

Impact of Changes in the Profiles of Patients' Backgrounds on the Indications of Thoracoscopic Lobectomy for Congenital Lung Malformations: A Multicenter Retrospective Study in Southern Japan.

INTRODUCTION: The number of patients with congenital lung malformations (CLM) is increasing because of prenatal diagnosis. We investigated the changes in the patient backgrounds and the surgical approach for lobectomy, evaluating the surgical outcomes of thoracoscopic surgery. METHODS: We enrolled 50 CLM patients who underwent lobectomy between 1995 and 2024. Patients were divided into three 10-year groups to evaluate the trends in patient backgrounds. We also divided them into the thoracotomy and thoracoscopy groups to compare the patient backgrounds and surgical outcomes. RESULTS: An incre

Pulmonary sequestration with a rare renal artery subtype: a comprehensive clinicopathological analysis of 20 pediatric cases.

OBJECTIVE: This comprehensive study of 20 pediatric pulmonary sequestration (PS) cases primarily aims to demonstrate the pivotal role of histopathological confirmation in achieving a definitive diagnosis, particularly in atypical, extrathoracic, or diagnostically inconclusive presentations where imaging alone is often insufficient. METHODS: We conducted a retrospective analysis of clinical data from 20 children with pathologically confirmed PS who underwent surgical resection at our hospital between October 2022 and March 2025. We conducted a comprehensive assessment of clinical, laboratory, i

Serotonin Associated Mechanisms in Sudden Unexpected Death in Epilepsy: A Review.

Sudden Unexpected Death in Epilepsy (SUDEP) is the leading cause of death in patients with epilepsy (PWE), although the mechanisms are unclear. Early studies have shown that abnormal cardiopulmonary function plays a key role in SUDEP. Cardiopulmonary activity is regulated by the autonomic nervous system. Serotonin (5-hydroxytryptamine or 5-HT) neurons significantly influence respiration and are also closely related to epilepsy. Therefore, serotonin is hypothesized to be involved in SUDEP, and a substantial amount of research has focused on it. Notably, serotonin signals through at least 14 kno

Revisiting LSDMCA: male lethality escape and genotype-phenotype correlations.

Mitochondrial disorders (MDs) are a diverse group of genetic conditions primarily affecting the oxidative phosphorylation (OXPHOS) system and cellular energy production. Among MDs, Linear Skin Defects with Multiple Congenital Anomalies (LSDMCA), or Microphthalmia with Linear Skin Lesions (MLS) syndrome, is a rare X-linked dominant male-lethal disorder characterized by ocular malformations, linear skin defects, and multisystem developmental anomalies. These features are associated with pathogenic variants in genes related to mitochondrial function, including HCCS, COX7B, and NDUFB11 or chromoso

Application of Acute Physical Therapy in Respiratory Failure Secondary to Bilateral Medial Medullary Infarction and Long-Term Follow-Up: A Case Report.

Bilateral medial bulbar infarction (BMMI) represents a rare yet critical form of posterior circulation ischemic stroke encountered in clinical practice. The condition progresses rapidly and is associated with a heightened risk of secondary central respiratory failure, severe pulmonary infection, and multiple systemic functional impairments, including dysphagia, limb paralysis, and sensory abnormalities. Consequently, both the disability and mortality rates remain elevated. Currently, systematic clinical studies focusing on early physical intervention, phased rehabilitation programs, and long-t

Subglottic stenosis in granulomatosis with polyangiitis: clinical, histopathological and therapeutic considerations with presentation of two cases.

Granulomatosis with polyangiitis (GPA), once known as Wegener's granulomatosis (WG), is an uncommon systemic vasculitic disorder characterized by inflammation of the vascular walls, particularly affecting the respiratory tract. One serious complication is subglottic stenosis (SGS), a narrowing of the airway just below the vocal cords. While uncommon, it can be life-threatening and may show up as stridor, dyspnea, or voice changes. Sometimes, it is the first or only sign that someone has GPA. Diagnosing this condition takes a combination of clinical evaluation, imaging, endoscopy, and blood tes

Association of obesity-related blood chemistry alterations with animal welfare outcomes in sterilized cats: A case-control study in Indonesia.

BACKGROUND AND AIM: Obesity is a common health concern in sterilized cats and may adversely affect animal welfare through metabolic disturbances and increased susceptibility to disease. Although sterilization offers several health and population-control benefits, it can predispose cats to excessive weight gain. This study investigated the association between obesity-related alterations in blood chemistry and animal welfare outcomes in sterilized cats by comparing obese and ideal body condition score (BCS) groups in Lampung, Indonesia. MATERIALS AND METHODS: An unmatched case-control study was

Mini-sternotomy as a life-saving access for thoracic endovascular aortic repair in the absence of peripheral options.

Managing descending thoracic aortic disease with thoracic endovascular aortic repair (TEVAR) in patients with extensive peripheral atherosclerosis remains highly challenging. A 66-year-old male was admitted to a district hospital with symptoms of upper respiratory tract infection, where emergency imaging [X-ray, followed by contrast-enhanced computed tomography angiography (CTA)] incidentally revealed a descending thoracic aorta pseudoaneurysm and diffuse atherosclerosis. The patient was urgently transferred to a cardiac surgery department. Past history included arterial hypertension, chronic

A homozygous splicing mutation in CCDC39 caused multiple morphological abnormalities of the flagella in an infertile man with primary ciliary dyskinesia.

Primary ciliary dyskinesia (PCD) is a severe sperm defect, leading to male infertility. PCD affects both respiratory function and sperm motility, as motile cilia and sperm flagella rely on axonemal architecture. Multiple morphological abnormalities of the sperm flagella (MMAF) is a distinct form of asthenoteratozoospermia, characterized by a heterogeneous spectrum of flagellar defects. In recent years, coiled-coil domain-containing ( CCDC ) genes have been shown to play crucial roles in both MMAF and PCD. In this study, a homozygous mutation in CCDC39 , c.1528-2A>G, was identified in a patient
