Management and outcomes of congenital pulmonary airway malformations and bronchopulmonary sequestrations diagnosed in the fetal period.

PURPOSE: To describe the prenatal and postnatal management and outcomes of fetuses prenatally diagnosed with congenital pulmonary airway malformation (CPAM) or bronchopulmonary sequestration (BPS). METHODS: This retrospective study included patients with a prenatal ultrasonographic diagnosis of CPAM or BPS who were treated at a quaternary referral hospital between January 2014 and December 2024. RESULTS: Twenty-seven fetuses were diagnosed with CPAM or BPS by prenatal ultrasonography. Five (18.5%) showed complete regression of the lesions during gestation. Among the 22 fetuses with persisten

Enhanced Recovery After Surgery (ERAS) in Neonatal Cardiac Care: A Case Report of Nursing-Led Management for Obstructed Mixed-Type Total Anomalous Pulmonary Venous Connection.

BACKGROUND: Obstructed mixed-type total anomalous pulmonary venous connection (TAPVC) is a rare life-threatening congenital heart defect requiring urgent, high-risk surgery. Postoperative care is complex and fraught with complications. Enhanced recovery after surgery (ERAS) is a multimodal, evidence-based approach to optimize perioperative care, yet its application in neonatal cardiac surgery is nascent. CLINICAL FINDINGS: A 12-day-old male infant presented with lethargy, poor feeding, and severe respiratory distress (SpO 2 80%-85%), requiring immediate intubation. Examination revealed dimini

Mild SARS-CoV-2 maternal infection in mice induces transient offspring neurodevelopmental aberrance.

Maternal viral infection during pregnancy has been identified as a risk factor for psychiatric disorders and neurodevelopmental abnormalities in offspring. With cumulative SARS-CoV-2 infections now numbering in the hundreds of millions globally, there is a need to evaluate the effects of maternal SARS-CoV-2 infection on offspring brain development and behavior. We developed a mouse model of mild COVID-19 during pregnancy in which SARS-CoV-2 infection is restricted to the respiratory tract. Infected mothers did not show weight loss or changes in litter size, but did exhibit detectable local and

Congenital Pulmonary Airway Malformation Volume Ratio in Fetuses With Congenital Cystic Lung Diseases to Predict Surgery Within 7 Days of Birth.

AIM: To identify congenital pulmonary airway malformation (CPAM) volume ratio (CVR) values that can predict the need for early neonatal surgery. METHODS: We retrospectively reviewed 70 infants diagnosed with congenital cystic lung diseases (CCLD) over the past 10 years at our center (10 with CPAM, 25 with intra- and extra-lobar bronchopulmonary sequestrations, and 35 with bronchial atresia). Perinatal features, the maximum CVR during pregnancy, and the final CVR before delivery were compared between the early surgery group (surgery within 1 week of birth) and the follow-up group (surgery afte

Echocardiography-Assisted Stenting of the Ductus Venosus in Infracardiac TAPVC: Report of a New Technique and Review of the Literature.

Neonates with infracardiac total anomalous pulmonary venous connection (TAPVC) frequently require urgent treatment. If surgical repair is contraindicated due to extreme prematurity, interventional stenting of the ductus venosus (DV) has been introduced successfully to postpone surgery. We report a new technique of transjugular stenting in an 890-g premature infant who presented with infracardiac TAPVC and functional closure of the DV. The DV was successfully crossed with a wire and stented under echocardiographic control. The case underlines the importance of echocardiography in diagnosing and

Prenatal radiofrequency ablation for bronchopulmonary sequestration: a case report of successful fetal intervention and longitudinal outcomes.

INTRODUCTION: Bronchopulmonary sequestration (BPS), a rare congenital pulmonary malformation, is characterized by nonfunctional pulmonary parenchyma with aberrant systemic arterial supply (typically from the aorta or its major branches) and absence of communication with the tracheobronchial tree. Large BPS lesions pose significant fetal risks, including hydrops fetalis, mediastinal shift, and polyhydramnios, necessitating timely prenatal intervention to prevent fetal demise. CASE PRESENTATION: We present a case of successful ultrasound-guided radiofrequency ablation (RFA) of the feeding arter

Surgical repair of extremely late presentation of supracardiac total anomalous pulmonary vein connection: a case series.

BACKGROUND: A late-presenting total anomalous pulmonary venous connection is extremely rare, and surgical repair remains a significant challenge. CASES PRESENTATION: We present two cases of surgical repair for late-presenting supracardiac total anomalous pulmonary vein connection: a 14-year-old female and a 32-year-old female. Both were successfully operated on and were discharged on postoperative days 9 and 29. The second patient had a prolonged hospital stay due to complications of acute kidney injury after surgery and the need for hemodialysis. Both patients had a small left ventricle and

Deep-Inspiration Breath-Hold Strategy Improves Image Quality of Transthoracic Lateral Shoulder Radiography.

Background/Objectives: Transthoracic lateral shoulder radiography serves as an important method for traumatic shoulder injuries and dislocations, but tissue overlap and breathing motion often compromise image quality and are particularly prominent in obese patients. This study aims to compare the image quality differences between the deep-inspiration breath-hold (DIBH) strategy and the routine free-breathing (FB) method in transthoracic lateral shoulder radiography for patients with different body mass indices (BMI), and to clarify the clinical application value of the breath-holding strategy.

Progressive electrocardiographic remodeling despite Tafamidis therapy in transthyretin amyloid cardiomyopathy (ATTR-CM).

BACKGROUND: Electrocardiographic (ECG) abnormalities are a hallmark of transthyretin amyloid cardiomyopathy (ATTR-CM), yet longitudinal ECG changes under disease-modifying therapy remain poorly characterized. METHODS: In this retrospective multicenter study, 140 patients with confirmed ATTR-CM receiving tafamidis underwent serial 12‑lead ECG assessment at baseline and after one year. ECG parameters were analyzed in relation to biomarker-based disease stage, clinical progression, and all-cause mortality. Longitudinal ECG changes were evaluated using Wilcoxon signed rank and McNemar tests. Surv

Lung-Cardiovascular Interactions and Derangements in ARDS.

The acute respiratory distress syndrome (ARDS) is fundamentally characterized by alveolar injury, leading to lung edema, severe hypoxemia, and poorly compliant lungs. Alveolar and cardiovascular pathology are deeply intertwined in the syndrome, which can be marked by clinical cardiac, pulmonary, and systemic vascular abnormalities. Pulmonary vascular dysfunction is common even in the era of lung-protective ventilation, is associated with right ventricular (RV) dysfunction, and worse mortality. Frustratingly, these cardiovascular abnormalities pose management challenges for which we have limite

Anesthetic considerations in surgery for early onset scoliosis: Challenges and advancements.

Scoliosis correction surgery in pediatric patients poses complex anesthetic challenges due to significant anatomical deformities, cardiopulmonary compromise, and the potential for substantial blood loss. Patients frequently exhibit significant cardiopulmonary compromise due to severe thoracic curvature and rib cage distortion, which impairs pulmonary gas exchange and increases the likelihood of perioperative complications. The severity of Cobb's angle, the number of affected vertebrae, and coexisting syndromes like kyphoscoliosis influence these cardiopulmonary changes and can lead to abnormal

Bi-allelic loss-of-function variants in TMEM63B cause syndromic surfactant dysfunction disorder.

Transmembrane protein 63B gene (TMEM63B) encodes a mechanosensitive ion channel expressed in alveolar type II epithelial cells, where it mediates stretch-induced surfactant secretion. While heterozygous gain-of-function variants in TMEM63B have been associated with developmental and epileptic encephalopathy, no human disorder has previously been linked to bi-allelic loss-of-function variants. Here, we report five individuals from four unrelated families with childhood interstitial lung disease and bi-allelic predicted loss-of-function variants in TMEM63B. Affected individuals presented with ea

Infracordal Stenosis: A Glucocorticoid-Responsive Subtype of Autoimmune Laryngotracheal Stenosis.

OBJECTIVE: As our understanding of autoimmune laryngotracheal stenosis (LTS) evolves, distinguishing patients who may benefit from systemic immunosuppression versus those needing only local treatment is increasingly important. In this study, we identify a distinct subset of autoimmune LTS characterized by edema of the inferior true vocal folds that extends to the superior aspect of the cricoid cartilage, termed "infracordal stenosis." The objective of this study is to characterize the clinical presentation and treatment outcomes of infracordal stenosis and compare it to typical autoimmune-rela

Identification of Histone Deacetylase Inhibitor Targeting Type I Interferon and B Cell Abnormalities in Systemic Lupus Erythematosus.

OBJECTIVE: Systemic lupus erythematosus (SLE) is characterized by increased Type I interferon (IFN-I) and autoantibody production. This study aimed to identify drugs that can inhibit both IFN-I and autoantibody production. METHODS: We identified an inhibitor of IFN-I production from a chemical library. Subsequently, we examined its efficacy and underlying mechanisms in suppressing the expression and phosphorylation of upstream signaling molecules for IFN-I production and the differentiation of B cells into plasma cells. Additionally, we examined whether it could alleviate disease severity in
